EE was assisting operation and placed finger on shim; finger was pinched causing a fracture and laceration of the left index finger tip requiring two sutures. Employee released to work without restrictions.

Mostrando los 22Miner experienced lower back pain while excavating sample material holes. Excavator occasionally hit hard material which caused a jarring sensation when front end of excavator would lift off the ground & land back down once bucket became free. Miner requested to visit WC Doctor on 8/10/18 due to lower back pain not subsiding. Physician placed miner on light duty.
Windshield wiper repair in progress. Turned wiper motor on to line up pin on sprocket with linkage. Sprocket rotated and pinched middle finger between the linkage and housing. Employee disregarded company policy and training on lockout/tagout procedures. Employee received 16 stitches to the left middle finger. No impairment, released for normal duty as a driller.
Employee sustained burn to right ankle when a piece of slag fell into a crease in jeans on top of boot trim; slag burned through the material and dropped down into the boot while performing welding repairs. Physician placed employee on 'off work' status.
Employee reported eye irritation, was taken to the clinic, employee had debris in right eye which required mechanical removal. Employee was released to return to work at full duty without restriction.
Employee was performing a belt splice, had a hand grinder in one hand and he was moving forward on the belt. The employee went to brace himself and hit the safety switch on the grinder powering the tool which cut his hand (7 stitches).
Employee was cutting welds to remove a cover plate to access the inside of a conveyor receiving box. Employee was using a grinder with a cutting wheel when the grinder kicked back striking and knocking off his hard hat and attached face shield. The wheel then struck the employees cheek causing a laceration from the edge of his lip to his cheek approximately 2.5 inches in length.
Worker was removing a bolt using a wrench and impact gun. Nut stuck and the force of the impact gun forced the wrench down and pinched off the tip of his finger
EE was grinding with a portable grinder, when the wheel slipped off the work surface and his forearm contacted a sharp edge of the conveyor frame.
Mud pump on clarifier exploded while EE was relieving preasure from top of pump causing hot rock and mud to splash on EE face and left arm. EE had light burns on face and 2nd degree burns on left forearm.
Employee was holding impact gun over his head to conduct maintenance on horizontal crusher. While doing this, he strained right shoulder.
EE was using gas welding torch to perform maintenance on conveyor, when a piece of hot slag entered around his wrap-around safety glasses and contacted his left eye.
While preparing to assist a 3rd party trucking company to load their damaged transfer trailer onto a flatbed our employee was injured. The transfer box fell off of the trailer ejecting our employee pinning him underneath the upside down transfer box.
MALFUNCTION OF THROTTLE CABLE ON 16H BLADE.
EMPLOYEE ALLEGEDLY WAS BUMPED EXTRA HARD BY A PUSH CAT DOZER. EMPLOYEE WAS OPERATING CAT 631 SCRAPER REMOVING OVERBURDEN.
EE USING HAND-HELD POWER TOOL WITH WIRE BRUSH WHEEL. HE WAS CLEANING OLD METAL PARTS. EE WEAR ING SAFETY GLASSES AND FACE-SHIELD FELT SOMETHING IN HIS EYE WHILE DRIVING HOME AFTER SHIFT.
EMPLOYEE WAS WELDING IN AN OPEN AREA WITH SOME LIGHT WIND. SPARKS ARCED CAUSING A FLASHBURN TO BOTH EYES.
REMOVING MOTOR WHILE USING A CRANE, EMPLOYEE PUSHED ON THE MOTOR TO FREE IT. THEN THE MOTOR SWUNG BACK AND SMASHED FINGER AGAINST STEEL FRAME.
AFTER CUTTING POLYLINERS FOR THE GRIZZLY WITH A SKILL SAW, HE RETRACTED HIS BODY OVER THE LENGTHOF THE LAST CUT. THE SAW WAS OUT OF THE CUT, ON ITS SIDE WITH GUARD FULLY EXTENDED. HIS HAND & FINGER WERE PLACED OVER THE TRIGGER BUT THE SAW WAS NOT IN MOTION, AS HE RETRACTED THE SAW WAS ACTIVATED & CUT INTO HIS KNEE.
WHILE LOOSENING A BOLT WITH AN IMPACT WRENCH THE BOLT SPUN AND CUT EMPLOYEE'S LEFT MIDDLE FINGER. FINGR REQUIRED STITCHES. EMPLOYEE WAS NOT WEAR GLOVES.
OPERATOR WAS RIPPING W PIT. WHILE LOOKING BACK OVER RIGHT SHOULDER AT RIPPER. THE DOZER TRAVELED OVER A HARD HUMP WHICH CAUSED THE DOZER TO ROCK FAST SIDE TO SIDE JERKING THE OPERATORS HEAD AN D NECK.
WELDING WHEN A FOREIGN OBJECT ENTERED HIS EYE.
