Employee was installing roof bolt and steel mat. While spinning bolt the mat turned pushing left arm back hurting back on left side.

Employee was changing the tire on the Water Truck using a 1" air wrench, wrench got caught and twisted his hand and wrist.

Tramming the continuous miner and placing himself too close to the miner and the coal rib, pinching himself between the body of the miner and the coal rib, breaking his left leg and bruising the right leg.

The victim was drilling top to install roof bolts and was using a 12" wrench to finish the hole and the wrench broke and struck his hand and thumb, (right hand) broke thumb.

The operator was lowering the head of the miner from the operator side as the employee was looking underneath the machine with his left arm lying on the miner pan, catching arm between the pan and head on the off side of miner, breaking employee's arm.

Mashed right wrist and fingers drilling hole to install roof bolt.

Victim was installing roof bolt, he reached to drop head but activated boom swing. causing boom to strike his head pushing him back into coal rib.

Employee was using a hand drill to drill a hole in a piece of metal. The drill bit hung causing the drill to twist employee's right hand.

Employee was drilling a hole. The steel stopped up causing dust to blow back in employee's face.

Employee was installing roof bolts when his glove became caught on the bolt while it was rotating & it twisted his right hand.

EE was moving a Steam Jenny when the jack pin broke causing the end to fall, striking EE on top of the foot.

While bending roof bolt, EE slipped and fell, striking his chin on a roof bolt plate, resulting in a cut on chin.

While operating roof bolter, EE caught left thumb on boom arm, resulting in mashing left thumb tip.

While operating roof bolter, employee caught left thumb on bolter head. Crush injury to thumb.

EE WAS REPLACING BELTS ON ROOF BOLTER BLOWER MOTOR. EE WAS ROLLING BELTS ONTO PULLEY WHICH ROLLED BACKWARD, PINNING HIS RIGHT HAND BETWEEN BELT AND PULLEY.

EE WAS CHANGING STEEL WITH HIS LEFT HAND AND ENGAGED THE WRONG LEVER, CAUSING THE MACHINE HEAD TO PIN HIS LEFT HAND AGAINST MINE ROOF.

THE EMPLOYEE WAS OPERATING COAL CRUSHER AND A LG. PIECE OF COAL FROM THE BELT MASHED HIS RT. THUMB.
