Deep Mine No 8 Coal

Employee fell from a ladder while building a stopping.

While putting up bolts in #6 Face a piece of rock the rib bolt to the rib and past the T bar the rock rolled employee to the ground.

Setting oil cans on back of rubber tire ride and twisted his back.

While bolting #2 heading EE slipped on a flat rock and twisted his right ankle.

Went to power center to use mine phone a rock fell and hit him in the back.

Operating scoop pulling tool car had hand out of scoop deck backing up smashing hand between scoop and material (rock and coal)laying on the bottom.

Shoveling #1 tail piece picked shovel load up twisted right shoulder.

EE was walking to back of bolter to tram the bolter from deck due to steep hill and he slipped and tried to catch his self from falling and caught right hand between the light guard and light

Was bolting #5 heading when a rock fell from top striking ee in right leg.

Another EE was running scoop unloading a new scoop bucket pulled bucket on the ground. EE pulled up to slack the chain, IE stumbled in front of scoop that EE was running mashing IE leg in between the buckets.

Walking back from roof bolter came through a fly pad rock fell from top striking him on head and right shoulder.

Hanging bolter cable stepped on a rock. Twisted left knee

Bolting top and plate got his thumb.

While bolting a rock fell from the roof and hit employee on the hand.

While changing a busted hose on miner rock fell out between bolts hit him in lower back and leg

Building a rock box on #6 belt head. Lifting a piece of sheet metal

Lifting guard on back of sccop. Let it down on left pinky finger. (This is not a lost time accident). Medical case

Employee was cleaning #5 heading the bottom was busted up scooped middle and right side was backing up out of heading to clean left side. Ran over a piece of rock scoop landed real hard putting head into canopy and sent sharp pain down into lower back and left leg.

Shoveling belt EE hit the belt with shovel and the shovel kick back into shoulder and arm.

Employee stated that while loading supplies onto the scoop, he felt pain in his stomach and groin.

While making belt splice and cutting beltline. EE cut his left hand with knife

EE was standing on top of the miner hanging the miner cable he was holding against the top, he strain his back. EE tried to work he called in on 1-27-12 and said he hurt his back. I didn't get to talk to him til 2-7-12. I tried numerous time to get in touch with him. EE didn't say anything to the men on the section or management at the mine.

Operator had drilled a hole he reached over to get a bolt and glue, a piece of rock fell from the mine roof, striking him on the right hand.

Employee stated that while bolting the roof, a piece of rock fell from the top, striking him on the head and leg.

Roof bolter operator was drilling hole for 10' rope bolt when steel clogged with mud. Operator removed 3' pusher from hole leaving 7' of steel in top. While beating 3' pusher on pot to remove mud from steel the remaining 7' of steel fell from hole hitting operator in knee.

Employee stated that while unloading supplies, he got his foot stuck in the mud, he twisted around injuring his back. (Continued working until 1/16/12, started missing work on 1/16/12).

Employee stated that while loading rock dust onto the scoop, the rib rolled off, striking him on the shoulder.

Employee stated that while operating the shuttle car, coal fell from the rib, striking him on the hand.

While installing roof support, a piece of rock fell from the mine roof striking him on the lower back.

While walking across the section, stepped on a rock and twisted his ankle.

An unintentional roof fall occurred in the belt entry one break outby spad no.5509. The fall measured 30'long, 20'wide and 8'thick. The fall does not affect the air flow or travel of men.

EE stated that while cleaning off the top of the cont. miner a piece of rock fell from between the bolts, striking him on the back.

Employee stated that while taping a hose, he cut his hand on a wire.

An unintentional roof fall occurred in the No.6 entry at spad No. 2577. The fall measured 20' high, 20' long and 6' high. The fall did not affect the air flow or the travel of men.

Walking around miner slipped and fell caught his fall with his hand.

Employee stated that while building an overcast, a beam started to fall and he caught it and strained his stomach.

While cleaning off the top of the continuous miner, slipped and fell off, twisting his right ankle.

Holding a ladder for another employee when a piece of draw rock fell and hit right wrist.

Reaching over his tray to get a bolter bit when a rock fell and struck him in the right forearm. Note: I was notified of the incident on 2/9/11 by Three Rivers Medical Center (asking for Insurance information) -Injury: Abrasion of right forearm...I called the mine to verify.

While jacking up the front of the tailpiece, the jack flew out striking him on the side of the face.

While loading supplies, slipped and fell backwards into the supply car.

Had a stopping half way built when it started leaning and trying to fall. The other employee who was helping removed a cap wedge that let the stopping fall. Employee was trying to get out of the way but a cinder block hit him on the right shoulder.

While loading pin supplies in a scoop bucket, a rib rolled off onto his right foot/ankle.

While moving a tailpiece across the track, foot got hung on the track jarring the tailpiece up, the jack flew out from tailpiece striking him on the leg.

A piece of rock fell from the rib, striking him on the foot.

EE stated that while he was moving a rail ride he got his leg pinched between the ride he was moving and another ride.

An unintentional roof fall occurred in the No.5 entry at spad No. 4636. The fall measured 7' thick, 19' wide and 40' long.

Employee stated that while getting off of his ride, he twisted around and injured his back.

An unintentional roof fall occurred in the 6 right and #7 entry at spad #5208. The fall measured 60'long, 19'wide and 9'thick.

An unintentional roof fall occurred off the no.2 section in the no. 5 neutral entry at spad no. 4544. The fall measured 20' long, 20' wide and 8' high. This fall did not affect the travel of men or the flow of air.

An unintentional roof fall occurred on the belt line two breaks inby spad #4361. The fall measured 50' long 20' wide and 6' to 7' thick. The fall did not affect the air flow or the travel of men.

EE stated that while loading timbers onto a scoop, he tripped and fell. The timber landed on his arm.

EE stated that while getting a knot out of the miner cable, it fell from the hanger and struck him on the head and right shoulder.

An unintentional rooffall occured 600' off no.2 section in the no.1 return. The fall measured 35' long, 18' wide and 4' to 5' high.

EE stated that while hanging a miner cable, he injured his stomach.

Employee stated that while carrying two five gallon cans of oil through mud he slipped and twisted his back and light leg.

EE stated that while lifting a miner cable, he injured his back.

An unintentional roof fall occurred in the No. 1 return entry at the drift opening. The fall measured 5' long, 16' wide and 9' high. The fall does not affect the air passage or the travel of men.

Employee stated that while walking on the section he stepped in mud and twisted his knee. Started missing work on 7/14/09

An unintentional roof fall occurred in the No. 7 entry at spad No. 2625. The fall measured 30' long, 20' wide and 3' high. Air passage or the travel of men was not affected.

An unintertional roof fall occurred 5,000 outby. The section, one break inby spad No. 3433 in the return entry. The fall measured 6' high, 40' long and 20' wide. The fall does not affect the travel of men or the passage of air.

Employee stated that while installing bolts in the mine roof, a piece of rock fell from the mine roof striking him on the lower back.

An unintentional roof fall occurred in the No. 6 entry one break outby spad No. 1892, 6000' outby the working section. The fall measured 40' long, 6' high and 19' wide. The faall did not affect the travel of men or the passage of air.

Employee stated that while striking a conveyor chain with a hammer, a piece of metal chipped off the chain and went underneath the bottom of his safety glass, striking him in the eye.

An unintentional roof fall occurred 8,000' outby the section at spad No. 2387. The fall measured 20' long 20' wide and 5' thick. The fall does not affect travel of men or the air flow.

An unintentional roof fall occurred 8,000' outby the section at spad No. 2386. The fall measured 20' long 20' wide and 5' thick. The fall does not affect the travel of men or the air flow.

An unintentional roof fall occurred 3,800' outby the section at spad No. 2718. The fall measured 20' long, 20' wide and 2' thick. The fall does not affect the travel of men or air flow.

Employee stated that while hanging the miner cable on a hook, the cable slipped off the hook and struck him on the head.

Employee stated that while standing on a ladder installing doors, he fell from the ladder and struck his arm on the door.

An unintentional roof fall occurred 1800' outby the No. 2 section in the no. 5 neutral entry at spad no 2980. The fall measured 60' long x 19' wide and 6' thick. The fall does not affect the travel of men or the air flow.

Employee stated that while shoveling coal from under the belt, he twisted his back while he shoveled the coal away from him.

EE fell against shuttle care and broke his right hand.

Wrecked manbus and he hit his right hand on the batteries while trying to catch himself. this accident was reported to me on 8/21/08 that why (late sending this reported in)

Employee stated that while operating the roof bolting machine, a piece of rock fell from the mine roof striking him on the head and shoulder.

An unintentional roof fall occurred in the No.4 entry, one break inby spad No.3194, 2000' outby the working section. The fall measured 80' long, 19' wide and 0' to 6' high. The fall did not affect the travel of men or the passage of air.

An unintentional roof fall occured 5,000 outby the active section in the #2 return entry. The fall measured 20' long 17' wide and 0'-5' thick. The fall does not affect the air flow on the travel by men.

Left ring finger on left hand, putting rock dust in machine. Guard was loose allowing finger to hit auger, had to have 13 stitches in finger.

An unintentional roof fall occurred 2500' outby the active working section in the No. 3 netrual entry. The fall measured 30' long, 20' wide and 6' thick. The fall does not affect the fravel of men or the air flow.

Employment stated that while walking around the roof bolting machine a pieces of rock fell from the mine roof striking him on the leg.

While operating the shuttle car, he ran over an object in the roadway and bounced into the canopy injuring his head and neck.

Employee stated that while operating the cont miner & loading a buggy, a piece of rock fell from between the bolts striking him on left foot.

Employee stated that while bolting the mine roof a piece of rock fell striking him on the arm.

EE stated that while bolting the roof, a piece of rock fell from between the roof bolts, striking him on the right arm and right leg.

Employee went to power center and put his bolter breaker in and it fired, burning his arm.

Changing hose on scoop. When fitting broke loose twisting body causing pain to rib cage.

Rock fell from top & struck the employee's right side, neck, shoulder and arm. 8 1/2 x 6 x 4.

EE was bolting top when a piece of rock fell from the top and hit injured.

Employee stated that while loading an oxygen tank into the cage, he felt a sharp pain in his lower back.

Slipped in the mud and broke right ankle.

Employee stated that while operating shuttle car, he had his hand on the canopy when he turned a corner and scraped the rib with his hand.

Employee said the seat bruised his back. I was not informed of this when the cituation occurred.

EE stated that while operating the roof bolter he lowered the roof bolter pot onto his foot.

Employee stated that while operating shuttle car he hit a hole in the roadway and bumped his head on the canopy.

A piece of rock fell from the mine roof striking the employee on the right shoulder and right arm.

EMPLOYEE STATED THAT HE INJURED HIS BACK WHILE LIFTING A TOOL BOX FROM A METAL BOX

Rock fell on his left hand while he was walking along the side of the shuttle car. He had his hand on the top of the canopy.

IN THE PROCESS OF LOADING AIRLOCK DOORS. DOORS SLID OFF THE SCOOP BUCKET, FELL AND SMASHED EE'S RIGHT HAND.

Employee was cutting a bolt down with a torch when a piece of slag fell through the corner of his glasses. (Started missing on 4/19/05 and returned to work on 4/22/05)

EMPLOYEE STATED THAT WHILE OPERATING THE MINER IN THE NO.5 ENTRY, A PIECE OF ROCK FELL FROM BETWEEN THE ROOF BOLTS STRIKING HIM ON THE HEAD.

Truck hit foot.

Employee stated that while operating the shuttle car he hit a hole in the roadway and bumped his arm and shoulder against the canopy.

EE STATED THAT WHILE LIFTING A MINER CABLE HE FELT PAIN IN HIS NECK AND RIGHT SHOULDER.

EE STATED THAT WHILE DISMOUNTING THE FORKLIFT HIS FOOT SLIPPED & HE FELL TO THE GROUND. (EE CONT'D WORKING DID NOT START MISSING WORK UNTIL 2/7/05.

Employee stated that while cutting a piece of mine belt the knife slipped off and cut his arm.

EE STATED THAT WHILE UNPLUGGING THE CHARGER BELL ON THE MANBUS HE FELT PAIN IN HIS BACK.

Ee was moving a timber from the side of the track, while lifting the timber he felt pain in his back (started missing work on 8-16-04).

EE WAS HANGING BELT CHAIN'S AND FOOT TWISTED AND SPRAINED LEFT KNEE.

EE STATED THAT WHILE TIGHTENING THE BELTS A CHAIN BROKE AND STRUCK HIM IN THE ARM.

EE STATED THAT HE INJURED HIS LOWER BACK WHILE LIFTING THE ROOF BOLTER SHIELDS.

THE LOADER OPERATOR TRIED TO REMOVE THE RADIATOR CAP FROM THE LOADER AND GOT BURNED.

AFTER THE MANBUS DERAILED, EE WAS JACKING IT UP TO SLIDE IT BACK ONTO THE RAILS WHEN THE JACK SLIPPED OUT STRIKING HIM ON THE KNEE.

EMPLOYEE STATED THAT WHILE OPERATING THE SCOOP, A PIECE OF COAL FELL FROM THE RIB STRIKING HIM ON THE NECK AND RIGHT SHOULDER.

THE EMPLOYEE STATED THAT HE INJURED HIS LOWER BACK WHILE LIFTING A MINER CABLE.
