Employee cut middle finger on Left hand while picking up trash.

Disconnecting a hose to stop a leak in the connection between the hose and customer's plumbing to a sulfuric acid storage tank. Back pressure from the customer's plumbing caused residue in the hose to spray the driver.

While relocating the old stacker belt using a loader and an extended boom, the boom operator lost control, causing it to tip onto its side. The boom operator jumped from the cab and sustained an injury to the left wrist. Employee went to a specialist for further evaluation. Specialist documentation from 7-7-2025 confirmed limitations and brace to left wrist.

Cat Operator was fueling Cat D9 Dozer at the Heavy Equipment Yard. Operator fell from the dozer and landed on the driver's side blade tilt cylinder. As a result, operator broke three left ribs and bruising to the left side of body.

One of the electrical systems failed on our primary escapeway shaft. There are redundant systems in place, the shaft was never out of operation. The hotline was called as a courtesy.

#2 hoist (Primary) went down due to electrical problem, Maintenance Coordinator and electrician used #3 hoist to reach surface. Maintenance conducted troubleshooting and found a bad coil on the main brake. Maintenance replaced the bad coil on the main brake and #2 hoist was up and operating at 9:50 pm. #3 & #6 hoist were still operable during this time.

In an attempt to close a liquor feed valve with a valve wrench, the valve broke the process, resulting in the wrench slipping and smashing the employee's right pinky finger.

Employee was isolating a steam leak to get repaired on #3 flash tank in GR2. Steam change directions when employee was isolating leak contacting employee's right-side forehead and cheek. Employee received first and second degree burn to right forehead and cheek.

while pulling on a come-a-long chain, the chain slipped and pinched the left middle finger.

This week we had a scheduled outage planned for the mine to do maintenance and upkeep. During this outage our main man hoist was scheduled for an upgrade taking it down until Friday. There are still 2 escape shafts to the surface in case of an emergency There was no accident or injury. The notification was just a courtesy call.

Employee notice product (soda ash) was not feeding adequately on to the 1410 belt. Employee began to rod out blockage. While rodding out though the gate on #2 Bin, the blockage broke free and hot soda ash overwhelmed employee trying to get out of the way of the product. Employee received burns to mid-thigh and calve areas on both legs.

Operator was washing out with hot condensate to unplug a chute going to #4 Dryer elevator. The material broke free in chute and hot condensate spilled over operator's left boot. Operator received 2nd degree burns to the top of left foot.

2 Hoist Replace Guides.

Two employees were troubleshooting pump on the new purge cyclone system in GR 3, when both employees were sprayed by hot liquor causing serve burns (1st, 2nd, and 3rd degree burns). One employee received 11% burns to left upper chest and arm. Other employee received 17% burn to the right back and shoulder, and neck.

Graveyard shift started on 05/05/2024 at 7:00pm. 2 Hoist tripped at 02:;40 am and could not be reset, 3 product hoist was concerted to man hoist to bring electrician and supervisor to the surface to troubleshoot the issue. Found that the battery for the universal power supply needed to be replaced.

2 Hoist faulted. Waited for electrician to troubleshoot problem. It was determined that a sensor was faulty, and it was replaced.

Employee was lifting pneumatic vibrator up to install on ore bin. Employee lost EE's grip and the vibrator started slipping towards the ground, EE caught it and doing so EE felt a pop in EE's right shoulder.

While bolting a rock fell hitting employee's right thumb causing a bone break and a laceration.

Found Broken Guide in #2 shaft. Taken out of commission at 4:40PM. Repairs were made and completed at 8:11PM. #6 and #3 Hoists were operable and available during this period maintenance was being conducted.

During normal mining operations the employee got dust in their right eye. Saw an optometrist and received prescriptions for a steroid and an antibiotic.

6 Shaft was taken out of service for maintenance work. Maintenance was completed at 12:56PM and returned to service then.

#2 hoist tripped out during inspections.

Employee working in EA32 tripped over a roller. Twisted their right knee and hurt lower back.

Employee was riding down frontend manlift and fell from 5th floor to the 4th floor. Employee received injuries to head, right ribs, right and left leg.

Working in 7 shaft area when employee felt something in eye. Thought it was a small lump of dust or rock. Flushed eye out & wiped with gauze. The material that felt like a lump in the eye was gone. Employee didn't feel further treatment was needed at the time. The employee went to see an off site doctor on 08/18/23. And employee received prescription medication for eye.

Employee was riding down frontend manlift in GR 3, employee looked up, and got something in their left eye. After an hour, employee started feeling pain in left eye. Went up to guard shack to get left eye washed out. Employee went to clinic, then went to eye doctor. Eye doctor prescribed steroid eye drops due to scratch to eye.

Employee tripped on last step going up stairs which cause employee to impact floor and handrail at 5 Bin at Product Load Out. Employee strained and injured right & left shoulder, and right side of body.

3 hoist tripped on slave fault. Faults were reset and hoist is operational.

Employee slipped in wash bay while dragging pressure washer and strained left knee.

Employee was carrying a spray bar and walking in #1 shaft return tunnel. Employee hit EE's head on the top of the tunnel, knocking EE's hard hat off. EE bent down to get EE's hard hat and hit self in the mouth with the spray bar chipping EE's tooth. Employee scheduled to have repair done on May 30, EE reported to Safety Dept. that the repair was complete 6/12/23.

Employee slipped walking up stairs going into the GR 2 Control Room at the being of shift. Employee rolled right ankle on stairs and felt a pop. Employee was evaluated at Town Clinic. X-ray's showed employee broke right fibula. Employee was placed in walking boot.

Workers were hoisting scaffold equipment to the 6th level. As the load was approaching, the basket spun and would not fit through the hoist opening. The hoist was stopped and one of the workers grabbed the cable just above the block to straighten it. The other worker accidentally raised the hoist. The worker's pinky finger was crushed in the sheave of the hoist cable.

When reaching the bottom of 2 shaft, employee pulled the cage gate up to open and felt a pop in arm. Visible bulge in arm.

Employee was trouble shooting the spout at the top of the 108 belt at the crusher. Walking down the 108 belt, employee stepped on a frozen piece of product on the 108 belt walkway and rolled left ankle. Employee was taken to the ER and found that employee had broke lower left fibula and tibia, and a torn tendon in left ankle.

Miner was using a floor drill putting in floor pins for WA08 drive. Drilled about half way down, miner stepped back to start kneeling down and ankle twisted then right knee popped and fell down. We were notified of medical review findings on 1/30/23.

#3 Production Hoist. Hoist had an electrical fault on the drive converter, an electrician from underground had to reset the fault. #2 and #6 hoist were still operational during this time.

#2 hoist power issues, troubleshoot electrical problems. Found problem and was able to run #2 hoist in manual mode. Cage is operable.

In GR 3, employee slipped in mud by the #2 flock tank and sprained right ankle.

Employee was working on forklift and removing hook from overhead crane. Employee pinched left ring finger between mast of forklift and hook on crane. The employee received a distal tuft fracture to the 4th finger on left hand.

While riding the GR2 Man Lift, a piece of debris flew into the employee's eye. Employee was wearing proper eye protection. Went to eye doctor, material was removed, and was prescribed medical eye drops.

EE was putting cover plate back on collection screw. Cover plate slipped & EE pinched rt pinky finger. EE got stitches to finger. Update: At time 7000-1 was filed, Mine understood EE had full use of finger & there was no bone involvement. On 1/3/22 MSHA issued citation, directing Mine to amend 7000-1. On 1/6/22 Mine learned EE had subsequent med treatment on or about 1/4/22.

Employee was cutting foam board with utility knife. While finishing cutting the board, knife slipped and caught employee's left ring finger. Employee received stitches in finger and returned to work.

#3 Hoist computer screens went down. Operator tried to fix issue. Management was notified and Dispatch called MSHA. Issue was fixed by resetting computer. #3 Hoist is a skip hoist. No person was entrapped or injured and no damage to hoist or shaft. Computer issue. MSHA was called back when issue was fixed.

Approx. 4:55a.m. in West A Mains between 4 and 5 Room at 6 XC, bolter operator was bolting top. Bolt sparked off pan and start fire from methane venting off floor. Immediately, bolter operator stopped machine and extinguished fire with muck and fire extinguisher. MSHA was called and MSHA Inspector investigated event.

While trying to align bolt hole with a bar on a take-up frame and ram frame on a belt drive, mine employee pinched right pinky finger between bar and ram frame. Mine employee broke tip of the top bone of the right pinky finger.

While stripping cable with utility knife employee cut left middle finger requiring three stitches.

Standing back 10 feet, employee burned left forearm from condensate splash. Employee was using condensate to unplug dust line on #2 Dissolver Tank. Employee receive 2nd degree burns to left forearm.

Employee was operating torque gun to tighten bolts on # 2 filter and pinched EE's left index finger to the shell of the filter. Left index finger was lacerated and required stitches.

Employee was grabbing cable bolt off of bolter when bolter operator signed machine was moving and trammed backwards over left foot.

Two Hoist at the bottom of shaft unloaded bottom of cage and would not jog down to unload the top of the cage. Issue was investigated and issue was fixed. Hoist was operational at 7:45 a.m. MSHA was called to let them know.

Surface lost power, which affected the mine. After generators were started, #2 hoist had a brake fault, which was found during checks. Miners were staged at 2 hoist. Issue on hoist was resolved and mine was evacuated. Mine still had utility power for 6 shaft. MSHA was called when issue was resolved on hoist.

Employee was grinding on material and a piece of debris went under face shield and safety glasses into right eye. Employee didn't initially know there was anything in the eye until the following day, employee had material removed from eye and received antibiotic drops. Employee returned to work for the next scheduled shift.

Production Hoist #3 went down on a limit error and shows parked. Issues were resolved. MSHA was called when hoist was came back operational at 12:30.

Employee was walking towards mine shop office, stopped to turn around, and foot caught on floor. From MRO report 7/14/2021, Employee fractured fibular head on left leg.

While an employee was unwiring a locked-out motor, employee dropped a nut behind the motor and reached for it. Employee cut arm on a sharp area on the guard for the motor, that was not visible to employee. Employee received stitches to the 2" laceration on right forearm cut.

Employee was tying down two supply trailers using a 3/4 Ton come-a-long, when EE tightened the chain the hook slipped hitting the employee in the back of the arm above the right elbow.

Employee working on ripper chain, hit the ripper chain pin with a hammer. A piece of metal shrapnel hit the mechanic in the left index finger going through glove, causing a laceration. Employee received stitches.

Load out operator traveling to dodge parking and struck a parked lowboy supply trailer. Results were hairline fracture in the upper tarsal bone in EE's right foot.

Employee was removing a stuck rock from the miner throat and felt pain in the left shoulder. Employee went off on surgery 3/10/2021. From our Independent Medical Evaluator report on 3/31/2021, this is injury is reportable.

Employee was washing down 201 Belt. A clamp came loose on condensate hose and sprayed employee under rubber gear burning left leg. Employee received second degree burns to left leg.

Lost power on surface, which affected the mine. Had problems starting back up generator for 2 hoist. Once generator started mine was evacuated, however, we still had utility power for 6 shaft.

Walking from Crusher to the Electrical shop, employee slipped on some mud and felt a pop in EE's left. After Doctor review on 2/8/2021, employee suffered ACL tear and is going into surgery to repair knee.

Supervisor was pulling reflector out of a muck pile and fell backwards on a pan stuck in a muck pile, cutting right forearm. Received stitches.

Production Hoist #3 kicked out on tail rope switch fault. Working to resolve issues. Operational at 6:16 p.m. called MSHA and let them know.

#3 Hoist 1 East Brake not applied. This is a production skip, #2 and #6 hoists were still operable.

#3 hoist West Brake fault, this was a production skip. #2 and #6 hoist were still operable.

Maintenance Employee working on tail roller. Employee lifted up the slide plate to swing into position. Felt a pop in LH Bicep. Employee went off on surgery 1/13/21 on bicep. From our Independent Medical Evaluator report on 3/31/21.

Electrician was cutting a hole in a electrical cover plate for a reset switch. Plate slipped out of EE's hold and cut EE's left thumb. Employee received stitches on left thumb.

2 Hoist Lower Track limit fault. Repaired and operable at 3:45 am.

Employee was using sledge hammer to unplug #1 ore bin and sprained EE's right shoulder. Employee went out 1/5/2021 for surgery on right shoulder. From our Independent Medical Evaluator report on 3/31/21, this injury is reportable.

Employee was inspecting a newly installed 13.8Kv. switch and came in contact with 1 Phase of 13.8Kv. Resulted in electric shock injuries and burns. Employee received medical care from Surface Rescue Team and Company Doctor. Employee was medi-evac to a Medical Center.

2 Hoist kicked mid shaft Electrical faults while running hoist checks. 2 hoist was running at 11:00 pm.

BO waste line in 2 shaft. Wet Electrical boxes. 2 shaft was fixed and running at 11:15 p.m. on 10/25/20.

Had electrical issues with 2 shaft, troubleshoot, found bad electrical components and changed them. We still maintained two escape way hoists at the mine.

Employee was replacing ripper chain on Joy miner, locked out, stepped from the miner drum onto the miner head and tripped or slipped causing EE to fall between the drum and head. Employee stated EE's leg twisted and was unable to put weight on it. Injury result employee broken right leg.

Employee was applying pressure to remove a drive sprocket off a shaft on #1 Calciner drive chain. Sprocket came off and struck employee on the left shin and left ankle. Specialist Doctor saw the employee and was placed with an airboot on left foot on 8-17-2020.

Drive Converter trip at 3 Hoist, Reset.

#3 Production hoist faulted, Electrical issues & mechanical issues.

Employee was installing a rib pan bolt, and a brick-sized rock fell on employee's left hand while operating roof bolter. X-rays showed employee had hairline fracture in left hand.

Broken air hose 3 hoist

Employee was cleaning old parts off maintenance truck and felt a pop in right shoulder. Employee went off for surgery on shoulder 8/21/20. From our Independent Medical Evaluator report on 3/31/21, this injury is reportable.

#2 shaft hoist had an unplanned outage. After trouble shooting issue, it was found hoist had electrical issue and manufacturer had to be called. Shaft came back up at 9:05. MSHA was notified.

Operator was opening a lid when right leg slipped forward causing the tie off line to catch jerking operator backwards causing strain to lower back.

Employee slipped in parking lot due to fresh snow. Went to Specialist about wrist and was casted on 1/27/2020.

Surface lost power, took out all of the mine power and hoists. Called evac. due to the lack of secondary escapeway.

Employee was moving some of the pans to the other side of the roof bolter, employee pushed the pans forward and ran right forearm across the cut edge of the metal banding. Employee received stitches from laceration on forearm.

Employee was greasing bearing with battery powered grease gun. Employee was rotating stone insure proper lubrication when gloved hand got caught in rotation stone and pinkie finger on right hand was pulled thru. Minimal clearance between stone and trunnion causing injury.

#2 shaft hoist had an unplanned outage involving counterweight guides. Shaft came back up and operable at 3:00 pm on 01/05/2019.

Employee was adjusting the tire grinder used to remove sharp edge on the tire. Grinder slipped and employee tried to catch the grinder resulting in contact between hand and grinding wheel.

Employee was removing unistrut (electrical strut channel) from roof in a building. Employee pinched right ring finger between unistrut and hand rail on scissor lift when EE was removing the unistrut. Employee noticed EE had smashed 1/2" tip of right ring finger after removing glove. Employee went to hospital and received stitches.

Roof fall at 5 XC Main South, collapsed overcast. Minimal ventilation disturbance. When issue was found, the mine was evacuated through 2 shaft. No gases were detected, no injuries.

Employee was hammering a pin in the roadway. Swinging a 4 lb. hammer employee missed pin and struck EE's left hand.

While using pry bar, Employee punctured skin between thumb and index finger on left hand. Employee received stitches and returned to work.

Employee was pulling apart ventilation tubing in section when they felt a pain in the right shoulder. Employee received surgery 9/20/2019. From doctor's report on 9/27/2019, this injury is reportable.

Tightening canopy bolts on a shuttle car, miner slipped off bolt and felt a pop and burning sensation in EE's left arm and shoulder. After testing and evaluation from the doctor, the doctor's report on 7/22/2019 to mine that this will be a reportable injury. Miner will get surgery 7/29/2019.

While turning a valve with an actuator employee felt a pop in right chest muscle.

A roof fall occurred above overcast at Main South Belt line at XC 27. The fall occurred when roof bolts pulled out of the strata in the roof. The material was 6" thick x 10' long x 15'wide in size on top of the crushed overcast. Damage to the overcast disrupted main ventilation in the area. MS belt line was damaged for approx. 15' and no miner was injured in the event.

Employee was holding high volt cable up while coworker was securing it to the mine roof when the cable slipped off of the cable wheel causing the cable to fall putting extra weight and strain on the left shoulder. On 9/5/2019, employee had surgery on shoulder. From doctor's report received on 9/27/19, this injury is reportable.

While changing an expansion joint on a polisher pump, the expansion joint on the pump behind employee unexpectedly burst spraying liquor onto employee's back.

Employee caught boot on grating and fell injuring right knee. Employee met with Doctor and was put in brace on March 22, after evaluation on knee. Employee is waiting for surgery.

Employee was mopping the floor when they cut a finger on an outlet box. We were notified on 2/6/19 from a doctor that there were stitches.

Employee slipped on ice and strain lower back and hip. Employee received surgery 4/30/19 and return on 8/20/19 on disc protrusion. After complete evaluation from doctor's report on 9/30/19. The surgery was needed due to the fall from accident.

At 7:55 am the surface operations lost complete power causing #1, #4 and #5 fan to shut down. All miners were evacuated out of #2 hoist.

#3 and #2 Hoist lost power due to surface power issues. Evacuated mine.

Employee was stepping of ladder when ee stepped on uneven ground and twisted rt knee. Employee went on MLOA on 11/11/2018 for surgery making this a LWT.

Employee stepped on steel plate with muddy boots and slipped, causing pain in EE's left knee. After being re-evaluated by occudigm on October 18, 2018, it was determined that injury was an occupational injury. Therefore it was changed from a non-cc injury to occupational injury.

IE was grinding, wearing all appropriate PPE and felt something in eye. Eye was flushed and IE finished shift. Next day IE eye was swollen and red and IE was taken to emergency room where a small sliver of metal was discovered in eye. Sliver was removed.

Employee was cutting FCT belt when the utility knife slipped and cut right thigh.

While trouble shooting the FD 103 screen, Employee tried to see how much more work was needed by lifting the top deck. Felt a tear/sprain the right upper arm. First test showed no injury to arm, reevaluated right arm was done later in April. Employee got surgery on 4/26/2018. Employee was off on Funeral leave and long change during recovery.

Lost power to #3 hoist due to surface power issues. #2 and #6 shaft was available for escape ways.

Slide gate support failed and liquor sprayed on the face of mechanic. Mechanic was working on a valve and the slide gate support failed. Liquor sprayed on the face of employee.

Walking. Employer notified on 26 Sept. of reportablitiy. It is ee position that incident described herein resulted from the miner's pre-existing condition and not as a result of a reportable occupational injury, re-injury, exacerbation or occupational illness pursuant to 30 CFR Part 50. However, out of an abundance of caution, ee nevertheless submits this.

Employee cut between right thumb and forefinger while picking up gasket material.

Employee was using a sledge hammer to unplug chute when ee felt a pop in left shoulder. Employee went to doctor on 7/20/17 for MRI. On 8/2/17 it was determined that ee had a torn muscle and that it was work related caused by the incident. The employee is currently off work.

Employee was opening a valve when they hit their right arm against a beam that had a burr on it causing a laceration.

Employee was standing next to a gear box that was on a pallet when the box rolled off and hit ee in the left leg.

Employee was operating bore miner when the back peeled and a chunk of rock hit employee's foot and breaking it.

Transformer for #3 production hoist went down. #2 and #6 hoist were available for evacuation.

Employee was operating the Beaver Bolter in the SB27 Panel when a piece of rock unexpectedly fell from the mine roof striking EE on the head.

Employee was walking in the parking lot when EE fell. On April 26, 2017 employee went off of work to have surgery on the left knee.

#3 Hoist with drive converter A. #2 and #6 was available for use.

#3 hoist went down on ems error. #2 and #6 was available for emergencies.

#3 hoist compressors went down. #2 and #6 was available for emergencies.

#3 hoist kicked power. Had #2 and #6 hoist for emergencies.

Miner tripped on grating, fell on hand.

While using a pry bar assisting in adjusting a pinion on a dryer the bar slipped and pinched the tip of the right hand ring finger. This action caused a nondisplaced distal phalanx fracture

#3 hoist kicked out of synch. #2 and #6 were available.

Planned power outage. No one working in the mine during that time.

Employee was lifting beaver bolter back in slot and felt a pain in abdomen. Employee was seen by doctor at later date and company was notified of hernia by doctor's note on November 2nd.

Millwright cut fifth digit on right hand working on pump.

Employee was standing on belt structure unplugging a chute when EE stepped back and fell 4 feet to the ground.

Employee was putting top roller on structure when right index finger was pinched between the two.

Employee was removing pipe when dust got in EE's eyes on 7/6/2016. It was later noted that the eye never healed and was flaring up. On 6/14/17 EE started LWD due to EE's eye and was giving drops. EE returned to full duty on 6/19/17.

Miner cut employee's hand on locker in change room and acquired an infection.

Miner stepped into a approx. 1 foot trench and twisted knee.

Employee was tapping on roller with hammer when right thumb was pinched causing a laceration resulting in 12 stitches.

Employee was opening a valve when they felt pain in their left ring finger. Employee notified company that they were going off on surgery on 4/18/17.

Employee was opening a feed valve when their right shoulder locked up and experienced pain. *Employee notified company on 4/14/2016 and was placed on leave on 4/18/2016.

Pillow block on bearing on head roller failed causing grease to heat up and catch fire.

Employee was putting door back on bucket elevator when they were burned by condensate.

Employee was taking foam off of overcast when a chunk of rock came down and struck employee's arm causing a laceration.

Employee was struck on right thumb pinching it into rotation unit of bolter and drill steel while removing the steel.

Employee was moving a spool of welding leads when they felt a pain in their groin and was later diagnosed with a hernia on a later date.

#3 Production Hoist kicked Power, Power was restored. #2 & #6 shaft were still available for escapeways

Hoist non-working due to extreme outside temperatures/ 6 shaft hoist is operational, out of use due to temperature.

Employee was doing advanced belt move when the splice on the belt was caught between the roadway and the tail roller. Employee tried to move the belt with his foot when the tension gave way and caught his right foot and sucked his leg into the roller.

Loading pocket fault. 2 Shaft and 6 Shaft were available for hoisting.

#3 hoist kicked power. #6 and #2 hoist were available for evacuation.

Working on top of Deca recovery, when expanded metal on door caught him on the left shin, causing laceration.

EMPLOYEE NOTICED TENDERNESS IN STOMACH DURING SHIFT ON 10/22/15. EMPLOYEE WAS DIAGNOSED WITH HERNIA.

During shift a metal flake went into subject's right eye causing an irritation.

Employee was swinging a sledgehammer overhead when he felt a strain in his groin. He was later diagnosed with a hernia.

Employee was walking next to rail road tracks and looking at rail car numbers and tripped over a piece of metal in the middle of the walkway. He fell and landed on his wrist.

#3 production hoist kicked power. #2 and #6 hoist were available for secondary escape routes.

Employee was changing bits on miner when he cut his finger.

Employee was removing cylinder from LHD when the jack fell and landed on his foot causing a minor fracture.

Employee was trying to separate vent tubes when he felt a pain in his left shoulder.

Employee was examining the working face when tripped on a rock and injured his knee.

#3 Hoist kicked power due to conveyor fault.

Due the heavy rains the ponds were inspected and 2 pond was found to be breaching and flowing into 3 pond. The series of impoundments had enough capacity to handle all of the water with plenty of freeboard to spare. No danger to persons or the environment.

Employee was walking on stairs when he slipped off a step and hurt his back. Employee notified the company on July 28, 2015 that he had been seeing a chiropractor.

#3 Hoist kicked power due to the EMS converter. #2 and #6 Hoists were available.

Employee was sitting his lunch bucket down on cage when he was struck on the back of neck by cage gate. *Employee went on LWD 7/28/15*

Employee was unplugging a hopper when he felt a pain in his lower back. Employee was evaluated by company doctor on August 3rd and he informed us that he has been seeing a chiropractor.

#3 Hoist kicked power on a loading pocket fault.

Employee was moving a guard when he smashed his left index finger which required stitches.

Stepped onto a soda ash pile. The pile had a crust formation and the employee's foot broke through the crust. The employee lost his balance, fell and tried to catch his fall with his left arm. Resulted in a partially dislocated shoulder.

Employee was operating drill during shift. He then was on vacation the next day when it became sore. Employee went to the doctor on the 27th and was put on restricted duty. On July 6th employee resigned from the company.

Employee reported an accident on 7/10/2015 which he said occurred on 6/14/2015. He initially complained about his neck from an old injury, then he changed his story days later saying it was his back. We opened an investigation feeling that this was not work related. On 7/10/15 employee was seen by a chiropractor and was given RD.

Employees were lowering mantrip down on #2 Hoist when the overhead crane on the hoist kicked power. Even though the Hoist was not down or damaged, the Hoist could not be used during that time if needed.

Employee was using a utility knife to cut conduit when the knife slipped at cut his left palm.

While washing out the cage, water got in breaker box and tripped the hoist.

Employee was getting out of chair when he felt pain in his back. The accident occurred on 5/20/2015. Employee then had another flare up from a previous back injury in 2010 and was seen by the doctor on June 23, 2015 and was instructed to take 3 days off. Employee returned to full duty on June 29, 2015.

On 5/16/2015 employee was carrying a piece of belt when he stepped on a rock and twisted his LT knee. On August 3, 2015 employee went on MLOA for surgery.

On 5/10/2015 employee was bolting and walking along windrow when he tripped and twisted his knee. Employee went on LWD 7/1/2015 for surgery.

Employee was lifting top roller off of lo track when he felt a pop in his wrist. 7000-1 is being reported from audit findings.

Employee was climbing ladders multiple times when he felt a pop in his right knee. Employee was placed on restricted duty. 7000-1 is being done from audit findings.

Employee was exiting the cab of LHD to unload stopping pans from the bucket when he stepped on a rock and twisted his ankle. 7000-1 is being done due to Part 50 audit.

On March 23, 2015 employee was evaluated and was diagnosed with carpal tunnel syndrome from repetitive motion of being a mechanic throughout the years on the job. Employee went off on surgery on 5/13/2015 making this a recordable injury. EE went on LWD on 7/17/15 for surgery on his other wrist. This 7000-1 needs to be reopened

Employee was installing top rollers when he went to turn and felt a pop in his back. Employee has been treated by a chiropractor. 7000-1 is being done due to Part 50 audit.

Employee was lifting one end of a board when he felt a pop in his back.

Employee was installing scavenger when the ratchet slipped off and he twisted his knee. On Feb 15th employee had fluid drained from his knee making it reportable. 7000-1 is being completed due to Part 50 audit.

Employee injured left shoulder while lifting patient into ambulance. Employee required surgery on 8/16/2017 on left shoulder due to injury on 02/04/2015.

Employee was installing roof bolt when the bolt didn't align with chuck wrench. He then had to slow feed it in the hole. Once he started feeding the rotation was too fast and it caught his glove and tore his thumb off.

#3 Hoist loading pocket fault and drive converter fault.

Employee was lifting up trailer to hitch when he felt pain on the right side of his ribs. 7000-1 is being done due to Part 50 audit.
