The bulk loader proceeded to clear the air slide and opened the clay belly to address a clog using an air hose. Upon opening the air valve, cement was expelled in the opposite direction, resulting in cement entering THEIR left eye. Eye wash station used, but followed up with eye doctor.

While making wooden storage boxes EE attempted to drive a 4 inch screw through a 2x4 using an unstable support, the screw pierced through the wood, EE's shoe, and into EE's right foot at the base of the middle toe.

Employee was using a 4" Dewalt Grinder to clean up a weld. The grinding disk broke apart and a piece of the the disk lacerated the back of left hand behind the middle finger. Employee was transported via company vehcile to an urgent care facility and received sutures

Metal shaving from lathe operation struck miner in left index finger. Miner received 3 stitches

Miner was using pneumatic jackhammer (rivet buster) on a sidewall performing refractory demolition when the bit broke, causing the tool to slip and impact EE's left hand against the wall. Miner has laceration and fracture on left index finger.

Employee was operating a Brokk machine in the kiln when a capped brick came loose from overhead and struck EE in the right shoulder. Employee had a right shoulder strain from the impact.

Worker lacerated skin on four fingers using a saw cutting wooden wedges

The operator tried to dislodge clay buildup from a return roller on the top return belt on the crusher. The crusher was running at this time. The operator tried to lay a 12" pry bar against the roller to knock the clay off. The operator inserted the pry bar too far which caused the belt to grab the bar and pull the operator's left hand and wrist in between the belt and roller.

EE was replacing a chain within the Primary Cone Crusher. As EE was kneeling on the concrete ring at the base of the cone cutting a chain link - the angle grinder they were using kicked back striking left thigh above the knee cap causing a laceration. EE was taken to the walk-in clinic where the wound was sutured - EE was released back to full duty.

Were removing interference fitted sprocket while installing bucket elevator sprocket under pressure with porta power ram, the bolt to the puller broke while employee was in front and struck employee left outer bicep.

Was breaking up hardened material with jackhammer, jackhammer broke causing loose material to fall and EE slipped on loose material. EMTs were called and EE was released from ER that night and followed up with DR who put them on restrictions:NO heavy lifting, pushing, pulling, climbing, overhead reaching, only sedentary work. Will be reevaluated 3/17/21.

Employee was grinding metal that had recently been welded in the hammer mill when a piece of metal flew up under EE's face shield and got into the left eye. EE's safety glasses were not fitted tightly due to wearing an N-95 mask.

The employee was using a cordless impact gun with a hobble head and 3/4" deep socket, the socket popped off and hit employee in the mouth.

The driller was tramming the drill off of the lowboy trailer, the driller had the door of the drill locked open in order to see the edge of the trailer. When the driller reached the point where it tipped forward onto the ramp, the driller reached out and grabbed the door frame, the jolt released the door lock lever, slamming the door onto the driller's hand.

The continuous miner (CM08) had backed out and was pulling back in for the final car, in the Final. The tail of CM08 was swung to the left, as CM08 started back in the miner helper was caught between the end of CM08 tail and rib. The operator positioned inby was unaware of the miner helpers location. Currently known injuries are broken arm, pelvis and possible organ damage.

Contractor was repair a coupling when the machine turned back on and caught EE's hand.

Right hand pointer finger smashed

Cable was bound up on the CKD load out spout, the employees finger/glove was pulled into a pinch point.

EE smashed EE's fingers while bricking. While using the ram to put pressure on the bricks, the tube broke and smashed EE's fingers against the shell.

Employee was attempting to weld a plate on the crusher, the plate slipped, EE tried to catch it and left a sharp pain in EE's right shoulder.

Employee was grinding off metal that had been cut on flop gate. A piece of metal must have got under EE's safety shield and safety glasses, causing an abrasion to eye. Employee did not notice discomfort until EE got home.

Emloyee was trying to put a radius on a twisted d-ring using the hyd press. When pressure was applied to the piece it slipped and sent one of the set up blocks flying . Hit the employee in both arms and lacerated the left elbow.

EE #1 was cutting C-Channel steel to remove it, while standing on a 6ft A-Frame ladder while EE #2 supported middle on another 6ft ladder. A piece of slag hit EE #1 causing EE #1 to turn, temporarily blinding EE #2 who stepped off ladder. EE #3 stepped forward to see if EE #2 was okay & steel broke off, fell 10ft. glancing EE #3's hard hat & hitting their shoulder.

Associate performing demo work with a jackhammer upward, a large piece of cement broke off and came down and smashed finger, the pressure created a gash.

Employee was cutting a board, using a pushed stick to push the material through the saw. Holding the material against the fence with left hand and pushing the stick with the right hand. The wood bucked in the saw and my fingers went into the blade.

Employee was using a sawzall to cut hilti bolts off. Employee had a reverse grip on the saw and employee's little finger on employee's left hand was pinched in the sawzall.

Employee was lancing off the tail pulley, lance caught in the tail pully and glove caught on the air lance and pulled hand into the guard. Broke and lacerated the little finger on employee's left hand.

Using welding equipment on grizzly bars and working in an uncomfortable position for an extended period of time.

Operating back hoe on uneven ground. (This is under investigation due to previous underlying degenerative discs)

He was jackhammering a concrete block. He had one leg behind the other to get good leverage on the jackhammer. When he stopped and stepped back and stood up straight, his hip felt like it got stuck and felt a sharp pain in his right hip and could not walk.

Employee was welding and grinding on a steel pipe at his work station. He felt irritation in his right eye around 1:00 PM on 1/15/2013

Employee was drilling out a hole in a guard. The drill bit caught in the metal causing the drill to catch and twist his arm.

Employee was loosening a bolt with an impact wrench and holding the nut with his fingers when the nut spun around and cut through his glove and slicing his right index finger.

While cleaning the tip of the burner barrel pipe & the front of the kiln & firing hood with a 15' long stainless steel air pipe with a 45 bend in the end of it. also a T handle set up upon turning on the valve the air pressure casqued the pipe to suddenly twist in a counter clock rotation and I was hit in the jaw. The blow broke some teeth loose and sheared them off at gum line.

Employee was bolting on right side of bolter #3. He stated that he had drilled his starter steel into the roof. He put the second drill steel in the box and was lining the 2 steel up. He bumped the feed up the bottom steel missed glancing over to the open jaws where the tip of his left thumb got crushed and tore off his thumb nail.

RED IRRITATED RIGHT EYE

Employee was walking to the bolter when it was backing out of the entry. As operator was pulling into #2 entry he left his cable reel on, pulling the cable off the roof. The cable swung from one rib to the other, striking employee below the left knee. Employee did not seek medical attention until 4/26/2010.

I was changing the bit in a 3/8 angle drill and forgot to unplug it. When I went to tighten the chuck I squeezed the switch on the drill and the chuck key went into my thumb. I had changed the bit several times during this job and always unplugged the drill but forgot to do it this time.

Ee was standing on the burner floor, air lancing a clinker pile build up, when hot material landed on his neck. The ee immediately reported the incident to his supervisor, but did not require medical attention at that time. The wound became infected, so medical treatment began on 7/20/2009. The injury became reportable on 9/30/2009 when received surgery.

Employee was roof bolting, swinging boom out. Got caught by head of bolter and rib, causing injury to right knee.

Employee was replacing the drive roller in Receiver 1. During testing he turned on the wrong valve & receiver came down on his hand - contusion, pain, swelling.

Effected employee was working in the clinker cooler to assess damage/required repairs, when hot material fell from above on his head, neck, arms and back. EE was assisted out by a fellow workers and taken to the shower to rinse off. Ambulance transport to hospital ER per company policy. All LOTO procedures on equipment had been followed.

While moving the miner, the employee was hit by the tail of the miner, when he tried to prevent the cable from sliding through the I hook on the tail of the miner. The tail of the miner hit the employee just below the rib cage.

Sub contract employee (Noel Company) was reaching over to shut down a portable generator, when he hit the switch of a 4" portable grinder which was nearby. The grinder spun around and struck him in the right hand between the thumb and index finger, resulting in a laceration.

Dust from dust collector became airborne and ignited, burning the face and arms of the employee.

The employee was blowing concrete debris out of a hole with pressured air, when something went into the eye.

Employee was cutting frame on continuous miner when a piece of slag blew back in under his glasses and got into left eye causing an abrasion to his eye.

Employee was under Mac 10 mantrip using air wrench to take bolts off leaf springs. Mud was packed around frame. Employee was wearing safety glasses w/side shields. Machine was blocked up from the weekend. Dirt got into left eye causing abrasion to eye.

Employee was grinding on a flange when something flew into her eye.

Employee was walking through the mill area where co-workers were grinding on fiberglass gaskets. The wind picked up and blew a strand on fiberglass into the employee's eye.
