While installing a roof bolt, the bolt wrench broke and struck the employee in the face.

Cutting a tree limb to clear pad site when chainsaw kicked back and cut knee.

The employee raised the roof bolter drill box and bent the drill steel which pinched the right middle finger.

The employee attempted to travel through a mandoor at Crosscut 14 West Mains 4-5 entry B-Seam when he stepped through the door he fractured his right ankle. The mandoor closed on his left left leg. He was found at 6:00 a.m. on 8/17/06.

VICTIM WAS ASSISTING WITH INSTALLATION OF NEW BEARINGS ON CRUSHER ROLLS. ONE BEARING HAD BEEN INSTALLED, MAKING PREPARATIONS FOR SECOND. REACHED UNDER THE SHAFT TO REMOVE SOME DEBRIS, WELD ON THE EYE HOLDING THE ROLL BROKE, FELL ON HIS RIGHT HAND. SURGERY PERFORMED ON HIS RIGHT HAND. THE 1ST AND 2ND FINGERS AMPUTATED TO THE KNUCKLE, 3RD AMPUTATED TO THE SECOND JOINT.

EE WAS TIGHTENING APPROX. 200 LUG BOLTS ON THE WHEELS OF CAT 793 TRUCK USING AN AIR GUN FOR APPROX. 45-60 MINUTES.

When removing wheel from water truck the cleat from the wheel ejected striking ee in the left hand and wrist area.

REMOVING PIVOT SHAFT RETAINER PLATE FROM DOZER BY APPLYING AIR PRESSURE. WHEN RETAINER PLATE EJECTED FROM ROLLER FRAME HOUSING, IT STRUCK EE IN THE STOMACH AND RT. SIDE.

Victim was tightening bottom roller caps when the impact wrench kicked back off the bolt. Pulled muscle or strain to shoulder. Missed one days work.

INSTALLING BOLTS IN SIDE PLATE ON DESLIME SCREEN, STANDING ON FRAME W/LEFT FOOT ON GEAR BOX. GEAR BOX ROTATED STRIKING EE ON HIS LEFT FOOT & ROLLING ONTO THE FLOOR.

EE was utilizing a Caterpillar 325-B excavator to clean loose material from the highwall along the road. EE states tried to pull a rock with the bucket and it slipped. This caused EE to be jarred around in the seat of the excavator. Complained of pain in lower back. Still being treated for pain in lower back radiating down leg.

A rock knocker was bent on the right rear tires. EE was attempting to cut it loose. When the final cut was made the rock knocker sprang out and struck the torch EE was using. The torch struck EE on the left cheek and forehead. Laceration to the forehead that required four stitches to close. EE is still off work due to a slight headache, awaiting doctors release to return.

This operator had been off work for several weeks with a back injury that happened at home during an ice storm. He returned to work on 2/2/05 and worked one full shift. The next day he notified his supervisor that he had hurt his back the previous night when he backed over a rock with the D-11 dozer he was operating. He is still off from work.

EE called on 1/19/05 & said he had hurt his back on 01/17/05 while operating a dozer.He states that when he backed over a rock, it caused him to be jerked around in the seat and strained his back.We have not been able to contact him since to get further information on his condition.Noone is sure of his length of experience in mining, so I have entered 0's for now.

The ee was roof bolting on the longwall recovery face when a piece of face coal rolled out and struck him on the knee.

Employee cut a tree when a grapevine dislodged a rock approx 75 ft uphill from the tree. The rock rolled down the hill striking employee on the knee.

Operator backed dozer over a rock while pushing in shot material. caused him to be jerked quickly. Complains of pain in lower back.

OPERATOR WAS USING THE EXCAVATOR TO PULL A ROCK BACK ONTO THE BENCH FROM THE OUTSLOPE. A SECTION OF THE ROCK BROKE OFF CAUSING THE EXCAVATOR TO JERK BACKWARDS. THIS CAUSED THE OPERATOR'S NECK TO HAVE PAIN DUE TO THE WHIPLASH EFFECT.

OPERATOR STATES THAT HE BACKED THE DOZER OVER A SMALL ROCK AND IT JERKED HIM CAUSING PAIN IN LOWER BACK AND LEGS. STRAINED MUSCLE.

OPERATOR RAN OVER A ROCK WITH DOZER AND JERKED HIS BACK STRAINED MUSCLE.

EMPLOYEE WAS USING A DISC GRINDER TO REMOVE WELDING SLAG. THE GRINDING DISC EXPLODED AND STRUCK BOTH LEGS BELOW THE KNEES. LACERATIONS TO LOWER LEGS REQUIRING STITCHES.

WHILE PUSHING A LARGE ROCK, THE BLADE SLIPPED AND THREW HIS RIGHT SHOULDER BACK AGAINST THE SEAT. POSSIBLE PINCHED NERVE OR JAMMED SHOULDER.

EE WAS GRINDING ON STEEL WHEN A PIECE OF STEEL FLEW IN HIS RT. EYE.

ONE EMPLOYEE WAS WORKING ON COAL DRYER WHEN FAN GUARD BROKE. HE TAPED IT OFF AND LEFT THE AREA. FOREMAN THOUGHT MOTOR WAS OUT & PLACED HIS HAND OUT TO CHECK TEMPERATURE. FAN BLADES HIT INDEX & MIDDLE FINGERS, LACERATING THEM. 13 STITCHES IN INDEX & 7 STITCHES IN MIDDLE FINGER.

EE WAS USING A ONE INCH IMPACT WRENCH TO REMOVE RETAINING NUT ON SCREEN BOLT. THE NUT WAS FROZE AND IT JERKED HIM CAUSING A PULLED MUSCLE IN LOWER BACK.
