While hanging structure, the employees fifth digit (pinky finger) on left hand got caught between the belt and belt chain, causing a laceration requiring stitches. Employee was not wearing gloves at the time of incident.

Employee was bent down making a cable splice with back turned to the rib. A rock fell hitting the employee in the back, resulting in a fracture to back. The size of the rock of 2' x 3' x 9"-14" thick. Incident occurred in the 3rd South Panel Entry 5 Crosscut 13.

Roof fall 5th main west entry 3 XC5 in the neutral beside belt entry. Area was bolted with 8ft TR and 12 foot cable bolts and had some supplemental support extra bolts, straps, Area was in a fault zone, ventilation was not affected. Stoppings were intact, area is dangered off on all sides and will not be cleaned up. Size 20ftX30FtX10Ft high.

Employee slipped and fell while carrying a roller frame. The roller frame fell on top of the employee causing a fracture to left rib.

Employee was assisting to flip over a piece of belt to roll up when the employee's channel locks slipped off belt, employee felt a strain to the right shoulder.

Employee let drill pot down and drill steel fractured finger.

The employee stepped near scoop to unload material, the employee did not realize that the scoop operator had not lowered the scoop bucket completely. The scoop bucket when lowered landed on employee's foot causing injury.

Roof fall occurred 4th east rooms in entry 3 crosscut 2 no equipment or people involved no damage to return stopping there were no timbers or cribs in this area there was extra support straps and bolts. The fall was 45X35 and 15 feet high. This Area was originally bolted with 8' TR bolts and 12' cable bolts area was dangered off and timbers set.

2nd S.Panel#2 Entry x-c9, roof fall was found intersection. Around 45ft x 25ft x 8-10ft hi in return airway where no one works or travels. This is same area last fall at x-c8 happened. The area was considered bad 9/1/2015 and lifeline rerouted extra support installed. Area originally bolted 8ft full grout rebar bolts & cable bolts in intersection. The ventilation not affected.

While employee was roof bolting, his glove caught the moving drill steel, causing wrist to twist.

Employee was cleaning out flap on hurricane of Fletcher Roof Bolter. Employee did not notify Roof Bolter partner prior to cleaning. Roof Bolter Partner started bolter, causing the flap to close on injured employees hand.

troubleshooting shuttle car and outlet arced. Burn to left hand.

Roof fall 2nd South Panel #5 Entry x-cut 8 out-by section in intake. Area working & dripping. Boss cleared area & dangered & let it fall. Set 2 rows of timbers around fall. No injuries, airways not affected. Depth of cover was around 685ft, area bolted with 8ft resin bolts and 5 12ft cable bolts & screened. The fall was around 10-15ft high x 25ft long x 30ft wide

Employee was walking and stumbled. Employee tried to catch himself when falling using his hand and landed on a rock, causing his right ring finger to dislocate.

EE was installing lagging at roof fall area. While building arch sets, he was holding a piece of lagging to bolt with another employee. The other emoloyee tightened the lagging before EE could clear his hand from between the lagging and structure. Resulting in a laceration to the left middle finger which required stitches, and an x-ray results was a cracked bone.

employee had his foot hanging out of mantrip and when traveling outside at end of shift, his foot hit a pallet of material and twisted his foot, causing a fracture.

employee was struck by shuttle car cable in the back of his legs and the force flipped him upside down and he landed on his head and neck.

Employee was moving a 480v pump and un-tied the trailing cable from the roof and a rock slid out and struck him in the head.

Shuttle car was stuck in entry #7, EE went to check on the situation. While walking around the corner to entry #7 and was struck by the trailing cable.

Employee was exiting the end loader and slipped off of the platform, falling to the ground and injuring his back.

Employee was roof bolting on offside at #2 face, he put up outside bolt was swinging over to put up 2nd bolt when rock fell from top and landed on his foot. Employee was wearing rubber steel toed boot. Jagged point of rock punctured through the upper portion of boot and punctured his foot.

Rock fell on to hand while putting wire mesh on to t-bar on roof bolter

installing a roof bolt into the wrench and inadvertently hit his rotation, causing the roof plate to spin and cutting his arm. he was taken to the hospital and received stitches. employee returned to work the next day with light duty restriction.

Worker walked around the back of the roof bolter to help his partner, and walked up to him while the partner was swinging a hammer and was struck in the nose. The injury required him to be taken to the hospital for stitches.

11/7 employee reported a shoulder strain while roof bolting. When he arrived at ER, his eye was hurting, but nothing was seen in his eye. 11/8 Employee returned to ER with his eye irritated. A fleck of metal was removed from his eye.

Roof bolting in 1st South #1 Entry, loose piece of rock fell between the ATRS and canopy striking the employee on the right arm.

Employee was drilling hole for rib bolt, when some rock fell out and struck him.

normal mining conditions, employee was operating a shuttle car, miner conveyor threw a rock and hit shuttle car operators right arm, shoulder and face.

9:58 am found a roof fall in an abandoned area, 3 panel, #5 entry, xcut 17, fell @ anchor zone @ 6' at the intersection, no one around, non working area, timbering & dangering off

While walking in a muddy area, employee slipped and fell to knees.

While roof bolting, loose piece of top/rib fell striking employee on left shoulder and arm.

While putting up 8 foot bolts and straps, employee was moving strap over to line up next bolt when a 2'4" L X 1'4" W X 1-1/4" H shale rock slid down strap and hit his right wrist

Employee trying to pull cable to the miner and felt right shoulder pop and starting hurting.

Normal mining conditions. Employee caught middle left finger between a roof bolt plate and the mine roof, crushing the tip of his finger.

While watching shuttle car cable slack get reeled up, ready to signal when to stop, the cable flipped over got behind employee grabbed his feet and back and slammed him face first into the rib.

Piece of sandstone fell out of the top, employee moved slightly and the sandstone grazed his neck, shoulder and back.

The area was wet and slick, employee fell and landed on his back.

Area of fall was in old works used for a sump area.The area was just outby another fall we had 12/29/2010. Fall was approx. 5' high 16'-18' wide and 20'-25'long. It was approx. 10'-15' inby the #7 Entry, East Panel of the 1st So. Panel. Seasonal change/humidity probably a factor. No damage or injury.

Wet/slick conditions. Employee was walking up #3 belt line and slipped and fell and tried to break his fall with his left arm injuring his left shoulder and forearm.

Employee was roof bolting in #3 face and a small sharp piece of rock rolled off of the rib and struck him on top of his left foot.

Employee is working as a bolter, he finished his shift on 3/20/2012 then went to the ER on 3/21/2012 complaining of shoulder pain. He was diagnosed with tendonitis of the right shoulder. Employee has stated that his right shoulder has been hurting for months.

IE and another EE were cutting roof bolts that were hanging down over #3 belt. IE handed other EE a lit torch as he was getting in position to cut the next bolt. The torch hose caught on the belt structure and turned the flame toward IE, he put his right arm up to block the flame from his face an received a burn to his right forearm.
