Employee states EE was welding on 3/27/2026 when a spark went under and in welding jacket. Employee states EE brushed it off and went about EE's normal day. EE reported that EE's injury may be infected on 3/30/2026 and was taken to a clinic for evaluation and prescribed antibiotic cream. No work restrictions were given to employee.

Employee sustained a laceration to the left ring finger while attempting to remove a grinding-aid lance from #3 Finish Mill. Employee was taken to the ER for further evaluation and received 4 stitches.

Production tech was removing a slurry hose for cleaning, EE had shut off all valves as well as open the pressure relief valve to get rid of excess pressure. When removing the camlock there was still residual pressure and caused slurry to spray on EE's face, chest and right wrist.

Two employees were doing dry wall hole repairs. Employees came across an abandoned copper water pipe. One of the employees used a Sawzall to cut the pipe while the other held it in place. While cutting the blade slipped and lacerated three fingers on employees left hand holding the pipe. The employee's pinky finger had the tendon cut and cut into the bone.

Employee was helping to remove an aqua ammonia (19% ammonia) spray lance. The tip of the spray was plugged up and pressure built in the closed, locked out line. When the connection was broke loose the residual line material blew out into the area contacting EE's head and EE inhaled some vapor causing irritation. Employee was monitored and prescribed an inhaler for the irritation.

EE was working with a jack hammer and was removing material with a wheelbarrow (with the assistance of other EEs). EE left the shift fine, woke up the next day with back soreness. Was treated initially and told to tend with self-care. On 3/22 went to see a doctor, doctor diagnosed with a potential bulging disc. Eventually was prescribed, prescription grade medication on 3/27.

An experienced carpenter of 15 years was hired to build forms for concrete. While cutting stakes the IW encountered a knot. Holding the Makita circular saw, in left hand, the saw bound up and kicked back, striking IW's right thumb. IW was taken to the ER, treated and released. On 3/16/24 IW received stitches.

Foreign object in EE eye while air lancing fly ash tank. Debris flushed out, but employee received antibiotic as preventative care.

A cable on an air tugger was being used to turn the mill, the eye bolt broke that was connected to the mill and the cable struck EE in the hard hat.

Our employee fell in a railcar EE was cleaning and as EE fell the pressure washer gun (who'd trigger was taped) slipped out of EE's hands and the barrel swung around and caused a water cut to EE's left lower bicep requiring 10 stitched and antibiotics for potential infection

Employee was cutting wear plate when it fell off the cutting table and struck the employee in the right foot causing a potential break, a prescription was issued by the doctor

On 2/22/22 the employee was using an air lance to clean a chute. In the process, EE hit a frozen chunk of material that was hidden behind non-frozen build-up. This caused a strain in EE's right shoulder from the jamming of the lance into the build-up. On 3/2/22 EE was placed on restricted duty for lifting capacity.

The employee was performing inspection functions of the job around the clinker cooler when cooler dust exited from a pinhole in the cooler sidewall. The dust got behind EE's safety glasses and into the left eye. The employee was treated for an abrasion to the left cornea and prescribed preventative antibiotic drops. No vision loss or permanent damage occurred from this incident.

EE was lowering mill liners into the mill to stage the liners for installation. While lowering the mill liner into the compartment, the liner became bound while passing through the opening in the mill. The EE began to pull on the liner to free it and allow it to lower through the opening. EE then felt pain in the muscle near EE's right shoulder.

As a production laborer was drilling a cardox port, the drill bit became stuck. As EE pulled back to get the bit loose, EE's weight shifted backwards and EE's thumb collided with a steel plate. The result from x-ray was closed fracture of the distal phalanx, thumb.

EE was using 1/2" impact equipped with a swivel adapter and 1 inch socket. While the socket was not seated on a bolt EE pulled the trigger. The socket came off hitting EE in the front center incisor, cracking it. A medical assessment was conducted on 3/5. Tooth repair did not occur until 5/17.

Injured employee was welding in the shop and in the field on what we believe is the day of the incident. EE does not recall being flashed or having issues with welding protective eye wear. That night, EE was awakened by a burning sensation in EE's eyes. EE went to the doctor and was diagnosed with ocular welder's flash. Prescription medication provided.

Employee was cleaning gunite machine. Placed hand inside pot and accidently bumped air valve. Feed wheel pinched tip of the ring finger on right hand.

EE was removing a liner bolt with an impact drill, when the drill slipped EE grabbed the drill by the socket, the split pin holding the head in place on the socket backed out 1/32" catching EE's glove & twisting EE's hand, breaking the sesamoid bone in the left thumb. EE went to the doctor on 12-30-19 where it was determined EE had a broken bone.

EE was using a wire wheel on a grinder. It did have a guard and the trigger lock is not present. EE claims that the wheel caught and torqued toward them. It made contact with their right leg. EE was taken to the hospital and given stitches. No restrictions or light duty.

Employee was cleaning mud from a tower using a water lance. They stepped backwards and slipped on material they just cleaned from the immediate work area. Employee was seen by a Dr. on 2/4/19 and placed on restricted duty.

Employee was using a pulley and winch snatch block to lower thimbles into a hole. When pulling the pulley/winch snatch block back up, EE grabbed the wire and it caught EE's left index finger and cut EE's finger below the finger nail.

Employee was preparing to grab a sample of kiln feed on the Pre-Heat Tower when an air cannon fired and blew dust out of the sample port and into the employee's right eye.

The injured employee was attempting to drill out a plugged purge line with a large hand drill. The bit caught in the hole and cause the drill motor to spin around and strike the employee in the right cheek. On Monday 02/19/2018 the employee experienced increased swelling and sought medical attention where they treated employee for infection.

Employee was performing an overhead patch repair on the kiln eyebrow. EE was tamping plastic using a hand held pneumatic pounder. The air exhaust from the tamper blew material behind safety glasses into right eye.

Employee was removing table liners from the coal mill. One of the liners had a strap around it so that it could be lifted with the hoist. The employee was taking up slack from the choker when the liner tilted and fell over, striking the employee's leg.

The employee was attempting to roll a piece of 12"x24" 13 gauge sheet metal in a sheet metal roll bender. The employees hand was positioned too close to the rolls which hand went through. This resulted in thumb being degloved and index finger broken.

Employee was lining up a punch and die set in a Piranha Ironworker. In doing so, the set up was not aligned and the punch came into contact with the inner edge of the die. As the employee bumped the machine to recheck alignment the pressure of the punch and die contact caused a small sliver of metal to break off and strike the employee's left cheek.

When chipping refractory wall using chipping gun, a small piece of refractor/slag fell hitting miner on tip of thumb.

Due to build up on bottom of vessel opening with limited access to this area men began cleaning from below bottom opening. Material buildup on vessel broke loose during operation and struck men causing bodily injury.

EE was working on an auto sampler. Air was turned off and the electrical equipment was turned off. Residual air pressure in the lines allowed the unit to cycle when the EE freed the blockage with his finger. The EE lost about 1/4 inch of his left index finger including some bone. Stitches to close the wound

EE was cleaning out a chute when the material broke free and forced the jackhammer into his neck. EE sustained a laceration to right side of neck that required stitches.

The injured employee was tasked to drill holes through the ACL Kiln pelletizer pan so new liners could be installed. Apparently, and unknown at the time, a piece of material got in his left eye. After work, he sought medical attention for eye pain and his eye doctor removed a foreign body and eye drops were prescribed. Incident reported/investigation began 04/21/15.

While machining parts the employee reached with his gloved hand to swipe away metal chips when the cutting head pulled his hand into the mill severing his right middle finger.

Employee guiding mixer unit slips down into tankers & agitates the load enabling pump out. Injury to right index, middle, ring & pointer finger. Smash with no broken bones. Faulty equipment design w/ no identified hand placement area & a floating cap on mixer is guided by 3 flat pieces of metal. Cap side loaded & entered into the tanker it jarred falling on employee's rt hand.

EE was grinding pipe when metal shavings blew under face shield and safety glasses into eye. Eye flush was done on site, but later taken to urgent care for removal of shavings. Initially the Dr. note was unclear on First Aid or Medical Treatment. Confirmation was given on 11/04/2014 that the procedure was Medical Treatment.

Employee was attempting to remove the bucket tooth on the 988H Cat loader with an air hammer. The air chisel bit on the air hammer broke off, causing the hammer to slip and pinned his left middle finger between the air hammer and the loader tooth.

Failure to control equ/roller

An employee was cutting metal with a small hand grinder with a cutting wheel attached. The grinder "kicked back" causing a three inch laceration to the top part of his middle thigh. The employee required stitches to close the wound.

Employee was rebricking inside the kiln and using a 4" portable grinder to smooth the metal surface on 12/10/2013. Safety glasses were worn at all time while in the kiln and safety glasses and a face shield were worn while grinding. Employee reported on 12/11 his felt material on his eye. Treatment was sought the following day.

While welding, the injured employee was chipping slag from the weld when a small piece of metal from the chipping hammer, about 1/16" long, came off and imbedded about 1 inch below the employee's left eye. The employee was wearing safety glasses. The employee was taken to the hospital to have the piece removed which required two stitches.

Employee was reaming a hole using an electric end grinder. The wind blew a foreign object into his left eye. Employee was wearing safety glasses and a full face shield. After flushing the eye with eyewash, employee stated that his eye was still irritated. Employee was taken to Care Now clinic. No foreign object was discovered in his eye, but the eye was slightly scratched.

Employee was welding and his eye felt irritated at the end of his shift. The irritation didn't go away so several days later he sought medical attention. A foreign body was found in his right eye.

EE was cutting a 2x4 and saw got in a bind and jumped back cutting his left thumb.

Employee was welding with improperly fitting welding hood. Employee noticed eye pain consistent with "flash burn". He was treated with antibiotics to prevent infection on 7/11/13

The operator & 2 helpers were attaching the jib to the boom of a 90 ton hydraulic crane. The operator removed the pin on the side of the boom before he installed the 2 pins that hold the jib in place. This caused the entire jib to drop to the ground. One side of the jib struck the operator's hard hat as it fell to the concrete paving.
