One employee received 11% burns to left upper chest and arm. Other employee received 17% burn to the right back and shoulder, and neck.

Working in 7 shaft area when employee felt something in eye. Thought it was a small lump of dust or rock. Flushed eye out & wiped with gauze. The material that felt like a lump in the eye was gone. Employee didn't feel further treatment was needed at the time. The employee went to see an off site doctor on 08/18/23. And employee received prescription medication for eye.
Workers were hoisting scaffold equipment to the 6th level. As the load was approaching, the basket spun and would not fit through the hoist opening. The hoist was stopped and one of the workers grabbed the cable just above the block to straighten it. The other worker accidentally raised the hoist. The worker's pinky finger was crushed in the sheave of the hoist cable.
Employee was grabbing cable bolt off of bolter when bolter operator signed machine was moving and trammed backwards over left foot.
Employee was grinding on material and a piece of debris went under face shield and safety glasses into right eye. Employee didn't initially know there was anything in the eye until the following day, employee had material removed from eye and received antibiotic drops. Employee returned to work for the next scheduled shift.
Electrician was cutting a hole in a electrical cover plate for a reset switch. Plate slipped out of EE's hold and cut EE's left thumb. Employee received stitches on left thumb.
Employee was greasing bearing with battery powered grease gun. Employee was rotating stone insure proper lubrication when gloved hand got caught in rotation stone and pinkie finger on right hand was pulled thru. Minimal clearance between stone and trunnion causing injury.
Employee was adjusting the tire grinder used to remove sharp edge on the tire. Grinder slipped and employee tried to catch the grinder resulting in contact between hand and grinding wheel.
IE was grinding, wearing all appropriate PPE and felt something in eye. Eye was flushed and IE finished shift. Next day IE eye was swollen and red and IE was taken to emergency room where a small sliver of metal was discovered in eye. Sliver was removed.
Employee was struck on right thumb pinching it into rotation unit of bolter and drill steel while removing the steel.
During shift a metal flake went into subject's right eye causing an irritation.
Employee was operating drill during shift. He then was on vacation the next day when it became sore. Employee went to the doctor on the 27th and was put on restricted duty. On July 6th employee resigned from the company.
Employee was installing roof bolt when the bolt didn't align with chuck wrench. He then had to slow feed it in the hole. Once he started feeding the rotation was too fast and it caught his glove and tore his thumb off.
EMPLOYEE WAS OPERATING A BEAVER BOLTER DRILL. LEFT HAND CAME IN CONTACT WITH DRILL STEEL, SEVERING LEFT RING FINGER.
Employee got "trona rash" on his legs from the #2 mud tank.
Employee was supervising a crew that was grinding metal when a piece of metal flew off striking him in his right eye.
Employee was cleaning ducon when steam blew out and burned his left side of face and left arm.
Employee bumped his elbow on storage tray on the roofbolter on May 1st. On May 8th during his seven days off he called in and reported the accident. He came back and worked his four days and went on days off. On May 15th employee went to the doctor and was diagnosed with bursitis. Employee went on MLOA FROM May 18TH through May 20th and had a full release to full duty on May 21st.
Employee was operating a pipe threader when it caught his right middle finger between the pipe and pipe threader.
Employee was cutting handrail when sawzall kicked and cut his index finger on left hand.
Employee was drilling a gas hole when he went to remove the steel and they fell out of the hole onto his hand.
Employee was installing roof bolt when pan fell out onto controls causing the bolter arm to swing over and striking the employee. Employee was put on MLOA on May 22,2011.
After inspecting rotation on H09 transfer screw, he was removing a rag from inside the inspection door when the transfer screw caught his glove resulting in partial amputation to his index, middle and ring fingers of his right hand.
Employee turned his head to find a bolt when the drill rolled off the block, pinching his left 5th finger. He had blood drained from nailbed.
Employee was drilling hole with fewer bolter when rotation stopped, and steel was stuck. He let go of trigger and bulter dropped and swung toward the rib. He then attempted to grab the bolter at which he then felt a pull in his lower back.
While drilling a gas hole, employee caught his thumb between drill chuck and the centralizer arm on the bolter resulting in a laceration to employees right thumb.
EE was opening valve using electric valve wrench and pinched his finger, causing a laceration requiring sutures.
Employee was cleaning up using a shop vac and strained his lower back while stretching the hose out.
EE was using a drill press in under ground elec. shop when he finished hole he started extracting bit od metal plate he had drill handel in right hand it slipped out of hand when drill struck his tip of his left thumb got caught between chuck and base of drill press.
Employee was closing Warman valve using an electric valve wrench and got his finger pinched between the trigger on the valve wrench and a 6" pipe.
EE WAS RELIEVED BY ANOTHER ROOF BOLTER OPERATOR. THE OTHER OPERATOR THOUGHT EE HAD LEFT THE AREA BECAUSE WHEN HE MOVED THE BOLTER THE FIRST TIME, EE WAS AT THE BACK OF THE BOLTER. WHEN HE MOVED THE BOLTER THE SECOND TIME, EE WAS BY THE RIGHT FRONT TIRE. RAN OVER EE'S RIGHT FOOT - FRACTURE.
Ee was operating fct-3 in panela fm02. fct3 has a breaker caps on the front a rock ricohet off. another rock, coming out of breaker shaft striking ee in head.
EMPLOYEE WAS REPAIRING A HOSE ON JCM-5 IN PANEL SB15. ANOTHER EMPLOYEE WAS GOING TO LET COVER FOR AUGER JACK DOWN AND TOLD EVERYONE HE WAS GOING TO DO SO. WHEN HE DID THE HEEL BAR KICKED OUT STRIKING THE FIRST EMPLOYEE IN THE RIGHT FOREARM.
While roofbolting, slab of material fell and hit ee on left shoulder.
EMPLOYEE STATED HE WAS INSPECTING THE FEED BOX ON #5 DRYER WHEN ONE OF THE SPRINGS CAME OUT STRIKING EMPLOYEE ON LEFT CHEEK CHIPPING A TOOTH AND BRUISING CHEEK.
EMPLOYEE STATED HE WAS WORKING ON GR III IMPACTOR CHANGING ANVILS WITH AN IMPACT WRENCH. THE SOCKET SLIPPED OFF OF SWIVAL AND STRUCK LEFT MIDDLE FINGER CAUSING LACERATION REQUIRING SUTURES.
EE WAS TROUBLE-SHOOTING CENTRIFUGE MOTOR CUBICLE WHEN THE PRIMARY BUS STAB PHASE LEADS SHORTED PHASE TO PHASE.
EE IS A BEAVER BOLTER OPERATOR IN PANEL, WHILE BOLTING ON THE 1/2 PAS SIDE OF 2 ROOM 18 X CURTAIN PANEL, EE NOTICED A CRACK IN SEAM. TRY TO BAR IT DOWN, PART OF IT CAME DOWN, DECIDED TO PUT BOLT IN IT, MOVED BOLTER UP, ROCK CAME DOWN STRIKING HIM ON THE RIGHT SIDE OF BODY.
EMPLOYEE WAS GRINDING ON A BEAVER BOLTER STEEL. HE HAD THE STEEL CLAMPED IN A VISE AND WHILE GRINDING, GRINDER HIT VICE AND JUMPED BACK. EMPLOYEE LOST CONTROL OF GRINDER AND IT GAVE HIM TWO ABRASIONS OR CUTS ON HIS LEFT FOREARM BICEP AREA.
EE WAS CLEANING #3 DRYER SCREW WHEN THE HANDLE ON THE LEVER OF THE DUCKON BROKE CAUSING THE GATE TO SLAM SHUT. THIS CAUSED STEAM TO EXIT THE DOOR WHERE EE WAS STANDING, CAUSING 2ND DEGREE BURN S ON THE NOSE & FACE.
EE WAS INSTALLING A WHEEL STUD ON A JOY SHUTTLE CAR. AS HE HELD THE STUD WITH HIS LT. INDEX FINGER, HE TURNED THE OUTER NUT WITH AN IMPACE WRENCH. HIS FINGER WAS PINCHED BETWEEN THE STUD AND A WELD.
EE WAS BOLTING IN THE #2 ENTRY IN A94 WHEN A PIECE OF SHALED FLAKED OFF FROM ROOF AND STRUCK HIS RIGHT BICEP.
EE WAS PULLING ON A STUCK DRILL STEEL. HE STEPPED UP ON THE BOOM TO GRAB THE DRILL STEEL, WHEN THE BOOM SWUNG OVER PINCHING HIS HAND BETWEEN THE BOOM & THE DRILL STEEL.
EE WAS WELDING ON A ROOF BOLTER IN THE SURFACE SHOP. HOLDING A PIECE OF METAL 2"X4"X24" WHILE WELDING TOOK HAND OFF THE PIECE OF METAL. METAL FELL SMASHING FINGER ON RIGHT HAND.
EMPLOYEE WAS WELDING ON HEAD OF LCM IN PANEL 028-3 WHEN CREW WAS PULLINGM STACK MINER CABLE BACK. CABLE WAS WIRED UP WHILE PULLING CABLE WIRES BROKE AND CABLE FELL DOWN STRIKING HIM IN BACK.
EMPLOYEE WAS OPERATING A CONTINUOUS MINER IN PANEL L17N BY REMOTE, HE WAS STANDING ON RIGHT RIB.A PIECE OF ROCK (SHALE) FELL BETWEEN BOLTS STRIKING HIM ON HARD HAT.
EE WAS RUNNING LCM-1 IN PANEL A92 WHEN A RIB SLAB CAME OFF, PUSHING EE INTO MINER CAUSING INJURYTO HEAD & RIGHT FOOT.
EMPLOYEE WAS INSTALLING A ROOF BOLT IN THE HOLE. WHEN HE PUT THE ROOF BOLT UNDER THE GLUE AND INTO THE WRENCH, THE ROOF BOLT PLATE STRUCK HIM ON THE RT FOREARM CAUSING LACERATION.
TWO EMPLOYEES WERE BRINGING ON A CONDENSATE PUMPTHEY OPENED THE SUCTION VALVE AND DID NOT SHUT THE DRAIN VALVE. THE HOT CONDENSATE WENT INSIDE OF INJURED'S BOOT CAUSING 2ND DEGREE BURN.
