The mine superintendent was contacted in regards to smoke on the edge of the mine property. The fire department was contacted and extinguished a brush fire. An employee called MSHA Sunday to report the fire. A response was received the following day with requests of this 7000-1. There were no miners working at the time, no injuries and the cause of the brush fire is unknown.

EE1 was picking up tools from his work area on the lower east side cat walk of primary #2 screen when EE2, who was located on the upper cat walk of Primary #2, picked up a swing gate he was installing. When EE2 picked up the gate it came apart in two pieces, one piece falling down and striking EE1 in the nose. EE1 suffered from a laceration on his nose.

Employee was fabricating a steel chute performing cutting, welding, and grinding activities. The employee was in the chute squatting or kneeling for most of the shift except to stop for breaks. There was no one event that caused the injury, it seems to be from cumulative work throughout the day. His knee did not start to swell and become painful for a few days over the weekend.

At the end of shift on 5-10-11 EE removed glasses to wipe his face off and got dust in his eye. EE flushed his eye out before going home and felt that his eye was ok. On 5-12-11 EE woke up and his eye was red and inflamed; EE was taken to the clinic where a foreign object was removed and antibiotic drops were prescribed.

EE was loading trucks and moving 1X4 material with the 980H loader all day. After work his fingers on his right hand started getting numb. He called his Dr. and they told him to contact his employer because they thought that it was a work related injury possibly carpultunnel.
Employee was cutting a 1/2 inch air hose off of a reel. The knife he was gutting it with slipped and the emoplyee cut his right index finger.

Employee was removing polyurethane screen panels from the dewatering screen; when screen came loose, screen popped out and disloadged a rock. Dislodged rock hit and entered employee's right ear.
Employee was changing screens; while changing screens employee twisted and hurt lower back.

EE was using porta power (Jack) to spread slat conveyor; block slipped and porta power jumped up and hit EE on the upper lip, cutting upper lip.
Walking around the screen catwalk and misstepped on slopped catwalk.
EE was sweeping a concrete slab, when the metal handle on the broom folded. EE continued to use the broom when the damaged area on broom pinched his left pinky finger (tip) and caused a laceration that required seven stitches.

INJURED WORKER ATTEMPTED TO DISCONNECT AND REMOVE 2 30' SECTIONS OF 8" DIA ALLUMINUM IRRIGATION PIPING, TO ALLOW A DOZER TO PASS THROUGH AREA. ON THE SECOND SECTION OF PIPE, ONE OF THE JOINTS WERE STUCK. THE IW LIFTED THE FREE END, PLACED IT IN A HEADLOCK POSITION WITH HIS RIGHT ARM AND MOVED SIDE TO SIDE WHILE PUSHING AWAY FROM THE STUCK JOINT. SUFFERED A HERNIA.

EE WAS ENTERING CHUTE BELOW #1 SCREEN OF PLANT WHEN HE LOST HIS BALANCE. HIS LEFT FOREARM STRUCK AGAINST THE ACCESS FRAME WHICH HAD BEEN SHARPENED BY MATERIAL.
