Employee was cutting a piece of belting with an oscillating tool. After completing the task they sat it down on the ground. While walking around to roll up belting they accidentally stepped on the battery portion of the tool and it flipped up and cut the back part of their calf. Supervisor took them to the hospital and they received 7 stitches.

EE was unrolling a roll of conveyor belting to splice onto an existing belt. While doing so EE felt something pop in right thumb. MRI showed tear in ligament. EE will have this repaired on 08/16/2024.

Sprained right knee when exiting mini-track loader. While exiting employee stepped off of equipment with left foot and slipped on dust particles on the concrete and twisted right knee which remained on equipment.

Employee fell while getting off conveyor and bruised their knee.

Employee dropped pry bar and caught on rotary valve which pinned their finger between the pry bar and cyclone flange.

Employee experienced eye irritation after cutting a chute in the Mill. EE was later diagnosed with a minor abrasion to the eye.

While walking across the blast pattern, Employee stepped on some loose material and lost balance causing pain to their knee and leg. Employee was put in a temporary leg brace for the 3 days.

EE had been arc gouging during shift. When EE got home and laid down for the night EE noticed right eye was tearing and hurting. EE was unable to identify any specific event that led to the injury. EE went to an ophthalmologist where two foreign bodies were removed, EE had a corneal abrasion and was prescribed an antibiotic.

EE had been working on the dryer kiln and noticed a pain in EE's right wrist at the end of shift. EE was evaluated by a doctor and placed on a temporary 10 pound lifting restriction. EE was unsure of an exact activity that caused the incident.

EE was making repairs under a fuel truck when dirt fell into right eye creating a corneal abrasion requiring a prescription antibiotic after removal with a cotton tipped applicator.

EE was walking in storeroom and tripped on corner of pallet and fell. EE was released back to work with no restrictions that day. EE was still having pain and saw DR on 12/22 and was diagnosed with muscle tear. Surgery was conducted on 12/23 thus constituting MSHA reportability.

EE was entering CAT long reach when EE's right foot slipped. EE felt pain in right ankle when EE's foot hit the ground. EE was diagnosed with partial Achille's tendon tear and has missed work, thus constituting MSHA reportability.

We are reporting a change in hearing sensitivity of an average of more than 25 dB at the 2K, 3K, 4K, Hz in the right hear relative to the miner's baseline. The employee is currently enrolled in a hearing conservation program. The condition is currently being investigated for work relatedness.

The employee was going to check for and clean up any material spillage under the 104 belt. As EE was descending the stairs, EE missed a step then several more. The employee maintained 3-point contact, preventing EE from falling down the stairs. The employee struck a side rail or mid rail with left knee before coming to a stop causing a dislocation of the left knee patellar.

EE was cutting piece of skirting with safety knife. Skirting moved and EE cut left forearm with knife resulting in laceration. EE received 3 stitches resulting in MSHA reportablity.

EE was operating a walk-behind skid steer and was pulling backwards to be able to use the bucket to scoop up another load of material off the floor when EE's left shoulder made contact with a steel I-beam causing arm to jolt forward into the equipment controls injuring the wrist.

EE was closing valve under slurry tank using 2' pipe wrench for leverage and felt a pain in back. EE was seen by a doctor and returned to full duty with a follow-up a week later. At follow-up, EE was referred to a physical therapist for an evaluation. On April 30th the EE attended the first of 6 PT sessions, thus constituting MSHA reportability.

Employee was inspecting area and made contact with poison ivy while walking out door. Employee observed rash upon waking on 08/26/2017. Employee received steroid injection on 08/29/2017 to assist with healing.

Employee was removing the cutting edge of a 990 loader bucket with an impact wrench. The employee removed glove to feel if the bolt was placed in the seated position. EE pulled the trigger on the impact wrench causing the bolt to spin in EE's hand cutting middle finger on the edge of the bolt.

While getting ready to start loading the blast, employee went to pull the hose from the truck. EE felt a sharp pain in groin area on right side.

Employee was guiding a walkway into place. Employee pulled on the come-along holding the walkway to free it up when hand popped. The walkway had hung on a brace. Employee experienced no pain in hand but lost gripping ability.

Employee helping to replace split riding ring on Kiln, employee was rolling the kiln drum manually with the drive belts, once it passed the ridge where the new section was added it rolled without effort surprisingly catching the employees glove pulling his hand into and thru the sheave.

Employee arrived at job site to begin shift. Employee had fatal heart attack prior to beginning work.

The employee was opening the blend chute of E finished product bin and struck his right hand on the front of an adjacent chute. The cut to the right hand required four sutures.

The employee struck his back against a structure causing him to stumble and fall to his knees. He struck his left knee on a rock causing a laceration.

The employee was installing a metal plate on the jaw crusher. While attempting to install a bolt, the plate shifted pinching the employee's finger between the plate and bolt head. The resulting wound required sutures. There is no lost time associated with this injury.

The EE struck his hand against the side of a rail car. The original incident occurred 2/05/13 though there was no apparent injury. A ganglion cyst formed at a later date. The cyst was removed April 4, 2013 making this a recordable injury. There is no lost time related to this injury.

Employee was working in the maintenance shop rebuilding a crusher. While completing this task, the miner strained his lower back.

Employee was cutting open a box and cut left palm below the thumb which required 4 sutures.

Employee struck left shin with a 2 pound sledge hammer on 5/15/12. On 5/21/12 employee went to the emergency room and was given an antibiotic prescription to treat Cellulitis.

Employee was cutting a piece of hose and cut left thigh which required 8 sutures.

Employee was repairing skirting and cut thumb which required four sutures.

Employee Strained Shoulder while lowering an air impact wrench to a lower level. Incident became reportable after doctor prescribed Physical Therapy.

Employee and co-worker were moving a piece of sheet metal. The co-worker released his end prior to the employee releasing the end that he held. The employee's right middle finger was pinched between the ground and the piece of metal. X-ray shows fracture of the distal tip of finger.

Employee has been diagnosed with Bi-Lateral Carpal Tunnel Syndrome which will require surgery

Employee was descending a stairway and his Fall Protection Equipment (Lanyard) caught a Handrail Post. This caused the employee to twist his right knee and fall to the floor level. Employee was later sent to a doctor, and the diagnosis was cartlidge and ligament damage which will require future surgury.

MSHA has indicated the incident at issue constitutes an accident requiring submission of a 7000 1 Report. The Company doesn't agree with MSHA's position, but is submitting this Report to provide the information MSHA believes must be submitted. By submitting this Report the company is simply cooperating with MSHA and is not admitting the Report is in fact required.

The employee strained his back while working on a D-8 Dozer. Employee reported the injury and is continuing to work as a mechanic. This incident became reportable when the employee began physical therapy on September 30, 2008.

The ee twisted L Knee while walking into Shop. Employee reported injury and continued to work. This incident became reportable when the employee had Arthroscopic surgery on Dec. 17, 2008.

Employee was using a small pinch bar to position radiator cover on Front End Loader. Bar caused laceration on right hand, which required sutures. No Time Lost.

EE experienced difficulty breathing while walking from his vehicle to the office building. He was transported by ambulance from the mine property to Iron County Hospital where he later died from his illness. Medical reports list cardiac arrest as reason for death. ISP Minerals believes this death is non-occupational. This is supported by independent evaluation of the ER physician.

Employee reported slipping on stairway and catching himself while falling backwards. Dr. Visit, four days later, shows muscle strain.

Employee was operating a walk behind skid steer loader and backed machine into an I beam used as a support column. His hand was caught in a pinch point and cut as a result of being pinched between machine and column.

Employee sustained an Electrical Flash Burn while working in a 480VAC MCC. Flash Burn area was to the Head and Neck area.

Employee alleges injury to left knee. Specific incident not known.

Employee alleges moving a space heater and straining his lower back on 12/19/05. EE continued to work and recently sought medical attention.

Employee was using an overhead crane to raise a piece of the jaw crusher. The wire rope chokers being used broke, and the recoil sent the sling hook upward toward the employee striking him in the side of the head. The employee remained conscious and was transported to emergency room. Medical treatment revealed a skull fracture.

Employee alleges injuried back shoveling rock. He stated felt pain after 10-15 shovels of rock. The Ee at work 35 minutes & this was first labor onshift. The Ee stated, he has a bad back, he was treat & released at Hosp. ER & sched to see Orthopaedic Spec & MRI. Diagnosis states has 5 bulging disk & that Phy. Therapy and anti-inflam drugs are Rx, followed by doctor visit

Employee was using a Plastic Fusion (Welding) Machine to join two pieces of Plastic Slurry Piping. As he was trying to position the machine and the pipe into the proper position, one of the handles on the machine moved and struck the employee's right ear which resulted in a laceration that required sutures.

Laborer was using a shovel to clean up area in which crushed rock had spilled. He pulled a muscle in the mid thoracic area. Laborer is a Temporary Laborer and he was lifting while twisting his back, this contributed to the pulled muscle.

Individual had chest X-rays that were examined by two certified "B" readers, each of whom made a 1/0 reading. Subsequent evaluation by a Pulmonary Physician indicated findings consistent with silicosis.

EMPLOYEE WAS WALKING IN PRIMARY CRUSHING BUILDING AND TRIPPED AND FELL AND BROKE HIS RIGHT LEG.

EMPLOYEE WAS WALKING IN FRONT OF THE MAINTENANCE SHOP TOWARDS THE STOREROOM. HE TRIPPED OVER A BEARING INSERT THAT HE WAS WORKING ON AND FELL INTO A PALLET OF STEEL AND HIT HIS FACE ON THE STE EL CUTTING THE BRIDGE OF HIS NOSE, REQUIRING SEVEN STITCHES.

SUPERVISOR WAS CHECKING THE NORTH COOLER FOR A LACK OF COOLING WATER. FROM THE PLATFORM'S LADDER, HE SAW WATER COMING OFF PIPE & HAD OPERATOR START BACK UP. A POOL OF WATER FORMED IN THE COOLE R WHILE IT WAS DOWN & WHEN THEY STARTED UP, THE HOT GRANULES MIXED W/ THE WATER CREATING A BURST OF STEAM THAT BURNED THE SUPERVISOR'S FACE & HANDS.

EE WAS INSTALLING BUCKETS ON # 4B ELEVATOR WITH AN AIR IMPACT WRENCH AND FELT A SLIGHT IRRITATION IN HIS RIGHT EYE. EE CALLED SAFETY COORDINATORAT HOME LATER THAT EVENING AND SAID HIS SEEMED T O BE GETTING WORSE. THE SAFETY COORDINATOR THEN SENT THE EE TO THE HOSPITAL WHERE THE OBJECT WAS REMOVED.

THE EE WAS USING A 6" SOCKET TO TIGHTEN BOLTS ON THE JAW CRUSHER. AT COMPLETION OF THE JOB, THE EE MANUALLY LIFTED THE SOCKET FROM THE BOLT. THIS RESULTED IN A STRAIN OF THE LOWER BACK. LIFTIN G EYES ARE PROVIDED FOR USE WITH A HOIST, BUT WAS NOT USED BY THE EE. THIS INJURY BECAME A LOST TIME INCIDENT ON AUGUST 5, 2002.

TWO MECHANICS WERE MANEUVERING A SINGLE CYLINDER HYDRAULIC JACK (APPROX. 75 LB) INTO POSITION UNDER A FRONT END LOADER. THE EMPLOYEE FELT A STRAIN IN HIS LOWER RIGHT ABDOMENT/GROIN AREA, A REO CCURRENCE OF A HERNIA WHICH HAD PREVIOUSLY BEEN SURGICALLY REPAIRED. THIS INCIDENT BECAME A LOST TIME INJURY ON 5/1/02.

THE EE WAS CHANGING A SCREEN CLOTH. AFTER REMOV-ING THE CENTER STRIP, THE EE WAS SLIDING THE STRIP THROUGH HIS LT. HAND, ATTEMPTING TO MOVE IT OUT OF THE WORK AREA. THE EDGE OF THE STRIP CUT THROUGH THE GLOVE AND LACERATED THE EE'S HAND. HE RECIEVED FOUR STITCHES TO CLOSE THE WOUND.

THE EE WAS REPAIRING A TRUNION TIRE. THE TIRE LODGED AGAINST A FRAME AS IT WAS BEING RAISED. THE EE REACHED TO MOVE THE TIRE, THE TIRE BROKE FREE AND THE LOAD SHIFTED, SMASHING THE TIP OF THEW EE'S LEFT INDEX FINGER. THE WOUND REQUIRED SUTURES.
