Mostrando los 5A bucket line was being held up using a hoist and a crane. The hoist broke and struck employee on the arm.
Raymond Roller Mill star feeder was blocked with large clay lumps. Employee shut the system down to remove the the blockage from the feeder. When he removed the lumps of clay causing the blockage, the feeder turned and rolled over his left hand causing severe lacerations to his ring and middle fingers.
Employee was cutting steel supports from a screening structure and came in contact with a hot piece of steel causing a burn to his upper left arm.
Employee was trying to locate a leak in the 8" vacuum line from the clay drum filters. When he ran his hand over the area of the pipe where he suspected the leak to be located, the vacuum sucked part of his left hand into a hole in the pipe. He subsequently pulled his hand out lacerating the middle finger of his left hand on the sharp edge of the rusty hole.
