Employee coming downstairs of a haul truck, their knee popped resulting in 2 tears that require surgery.

Employee was exiting a Bobcat Skidsteer loader and rolled their left ankle.

An employee was working on an Auto Valve, when the handle was pulled, it came loose and the employee lost their balance causing them to fall. Resulting in a fracture on fibular head.

While unbolting a flange, a faulty bolt with threads coming off contacted the right index finger, resulting in 13 stitches.

Employee was operating a haul truck and was jarred while being loaded by a shovel.

Employee was lifting an equipment part out of a storage bin and felt a pull in their lower back.

Employee felt a pop in their hip while picking up a paint bucket on 2/23 and then again while changing filters on 2/24 that resulted in additional back pain.

Employee was putting a wheel chock into a chock holder when they smashed their finger resulting in a fracture.

Employee was attempting to seat a clamp and struck right thumb with brass hammer. Employee has a fracture in thumb.

Employee was checking fire extinguishers and stepped on a rock rolling EE's right ankle.

Employee was performing work with a sledgehammer on 1/25/25. Due to increasing pain, medical attention was requested on 1/27/25 and the employee was taken off of work.

Employee had a pipe wrench up to chest while working on a pipe and was trying to use body weight for leverage when employee felt a pop in ribs. On 1/13/25, the employee was taken off of work after further medical evaluation.

A haul truck operator was jarred, while being loaded by a loader, when the loader bucket made contact with the bed of the haul truck. On 1/16/2025, the employee was taken off of work due to continuous pain.

Employee was entering mine site for start of shift and felt something hit their eye. Prescription medication was later given to prevent any infection due to evidence of a foreign object that had been in the eye.

Contributing factor are trip hazard, multiple people in station.

When moving the rod holder jaw on a drill, the employee pinched their finger resulting in a partial finger amputation that required sutures.

Employee was racking out a substation and grabbed a live stab resulting in an electrical shock and burns.

Employee felt a pop in their chest while lifting a pipe to screw in spin locks.

Employee was exposed to nitrogen dioxide on 5/15/24. On 6/6/24, it was determined that the employee was experiencing respiratory related symptoms due to this incident and additional medical treatment was needed.

Empire Employee twisted knee after it was caught in between ice chest and other items in the back of service truck.

On 1/8/2024 employee was assisting with lifting an oven onto a cart when they felt pain in their shoulder. It was later determined that surgery was needed and was completed on 6/13/2024.

Employee was walking down a 6060 cat shovel with a small box. EE fell approximately 7 steps to the ground. This resulted in the employee sustaining some scratches to their stomach, knees, and face. Along with breaking their jaw.

Employee's hand made contact with a sharp spinning bolt resulting in a laceration.

Employee's hand made contact when wrench slipped off bolt causing a laceration.

Individual went to tighten the pipe collar with an impact. This was when the right index finger got caught between the impact socket and the table bushing bolt. Resulting in finger laceration.

Employee slipped and fell due to snow on the ground resulting in a distal fibula fracture.

Employee was walking when they stepped wrong and twisted their knee on 11/28/2022. On 12/21/2022, it was determined that the employee needed surgery due to a torn meniscus.

Employee was using a hammer and made contact with their hand resulting in a laceration.

Employee lost their balance and fell forward resulting in a laceration on their forehead.

Employee was installing pump drive. Lining up in proper position. While moving to desired position employee had right pinky finger between pump drive and housing. The pinch resulted in a laceration requiring 2 stitches.

Employee's hand was pinched between the top edge of a distributor and dart causing a laceration and fracture.

EE was finishing blowing off load of Cyanide into ADR tank. Went to back of truck and Cyanide Crystals fell from top of Storage tank and hit EE, the trailer, and the ground. The wind was blowing toward back of tanker. EE was not sure if EE had inhaled any or not but had a co-worker watch EE disconnect then go get the Supervisor. EE then called mayday.

Employee received a laceration that required two stitches when a rubber tire dozer and scraper made contact.

Employee's foot was smashed when raising the shovel ladder.

An employee's finger was pinched resulting in an avulsion that required a skin graft surgery.

An employee's finger was contacted causing a fracture.

Employee was installing a bushing in a haul cylinder and struck left hand with a hammer. The injury resulted in a non-displaced fracture of the index finger. Employee has been released to "work to comfort level" and is performing light duty and computer work.

An employee smashed their left hand index finger.

An employee attempted to move a large rock and felt pain in their bicep on 6/25/2021. On 7/19/2021 it was determined that the employee had a bicep rupture and surgery was needed.

Employee felt pain in their shoulder while using an impact wrench on 5/14/2021. On 7/9/2021, it was determined that the employee had a tear in the shoulder consistent with the 5/14/21 incident.

Employee smashed their thumb in the hinge side of the cab door.

Employee was pulling PVC casing off the truck at the end of shift. While the casing was being removed, it started to fall. The employee reached to catch the falling casing and caught finger between the casing and a pipe wrench. It was at this time EE pinched finger between wrench and casing.

As an employee was walking they slipped and fell on their shoulder due to ice on the ground. On 1/2/2021 it was determined that the employee had a fracture of the humeral head.

Employee cut hand.

An employee was stepping out of a blue room and twisted their knee. On 1/6/2021 the employee was given work restrictions due to a torn meniscus.

Driller's helper attempting to remove cap from drill pipe. Pipe shifted and pinched helper's pinky finger causing a laceration requiring seven sutures.

Employee was working out of a manlift to remove wear plate. Tension was taken out of the rigging and the crane motion stopped and then started back in motion. Employee immediately tried to move the man-lift to get away from the pick. The picking eye failed dropping the wear plate. Struck the employee in the side of the face causing two fractures that do not require surgery to fix.

Employee felt pain in their knee when they were walking down a slope and stepped onto a treaded liner on 7/24/2020. Employee felt reoccurring pain and on 1/27/2021 it was determined that the employee had a torn meniscus consistent with the 7/24/2020 incident that needed medical treatment.

Employee was conducting maintenance on a haul truck. While utilizing a torque gun on a wheel lug the employee pinched pinky finger between the torque gun and the wheel well. The pinch resulted in a laceration and fracture to the tip of the pinky finger.

An employee's hand was contacted causing a laceration.

An employee stepped out of a truck and twisted their ankle.

An employee fell and contacted the ground causing pain in their back.

As an employee was walking backwards they tripped and fell landing on their arm. It was determined that a tendon was torn. On 2/3/2020, the employee had surgery to repair the tendon.

The employee had finger pinched between a steel bar and valve handle causing a fracture.

The employee's left hand middle finger and ring finger were pinched causing a fracture to both.

The employee was carrying a loop of wire weighing approximately 50lbs to the work area when they felt a "pop" in their back and some pain.

The employee's right thumb was pinched resulting in a partial surgical amputation. (distal phalange)

Employee slipped on piece of ice laying on concrete pad and severely sprained EE's right ankle, requiring surgery.

Employee felt pain in their shoulder when they slipped and fell while stepping across a ditch on 1/2/2019. Employee felt reoccurring pain and on 1/27/2021 it was determined that the employee had a rotator cuff tear consistent with the 1/2/2019 incident.

As an employee was moving a bottle jack, they felt pain in their arm. It was determined that a tendon was torn. On 11/26/18, the employee had surgery to repair the tendon.

While winching down the lifter got between rings on the mast ladder. Winched back up and hand was between winch line and rig mast.

An employee was walking down the slide slope of a leach pad when they tripped and fell, twisting their leg during the fall.

While removing 9 5/8' bottom hole tooling support wrench the driller started raising wrench while helpers were in position to assist the wrench off of tooling to deck. The wrench tipped and helper two removed EE's hands as wrench started tipping in casing. As wrench tipped helper one's left ring finger was caught between wrench handle and casing.

Employee felt pain in their hip when their haul truck bed was contacted by a loader bucket while being on loaded on 2/1/2018. Due to a reoccurring pain the employee was seen on 7/23/2020 and it was determined that they needed to work on restricted duty.

An employee stepped up to access a screen deck and felt a pop and then pain in their left knee.

An employee's right thumb was pinched in between a valve and pipe. The employee was transported to hospital and an orthopedic clinic and was released back to work.

An employee was opening a parts container in the Cashman warehouse. Employee was using a razor knife to break a zip tie on the container. They pulled the knife toward themselves and the container slipped, causing the knife to lacerate their wrist between the coat cuff and cut resistant glove cuff. The laceration required 3 sutures to close.

Employee was injected with grease in both hands requiring surgical debridement.

An employee stepped off of a forklift and twisted their left ankle. Due to continuing pain, the employee was seen on 7/5/17 and was given work restrictions.

An employee's right foot was pinched in between a diverter plate and the grating. The employee was transported to Banner Churchill Hospital and was released back to work. Due to continuing pain, the employee was seen on 7/12/17 and it was determined that they needed to work on restricted duty.

Employee pinched their right index finger causing a laceration.

An employees hand was contacted causing a fracture.

The employee's right hand middle finger and ring finger were pinched causing a fracture to both and a laceration to the middle finger.

While walking at a Q-point, an employee tripped and fell. Original x-ray results were negative. Due to the employee continuing to experience pain additional scans were taken. On 6/10/2016, it was determined that 2 left side ribs were fractured.

Employee felt a pop in their shoulder on 3/22/2016 and later noticed a deformity with their arm. It was determined that EE had a rotator cuff and biceps tear but surgery was not needed at that time. Due to continuous loss of strength in that arm, the employee requested surgery which occurred on 5/18/2020.

An employee's hand was pinched causing the employees right index finger to be amputated at the middle phalange, the right middle finger to be lacerated and wrist to be fractured.

An employee smashed their right hand middle finger.

Operator was cleaning a screen chute when he caught his picky finger in a pinch point fracturing it.

Walking outside the Truck Shop and slipped on a slick area, he did not fall but twisted his knee.

An employee stepped wrong and twisted their knee. The employee was released to full duty until 4-13-15 when they underwent surgery for their knee injury.

EE working on conveyor system at ground level had foreign debris blown into his left eye by a strong gust of wind, resulting in a scratched cornea.

Employee was inside of a feeder to replace the head pulley. His FLRA was completed, SOP was followed, and he was using fall restraint. After the pin was removed the rigging failed causing the flights within the feeder to slide downward. The employee fell and was caught by his fall restraint but in the process something hit his leg causing his injury.

Employees noticed a fire on the screen deck of the Mill. The Mill was evacuated and code blue was initiated. The Round Mountain Volunteer Fire Department responded to extinguish the fire. The cause of the fire is still under investigation.

A wedge (block of rocks) came out of the wall along side of the haul road and struck Loader 377 while it was being used for high wall cleanup

An employee was carrying and spreading two 25 pound bags of floor dry. The employee drove to the shop and was unable to move. The employee was transported to Nye Regional Medical Center for observation. On 09-03-2014, MRI results showed a spinal injury and the employee was taken off of work.

As an employee was removing GET, they were holding a bolt while loosening bolts with an impact drill. The bolt spun caused a laceration on the employees left middle finger. The employee was transported to Nye Regional Medical Center where they received 7 stitches.

Chipping of Nordbak material out of crusher when a piece of material fell off and pinched thumb between fallen material and air hammer that was being used to removed material.

An employee was cleaning out carbon tanks and his feet got wet. The employee was seen at the RMGC Clinic and was released back to work in a dry environment.

A technician was pushing a 90 degree glass tube into a rubber stopper. The glass tube broke, puncturing the technicians palm.

After removing a chain wrench from the drill, the operator stepped backwards, falling between the cab and the deck of the drill (a space approx. 4.5 inches wide). His right lower leg was cut on a bolt sticking out of the deck.

Employee was unscrewing a blind flange end cover to remove it, when it fell on his thumb, smashing it.

An employee reported lime in the cab of a haul truck. The employee was driving the truck to the shop and began feeling burning in their lungs. The employee was transported to the RMGC Clinic and was released to full duty.

As an employee was cutting zip-ties off of PVC pipe, the knife slipped, puncturing the employee's left hand. The employee was transported to Nye Regional Medical Center where they received 1 staple. The employee was released to full duty.

An employee inhaled lime dust in the cab of a haul truck. The employee was transported to Nye Regional Medical Center and was released to full duty.

An employee was walking down a ramp and twisted their knee. The employee was transported to the hospital where they were released to restricted duty.

While removing a differential from a 789C haul truck the crane was moved off center of load to swing differential out of axle housing. Differential got hung up and when it came free the differential swung farther than expected and hit EE's causing laceration and contusion to right leg below knee.

As an employee was walking down liner, they slipped and fell causing pain in their right knee. The employee was transported to the hospital where it was determined that they have a sprain. The employee was released to restricted duty with the use of crutches.

An employee was moving a wheel barrow full of floor dry and pinched their finger in between the handle and a metal bin causing a laceration. The employee was transported to the hospital where they received 10 sutures to close the laceration.

As an employee was working on the S Dedicated pad, a piece of 8 inch pipe contacted her in the leg causing a laceration. The employee was transported to Nye Regional Medical Center where she received 1 suture to close the laceration.

As an employee was exiting the Admin Building, they tripped and fell. The employee felt pain in their left wrist. On 8-26-13 it was determined the the employee's wrist was fracured.

As an employee was walking down the Gold Hill heap side-slope, she lost her footing. In an attempt to break her fall, she extended her arm and landed on her out-stretched right hand. The employee was transported to Nye Regional Medical Center where it was determined that her right wrist was broken.

An employee was on the deck of a drill and walked into a stake that was sticking out of the tool box. The stake punctured their right leg. The employee was transported to Nye Regional Medical Center where they received 5 sutures. The employee was released to full duty.

Employee was breaking fittings loose on bottom winch line on deck with a 1 1/4" wrench to repair a hydraulic leak when wrench slipped and hit him in the face chipping two front teeth.

An employee was preparing to install cutting edges (Ground Engagement Tools) on a motor grader, when his right-hand middle finger was caught between two cutting edges. The employee was transported to Nye Regional Medical Center where it was determined that his finger tip was fractured and 4 sutures were required to treat the laceration.

An employee was lifting pipe and felt a pop in his left sholder. The employee was transported to the RMGC Clinic and was released on restricted duty.

As an employee was turning on heater valves on a dozer, their hand slipped and their glove got caught in the pulley. The employee's right thumb was smashed in the spring next to the pulley. The employee was transported to Nye Regional Medical Center were it was determined that their thumb was broken and they received 6 stitches.

An employee was performing clean up at the Primary Crusher apron stairs. When they reported to work on 08-15-2012, they were complaining of back pain. The employee was transported to Nye Regional Medical Center and was released to restricted duty.

While removing concaves (metal liners-1500 lbs each) in the primary crusher bowl, an employee was on the wall of the crusher attempting to loosen a concave. As the concave fell, it rotated out of position contacting the employee's right foot. The employee was transported to Nye Regional Medical Center where it was determined that the employees foot was fractured.

An employee was climbing into a rubber tire dozer and felt a pop in his left shoulder .The employee was transported to Nye Regional Medical Center and was released with temporary restrictions.

Three employees were carrying a barrel handler that weighed over 300 pounds. One employee was walking backwards & tripped over a rock. The employee fell down & the barrel handler fell on top of him. The employee's left leg was under his body, & he began feeling pain in his ankle. The employee was transported to NRMC where it was determined that his ankle was fractured.

The employee pinched the tip of her finger under a material handling magnet while putting it in storage. Employee was wearing all required PPE for the task including safety glasses, gloves & hard hat.

While changing out a belt on the drill rig, helper stepped down off the track of the rig to the ground in doing so he stepped onto the six inch discharge hose causing his ankle to roll. After being examined at the ER it was determined his ankle to be Strained.

An employee in a haul truck was being loaded by a loader. It is believed that the loader tire contacted the haul truck tire causing the employee to be jolted in the cab. This caused the employee to chip their tooth. The employee was transported to the dentist, where it was determined that the chipped tooth would require repair.

An employee was stripping PVC wire insulation away from the inner conductor with a razor knife. The employee cut through their leather glove and lacerated their left palm and wrist. The employee was transported to the RMGC Clinic where they received 11 sutures. The employee was released to restricted duty.

While attempting to break core, employee put his hand on the water swivel and lowered the drill steel and caught his right index finger between the power head and water swivel. Employee sustained a dislocated and laceration injury.

Employee was tripping in and as the plug was being screwed into new rod the plug slipped out of thread, as plug was falling employee grabbed plug and finger was pinched between plug and head slide.

While changing out an equalizer bar on a D10R Track Dozer, an employee had his hand in a pinch point between the equalizer bar and Dozer frame while he was using a wrench. The employee's left pinky and ring finger were pinched between the frame and equalizer bar resulting in a crushing/amputation injury to these two fingers.

Employee was air arcing on a mantle liner, when a piece of molten slag burned through his pant leg and fell into his boot. The slag caused 2nd and 3rd Degree burns to his ankle before he could get his boot off.

An employee fell off of the base of a "candlestick" (used to bridge electrical cable over haul roads). (Base is approximately 36" above ground level. The employee received a laceration to the left side of their head requiring eight sutures.

A mechanic was installing a shank on a dozer ripper. The dozer operator which was standing nearby was struck by a piece of frag metal.

On April 4, 2011, an employee was performing normal job functions and began experiencing pain in the employee's groin area. The EE was transported to the RMGC Clinic and diagnosed with a possible hernia. The EE was released back to work and a follow-up scheduled with a specialist. On May 6, 2011 the employee had surgery to repair the hernia.

Employee was tripping in pipe and hand was caught pulling up and twisting arm. Injury occurred on May 3, 2011 but employee did not see a physician or receive medical treatment until May 31, 2011.

Employees were lowering a full core tube at a slow speed. Employee was holding the tube about to put it on the rod rack when the wire line sped up causing the employee to lose control of the core tube. While the employee was trying to control the tube he felt a pull in his left shoulder. On 03-24-11 employee stated his arm was hurting and received medical treatment.

As an employee was adjusting pipe in the fusing machine, the loader operator pushed the pipe further into the machine and pinched the employee's left hand middle finger. The EE was wearing gloves at the time of the incident. The EE received 6 stitches and their finger was fractured.

Employee strained shoulder while lifting 20' casing from rack and placing it onto truck.

An employee was opening a container of drill steel coupler anti-seize with a knife and the knife slipped causing a laceration to the employee's left inner thigh. The employee was transported to Nye Regional Medical Center where they received 3 staples. The employee was released to full duty.

An employee was working the C-5 conveyor, replacing a return idler. A rock fell from the belt and contacted the employee on the mouth, causing one tooth to be chipped and one tooth to be cracked.

A Cat 16H motor grader was blading the West Waste Dump area as a loaded Cat 789C haul truck approached to dump. Just prior to stopping forward motion, the haul truck contacted the motor graders front left wheel.

Employee was walking down warehouse stairs, where he blacked out and woke up on the concrete floor. Employee suffered a 1 inch laceration that took 5 staples.

EE was using an eight foot fiberglass ladder to install coax cable above the suspended ceiling in the office area of the RMG truck stop.When the EE came down the ladder and his left foot touched the floor his leg would not support his weight and he fell to the floor.

An employee was loading a 12 foot section of 6 inch rubber hose onto a flat bed truck. The employee's left thumb got pinched between the flat bed and the swivel end of the hose causing a laceration to their thumb. The employee was wearing gloves at the time of the incident. The employee was transported to Nye Regional Medical Center where they received stitches.

Employee was drilling holes with a mag drill and power failed which released magnet and caused the drill to drop hyper extending the thumb.

Employee was grinding on a table, grinder slipped, caught in pants, cut through his pants and underwear. Resulting in torn epidermis x 2 on shaft of penis, pinch on eccymosis across glanis and no break in skin.

An employee was removing a pump impellar by using the drive belts and a bar. The bar slipped and the employee's hand went into the sheave causing their little finger to be pinched in between the belt and sheave. The employee was transported to Nye Regional Medical Center where it was determined that their finger was fractured and they recieved 7 stitches.

As an employee was opening a Grieve Oven, their arm contacted the metal shelf behind them and the employee received an electrical shock. The employee was transported to Nye Regional Medical Center and released to full duty. After investigation of the oven, it was found that an element wire had come loose and contacted the frame.

A loader operator backed away from the dig face with a full bucket. When the operator moved forward, they articulated with bucket in the air. The loader began to rock side to side. While applying the brake, the operator went to put the bucket on the ground and the loader came over on its left side. The operator was wearing their seat belt at the time of the incident.

Employees were moving an eyewash station building with a fork lift. As the fork lift operator began lifting the building, an employee bent down to pick up an electrical cord. The building tipped over and pinned the employee against a concrete barrier. The employee was transported to the hospital, where it was determined that they had 2 pelvic bone fractures.

As an employee was flipping over a cutting edge, the cutting edge slipped causing the employee's fingers to be smashed between the floor and the cutting edge. The employee was transported to Nye Regional Medical center where it was determined that the tips of their right hand middle finger and ring finger were fractured.

As an employee was using a spring attached to a hand held drill to clean out the valve stem on a haul truck tire, the spring broke. The spring contracted and became embedded in the employee's left hand little finger. The employee was transported to Nye Regional Medical Center where it was determined that the employee's finger was fractured and the employee received two stitches.

As an employee was setting down the lid of a screw feeder, they smashed their left hand ring finger and little finger causing a laceration on both fingers. The employee was transported to the hospital where they received six stitches in their little finger.

As an employee was cutting a piece of steel out of a crusher, the steel shifted and contacted their right leg causing a laceration. He was transported to the hospital where they received eight stitches and seven staples.

While installing a lock ring on a haul truck, the employees finger was pinched between the lock ring and wheel.

On 5-25-2009 an employee was installing a lock ring on a tire. The employee felt a pop in their right shoulder. The employee was transported to the hospital and returned to work. The employee continued to have pain in their shoulder due to a torn bicep. The had surgery on their right shoulder on 10-30-2009.

An employee was using a Fluke T5-1000 meter to check for stray current/static electricity in the housing of a filter resistor bank. The employee placed one lead on the ground wire. When they placed the other lead against the bottom left corner of the housing, an arc occurred destroying the meter and causing burns to the employee's hands and wrists.

As an employee was installing a wheel group on a haul truck, the pry bar they were using slipped and contacted the employee in the face, causing a laceration. The employee was transported to Nye Regional Medical Center where they received stitches.

As an employee was bending down to adjust the collector feed pump, the discharge line broke at the union. When the line broke, MX-6205 was sprayed into the employee's eyes. The employee was wearing safety glasses at the time of the incident.

Employee was cutting through a 12" SDR 11 pipe that was being supported with a backhoe bucket. When the employee had cut apprx. 3/4 of the way through the pipe it broke and struck the employee in the face. The employee suffered a lacerated and broken nose.

Employee was walking down the face of a heap that was actively leached when he tripped over a large piece of rock and fell. As he fell, he attempted to catch himself entirely with his left hand. His left hand took the entire force of the fall and his forearm was broken just above the wrist. One of the two bones in the forearm broke.

Employee was putting away the knives in the sample prep room when one slipped and lacerated her left hand. Employee was seen at Nye Regional Medical Center where she received stitches to close the the laceration.

As the employee was operating a rubber tire dozer, they began feeling pain in their neck and back. The employee was seen at the hospital and released to full duty. At a follow up appointment on 11/26/08 the employee's status was changed. Employee was ordered off of work until 12/26/08.

Employee jumped from deck of truck and fell causing a shoulder separation.

As an employee was closing the lid of a pulverizer, the lid slipped out of her left hand and smashed her right hand little finger causing a fracture to the finger.

While working on the work platform of a boom truck the employee tripped and fell backwards striking his head, neck and back on the ground.

Employee was walking down the stairs of a rubber tire dozer and slipped landing on his right hand. Employee received a hair line fracture to his right hand.

Employee was climbing a ladder to hand another employee a 55 drum lid. He was approx 3 to 4 rungs up on the ladder when he reached to hand up the lid, the ladder then kicked out, causing him to fall off approx. 8 ft. The ladder was tied off on one side of the ladder.

As an employee was responding to a code blue, they lost their footing while going down the stairs. When the employee grabbed the handrails, the backpack that they had over their right shoulder, swung forward, causing their shoulder to dislocate.

Employee was climbing the ladder on a RTD, his foot slipped and he fell to the ground.

Employee climbed to the top of a berm to look at marking stakes at the toe of the dump. When employee turned to come down from the berm, a rock rolled under his foot, causing him to fall. When employee fell he struck his knee on a sharp rock, causing an abrasion and puncture wound.

Operator lacerated his right little finger while pulling on a rope used to hoist his lunch box from ground level to the cab of his haul truck.

Employee claims that while lifting a sample tray when experienced discomfort in her lower back. This employee had recently been released from short term disability with a lifting restriction due to an off the job medical condition.

While unloading boxes the employee caught his hand between a package and the door jam.

Employee was attempting to ignite a propane heater in a lunch room when the heater flashed.

The employee was exiting the mine bus to begin shift and lost her footing falling and striking her right side (The date of incident was 09/01/07 but this individual has been working with a full release. It was determined that it was a Reportable Incident on 11/12/07)

Operator was attempting to replace a return roller when the roller slipped from his grip and pinched his finger between the roller and bracket.

The employee was moving a mold bar off the table and a piece of slag that was on the table went through her leather glove and into her finger.

The employee was picking up a roll of paper towels that had been dropped during a pre-use inspection and slipped and fell. Attempting to catch himself, the employee landed on his wrist and fractured it.

While using a Drisco fusing machine, the operator caught his gloved hand on a sharp section of pipe.

An employee was tightening a drill bit when the mag drill fell causing a laceration to his right hand.

Employee was demobilizing from site when the employee sufered a heart attack. At the time of the event the employee had been assisting the forklift operator by guiding him into place. Employee died of natural causes due to heart disease.

The Employee was exiting a Haul Truck and hit her wrist on the steering wheel. She was taken to the Dr. and given a full duty release. Later a growth was removed that was determinded to be from the incident; minor surgery was done 02/12/07. This minor surgery WAS NOT REPORTED to the Loss Control Dept. until 03/13/07.

The employee is a blast-hole driller. He reached down to retrieve a blast-hole sample bag and felt a pop in his left shoulder. He was seen by a Dr. & received a full duty release the next day. During follow-up visits it was determinded surgery was needed. The surgury was performed on 03/07/07.

While moving a power cable for an electric shovel , the cable got hooked on a rock. When it came undone it stuck an Employee in the legs, knocking the individual to the ground. The Employee reported the incident then was on normal days off. The Employee was taken to the Doctors for evaluation and placed on restricted duty on January 12, 07.

While unhooking a drain hose from a carbon truck, an employee felt what he thought was an eye lash in his eye. The employee rubbed his eye with his hand. The object did not move and had become embedded in the eye.

An ore process employee was cleaning a discharge chute with a spader. The spader caught his hand in a pinch point.

Wind was blowing extremely hard - when employee got out of haul truck at fuel island & felt something hit her eye.

Operator was helping load a utility truck with 4x12 sheets of sheet rock. (Operator continued to work until 2/13/06) (2007100030-the operator was off again on 10/31/06 Short term Diability. Negotiation have been ongoing between the Workman's Comp. Carrier and the Doctor as to whether or not this injury is related to the initail incident.)

Employee was detaching liner from liner handler, balancing himself by placing his hand on handler boom. The handler's operator retracted the boom before employee could remove his hand, catching employees thumb in a pinch point on the boom.

Attempting to replace by idler wheel on a D-10 dozer, the mechanic got his finger in a pinch point between the idler wheel and a hydrautic jack.

DURING THE ASSEMBLY OF A BEARING INTO A CYLINDER BORE, THE BEARING BECAME LODGED. A 2X4 & HAMMER WERE BEING USED TO DISLODGE IT. THE 2X4 SPLIT CATCHING THE MECHANICS HAND IN A PINCH POINT.
