Employee was troubleshooting an issue with a California Pellet Mill. Two employees were rotating the pellet die when the glove of one individual became caught between the rolls and the die pulling the employees left middle fingertip into a pinch point causing a laceration and fracture of the fingertip.

Removing drive pulley off of Jet Mill #01 classifier, using hydraulic puller to remove pulley from classifier off of JM#1. While coworker was applying more pressure to hydraulic puller; puller stripped/kicked off to the side, catching employee's right hand.

Operator was feeding raw material into a feed bin when a small granular piece of graphite flung out of the feed bin striking the operator in the eye. The operator was wearing prescription glasses at the time of the accident. Operator requested a hospital visit due to pain in the left eye.

Employee was grinding a metal bracket with a Milwaukee 2785-20 angle grinder and a cutoff wheel. EE was wearing eye protection. EE awoke in the morning of 4/16 with an irritated eye and it was determined EE had a small piece of metal in EE's eye. EE sought medical attention to remove the piece and follow up care including antibiotics.

Employee was operating a stretch wrap machine when the material they were wrapping became unbalanced toppling over and pinning the employee against a nearby railing. Employee requested medical attention, ER doctors report minor swelling on employees back.

Employee placed a pallet of material on stretch wrapping machine off centered. As the pallet rotated around the wrapping machine it grazed the top of the employees foot due to overhanging the wrapping platform. This caused a contusion on the top of right foot, employee was placed on restricted duty due to injury.

Employee was riding along on a stretch wrapping machine turntable when EE lost footing and wedged foot between a wall and stretch wrapper. This resulted in a fracture in the right foot.

Employee was running a bead on a finished product bin and got a piece of metal in eye. Employee had on a hood, along with cap and welding gloves.

Employee was operating a slide gate at main plant screw packer #2. EE improperly grasped the slide while EE was operating it and pinched EE's finger between the hammer portion of the slide and the hammer stop fracturing the end of EE's thumb and causing a hematoma.

Employee was changing a dust collector sack without turning off the rotary airlock, he had been trained and understood that the airlock must be turned off prior to changing sacks, he had properly performed this task many times previously.

right elbow pain caused by employee was drilling holes in a cement wall, with a hammer drill and it got caught on something and it twisted his right arm. injury was on 7/22/15 first dr. apt. 8/28; x-rays on 8/28; MRI on 9/9; put on restrictive duty on 9/10/15.

Pressing pallet in hydraulic press and bag blew out and material lifted safety glasses and went into eyes.

He was cutting steel plate with a torch and a spark got behind his cutting glasses and burned his right eyelid.

THE EMPLOYEE WAS BAGGING MATERIAL AT THE PACKER, WHEN THE BAG BLEW APART, BLOWING MATERIAL UNDER HIS SAFETY GLASSES AND INTO HIS EYE.

Ee was in the maintenance shop fabricating a guard for the blower at the dual bins. When he was welding the pieces together he was not putting his welding hood down fast enough when he started welding.

The employee was driving the sweeper by the jet mill room. The forklift came out of the jet mill room and the two collided. The injured employee banged his head off the frame guard of the sweeper.

Employee was driving the sweeper, when the steering wheel spung around and caught the ee's thumb.

Coworker used threading machine to cut threads on pipe. EE was holding backup pipe with a pipe wrench so coworker could remove thread cutter. EE accidentally stepped on the On/Off switch, engaging machine. EE was pulled up & over rear of machine; fingers pinched between wrench and machine housing. Amputation distal phalange R. ring finger & broken L. index finger - severe inj.

EE was using a cutting torch to cut out and replace a section of chute. A piece of slag fell into the rolled up cuff on his right pant leg. The slag burned through the cuff and fell into his boot.

EMPLOYEE'S HAND GOT CAUGHT BETWEEN BELT AND PULLEY OF GRINDING MILL THAT WAS SHUT DOWN, BUT WAS FREE SPINNING.

EE'S WERE WORKING ON OPPOSITE SIDES OF A MACHNE INJ EE WAS WELDING . NEITHER COULD SEE WHAT THE OTHER WAS DONG. EE ACCIDENTLY GOT HIS HAND IN THE PATH OF TODD'S TORCH & WAS BURNED.

EE WAS BAGGING MATERIAL WHEN BAG BLEW UP ON #2 EMILL (RIGHT HAND BAGGER). GRAPHITE BLEW IN LEFT EYE.

RUNNNING TRASH COMPACTORS & PIECE OF METAL CAM EDOWN SLIDE AND HIT BACKING LATE AND BOUNCED OFF OF COMPACTOR AND HIT EMPLOYEE IN FACE.

EMPL DOING OVERHEAD WELDING WHEN HOT SLAG FELL INTO TOP OF RIGHT STEEL-TOED WORK BOOT.EMPL DIDNT FOLLOW INSTRUCTIONS OF COMPANY EMT. DEVELOPED SEVERE INFECTION AND HAD TO SEEK MEDICAL ATTENTI ON.

EMPLOYEE WAS RUNNING SUPER SACKS ON SCREEN II/HE ALLEGES THAT MATERIAL LEAKING FROM THE SYSTEM ENTERED HIS RIGHT EYE.

GRINDING CASTING OFF ROLLER MILL (WAS WEARING SAFETY GLASSES) AND GOT A PIECE OF METAL IN RIGHT EYE.

HOT WATER LINE 5ET GO FROM CLAMP ON NOZZLE OF STEAM JENNY. HE HAD SECOND DEGREE BURNS TO THE STOMACH AREA OF HIS BODY.

BURNING STEEL P3ECE WENT DOWN PANTS AND BURNED LEG AND LEFT ANKLE,TOP DIDN'T APPEAR TO BE BAD WORKED THROUGH JULY 6 AND WENT TO DOCTOR ON JULY 6.

THE TUBE WHEN HIS MIDDLE FINGER OF HIS RIGHT HAND CAME INTO CONTACT WITH THE AIRLOCK FAN BLADE OF THE DUST COLLECTOR. MIDDLE FINGER WAS LACERATED. EMPLOYEE DID NOT ASK SUPERVISOR IF HE COULD C HECK THIS MACHINE NOR DID EMPLOYEE SHUT OOF THE MACHINE.

WORKING ON PACKER SOMEONE TURNED SWITCH ON SPINNING BLADES IN PACKER CAUGHT FINGER

WAS TOLD TO JUST WATCH WHILE LEARNING STOCKRUNNER JOB FEEDING 5 & 6 BALL MILL, BUT HE THOUGH HE COULD DO JOB, HE DROPPED HIS KNIFE IN FLOOR HOPPER, WHICH DIDN\T HAVE SCREEN OVER OPENING WHICH IT USUALLY HAS. HE CAUGHT MIDDLE FINGER LEFT HAND IN SCREW. FINGER BROKEN AND LACERATED AND HAD 12 STITCHES.
