Operator was trying to clean a blockage out of the bottom of a bag house compartment. A chunk of material fell onto the operators hand pushing it into the roll feeder cutting the ends of two fingers on EE's left hand.

was blowing out plant air compressor radiator with air hose...this location is contained within a small room...some debris generated by this process got behind safety glasses...washed out but believed some remained...ER staff found nothing and blue light revealed nothing...prescribed anti-biotic as a precaution and released to return to work with no restrictions

Cutting bolts with torch to facilitate replacement of drive motor bases on screen when torch being re-positioned for the task brushed by left hand burning area of hand where thumb and hand are joined resulting in a second degree burn that has been seen by a Doctor several times and continues to be treated with Silver Sulfadiazine...free to continue normal job routine as of Monday

Roller mill operator was finishing the routine cleaning of #1 roller mill dust collector system...was standing on the deck beside the collector chute, reached inside access door and down the chute with right hand feeling for material... made contact with operating roll feeder...result is partial amputation of two fingers and laceration to a third

Employee using band saw and allowed right ring finger to get too close to blade...resulted in (4) stitches...returned to work with no restrictions.

Using a backhoe to clean mud, clay & rock out of raised bed of articulated haul truck...working backhoe bucket near midsection of bed...glob of mud fell from the side of the inner bed striking backhoe bucket...rock that was part of the glob of material rolled over the bucket stricking the web of EE's left hand resulting in seven (7) stiches...no lost time or change of duties

Cleaning out kiln transfer chute using a jackhammer through an opening while standing from a position outside of the transfer chute when a large piece of material fell striking the jackhammer bit causing it to go down resulting in the hammer end rising up and this hammer struck employee in the forehead resulting in (9) stitches but no lost time.

using cutting wheel on a Milwaukee 4.5" grinder to prepare area for welding when cutting wheel slipped/grabbed and kicked back striking left wrist causing a laceration requiring (3) three stiches to close

EE was drilling holes in concrete panels when the drill bit hit rebar. The dill bit stopped and drill turned striking EE in the face and mouth.

While using cutting torch a piece of hot slag was trapped under wristwatch band resulting in what was later determined to be a second degree burn.

Supt.was helping with positioning of roll mount for No.1 kiln/No.5 Pier when a hydraulic jack started leaking. Pressure on jack was released allowing jack/shims to lower, remaining jack/shims had all pressure on it & being to move. EE had his left hand on handrail and this hand was struck by a piece of shim stock 11 1/4x3x1" weighing 11 1/2lbs. laceration 4 sitches.

EMPLOYEE WAS USING A TORCH TO CT AWAY SUPPORT METAL FROM WORN END OF CRUSHER DUMP HOPPER. WHILE HOLDING TORCH WITH RIGHT HAND, HE HELD ONTO ANOTHER PIECE OF SUPPORT METAL WITH HIS LEFT HAND TO BRACE HIMSELF. PIECE BEING CUT WITH TORCH CAME LOOSE SOONER THAN EXPECTED STRIKING LEFT FINGERS. HOSPITAL X-RAYS WERE NEGATIVE FOR FRACTURE BUT LACERATION REQUIRED FIVE STITCHES TO CLOSE.

EMPLOYEE WAS HOLDING WRENCH THAT WAS HOLDING THE BOLT USED TO ATTACH TRACKS TOGETHER ON DOZER, THE WRENCH WAS TO BE EASED AGAINST SIDE OF THE TRACK TO PREVENT THE WRENCH FROM SLIPPING AND AFTE R THE WRENCH WAS AGAINST THE TRACK THE EMPLOYEE WAS SUPPOSED TO REMOVE HIS ENTIRE HAND FROM THE AREA BEFORE THE AIR WRENCH WAS ACTIVATED, HIS RIGHT FINGER WAS IN THE WAY, FRACTURE RIGHT PINKY

EMPLOYEE WAS USING BACKHOE AROUND NEW QUICKLIME STORAGE LOADOUT FACILITY WHEN HE BACKED MACHINE TO NEAR RAILROAD TRACK AS THE COMPANY LOCOMOTIVE WAS MOVING SOME CARS INTO POSITION FOR LOADING AT THE FACILITY...TRAIN WAS MOVING VERY SLOWLY BUT CORNER OF CAR STRUCK THE BUCKET OF THE BACKHOE...BUCKET AND SWING ARM ABSORBED MOST OF THE IMPACT BUT ENOUGH ENERGY REMAINED TO TIP IT OVER.

EE WAS MAKING A PUNCH USING OUR PLANT STATIONARY GRINDER. THE GUARDS WERE IN PLACE! HIS LEFT HAND SLIPPED BETWEEN THE INSIDE OF THE LEFT WHEEL AND LEFT SIDE OF THE MOTOR. THE INSIDE EDGE OF TH E GRINDING WHEEL CUT INTO THE TOP OF HIS LEFT HAND. A TENDON HAD TO BE STITCHED BACK TOGETHER AND STITCHES WERE USED TO CLOSE THE WOUND. I EXPECT THE EMPLOYEE TO MISS THREE TO FOUR WEEKS.

EE WAS CUTTING OFF TEMPORARY WALKWAY SUPPORT. AS THE LOST CUT WAS MADE. THE BRACE SURING AROUND AND STRUCK EE.

PARTNER WAS LOOSENING LUGNUTS WITH AN AIR WRENCH. INJURED WAS REMOVING LUG NUTS BY HAND WHEN TIRE WHEEL COMBO ON A 35 TON HAULER MOVED MASHING FINGER BETWEEN WHEEL AND BACK SIDE OF LUG NUT. FI NGER WAS NOT BROKEN AND NO STITCHES WERE REQUIRED. HOWEVER, PRESCRIPTION MEDICATION IS BEING USED FOR SEVERAL DAYS.

USING A BACKHOE TO SCALE QUARRY WALL AFTER SHOT...PRYING ROCK WITH BUCKET GOT BACKHOE IN A BIND AND THE BACKHOE RAISED FROM ITS SUPPORTED POSITION...WHEN IT SUDDENLY RETURNED TO ITS SUPPORTED POSITION THIS ACTION JARRED MACHINE AND EE STRAINING HIS BACK.

EE ALONG WITH 3 OTHERS WERE REASSEMBLING TRUCKS FOR 50TH LOCOMOTIVE. FRAME BEING RAISED/POSITIONED BY CRANE BECAME HUNG ON WHEEL. WHEN RAISING LOAD INSIDE OF LOAD WAS STILL HUNG & OUTIDE OF LOAD TILTED UP RESULTING IN CHANGE OF BALANCE. BRAKE ARM THAN HUNG ON WHEEL WAS FINALLY FREE BUT UNBALANCED LOAD WAS DRAWN TO FRONT OF CRANE AND EE WAS CRUSHED BETWEEN LOAD AND CRANE.

EE WAS HOLDING A PIECE OF METAL TO BE WELDED BY HIS WORK PARTNER ADN WAS PLANNING TO LOOK AWAY BEFORE WELDER STRUCK ARC BUT WAS TOO LATE.. FLASH RSULTED IN FLASH BURNS TO EE'S EYES...HE CALLEF IN THE NEXT DAY REPORTING OFF WORK DUE TO DISCOMFORT...NO OUTSIDE TREATMENT BY ANYONE HOWEVER LOST ONE WORK DAY.

EE WASUSING A CUTTING TORCH TO CUT A PATTERN FROM A PIECE OF METAL WHEN A PIECE OF RUST FRM SLAG POPPED UP INTO HIS EYE CAUSING A SCRATCH AND IRRITATION.

EMPLOYEE SAID MATERIAL(HYDRATED LIME) SPILLED ON HIM CAUSING MINOR BURNS TO HIS LEGS AND AROUND HIS WAIST.HE LATER BECAME SICK AND WAS DIAGNOSED WITH EVERYTHING FROM AN ANXIETY ATTACK TO BRONC HITIS FOLLOWED BY PNEUMONIA. HE HAS BEEN OFF WORK SINCE AND SEES A DOCTOR ONCE PER MONTH.THE COMPANY HAS DNIED WORK RELATED UNTIL OF INDEPENDENT INVESTIGATION.

EMPLOYEE WAS OPERATING SANDER WHILE CLEANING OFF HYDRATOR DRUM WHEN HE LOOKED AWAY. SANDER DISC TOUCHED DRUM AND KICKED BACK. EMPLOYE ELOST CONTROL OF THE HANDLE (SUPPORT) ON THE LEFT SIDE O F THE SANDER AND THE DISC STRUCK HIS LEFT FOREARM RESULTING IN A LACERATION REQUIRING STITHCES TO CLOSE.

POWER HOIST WAS LIFTING COWLING OFF OF FRONT END LOADER...CORNER OF COWLING CAUGHT ON MACHINE...AS HOIST OPERATOR WAS REDUCING PRESSURE ON COWLING THE COWLING RELEASED FROM THE OBSTRUCTION STR IKING EMPLOYEE IN THE TEETH. NEITHER THE MACHINE OR HOIST WERE AT FAULT...EMPLOYEE WAS TO CLOSE TO THE ACTION.
