Removing drive pulley off of Jet Mill #01 classifier, using hydraulic puller to remove pulley from classifier off of JM#1. While coworker was applying more pressure to hydraulic puller; puller stripped/kicked off to the side, catching employee's right hand.
Operator was bagging pallets on Screen 5, picked up pallet to place under new sack and felt pain in left shoulder. Continued to work through shift. Did not report to foreman until pain persisted through to new shift next day that started at 7am. Went to foreman at 11am due to pain not subsiding and affecting work.
Employee was climbing up ladder outside screen #5 to check bin for fullness with headlamp, when climbing down ladder, employee felt sharp pan in left shoulder. Ambulance was called at 9 pm and was taken to emergency room to be evaluated.
Was doing a screen change on Screen #1 and bottom screen needed stretch out and employee left middle finger popped.

Employer was advised that there were three MSHA reportable audiogram shifts per annual med bus testing. Employee will be informed and any future testing may be advised.

Was getting off forklift at roller mill 1 when right knee popped. Fell onto right knee after it popped.
Went to step up on forklift and right foot slipped off of forklift. Grabbed handle to keep from falling, twisted, and felt a pop in the right side of back.
Was removing a pipe nipple from Jet Mill 2 cooler system with a pipe wrench using my legs to hold me in place.
Doing screen change on Screen 5, EE got right pinkie caught under deck while letting it down.
Going down vertical ladder. Slipped going down and jammed right foot.

By compactor, employee was pulling a box full of junk, box ripped and employee fell backwards. When Employee fell backwards employee jammed left wrist. Employee was given a velcro splint to wear for two weeks.
Employee was helping to position a gear box onto a screw conveyor when the gear box fell from the chain fall it was being lifted by contacting the employee on the left thigh.
Locker room at Asbury; felt something in right eye. Used eye wash station and also the squeezable tubes of saline solution to try and wash out right eye. Couldn't seem to get out was was in eye.

Employee was helping the loader operator make a mix when EE picked up a pallet and experienced pain in EE's back.

Working on trying to remove pulley from JM1 either dirt of piece of metal got in eye.
Employee was working at Ball mill #1 tightening the door on the top. When EE went to turn the bar, EE "felt a pop and a tear in the groin area". EE was taken by ambulance to the ER. released "off work" until seen by a surgeon. EE's first appointment is on 4-12-19.

After annual audiometric testing one employee was found to have a reportable change in hearing.
Employee was cleaning bathroom and was stung by a bee. Taken to hospital on 7/21 and released on 7/21. Returned to work 7/24 (Monday).
Employee was removing strap and shackle, bar or strap moved, pushing left hand into the sheet metal side of the cooler. Causing laceration to 4th finger on L hand .Employee received medial treatment, stitches and x-ray, on 7/20.17 immediately after injury occurred.

While helping the screen #3 Operator hang a sack at the floor dump, the forklift Operator lowered the forks to quickly and the employee's finger got caught between the hooks of the forklift and the sack of material.
The employee was getting dressed for work. When he put on his shirt, he hit a built-in wooden shelf in his locker and got a splinter under his thumbnail. He left early and went to his family doctor, who removed the foreign object. When I received the doctor's report, it stated that it was a metallic splinter.
THE EMPLOYEE WAS PULLING WIRE AT A JUNCTION BOX THAT RUNS FROM THE AC MILLS, HE HELD UP SOME LOOPED WIRES TO MOVE ANOTHER WIRE, WHEN HE RECEIVED A SHOCK. DURING THE INSTALLATION, THE INSULLATION ON THE WIRES MUST HAVE BEEN DAMAGED, LEAVING A KNICK IN THE WIRE.

Two employee went upstairs to do a screen change. One felt weak and had trouble breathing. He laid down on the floor and sent his co-worker to get their foreman, who then called 911. The temperature has been really hot and humid for the past week.
EE was moving drums at the blender when he noticed a sharp pain in his right shoulder. When he tried to finish bagging at the magnetic, the pain go worse. Became reportable on 11/27/07 after surgery.
While attempting to open a thief sampler, the employee's pinky finger slipped into the slot on the sampler. While sliding it apart, it stuck and when the employee pulled harder, the tool sliced off the tip (1 centimeter) of his pinky finger
