Employee was operating miner on 004 section EE was cutting coal in the 7R crosscut EE was cutting the first sump in on the left side loading a shuttle car when a piece of bottom rock 24"x30" flipped out from under the ripper head and struck EE in the lower right leg.

Employee was starting to drill a new hole when the steel hit them on the back of hand.

Employee was installing roof straps and turned and struck strap causing laceration to left side of face

Employee changing sprocket on high wall drill when boom on mechanics truck extended casing EE's left hand to be pinched causing laceration to pinky finger on left hand.

Employee was running dozer ran over rock and it shook them around causing lower back pain radiating down into the left leg.

Employee was putting up flypad boards & hit the wrong lever on the roof bolter causing the canopy to hit employee on the right side of head.

Employee was roof bolting. EE had put in finish and push steel. As EE began to drill the hole, loose shirt sleeve got entangled in the drill pad.

Employee was pushing with a dozer in rock, felt pain in lower back.

While employee was installing a roof bolt a damaged roof bolt struck the employee on the left side of face cutting it beside the left eye. Employee received 4 stitches. Came back to work the next day.

Employee was putting a liner in a wash box when he burnt him self with a plasma cutter.

Employee was helping replace segment on an excavator when they set the excavator back down a piece of metal sheared off the segment and struck him in the right leg. He received 5 stitches.

Employee was operating a continuous miner loading a shuttle car when a piece sand stone came out of the conveyor on the miner and struck him on the right side of his hard hat.

Employee was roof bolting and the bolt plate caught his middle finger on he lefty hand and cut it he received 5 stitches.

Employee was drilling a hole for a roof bolt when the drill steel fell out of the hole and hit him on his right thumb he recieved 6 stitches on his right thumb. Came back to work the next day.

Employee was using oxygen and acetylene torch doing repairs. The acetylene hose was leaking and ignited in side of his welding glove and burn his left hand.

While bolting top drill steel fell out of top hitting the top of his right hand

Employee was roof bolting when he lowering canopy on the roof bolter the canopy hit his head.

While running roof bolter he cut his ring finger tip off when lowering the boom had hand on the boom when pot came down cut the ring finger tip off and smashed middle finger of the right hand

Employee was operating a D-9 Caterpillar Bulldozer when he backed up over a rock causing pain in back and numdness in arm.

Employee was installing 6' torque tension bolts when he felt pain in his back

Tightening a bolt on the valve chest caught little finger behind metal tag when pulled finger out cut pinky finger.

Employee was roof bolting when letting down the canopy it caught his left hand on bolts in his tray with the canopy it mashed his middle finger and index finger on his left hand.

While operating a dozer, a hose busted, spread fluid on engine causing smoke to get in the cab.

Employee was standing next to roof bolter when the roof bolter operator moved the bolter forward. The tire on the roof bolter hit employee's left foot.

EE stated that while installing roof support, the drill steel flew out and hit his hand.

Bolting top and plate got his thumb.

Employee was operating Dozer running the fill and said his back was hurting him.

Employee was roof bolting when the bolter pulled up to the next roll of butts the bolter tire hit employee left ankle.

Roof bolter operator was drilling hole for 10' rope bolt when steel clogged with mud. Operator removed 3' pusher from hole leaving 7' of steel in top. While beating 3' pusher on pot to remove mud from steel the remaining 7' of steel fell from hole hitting operator in knee.

Employee was hoisting a beam for a highwall miner. Was swinging beam around pinning his left hand between beam & loader bucket.

Employee was roof bolting when his right index finger was pinched between pin steel and pot.

EE was welding on a end loader bucket using a bottle jack to hold a piece of metal in place when he cut his ring finger on hand on the jack handle. 12 stitches.

EE was changing out suction hose on the roof bolter he put his left hand over pot to check suction when his right hand slipped hitting joy stick allowing pot to raise and pinch left hand between pot and drill steel. It fractured top of his left hand.

Employee had drilled a hole for roof bolt. When he took bottom section of drill steel loose, the top section of steel fell out of hole striking him on his right index finger cutting it. He received 4 stitches.

EE was using air impact wrench on belt tailpiece when dust blew into eyes.

EE was operating CM when he stood up he had pain in his lower back.

Employee stated that when the roof bolter operator took his cable up, it struck him, knocking him down.

Take up pin on continuous miner struck EE on top of his left foot.

Employee was roof bolting when the drill steel got stuck in the roof. Then the steel fell out of the roof striking him on the right hand little finger. He received 17 stitches on his finger.

EE was moving the roof bolter when he stepped and his arm hit the slide lever, and the slide came out and mashed his middle finger on his left hand. Received 15 Stitches on his finger. No lost time.

EE stated that while operating the excavator, a rock fell from the bucket causing the boom to hit the ground, jarring him.

EE was roof bolting and had a piece of drill steel in his hand and was dropping boom of bolter and the boom caught the drill steel and pushed it down on his right foot. This fractured his 2nd toe next to his big toe.

He was installing roof bolts and pulled a muscle in his chest.

EE was roof bolting when his glove caught on drill steel and twisted finger he had a laceration on 2 middle fingers on his left hand. 14 stitches.
