Employee pinched finger of left hand between frame and wrench of lathe.

The affected employee had their annual audiogram completed and it was reported to the mine that the employee has hearing loss in their left ear.

The affected employee slipped and fell, injuring their right shoulder. The employee has been placed on restricted duty.

EE was walking and slipped/tripped/fell onto left knee, fracturing patella constituting MSHA reportability.

The crusher utility was in the work area picking up small trash on the ground. EE was just bending over when EE felt a slight shift and a shooting pain in the back. EE sought treatment on 11/4 and was placed on restrictions, thus constituting MSHA reportability.

PRESHIFT INSPECTION ON EXCAVATOR, THE STEP CAME OFF FRAME WHILE CLIMBING ONTO EXCAVATOR. EE CAUGHT SELF WITH THE HAND RAIL USING THEIR ARM.

EE was tramming boom lift to area and hit a dip in the road and it bounced the EE around the basket and the EE lost balance and injured shins which required stitches thus constituting MSHA reportability.

EE was conducting change over duties and had dust get in EE's eyes. EE was wearing safety glasses. EE had embedded material removed from EE's eye and was given prescription eye drops, thus constituting MSHA reportability.

A mechanic cut finger while changing belt splice. EE was holding the back of the clip with the other hand to apply pressure to the back of the clip, the bolt came free and contacted EE's right middle finger resulting in a laceration, requiring 4 stitches.

EE stepped off a platform and twisted right knee resulting in a knee strain. 1/15/19 EE was seen by doctor for observation 1/28/19 EE received physical therapy which resulted in the reportable injury.

EE was starting shift and felt dizzy when getting out of chair in breakroom. EE had worked in heat most of the previous day and worked overtime as well. EE received IV's for heat related illness, thus constituting MSHA reportability.

8/3/17 Electrician pulling new wires for a baghouse and had left shoulder muscle strain. 8/4/17 employee received post urinalysis testing. 8/7/17 employee was seen by the doctor and released. 8/9/17 Urinalysis results came back and employee return to work. 9/25/17 received physical therapy which resulted in the reportable injury.

After working EE's shift, the employee took a hygiene shower and slipped and fell on the floor. This resulted in a fractured right collar bone. Root Cause- Worn Anti-Slip floor paint causing a smooth surface along with the floor being wet from the shower.

Individual went to start air compressor, it failed to start. Individual went and opened engine cover/guard to check for loose wires on alternator. As he was checking for loose wires, another individual came over and tried to start air compressor. This time, air compressor engine turned over causing individual's glove to get caught in fan/alternator belt.

Liquid sodium silicate was splashed into left eye of employee. Medical providers concluded that no burning, scarring, or permanent damage resulted from this incident.

Small foreign particle stuck to cornea of right eye

Employee was installing a lifting lug and struck the rear portion of his left shoulder on a stationary object while trying to get up. The employee broke his left scapula as a result of this incident.

Employee slipped on ice and injured head/back regions.

A mill worker received dust/debris in both eyes when overloads were tripped on a circuit breaker inside the MS11 conveyor panel. The mine believes this incident did not and does not require reporting pursuant to 30 CFR 50.20.

Employee was sitting in a benefits meeting when he felt an irritation in his eye. Sent to doctor, foreign body removed, prescription antibiotic given

Employee had a chunk of ice hit the tip of his finger.

Employee got dust in their eye. The eye was flushed at the plant and the employee was brought to a medical professional, where he was prescribed an antibiotic eye drop as a precautionary measure. PPE was being utilized.

Employee has a hearing threshold shift in their left ear. The employee is currently enrolled in a hearing conservation program. The condition has not been diagnosed as a work related injury/illness.

We are reporting a hearing threshold shift of 30 dB in the employee's left ear at the 4k range. The employee is currently enrolled in a hearing conservation program. This condition has not been diagnosed as a work related injury/illness.

The Blast Crew was preparing for a development shot. Terrain was rough, with steep slopes and numerous loose rocks. Employee was standing on a slight incline when his feet slipped on loose material and he started to fall backwards. As he stuck his hands out to brace himself, his left wrist made contact with a sharp rock, causing a laceration that required 4 stitches to close.

Employee was walking down a flight of stairs on or about 7/2/2012 and felt a pain in her knee. Employee was seen by a doctor on 7/3/2012 and returned to full duty. After additional complaints the employee was returned to the doctor on 7/16/2012 for further evaluation and was removed from work.

An employee stepped over a chain, fell, and broke his leg.

An employee was dumping at a stockpile when he backed over a berm. The truck slid backwards and turned over, landing on the passenger side at the base of the stockpile. The employee was taken for medical examination, where he was released to work. On 9/6/11, he had a follow up appointment where he was excused from work.

While stripping old wires, knife slipped and hit finger

Employee lacerated hand requiring 4 stitches while cutting through a zip tie.

Twisted knee while walking to work. Note: The knee injury originally occurred on 6/23/10. It did not become reportable until he had surgery on 12/27/10.

While performing clean up, employee hit leg on framework, causing laceration and requiring stitches.

Employee slipped while carrying a ladder in snow. This incident became recordable on 1/7/09 when employee received a cortisone shot. Strain

Employee was going to step onto the ladder of his 990H CAT Loader from flat concrete ground, when he lifted his left leg up he felt a pop in his right knee.

Employee was adjusting load on forks of fork truck and metal shifted causing laceration on finger

Employee stepped onto then off of a pallet rolling her ankle.

Employee became engaged in a misconduct (fighting) with another hourly employee causing him to strike his arm on the wall, resulting in a laceration requiring 5 stitches.

Employee was vibrating concrete. When employee removed equipment, it was stuck and he pulled hard injuring his back.

The employee allegedly felt a strain in his arm while removing a rock from the haul road. This incident became a reportable injury when he when he was placed on restricted duty 10/24/07. DCN: 220080170034-E-doc-560436-missing time on 12/4/07

Employee was changing a 3.5'X5.5"X1/4" perforated screen. While making final adjustments, the employee allowed his finger to fall into a pinch point, and pinched the tip of his left pinky finger, causing a laceration that required stitches.

Employee was rapping out an aerator chute with a hammer, when she missed the chute and struck her right hand, resulting in a fracture and laceration to her right index finger.

Employee stated he strained his back while operating a Caterpillar loader. The bucket struck a rock pile which resulted in a jolt.

Employee and partner were replacing section of cable. Employee was holding cable while his partner was cutting with a hacksaw. His partner cut through the cable and the hacksaw struck employee in the upper lip resulting in a laceration which required stitches.

Employee tripped while walking on top of railcar and fell to the ground resulting in numerous abrasions and strain to hips, shoulders, elbows, ankles, shins, and lower back.

Employee was going up a flight of stairs to check a screen that is fed from primary crusher when a 27 lb. rock fell from above, striking him on the hard hat and then bouncing onto his left shoulder, resulting in lacerations and contusions and possible concussion.

Employee was using a J-wrench (40 - 50 lbs) to break the lock on the thread between the hammer and the back head on the drill. He was using the supports of the broom as a block to push the wrench against, and was next to the cab when the wrench broke loose under pressure and shot back at him, striking him in the nose. This resulted in a laceration and a fracture to the nose.

Ee was shoveling granules from hot elevator pit into elevator chute. a bracketed 90 degree piece of metal used to divert granules was not in it's proper place and ee bumped the diverter and it fell, striking him in the right foot, resulting in a contusion.

EE WAS USING WALK BEHIND BOBCAT LOADER AND SLIPPED ON THE ICE. THE BOBCAT RAN OVER HIS RIGHT FOOT, RESULTING IN A FRACTURE. THIS IS A LOST TIME INJURY.

Employee was putting the upper office side tailpulley guard back in place when she felt pain in her lower back. She began losing time on January 3, 2005.

EMPLOYEE WAS POSITIONING A PIPE STAND INTO PLACE WHEN IT MOVED AND CAUSED A LACERATION TO HIS RIGHT HAND.

EMPLOYEE AND PARTNER WERE CHANGING #6 ROLL BEARING. EMPLOYEE WAS USING A PRY BAR TO POSITION BEARING INTO PLACE WHEN HE FELT STRAIN IN LOWER BACK. THIS HAS RESULTED IN A LOST TIME INJURY.

Ee was lifting floor sections when he strained his lower back. This became a reportable injury on 9/16/04 when ee sought follow-up treatment.

EE WAS CLIMBING OUT OF MS-35 PIT AND STEPPED ON THE GUARD TO EXIT. THE GUARD FLIPPED UP AND STRUCK HIM ON THE LEG. THIS RESULTED IN A LACERATION WHICH REQUIRED STITCHES.

EMPLOYEE WAS INSTALLING REPLACEMENT CABLE THROUGH DROP BALL. HE WAS ATTEMPTING TO SECURE THE CLAMPS WHEN THE CABLE SLIPPED & STRUCK HIM ON THE LEFT SIDE OF HIS HEAD, RESULTING IN LACERATIONS.

EMPLOYEE AND HIS PARTNER WERE TRASPORTING ROLL SHAFT FROM MIANTENANCE SHOP TO MAINTENANACE QUONSET BUILDING. THE ROLL SHAFT WEIGHS APPROX 2000 LBS. WHILE A 3RD EMPLOYEE WAS LOWERING THE SHAFT ONTO A WOODEN PALLET USING A FORKLIFT, THE SHAFT ROLLED OFF PALLET GAINING MOMENTUM. EMPLOYEE ATTEMPTED TO CONTROL THE SHAFT WHEN IT STRUCK HIM AND PINNED HIM TO THE GROUND.

Employee was shoveling wet heavy material underneath a belt and felt muscle strain in her right arm. This injury became reportable after ee went for treatment in late May.

EE STEPPED ON BUMPER OF TRUCK TO REACH PORTABLE TORCH. BUMPER WAS COVERED WITH SNOW AND ICE AND EE SLIPPED AND FELL TO THE GROUND, CAUSING STRAIN TO HIS BACK & RIGHT LEG. HE DID NOT SEEK TREATMENT UNTIL 3/19/04.

EMPLOYEE WAS HAULING CASES OF OFFICE PAPER DOWN TO THE BASEMENT FOR STORAGE. HE FELT A STRAIN IN HIS LEFT GROIN AREA THAT BECAME PROGRESSIVELY WORSE WITH TIME. HE BEGAN LOSING TIME ON FEB. 23 WHEN HE HAD HIS SURGERY.

EE STATED HE WAS UNPLUGGING CHUTE BY HAMMERING A BAR INTO THE BLOCKAGE WHEN HE MISSED THE BAR AND HIT HIS LEFT INDEX FINGER RESULTING IN A LACERATION.

EMPLOYEE WORKS ON THE MAINTENANCE CREW AND HAS NOTICED INCREASED PAIN AND LOSS OF STRENGHT AND FEELING OVER SEVERAL MONTHS. HE WAS DIAGNOSED WITH REPETITIVE MOTION SYNDROME. HAD SURGERY ON 4/30/04 AND BEGAN LOSING TIME ON 5/3/04.

EMPLOYEE STATED SHE WAS CLEANING MIXER/ AERATOR CHUTE WITH CHISEL. CHIPPING HAMMER SLIPPED, CAUSING HER TO TWIST HER BACK, RESULTING IN STRAIN OR POSSIBLE PINCHED NERVE.

EE STATED HE WAS OPENING THE PAINT VALVE ON #4 TANK WHEN HE FELT A POP IN HIS LEFT BICEP. THIS BECAME A LOST TIME ACCIDENT WHEN EE BEGAN LOSING TIME ON 7/8/03.

EE WAS CUTTING A PIECE OF STEEL WITH A TORCH WHEN HIS CLOTHING IGNITED, CAUSING BURNS TO HIS STOMACH.

EE STATED HE WAS CHANGING A TIRE AND FELT BACK PAIN WHILE LIFTING THE TIRE. THIS BECAME A RECOREABLE INJURY WHEN EE BEGAN PHYSICAL THERAPY IN AUGUST.

ROCK WAS HUNG UP ON SIDE OF CRUSHER. EE PUT A CABLE AROUND ROCK & JAW CRUSHER OPERATOR WAS BRINGING THE 12 TON HOIST OVER TO LIFT ROCK WHEN IT DROPPED APPROX 3 FEET INTO CRUSHER. EE SLID OFF R OCK & ATTEMPTED TO CATCH HIMSELF WITH HIS ELBOW ON SIDE OF CRUSHER OR WALKWAY & HYPEREXTENDED HIS SHOULDER. EE STARTED LOSING TIME ON 7/22/02.

EE STATED HE WAS DUMPING SCRAP METAL INTO STEEL DUMPSTER USING A SKID STEER WITH BUCKET. WHILE RAISING THE BUCKET, THE LOAD SHIFTED, CAUSING THE SKID STEER TO LEAN FORWARD. EE REACHED HIS HAND FORWARD FOR BALANCE AND HIS FINGERS BECAME PINCHED BETWEEN THE LOADER AND THE EDGE OF THE DUMPSTER.

EE WAS TURNING SWITCH FROM ONE TRACK TO THE OTHER.

EMPLOYEE STATED HE AND HIS PARTNER WERE SETTING UP FOR BAGGING 100# BAGS. THE BAGGING SYSTEM HAS A SCALE, AN AIR TRIP MECHANISM, AND CAN BE SET FOR DIFFERENT HEIGHT BAGS. PARTNER ACCIDENTLY DROPPED THE HEIGHT OF THE SCALE AND EMPLOYEE'S HAND BECAME PINCHED BETWEEN THE BAG TRIPPER AND THE FRAME RESULTING IN A LACERATION REQUIRING STITCHES.

EMPLOYEE STATED HE WAS TAKING STEEL OFF UPPER PART OF STEEL RACK WHEN HE SLIPPED ON ICE AND FELL BACKWARDS.

EMPLOYEE WAS INSTALLING NEW SCREENS AND WAS KNEELING FOR A LONG PERIOD OF TIME. WHEN HE STOOD UPHE STRAINED HIS RIGHT KNEE. THIS INJURY BECAME REPORTABLE 6/19/00 WHEN HE SAW A DOCTOR FOR A FOLLOW UP VISIT.

EE STATED HE WAS CLIMBING A LADDER TO OPEN A CHUTE WHEN HE FELT PAIN IN HIS BACK.

EMPLOYEE STATED HE WAS TRYING TO UNPLUG FINISHEDPRODUCT ELEVATOR. HE FOUND A CERAMIC TILE AT THE BOTTOM AND ATTEMPTED TO REMOVE IT WITH A 3 FT SECTION OF LUMBER. AS THE TILE WAS DISLODGED, TH E ELEVATOR SPUN BACKWARDS FORCING THE LUMBER OUT OF THE ELEVATOR CASE. THE ACTION PUSHED HIS ARM BACKWARDS WHERE IT STRUCK A STEEL ANGLE SUPPORT. RESULTED IN FOUR INCH LACERATION AND FRACTU
