Two employees were performing patch work on a chute while cutting a existing piece of metal approx 1'x1' approx 3/8" thick off the piece of metal hinged and swung roughly 4' and fell 4' onto the EEs foot. The sharpe corner of the metal penetrated their boot and cut the top of their foot.

Employee pinched finger of left hand between frame and wrench of lathe.

Employee got foreign material in right eye.

During welding operations on the Primary Feeder, a mechanic sustained welder's flash.

EE removed a cover on a slide reached inside and had a finger struck by a rotary feeder.

Employee experienced eye irritation after cutting a chute in the Mill. EE was later diagnosed with a minor abrasion to the eye.

EE inserted an air wand into an elevator making contact with the buckets pinching a finger on the lip of the chute.

Working on haul truck engine. Removing number 1 cylinder head bolts. Hand was caught between bracket and air gun while removing head bolts.

EE was changing out a steel plate in jaw crusher. Weld came loose during the work and plate landed on EE's foot resulting in a fractured metatarsal.

EE was sitting on a platform beside the aerator using a grinder to clean up the shaft when EE lost control of the 4 1/2" angle grinder causing the spinning disc to come in contact with the inner area of EE's left thigh. Injury resulted in a laceration requiring stitches.

Employee was welding and a foreign object got into EE's right eye. EE didn't notice it until later that day.

Lowering DTH Hammer in to service truck using a winch line. While repositioning from vertical to horizontal for transport, the hammer shifted against truck bed. This action pinched the gloved hand of the victim as ee was attempting to guide the hammer in to position. the small finger of the left hand was caught between the hammer and truck bed surfaces

Employee was removing the cutting edge of a 990 loader bucket with an impact wrench. The employee removed glove to feel if the bolt was placed in the seated position. EE pulled the trigger on the impact wrench causing the bolt to spin in EE's hand cutting middle finger on the edge of the bolt.

Individual went to start air compressor, it failed to start. Individual went and opened engine cover/guard to check for loose wires on alternator. As he was checking for loose wires, another individual came over and tried to start air compressor. This time, air compressor engine turned over causing individual's glove to get caught in fan/alternator belt.

Employee was using a pressure washer to clean the outside of a scrubber prior to welding repairs. His face shield became fogged, and he accidentally struck a scrubber support with the nozzle, causing it to change direction and strike his left leg with hot water. He received a second degree burn to his left shin, just slightly above his boot.

Employee is a mobile mechanic and was grinding and welding all day, wearing goggles under his face shield. He had no incident at work, but on arriving home and taking off his cap, a small fragment of metal fell into his left eye. He reported something in his eye the following morning and was taken to a doctor, who removed the fragment and some rust that had formed.

Worker was welding a small connecting beam to a bridge beam, when hot slag fell between the leg strap of his harness and his leg. The hot slag was able to burn a hole in his pants before he could get it off. The worker received a minor burn to his leg where the slag had fallen.

Checking threads on drill rods while head was in motion. Employee put his hands in the tower of drill accidently and was winched by a pulley, causing partial amputation of middle finger and breaking adjoining fingers.

Employee was using an overhead crane to raise a piece of the jaw crusher. The wire rope chokers being used broke, and the recoil sent the sling hook upward toward the employee striking him in the side of the head. The employee remained conscious and was transported to emergency room. Medical treatment revealed a skull fracture.

Coworker was walking holding a circular saw. Extension cord got hung up on something, so he gave it a tug. In doing so he bumped into EE & accidentally squeezed the trigger on the saw. EE was startled & jumped. EE's hand contacted the saw blade and his right middle & ring fingers were lacerated, requiring medical attention (stitches).

Employee was using a Plastic Fusion (Welding) Machine to join two pieces of Plastic Slurry Piping. As he was trying to position the machine and the pipe into the proper position, one of the handles on the machine moved and struck the employee's right ear which resulted in a laceration that required sutures.

Employee was pulling broken screener guard back to keep it from banging around. The screener was still running and the guard caught on the moving drive and pushed foreward hitting his left knee.

Employee was priming a wet scrubber pump #2 by turning on fresh water intto the pump casing. When the pump primed and built pressure the threaded PVC adaptor pulled out of the coupler just above the pump outlet and sprayed hot water on the employee

EE WAS CLIMBING OUT OF MS-35 PIT AND STEPPED ON THE GUARD TO EXIT. THE GUARD FLIPPED UP AND STRUCK HIM ON THE LEG. THIS RESULTED IN A LACERATION WHICH REQUIRED STITCHES.

EMPLOYEE CUTTING OUT DISCHARGE CHUTE - CONVEYOR #104- UPON COMPLETING CUT, CHUTE FELL AGAINST HIS CUT, CHUTE FELL AGAINST HIS RIGHT HAND.

EE CAME INTO CONTACT WITH ROTARY DRYER. HE RECEIVED MINOR ABRASIONS, CUTS & CONTUSIONS.

EE WAS CUTTING A PIECE OF STEEL WITH A TORCH WHEN HIS CLOTHING IGNITED, CAUSING BURNS TO HIS STOMACH.

SUPERVISOR WAS CHECKING THE NORTH COOLER FOR A LACK OF COOLING WATER. FROM THE PLATFORM'S LADDER, HE SAW WATER COMING OFF PIPE & HAD OPERATOR START BACK UP. A POOL OF WATER FORMED IN THE COOLE R WHILE IT WAS DOWN & WHEN THEY STARTED UP, THE HOT GRANULES MIXED W/ THE WATER CREATING A BURST OF STEAM THAT BURNED THE SUPERVISOR'S FACE & HANDS.

EE WAS INSTALLING BUCKETS ON # 4B ELEVATOR WITH AN AIR IMPACT WRENCH AND FELT A SLIGHT IRRITATION IN HIS RIGHT EYE. EE CALLED SAFETY COORDINATORAT HOME LATER THAT EVENING AND SAID HIS SEEMED T O BE GETTING WORSE. THE SAFETY COORDINATOR THEN SENT THE EE TO THE HOSPITAL WHERE THE OBJECT WAS REMOVED.

EE WAS TURNING SWITCH FROM ONE TRACK TO THE OTHER.

EMPLOYEE STATED HE AND HIS PARTNER WERE SETTING UP FOR BAGGING 100# BAGS. THE BAGGING SYSTEM HAS A SCALE, AN AIR TRIP MECHANISM, AND CAN BE SET FOR DIFFERENT HEIGHT BAGS. PARTNER ACCIDENTLY DROPPED THE HEIGHT OF THE SCALE AND EMPLOYEE'S HAND BECAME PINCHED BETWEEN THE BAG TRIPPER AND THE FRAME RESULTING IN A LACERATION REQUIRING STITCHES.

EE STATED HE WAS HELPING WITH CLEANING A TANK CAR WHEN SOME SILICATE SPLASHED FROM THE AIR HAMMER SIDEWAYS INTO HIS EYE. EE WAS WEARING SAFETY GLASSES AT THE TIME.

EE STATED THAT HE WAS BURNED BY STEAM FROM HOT GRANULES AFTER CLEANING A PLUGGED CHUTE AND RESTRARTING EQUIPMENT.

GROUND CONDITIONS CAUSED DRILL BAR TO TIGHTEN. AS A RESULT, THE WRENCH BROKE, FLYING OUT AND STRIKING DRILLER.

THE EE WAS GRINDING ON A TURNBUCKLE. THE TURNBUCKLE CAUGHT ON THE GRINDING WHEEL AND THE OPPOSITE END CAM EUP AND PUNCTURED THE EE S RIGHT HAND, THROUGH HIS GLOVE.

EE WAS OPERATING STANDING MOTORIZED LIFT TO MOVE MATERIALS INTO PLACE. PINNED RIGHT FOOT BE6TWEEN METAL COLUMN AND LIFT. FRACTURED RIGHT FOOT.

EMPLOYEE WA SWELDIGN WHEN A ROD STUCK TO WELD. TO BRAKE ROD LOOSE, EMPLOYEE PULLED ON ROD WITH LEFT HAND. WHEN THE ROD CAME FREE IT INSERTED ITNO RIGHT ARM. EMPLOYEE REQUIRED 14 STITCHES.

EMPLOYEE STATED HE WAS WELDING A CHUTE IN TIGHT QUARTERS THAT INVOLVED TWISTING AND BENDING.AFTERWARDS,HE FELT A STRAIN IN HIS LOWER BACK.

EMPLOYEE STATED HE WAS REMOVING CMENT WITH AN AIR HAMMER AND STRAINED HIS LOWER BACK.

EMPLOYEE WAS USING POWER NAILER WHEN IT DISCHARGED & STRUCK EMPLOYEE IN LEFT HAND

THE EMPLOYEE HAD JSUT FINISHED CHANGING OUT 2-A ROLL CRUSHER. WHEN THE ROLL CRUSHER WAS STARTED THE WEST SHEAVE WAS RUBBING THE GUARD. THE EMPLOYEE PULLED THE GUARD AWAY FROM THE SHEAVE. THE G UARD FELL INTO ALIGNMENT WITH SUPPORT PINS AND MASHED EMPLOYEE'S FINGERS.

THE EMPLOYEE WAS UNHOOKING A 1" THICK METAL SHIM PLATE FROM AN OVERHEAD CRANE TO PLACE IT IN STORAGE. THE HOOK ON THE CRANE WAS HUNG IN THE LIFTING EYE ON THE SHIM PLATE. WHEN THE CRANE BECAME UNHOOKED THE EMPLOYEE MASHED HIS FINGER BETWEEN THE SHIM PLATES.

EMPLOYEE WAS WELDING IN AN AWKWARD POSITION AND THE NEXT DAY HIS NECK WAS VERY SORE.

EMPLOYEE STATED HE WAS CUTTING A DECK WHEN MOLTEN METAL LANDED ON HIS MIDDLE RIGHT FINGER CAUSING A BURN

EMPLOYEE STATED HE WAS GRINDING IN THE MAINTENANCE SHOP WHEN SOME METAL FRAGMENTS ENTERED HIS RIGHT EYE.

EMPLOEYE STATED THAT HE WAS WELDING WHEN A PARTICLE FLEW INTO HIS EYE.EMPLOYEE WAS WEARING SAFETY GLASSES AND WELDING HELMET AT THE TIME.

EMPLOYEE STATED THAT SHE WAS RAPPING ON #4 AERAT OR & FELT A PAIN IN HER BACK & LEFT SIDE

EMPL HOLDING CABLE SPREADER HOOK IN LIFTING EYES LOCATED ON EACH SIDE OF CRUSHER FEED CHUTE.AS 2ND EMPL RAISED THE HOIST,CABLES PULLED TIGHT PINCHING LEFT THUMB BETWEEN HOOK AND CRUSHER FEED C HUTE.

EMPL TURNING #3 KILN WITH V-BELTS.RIDING RING HAD LOW SPOT ON IT WHEN LOW SPOT PASSED TRUNION THE KILN FREE-WHEELED AND PULLED EMPL FINGERS INTO REDUCER SHEAVE.

EMPL EMPLOYEE WAS CUTTING OFF A SECTION OF KEY STOCK FASTENERS WHICH ATTACHES THE RIDING RING TO NO5 KILN.DURING THE WORK PROCESS,A PICE OF SLAG AND OR SPARK GENERATED FROM THE AIR ARC CUTTING PROCESS LANDED ON THE BACK OF HIS SHIRT.THIS RESULTED IN BURNING A HOLE IN HIS WORK SHIRT WITH A SUBSEQUENT BURN APPROXIMATELY 1" IN DIAMETER TO THE
