After tightening a casing connection, the employee put EE's hands in the derrick before the pipe settled, pinching EE's thumb between the string and the table. Resulting in a half inch laceration.

Employee was operating a haul truck and was jarred while being loaded by a shovel.

Employee's hand made contact with a sharp spinning bolt resulting in a laceration.

Individual went to tighten the pipe collar with an impact. This was when the right index finger got caught between the impact socket and the table bushing bolt. Resulting in finger laceration.

An employee's finger was pinched resulting in an avulsion that required a skin graft surgery.

Employee felt pain in their shoulder while using an impact wrench on 5/14/2021. On 7/9/2021, it was determined that the employee had a tear in the shoulder consistent with the 5/14/21 incident.

Employee was working out of a manlift to remove wear plate. Tension was taken out of the rigging and the crane motion stopped and then started back in motion. Employee immediately tried to move the man-lift to get away from the pick. The picking eye failed dropping the wear plate. Struck the employee in the side of the face causing two fractures that do not require surgery to fix.

Employee was conducting maintenance on a haul truck. While utilizing a torque gun on a wheel lug the employee pinched pinky finger between the torque gun and the wheel well. The pinch resulted in a laceration and fracture to the tip of the pinky finger.

An employee's hand was contacted causing a laceration.

While winching down the lifter got between rings on the mast ladder. Winched back up and hand was between winch line and rig mast.

An employee's hand was pinched causing the employees right index finger to be amputated at the middle phalange, the right middle finger to be lacerated and wrist to be fractured.

As an employee was removing GET, they were holding a bolt while loosening bolts with an impact drill. The bolt spun caused a laceration on the employees left middle finger. The employee was transported to Nye Regional Medical Center where they received 7 stitches.

Chipping of Nordbak material out of crusher when a piece of material fell off and pinched thumb between fallen material and air hammer that was being used to removed material.

While removing a differential from a 789C haul truck the crane was moved off center of load to swing differential out of axle housing. Differential got hung up and when it came free the differential swung farther than expected and hit EE's causing laceration and contusion to right leg below knee.

An employee was moving a wheel barrow full of floor dry and pinched their finger in between the handle and a metal bin causing a laceration. The employee was transported to the hospital where they received 10 sutures to close the laceration.

As an employee was turning on heater valves on a dozer, their hand slipped and their glove got caught in the pulley. The employee's right thumb was smashed in the spring next to the pulley. The employee was transported to Nye Regional Medical Center were it was determined that their thumb was broken and they received 6 stitches.

Worker was holding sign when Backhoe operator went to move Backhoe and back of Backhoe swung and hit his finger

Employee was air arcing on a mantle liner, when a piece of molten slag burned through his pant leg and fell into his boot. The slag caused 2nd and 3rd Degree burns to his ankle before he could get his boot off.

Employee was drilling holes with a mag drill and power failed which released magnet and caused the drill to drop hyper extending the thumb.

Employee was grinding on a table, grinder slipped, caught in pants, cut through his pants and underwear. Resulting in torn epidermis x 2 on shaft of penis, pinch on eccymosis across glanis and no break in skin.

As an employee was using a spring attached to a hand held drill to clean out the valve stem on a haul truck tire, the spring broke. The spring contracted and became embedded in the employee's left hand little finger. The employee was transported to Nye Regional Medical Center where it was determined that the employee's finger was fractured and the employee received two stitches.

As an employee was cutting a piece of steel out of a crusher, the steel shifted and contacted their right leg causing a laceration. He was transported to the hospital where they received eight stitches and seven staples.

Employee was cutting through a 12" SDR 11 pipe that was being supported with a backhoe bucket. When the employee had cut apprx. 3/4 of the way through the pipe it broke and struck the employee in the face. The employee suffered a lacerated and broken nose.

As the employee was operating a rubber tire dozer, they began feeling pain in their neck and back. The employee was seen at the hospital and released to full duty. At a follow up appointment on 11/26/08 the employee's status was changed. Employee was ordered off of work until 12/26/08.

While using a Drisco fusing machine, the operator caught his gloved hand on a sharp section of pipe.

Employee was detaching liner from liner handler, balancing himself by placing his hand on handler boom. The handler's operator retracted the boom before employee could remove his hand, catching employees thumb in a pinch point on the boom.

The employee was preparing a tank for liner by grinding the loose material off the tank.

ROLLING BERM, ROLLER WAS ON TOP OF SLOPE ON LOOSE GROUND. IT STARTED SLIDING SIDEWAYS AND ROLLED OVER ONTO ITS SIDE.

Employee was smelting mill concentrates and a piece of slag popped out of the melt and landed on the ee's arm burning through his coveralls.

THE OPERATOR OF A IR-DML PIT BLAST HOLE DRILL RIB WAS POSITIONING A CHAIN WRENCH TO LOOSEN A STEEL TOOL. THE OPERATOR SIGNALED THE HELPER TO MOVE THE CHAIN WRENCH CYLINDER. WHEN THE CYLINDER R ETRACTED THE OPERATORS FINGER WAS CAUGHT IN A PINCH POINT.

WHILE REMOVING A NUT FROM A WEARPLATE, THE STOP-NUT SLIPPED & THE MECHANIC PLACED HIS FINGER AGAINST THE SPINNING BOLT.

THE EE WAS TAKING A D-5 DOZER FROM THE TOP OF A REUSEABLE LEACH PAD TO THE BOTTOM OF THE PAD TO BE FUELED. THE EE DROVE THE DOZER OVER THE BERM WITH THE BLADE IN THE RAISED POSITION. THE DOZER ROLLED 1 CIRCLE END OVER END COMING TO REST ON ITS TRACKS.

WHILE EE WAS REMOVING WARE PLATES INSIDE A SHOVEL BUCKET. A PIECE OF HOT AIR ARCH ROD BROKE OFF AND BURNED THROUGH THE EE S COVERALLS.

EE WAS HOLDING THE CABLE UP FOR THE LINER HANDLER OPERATOR TO POSITION THE HOOK TO LIFT THE LINER. HE PUT HIS RT HAND ON THE BOOM TO SUPPORT HIMSELF UNAWARE OF THE PINCH POINT. HIS HAND WAS CA UGHT IN BETWEEN THE BOOM SECTIONS AS THE BOOM WAS BEING POSITIONED.

EE WAS CUTTING/GRINDING DUST COVERS AND STOPPED TO WIPE THE SWEAT WITH HIS GLOVED HAND & FELT A PIECE OF METAL IN HIS RT. EYE.

SHOVEL OPERATOR WAS LOADING A TRUCK. THE SHOVEL WAS ON UNEVEN TERRIAN AND AS SHE SWUNG THE SHOVEL, IT DIPPED SO THE OPERATOR MOVED THE CONTROLS IN A RAPID MOTION TO STOP THE MACHINE. THIS CAUS ED HER TO SPRAIN MUSCLES IN HER WRIST.

EMPLOYEE WAS ATTEMPTING TO REMOVE A HEAVY FUSING MACHINE WHICH HAD WHEELS FROM A TRAILER. THE MACHINE WAS HOOKED UP TO A WINCH TO ASSIST IN REMOVING THE MACHINE FROM THE TRAILOR. THERE WAS STO CK IN THE WINCH CABLE AND WHEN MACHINE ROLLED OFF TRAILER, THE CABLE TIGHTENED UP. THIS ACTION CAUSED THE EMPLOYEE'S FINGER TO BE CAUGHT AND LACERATED.

THIS ACCIDENT TOOK PLACE WHILE BLDING A HYDAULICHOSE. ARDMORE FOR THE HOSE WAS BEING CUT TO LENGTH USING AN AIR POWERED DIE GRINDER. THE GRINDERSLIPPED & WHILE TRYING TO CATCH THE GRINDER, IT CAME IN CONTACT WITH THE THUMB OF THE RIGHT HANDRESULTING IN A LACERATION. THE LACERATION REQUIRED 5 STITCHES Y

THE EMPLOYEE WAS OPERATING A HAUL TRUCK, BEING LOADED BY A SHOVEL WHEN THE TRUCK MOVED SIDEWAYS SUDDENLY CAUSING THE EMPLOYEE TO JAR HER SHOULDER. A ROCK ROLLED OUT OF THE SHOVEL BUCKET STRIKI NG THE REAR CORNER OF THE TRUCK BOX CAUSING IT TO MOVE SUDDENLY.

THE ASSAY LAB EE STATED SHE WAS OPERATING THE PULVERIZER IN THE LAB WSHEN SHE BEGAN TO FEEL EYE IRRITATION. THE EE WAS TREATED WITH EYEWASH BY EMT'S THE DAY OF THE INJURY, THEN WENT ON DAYS OF F. THE EE RECEIVED FURTHER MNEDICAL ATTENTION ONHER DAYS OFF AND WAS NOT ABLE TO RETURN TO WORK ON 7-2-99.

WHILE USING AN OCY/ACETYLENE TORCH TO CUT HOLES IN STEEL PLATE, THE EE REACHED TO PICK UP A GRINDER WITH HIS RIGHT HAND WHILE HOLDING THE LIGHTED TORCH IN HE LEFT HAND. AS HE REACHED FOR THE G RINDER, THE EE LOST HIS GRIP ON THE TORCH IN HIS LEFT HAND. IT ROLLED IN HIS LEFT HAND CAUSING THE TORCH FLAME TO FLASH ACROSS THE PALM OF HIS RIGHT HAND.

WHILE ASSISTING OTHER EE'S CHANGING CUTTING EDGES ON A GRADER BLADE, THE EE PINCHED THE END OF HIS FINGER BETWEEN A CUTTING EDGE BOLT HOLD AND THE CUTTING EDGE. THE EE WAS USING AN IMPACK WREN CH TO TIGHTEN THE BOLTS AT THE TIME OF THE INJURY

THE EMPLOYEE WAS CROUCHED INSIDE THE BED OF A 785 HAUL TRUCK AIR ARCING OUT LINER PLATES. WHEN THE EMPLOYEE STOOD UP HE FELT PAIN IN HIS LOWER BACK.

EE WAS ATTEMPTING TO REPAIR A HEATER SWITCH. HE STUCK HIS FINGER THROUGH A VENT HOLE TO FEEL IF SWITCH WAS TIGHT & ACTIVATED THE FAN SWITCH WITH HIS THUMB.

WHILE CUTTING DRILL CORE, THE EMPLOYEE TURNED THE CORE SAMPLER TO GET A BETTER CUT AND SAW. BLADE NICKED EMPLOYEES FINGER. STITCHES WERE REQUIRED.

EE WAS INSTALLING SIDING PANELS & HIS DRILL GOT TO CLOSE TO A BANDING STRAP. THE STRAP GOT HUNG UP IN THE DRILL & CUT HIS FINGER. FINGER REQUIRED SEVERAL STITCHES.

WHILE REMOVING FINAL DRIVE FROM 214 PIT DRILL. FINAL DRIVE DROPPED PINNING LEFT THUMB BETWEEN FINALDRIVE AND DRIVER PIN.

EMPLOYEE WAS CUTTING ON GALVANIZED STEEL (2'X2") WHEN IT FELL AND HIT HIM ON THE LOWER LEFT SIDE OF HIS LEG. EMPLOYEE WAS TREATED AND RELEASED. THE INJURY BECAME INFECTED AND THE EMPLOYEE WA S TAKEN TO THE HOSPITAL ON 2-27. THE WOUND WAS CUT OPEN TO CLEAN. EMPLOYEE WAS STAYED IN THE HOPITAL OVERNIGHT.

SEMITRAILER USED AS TOOL ROOM AND FIELD OFFICE. CONTAINED HEATER. HEATER STARTED LATE AFTERNOON AND THEMOSTAT SET. 0700 EES ENTERED TRAILER TO BE GIVEN JOB ASSIGNMENTS FOR THE DAY. HEATER APPE ARED TO BE FUNCTIONING WELL. AFTER BEING IN THE TRAILER FOR TIMES OF 10-40 MINUTES EES; BEGAN TO FEEL ILL, NEAUSE, HEADACHE, PALPITATION.

HEATER APPEARED TO BE FUNCTIONING WELL. AFTER IN THE TRAILER FOR TIMES OF 10-40- MINUTES EES BEGAN TO FELL ILL, NAUSEA, HEADACHE, PALPITATIONS.
