Employee was running dryer. Needed to grease bearing. Didn't shut down dryer. Went under guard & tried to grease while dryer was running. Fore arm was caught on something and EE's skin was removed.

Contractor was cutting a piece of tin with a grinder when a small piece of metal struck them in the eye.

A bucket line was being held up using a hoist and a crane. The hoist broke and struck employee on the arm.

Employee climbed on excavator and operated it for approx 3 hrs & when EE climbed off EE felt pain in lower center to right of back. EE thought it was just stiff from operation of excavator but has gotten worse over the last 3 days.

Raymond Roller Mill star feeder was blocked with large clay lumps. Employee shut the system down to remove the the blockage from the feeder. When he removed the lumps of clay causing the blockage, the feeder turned and rolled over his left hand causing severe lacerations to his ring and middle fingers.

Employee was cutting steel supports from a screening structure and came in contact with a hot piece of steel causing a burn to his upper left arm.

Employee was trying to locate a leak in the 8" vacuum line from the clay drum filters. When he ran his hand over the area of the pipe where he suspected the leak to be located, the vacuum sucked part of his left hand into a hole in the pipe. He subsequently pulled his hand out lacerating the middle finger of his left hand on the sharp edge of the rusty hole.

Employee was washing deck of dredge with a 2" water hose, another employee turned the valve open. Employee could not hold the hose and the hose hit him in the head. Employee was wearing a hard hat.

AFTER CUTTING A PIECE OF POLY-PIPE, EE STEPPED ON THE OTHER SIDE OF THE PIPE TO DETERMINE WHY THE PIPE HADN'T RELEASED. JUST AFTER HE WAS ON THE OTHER SIDE, THE TENSION RELEASED AND THE PIPE HIT EE IN THE LEFT LEG.

EMPLOYEE WAS USING A TORCH TO SHRINK-WRAP A PALLET OF SAND BAGS. HE HELD THE TORCH TOO CLOSE FOR TOO LONG AND THE FILM CAUGHT FIRE. HE USED HIS HAND TO PUT THE FIRE OUT, BURNING HIS HAND.

EMPLOYEE WAS GUIDING AUTOMATIC SWEEPER FELT PAIN IN LEFT SHOULDER.

WHILE SANDBLASTING HOSE FROM SAND RELEASED & HIT RIGHT WRIST

GRINDER SPIT METAL INTO RIGHT EYE.

INJURED WAS STANDING TOO CLOSE TO WELDERS AND RECEIVED THE GLADE DURING THE EVENING HRS.

WHILE NEAR SANDBLASTING PIECES OF METAL ENTERED RIGHT EYE

WATCHING INDIVIDUAL WELD WITHOUT WELDING GLASSES

WHILE WELDING PIECE OF METAL FEEL ON FOOT.

PC OF METAL ENTERED L EYE CAUSING AN IRRITATION

BAGGING SAND, PIECE OF SAND ENTERED EYE.

WORKING NEAR BURNER,PIECE OF HARD OIL CAME OUT AND GOT INTO EYE.BLINDNESS IN RIGHT EYE,SEEING 2 DRS.

WHILE GRINDING METAL FELL IN EYE.

WHILE WORKING IN SHAKER AREA PIECE OF HOT METAL FELL ON LEFT FOOT CAUSING BURN TO INSTEP OF FOOT.

WHILE CUTTING METAL, A SUDDEN JOR TO WRIST CAUSED PAIN.
