Employee was welding at the Plant #1 Gyro and while doing so a piece of "slag" burned through EE's pants and went into EE's left boot and burned the bottom of EE's left foot.

Mostrando los 18Employee was assisting with supplies in the area where two welders were welding. Employee stated they did not look directly at the welding light but due to being in area while welding was taking place, the reflection of the welding light caused their eyes to be subjected to arc flash.
Night crew was rebuilding 5.5' Standard Crusher, which required the use of a 60- Ton crane, which was an outside contractor. After completion of the rebuild, the employee was assisting the crane operator with stowing away the outrigger foot pads. The employee placed their right hand on top of outrigger and crane operator extracted outrigger, thus crushing the employee's right hand.
Employee was holding bolt head to enable tightening of lock nut. When the impact wrench was activated to tighten the nut, it caused the bolt to spin and crush employee's right middle finger between bolt head and crusher housing.
Temp worker was un-bridging rock from C4X front feeder. After they un-bridged the feeder and was standing to the side and observing the feeder, a rock rolled off the conveyor belt and hit temp worker in left leg. The temp did not seek medical attention and finished shift. The next morning 11/8/2022, temp went to dr. and returned to work on light duty.
EE was cutting a tac weld with grinder that had a cutting wheel. The grinder kicked back and cut EE's finger. EE was wearing gloves
He is a welder helper after he had gotten off work at 2:30 a.m. he went home and then around 5:00 a.m. his eyes started hurting. He had gotten a flash burn. He received a prescription for his eyes
He was grinding inside the main frame liners in the simmon crusher when a piece of metal got in his right eye. He was wearing all of his PPE, safety glasses, and face shield. He also washed his eye out several times thinking that he had gotten. On Friday March 1st is when he when to the doctor.
Was backing the take up off with 3/4" impact wrench when the bolt broke causing the impact to smash his fingers against a piece of metal on the frame.
Was cutting metal when a fireball fell on his blue jeans over his left knee causing a burn on his knee. Didn't go to the doctor till 7/30/08.
