Index finger bent backwards while pulling cable off when cable suddenly started going the opposite direction due to cable on tugger was wedged in a spot and started backspooling. Employee went to Doctor on 7/25 and was put on restrictions starting 7/26.

Elbow pain after jackhammering and sledge hammering on plugged line.

While attempting to loosen the nut the pneumatic wrench caused the straight wrench to turn pinching the employees hand between the straight wrench and the adjacent bypass valve. The injured cleaner received a contusion to the top of the hand.

EE was grinding tacs off of the north elevator door, the grinder jumped and caught the glove and pulled thumb between guard and grinding disc causing laceration to left thumbnail.

Employee was utilizing the flex lance and he thought that he felt the flex lance build up of pressure in the tube and it pushed the flex lance upward. Water sprayed on his face shield and his face shield fogged, which caused him to temporarily be unable to see. He then felt water pressure hit his right foot.

ee reported pain in left hand after using impact wrench

employee was running an impact tool, the impact slipped off the nut, the socket hit the grating and bounced off grating and struck employee in mouth breaking 2 teeth and lacerating lip requiring sutures

Employee was jack hammering and had weakness in wrist.

finding from enforcement was jackhammering and the hammer jerked causing pain to shoulders and neck

grinding on suction valve for about 2 hours alternating with co worker and supervisor, minimal soreness at end of day, woke with pain and weakness to lateral right shoulder with raising arm and overhead motion, right side of back sore

While hydroblasting the airlift on R-45 #80 precipitator tank, the employee went to top floor of the precipitator tanks to monitor progress of the #80 tank. The tank "burped" splashing the employee with effluent.

Employee was washing a press and the pressure of the water hose knocked him off balance and he struck side of the his back against the press causing a contusion.

Employee was grinding a valve and the valve handle broke. The broken handle struck him in the lip and lacerated his upper lip and loosened two teeth.

Employee was trying to free the bit of a jackhammer and when it came loose, the employee caught his thumb between the jackhammer and the wall. It resulted in a fracture to the thumb tip.

Employee was welding and was splashed with hot water on his right calf. He received a 3in x 5in 2nd degree burn.

Employee was scaling a tank with a pneumatic tool and pinched right thumb between tool and doorway of tank causing a contusion to thumb.

Employee says he was taking the bolts off of the number 7 heater when the wrench came loose from a bolt, employee put the wrench back on the bolt when another employee engaged the empac, causing employee to smash his middle finger between the wrench and the heater.

EE was lifting face shield when dust blew into his left eye.

Employee was assisting another employee to move a heater drill and the drill moved unexpectedly and struck the employee in the right arm. Employee was being evaluated in Medical Dept on Tuesday, 11/2/2010 and nurse noticed that abrasion was becoming infected. Rx meds prescribed on 11/2

Employee was on top of presss #14 standing on work platform washing inside of press. Press malfunctioned and the door started to drift. Employee braced himself as the door impacted the door frame. Employee was not struck, but the impact jolted his shoulder. Patient has been prescribed Rx meds, restrictions, physical therapy on 7/21.

Employee was using a jackhammer to remove scale for the interior surface of a filter press in Clarification. He got his left hand caught between the jackhammer and the surface of the shell causing a severe contusion to the hand.

Working with a co-worker using a jackhammer to loosen scale on gate valve. Jackhammering on top plate of valve. Sliver a metal fractured and became projectile embedding in forearm. Sent to external physician on 6/8 for evaluation and removal of sliver. Suture required to close wound.

Employee was operating valves in plate exchanger throughout the day. He awoke the next day and felt soreness and tightness in his lower back and reported to medical on 5/11/2010. He was referred to external doctor and work restrictions were recommended.

EE was impacting bolts next to another EE who was grinding. His left eye became irritated when he removed his goggles.

Employee was preparing to swap pumps and was exposed to hydrocholric acid mist from a leaking flange that was approximately 15 feet away on adjacent equipment to what he was working. He had difficulty breathing

Employee was using a pneumatic air wrench and was pulling on the hose to move the hose. The hose unexpectedly loosened and the employee was pulling toward her face and the air wrench hit her in the mouth breaking a top tooth.

EE's (L) middle finger was amputated between the nail bed and 1st joint due to the impact from a "T" handle while a valve was being worked on. The last time he engaged the air gun, the pressure from liquid entering the line caused the "T" handle to swing down contacting his finger.

Employee was cutting a piece of angle iron. He cut a 5' piece of angle when the piece fell, hit the ground, bounced up and hit his (L) foot. Employee continued to work, but was taken to the doctor on 10/9/07 where a fracture was discovered. He was released to return to work on modified duty.

EE received injuries while attempting to restart No. 2 Aluminum Floride Unit in the R-84 Chemicals facility. During the restart, vacuum was lost and the HF was not being pulled through the converter. As this happened, HF gass was released from around the 8 & 9 feed screws and around the seal of the kiln. EE was transported to a hospital for treatment and was released the next day.

As this happened, HF gass was released from around the 8 & 9 feed screws and around the seal of the kiln. EE was transported to a hospital for treatment and was released the next day.

EE sustained a laceration to the right side of his forehead while operating a D10 dozer. A pathway had been cut during the previous shift by the D10 dozer. After the run was completed, the dozer operator decided to help push bauxite to the other hopper. When he crossed the mound with the dozer, it came crushing down to the ground. He was unaware that the ramp had been dug out.

EE & co-worker were using pneumatic jacks to position motor for #4 rod mill in R-25. Jack was placed between fabricated backstop & base of motor. EE had positioned his jack in preparation to move motor & had enough pressure on jack to keep it in place against backstop. While waiting for co-worker to position other jack, backstop failed. Base of jack slipped off & hit EE in head.

Employee was removing scale from inside the digester when a large piece of scale fell from above him and struck his body in several different areas.

Employees deviated from normal tube demoliton procedure, they used a tool that was not designated to be used in tight quarters. Resulted in injury to employee's right forearm. The injury resulted in the foreman having to be sutured by an MD. The injury did not result in any loss of mobility or impairment to the arm.

Employee was pulling blinds which required use of an impact wrench, resulting in a strain to his neck. He was treated with over the counter medication. As a result of this injury, he was given an injection for pain on 3/6/06 and resulted in restriction being issued.

Employee was working in R-25 rod mill bldg. & sustained an eye injury that required the use of prescription medication. He received the prescription after he was sent to the hospital for further evaluation. He was lowering his welding hood when a fan blew something into his left eye. There was dust behind him & in front of the fan.

Employee was guiding a press filter screen into the storage rack. As it was being lowered by the crane, it hung up on the opposite end from the employee and when released, struck his (R) thumb fracturing it. Employee was released by the doctor to return to work with no restrictions.l

Employee was working on #2 meter when restriction caused water and grit to break loose resulting in a chemical burn to both eyes. She was taken to Memorial Medical Center where medical treatment was given.

WHILE TIGHTENING THE BOLTS ON THE BOTTOM COVER OF HEATER #4 IN R42 WITH AN IMPACT WRENCH, THE EE RECEIVED A FOREIGN BODY IN HIS RT.EYE WHEN THE IMPACT BLEW AIR UNDER HIS FACE SHEILD AND SAFETY GLASSES.

EE SUFFERED A DEEP LACERATION TO HIS THUMB AND INDEX FINGER OF HIS RIGHT HAND RESULTING IN OUTPATIENT SURGERY. BECAUSE OF THE SEVERITY OF THE DAMAGE DONE TO THE NAIL AREA OF HIS INDEX FINGER, THE PORTION OF HIS FINGER FROM THE NAILBED TO THE JOINT WAS AMPUTATED DURING SURGERY. HE WORK ON LOADING DOCK AND HAD GONE OUT ONTO THE BOOM. VIA THE WORK PLATFORM, TO VERFITY THAT BLOWER FAN

EE WAS DISCONNECTING AN AIR HOSE FROM A PORTABLE AIR COMPRESSOR. HE THOUGHT HE HAD BLED THE PRESSURE OFF THE HOSE, BUT WHEN HE BROKE THE CONNECTION AT THE COMPRESSOR, HYDRATE AND LIQUOR BLEW O UT ONTO HIS FACE AND UPPER BODY. THE BURN TO BOTH EYES RESULTED IN USE OF RX MEDICATION AND A PATCH TO HIS RIGHT EYE, WHICH RESULTED IN TEMPORARY RESTRICTIONS BEING ISSUED.

A RECLAIM OPERATOR FROM RAW MATERIALS SUSTAINED A LACERATION INJURY TO HIS LEFT HAND WHILE WORKING AT THE TOP OF THE R25 ROD MILL BUILDING. HE WAS JACKHAMMERING AT #15 TRIPPER TO CLEAN DIRT OF F THE SIDE OF THE TRIPPER. SOME MUD ON THE SIDEWALL FELL OFF AT ONCE TAKING THE JACKHAMMER AND HIM ALONG WITH IT. HIS HAND STRUCK AGAINST AN UNKNOWN OBJECT TO CAUSE THE LACERATION.

A LOOSE PIECE OF SCALE ESTIMATED TO WEIGH 200 TO 250 LBS FELL 2 FEET FROM THE SPIRAL WALL OF THE AREA 31 F3 FLASH TANK. THE SCALE FELL DUE TO VIBRATION CREATED WHILE REMOVING SCALE WITH A 15 LB CHICAGO PNEUMATIC HAMMER GUN. SCALE STRUCK EE ON LEFT SHOULDER AND BACK. RT ELBOW STRUCK TANK WALL ON IMPACT.

EE WAS CONTACTED BY LIQUOR WHILE CHANGING THE NOZZLES ON THE BACKWASH SPRAY BAR AT THE 56-2 HORIZONTAL FILTER. HE SUFFERED A SECOND DEGREE BURN ON HIS LOWER LEFT LEG AND WAS TREATED WITH RX ME DICATION.

EMPLOYEE RECEIVED A LACERATION TO THE RIGHT SIDE OF HIS FACE WHICH REQUIRED SUTURES. HE WAS WORKING BETWEEN #7 AND #8 PAN FEEDERS IN R-25 USING A JACKHAMMER. THE JACKHAMMER GOT STUCK AND WHILE ATTEMPTING TO FREE IT, IT SLIPPED OFF THE BIT & STRUCK HIM IN THE FACE.

WHILE CHIPPING SCALE INSIDE THE VESSEL WITH A JACKHAMMER, A PIECE OF SCALE WEIGHING APPROX 25 # BROKE LOOSE AND FELL ON EMPLOYEE'S FOOT.

EMPLOYEE SUSTAINED A TUFT FRACTURE AND ALSO REQUIRED SUTURES TO HIS LEFT INDEX FINGER. HE WAS WORKING AT 18 JUNCTION USING A JACKHAMMER TO TRY TO CLEAR A CHUTE PLUGGED WITH BAUXITE. AS HE WAS WORKING THE JACKHAMMER BACK AND FORTH, HIS FINGER WAS CAUGHT BETWEEN THE CHUTE AND THE JACKHAMMER.

TWO EMPLOYEES WERE REMOVING THE HEAD FROM A HEATER AFTER IT HAD BEEN TAKEN OUT OF SERVICE. THEY HAD CLEANED THE TOP BY BLOWING IT OFF WITH AIR & WERE READY TO REMOVE BOLTS. THEY LEFT FOR A SHO RT TIME & UPON RETURNING BEGAN REMOVING BOLTS. HOT LIQUID BEGAN SPRAYING OUT CONTACTING BOTH. INVESTIGATION REVEALED THEY HAD BEGUN WORK ON AN ADJACENT HEATER THAT WAS STILL IN SERVICE.

THE EE WAS WATERBLATING A PUMP PART AT THE GATE 9 WASH PAD. THE EE RELEASED THE SURFACE CLEANING TOOL TRIGGERS TO RELIEVE AIR AND WATER PRESSURE TO FREE THE ROTATION OF THE LANCE. WHILE MANUAL LY ROTATING THE LANCE, THE EE S HAD MADE CONTACTWITH THE RESIDUAL WATER STREAM AT THE LANCE TIP CAUSING A MINOR LACERATION TO HIS LEFT INDEX FINGER.

EE FROM SERVICES MAINTENANCE SUSTAINED A THERMAL BURN TO HIS EARDRUM. THIS CASE IS BEING CLASSIFIED AS A RECORDABLE DUE TO THE LEVEL OF THE MEDICAL TREATMENT REQUIRED FROM A PHYSICIAN. THE EE WAS USING A CUTTING TORCH OVERHEAD AND SOME SLAG DROPPED INTO HIS RIGHT EARDRUM. NOTE: INJURY OCCURRED ON 3-22-02, BUT WASN'T REPORTED UNTIL 3-25-02.
