Employee was cutting a 4" black iron steel pipe with a portable band saw. The saw grabbed the pipe and jumped up and the employee's hand went under the spinning blade cutting employee's fingertip off just behind the fingernail.

Mostrando los 15Removing the rock breaker from above the primary crusher. while hooked to the breaker with the crane the employee cut off the last bolt holding the breaker in place. once cut the breaker shifted backwards pinning the employees legs against the hand rail around the platform the breaker was bolted to.
Employees right palm of hand was cut while using a impact wrench to install bolt and nut fasteners retaining screen media.
On 8-18-11 at 4:50 pm employee was struck in the right hand by a falling rock causing a laceration to hand and middle finger.
Employee was operating an electrically powered hand drill, boring out a piece that was secured by a vice. The drill bit caught the metal piece dislodging it from the vice striking the employee on the left side of the face around eye area. The employee was wearing safety eye protection. The employee sustained a slight fracture of the left orbital.
While trimming metal tail pulley safety guard with a torch, a small piece of molten metal fell into employee's right ear. He had appt. w/ear doctor on 6/26/06 and is scheduled for ear surgery on 7/18/06 for perforated ear drum. He went back to the doctor on 7/12/06 because of irritation & received antibiotic on 7/12/2006.
THE EXCAVATOR WAS LOCATED @ BASE OF A SHOT THAT WAS BLASTED THE PREVIOUS DAY, W/THE BOOM POSITIONED PARALLEL TO THE MUCK PILE & THE CAP RIGHT NEXT TO THE PILE. THE EE WAS USING THE HYD HAMMER TO BREAK SOME OVERSIZED ROCKS WHEN A ROCK SLID OFF THE SHOT & ROLLED INTO CAB OF MACHINE. CAB OF EXCAVATOR SUSTAINED DAMAGE & EE RECEIVED 6 STITCHES IN RIGHT LEG BELOW KNEE.
EE & ANOTHER EE WERE DRILLING A HOLE FOR SHOOTING A PIPE LINE DITCH, THEY WERE UNPLUGGING THE DRILL AND NOTICED THE STEEL HAD COME LOOSE FROM THE DRILL, THEY STOPPED THE ROTATION AGAIN THEY BOTH SCREWED THE STEEL PART OF THE WAY BACK IN WHEN THEY DECIDED TO START THE ROTATION SINCE THEY HAD IT SCREWED FAST. THIS IS WHEN EE'S COAT GOT CAUGHT IN BIT & ROD WHICH IN TURNED SLAMMED H
EE WAS GOING TO PUG OUT AT PUG MILL WHEN HE MUST HAVE FOUND THE FEEDER PLUGGED AND ATTEMPTED TO UNPLUG THE FEEDER WITH A SCALING BAR, THE PUG MILL BEATERS MUST HAVE BEEN RUNNING WHEN THE BAR G OT CAUGHT INTHE HEATER AND CAUSED THE SCALING BAR TO STRIKE HIM SEVERAL TIMES ON THE HEAD.
THE EE WAS RETORQUING HEAD BOLTS ON TRUCK WHEN HE GRAVOUNTED HIS LUMBAR SPINE.
EMPLOYEE WAS GRINDING STEEL WITH A PORTABLE GRINDER. THE GRINDER KICKED & HIT THE EMPL ON THE FINGER. THIS CUT TO THE FINGER REQUIRED SURGERY TO REPAIR THE TENDON ON 7/26/93.
THE EMPLOYEE AS OPERATING A D10N DOZER PUSHING AMUCK PILE DOWN TO A LOADER. WHEN HE WAS HALF THE WAY DOWN THE MUCK PILE HE HAD SEVERE LOWER BACK PAIN. TALKING TO THE OPERATOR HE SAID HE DID NTO HIT ANY LARGER OR SOLID MATERIAL WITH THE DOZER CAUSIGN ANY SUDDEN STOPS OR SHIFT OF HTE MACHNE. HIS LOWER BACK PAIN SEEM'S TO BE AN IRRITATION OF A PREVIOUS STRAIN. EMPLOYEE HAD MEDICAL
54736YEE WAS REMOVING A TOP FROM THE #2 WHIZZER MILL AND LIME BLEW INTO LEFT EYE. DR. PRESCRIBED MEDICATION FOR SECONDARY INFECTION AND REMOVED DIRT FROM THE EYE LID.
EMPLOYEE WAS TRYING TO UNPLUG BLOW BACK SYSTEM AND LIME WAS BLOWING IN HIS FACE. LIME WAS REMOVED FROM HIS EYE BY A DOCTOR AND HE WAS GIVEN AN EYE PRESCRIPTION TO RELIEVE PAIN FROM THE IRRITAT ION.
