While working on North wall mitigation, dozer ( Cat D10T) slid down the dig face and rolled over landing back on it tracks. Employee was seat belted in and received small laceration to forehead and minor bruising to the shoulder.

Mostrando los 22While placing 4" HDPE pipe into the fusing machine to be welded, the jaw that holds the pipe into place fell. It struck the middle knuckle of the miner's left middle finger causing a laceration.
Employee was cutting expanded metal and saw kicked back coming down on employee's left thumb cutting thru his glove and into his thumb. Employee received 3 stitches.
Mill maintenance employee was working on a 14' pipe line and a pump that sanded up. Discharge line slipped out of the upper fitting. Employee was performing maintenance on shovel ropes on P&H shovel. While trying to unspool tugger, finger was placed in wire loop and tugger was retracted pulling finger into spool, pinching finger between wire rope and spool. Employee lost one centimeter of right index finger.
Employe was standing behind co-worker while co-worker was welding and received flash burn to his eyes. EMPLOYEE RECEIVED EYE DROPS FOR TREATMENT. ***EMPLOYEE DID NOT RECEIVE MEDICAL ATTENTION UNTIL 12-27-08. EMPLOYEE WAS ON DAYS OFF UNTIL 1/2/09.
While trying to secure a 30" SDR-17 pipe into a fusing machine, the pipe came out and over the "jaws" of the machine, striking the worker on the right side.
Employee was using floor jack and dunage to straighten a clip on truss, dunage slipped striking employee in the face.
Employee was operating a nail gun with one hand and holding a board with the other hand. Nail went through the board and punctured his finger.
Employee was tightening bolts with tone gun in 400 area, he lost his footing and slipped. Employee placed his left hand on beam, between tone gun handle, pinching his left index finger. Employee panicked and pulled hand out, lacerating left index finger.

The employee who was injured was operating a 240 ton haul truck and was being loaded by a hydraulic shovel when the bucket struck the top portion of haul trucks canopy bed (drivers side). The next day the operator stated that his shoulders hurt due to the haul truck being jarred when the bucket struck the truck.
After a "dog" was welded into place the employee put a wedge in, put his foot on the "dog". Hit the wedge in until liner was as far as possible, got down to check the gap, looked good stood up and was turning to tell another employee to go ahead and stitch it in. When the weld broke and the "dog" struck the employee in the face.
Employee grabbed the pipe and put it in position to line it up with the top head so the driller could stab it and screw the sub onto the pipe. The driller came down a little too fast and the savor sub caught the edge of the pipe pushing down on the pipe to kick outward at the bottom causing a crush injury to his right small finger.
Driller helper was assisting in loading a pneumatic drill hammer onto service vehicle, when hand slipped and pinkie finger was pinched where hammer was designed to close causing injury.
The truck employee was operating was parked under the shovel, a boulder fell out of the shovel into the bed of the truck. Second incident - shovel dropped boulder on tail end of truck causing the bed of the truck to lift up and bounce back down. Employee's neck was jarred both times.
While pulverizing samples, lifting the pulverizer lid and knocking off excess sample, ee's neck popped at the base between shoulder.
Backing under shovel. Shovel spotted bucket too low. Employee hit dutchman, causing bed to go up and come down hard, jarring her.

WHILE USING A HANDHELD ELECTRIC DRILL THE BIT LOCKED UP IN THE MATERIAL AND THE DRILL JERKED IN THE OPERATORS HAND CAUSING A FRACTURE OF ONE OF THE BONES
EMPLOYEE OPENED THE DOOR ON THE FILTER VESSEL BEFORE ALL OF THE PRESSURE WAS RELEASED. THE DOOR OPENED, STRUCK THE VESSEL HOUSING AND RECOILED, STRIKING THE EMPLOYEE'S ARM.
