Employee was cleaning out flap on hurricane of Fletcher Roof Bolter. Employee did not notify Roof Bolter partner prior to cleaning. Roof Bolter Partner started bolter, causing the flap to close on injured employees hand.

normal mining conditions, employee was operating a shuttle car, miner conveyor threw a rock and hit shuttle car operators right arm, shoulder and face.
Normal mining conditions. Employee caught middle left finger between a roof bolt plate and the mine roof, crushing the tip of his finger.
Employee is working as a bolter, he finished his shift on 3/20/2012 then went to the ER on 3/21/2012 complaining of shoulder pain. He was diagnosed with tendonitis of the right shoulder. Employee has stated that his right shoulder has been hurting for months.
IE and another EE were cutting roof bolts that were hanging down over #3 belt. IE handed other EE a lit torch as he was getting in position to cut the next bolt. The torch hose caught on the belt structure and turned the flame toward IE, he put his right arm up to block the flame from his face an received a burn to his right forearm.
East panel #2 entry, approx 20' inby center line of #7 Xcut, top had fell out apporx 12-14" semi-domed, employee ran drill steel up against roof. Pressure on steel cause a piece of rock to break off causing drill steel to shift/slide sideways striking index finger on left hand. Employee had moderate bruising/swelling & pain. Doctors results showed a dislocation.
Submitted online 10/16/07 submission # 526437 but had mistake filing so resubmit. Roof bolter operator slipped while moving the drill station, right hand was on canopy post and left hand hit canopy lever, dropping canopy and cutting little finger on right hand.
Injured was installing roof bolts using roof bolter and had hand in a pinch point area and when he lowered the boom it caught his hand and cut his pinky finger on left hand.
Drilling hole for roof bolt, the operator reached out and grabbed while still spinning, twisting left hand, fracturing hand. Note: Empoyee took drug test on 2-6-06 and results came back positive for drugs on 2-10-06 and employee was terminated per company policy.
Injured was chipping weld slag and a piece went into his eye. Injured was wearing glasses but it entered through the side of his glasses. The injured was treated with cream for his eye and prescriptions for pain and anti-biotics.
Injured was welding, using only glasses and a lens for eye protection,and his eyes were burned. Injured went to the ERand was given cream for his eyes and a prescription for pain medication.
INJURED WAS OPERATING THE OFF SIDE OF THE RRII TWIN BOOM FLETCHER ROOF BOLTER, WHEN INJURED ATTEMPTED TO DRILL THE HOLE FOR THE #1 BOLT, A SHARP PIECE OF ROCK DISLODGED FROM THE ROOF, AROUND THE DRILL STEEL AND STRUCK INJURED ON THE RIGHT FOREARM, CREATING A LACERATION.
