Upon opening the air valve, cement was expelled in the opposite direction, resulting in cement entering THEIR left eye. Eye wash station used, but followed up with eye doctor.
The miner was moving a new wire reel to power the hoist. Miner was using a socket to remove a bolt when the socket slipped and hit the miner in the mouth. The bottom of the miner's two front teeth was chipped off.
After shift ended, employee left the main building to the parking lot where there is one set of stairs before reaching the lower level. It had snowed about 6"-8" the evening of their shift. Employee slipped on the second to last stair landing on their back. On their way home employee felt a sharp strong pain in their mid-back when moving.
Was Changing support rollers on 57101 Clinker Pan conveyor. Was using standard steel 3lb sledgehammer to drive the rollers into the bracket on the pan. The hammer glanced off the end of the roller and struck the heat-treated flange of the roller causing a shard of steel to break off and lodge in leg. The employee was taken to clinic and released back to work with no restrictions.
The employee was lifting a wheelbarrow to dump contents into the hopper. The ring on EE's left ring finger got snagged and popped the finger, causing pain and swelling.
Employee was installing a flange, when chip plate fell, causing the flange to fall, striking EE's left thumb.
Miner was cleaning the tower using a bar and a sledgehammer. Miner hit the bar with a hammer; a piece of metal broke off and flew into the miner's eye.
Employee was fueling front end loader and walked away from the equipment and when EE heard the fuel overflowing ran to stop it, stepped on a rock and employee sprained the ankle.
Miner was shoveling rock into buckets at the tail end of the limestone tunnel and hauling the buckets outside the tunnel to dump. Miner hurt their lower back in the process.
The employee was assisting in installing a new drop pipe. The pipe remained secured but shifted from its placement, and the employee on the ground had a hold of it to keep it from swinging and felt the impact of the pipe shifting and felt discomfort in EE's back.
Lost the kiln mcc and lost power to kiln draft, and the atox feed belt caught fire. Fire was controlled in about 15 minutes.
The Employee was caring an electric winch onto the Raw Mill to help with the Discharge grate job. While descending the steps to the mill The Employee trip and fell on top of the mill with the winch in hand. They injured their left ankle during the fall.
The employee was moving thin sheets of metal (not wearing the appropriate PPE, cut-proof gloves) and cut EE's finger, resulting in five stitches.
The employee was bent down picking up extra bolts when they felt a pop in their knee as they stood up. It was determined that the employee tore their meniscus and needed surgery.
Miner was replacing rear belt skirting. After removing the old skirting, the miner used it to trace and cut a new piece. As the affected miner was starting to cut the new piece, the knife slipped and embedded itself about an inch into their leg. The miner was taken to urgent care, where they received four stitches just above the knee.
Miner was called in to repair the splice in a belt. After completing the repair and reassembling the equipment, the miner checked the belt's operation. While inspecting the belt, the miner accidentally stepped on some clinker on the ground, causing them to slip and hyperextend their knee.
Mechanic was climbing out of bucket elevator and felt a pop in the right knee. Employee was still able to perform task and did not miss work until surgery date of 12/4/2025.
Employee was pushing forks together on a telehandler when EE felt a pain in EE's left bicep.
While descending a portable ladder, an employee missed the last steps and landed awkwardly on the ground, resulting in a knee injury. The employee was promptly attended to and received medical evaluation.
A delivery driver employed by a water supply vendor was in route to the facility when the tanker truck overturned and slid off the road. Investigation ongoing
While making wooden storage boxes EE attempted to drive a 4 inch screw through a 2x4 using an unstable support, the screw pierced through the wood, EE's shoe, and into EE's right foot at the base of the middle toe.
The employee chose a 4-foot pry bar instead of a 6-foot pry bar to open a valve on a silo pump. The employee strained and felt a pop in their shoulder.
2 NSM XC4, 7 Entry. A roof fall occurred and was found by a weekly mine examiner. Approx. 27 feet long x 17' wide. The fall was above the 7' bolt anchorage zone.
While exiting a man lift the employee stepped on a impact and twisted right ankle.
A 45 ton haul truck pulled away from the dump area with the bed still partially raised, it then snagged a powerline, pulling it down. Personnel were evacuated to a safe distance, power was disconnected and the equipment removed
On 5/7/2025, an employee was injured while attempting to remove an Easy Skirt from the Feed Belt of Finish Mill #4. The worker was using a pry bar to detach the skirting from its mounting plate. During the task, the bar slipped while being forced downward, and the sharp end punctured the employee's right palm. The injury required medical attention, including three stitches.
Employee was using a 4" Dewalt Grinder to clean up a weld. The grinding disk broke apart and a piece of the the disk lacerated the back of left hand behind the middle finger. Employee was transported via company vehcile to an urgent care facility and received sutures
The employee was tightening bolts and aligning a motor on a compressor. EE felt fine during EE's shift but felt discomfort 27 hours later, after driving 5 hours on personal time. EE felt discomfort in EE's right rib area. The injury is still under investigation to determine whether it is work-related or not.
Employee was in the process of putting a bolt in a steel beam. With EE's left hand holding the bolt in the top bolt hole. As the left structure piece was closing the gap between the end plate and the main beam the tip of EE's left thumb was briefly caught in the gap.
An employee sustained an injury to left thumb. The injury occurred when employee was using a pry bar to remove a rock lodged between the conveyor belt and its supporting structure.
Employee was torqueing studs at Finish Mill. Employee dropped nut on foot.
Right thumb tip was pinched between pallet jack and dunnage.
Mill Manlift gearbox broke. Miner was entrapped in manlift for 38 minutes. Miner egressed out emergency exit of manlift roof.
A miner received fatal injuries. After a bridge collapsed that the miner was operating a skid steer loader on. The miner was clearing refractory brick form a rotary kiln. The bridge that was installed by the property owner collapsed. The skid steer fell approximately 10 feet into a hopper, causing the payload of brick to engulf the operator causing fatal wounds.
Employee was getting out of haul truck to load production and experienced spasms in their back. Called supervisor for assistance and was taken for medical attention. Placed off work for 1 week.
Employee lifted a new cutting blade into place on the continuous miner gathering head to weld. A floor jack was used to pressure it into place. The jack extension was side loaded not center loaded causing it to break free, flying out and hitting the employee above the right eye. The extension cut through the right eyebrow onto the eye lid. It required 6 stitches to close it up.
Employee was climbing ladder in the reclaimer when they felt a pinch in lower left back.
Employee was welding on the ground level, suddenly EE turned to the opposite side of work area and tripped over a piece of wood lying on the ground. EE fell placing hand on the edge of a structural component H-Bar 3" flange x 4" x 6" base, the sharp edge of the material caused a deep cut in EE's right hand. EE was wearing all the PPE and proper gloves.
EE was replacing a wheel on the pan conveyor and wheel needed to be realigned to remove. EE hit the wheel with a hammer and a metal piece fractured off the hammer and hit the EE in the chest. EE was taken to urgent care where they were able to remove the metal piece and stitch up the puncture.
Employee was at TT1 Transfer belt when EE's head came into contact with a cross member beam. The employee tripped and fell to the ground where EE suffered an injury to their ankle.
The Employee was blow drying the ceiling in the air compressor, cleaning the debris
Metal shaving from lathe operation struck miner in left index finger. Miner received 3 stitches
A Contract Truck Driver arrived on the Pueblo Plant South Truck Bay Shipping Loadout. Two GCC employees noticed this driver was laying on the platform with their head and neck area on the midrail of the gangway. After assisting the drive to their feet, driver was able to climb up the stairs under their own power.
Miner was adjusting a belt scraper using an adjustable wrench. The wrench slipped off the scraper with a lot of force and hit the miner in the mouth.
Employees were using a chain hoist to lower parts down into the tunnel. One employee grabbed the chain on the hoist and had EE's hand on the handrail, which created a ground, causing EE to receive a shock of 270 volts. The 480-volt outlet was wired incorrectly. The employee was taken to the clinic, evaluated, and returned to work with no ill effects.
Crusher Operator was moving a wheelbarrow full of material and as it was tipping over they tried to catch it and strained a shoulder and bicep. The EE is still able to perform normal task and has not missed work. MRI results came back in November showing torn rotator cuff and EE will need surgery.
Miner was working in the quarry, exited excavator. While walking back, miner tripped over a rock. Miner landed on shoulder injuring the shoulder and thumb. Miner was treated and released to work restrictions within the miner's normal work duties. On October 4th, miner had surgery to repair the shoulder injury and placed off work.
Grinding on duct, raised face shield, debris got in right eye
Employee was holding a piece of rebar vertically with left hand to drive into ground. Employee was holding a sledgehammer with their right hand, chocked up to the head. As they were hitting the rebar, they missed and smashed the right index finger between the handle and the top of the rebar.
