Maintenance employee was welding an inspection port in mill room 1 on feed chute when a piece of metal or slag came off the weld and down into EE's welding hood and underneath EE's glasses.

While testing grounding systems an arc flash occurred at the disconnect causing a burn to the employees left hand and flash burn to the eyes.

Operator was trying to clean a blockage out of the bottom of a bag house compartment. A chunk of material fell onto the operators hand pushing it into the roll feeder cutting the ends of two fingers on EE's left hand.

Operator was using a T-Bar to dislodge material from a head chute when the material gave way the weight of the bar traveled down and the operators thumb was struck between the T-Bar and the head chute causing the operator to suffer a broken thumb.

Miner missed a step climbing onto a locomotive.

Train of moving railcars struck a train of stationary railcars resulting in a sudden stop.

Damaged boot allowed hydrated lime to contact skin.

contract labor engaged in cleaning activity...windy day...got foreign material in eyes...rubbed them repeatedly in disregard to both advice and instruction...washed out with eye specific solution...EE went to WRMC ER later that evening...ER staff flushed eyes...prescribed an antibiotic as a precaution...released EE to return to work with no restrictions 04/21/2018

was blowing out plant air compressor radiator with air hose...this location is contained within a small room...some debris generated by this process got behind safety glasses...washed out but believed some remained...ER staff found nothing and blue light revealed nothing...prescribed anti-biotic as a precaution and released to return to work with no restrictions

EE was cleaning dried (mostly) mud from around the pedals of the plant Bobcat loader prior to use...EE was using a trowel to dig the mud and chip away at the dried material...EE's hand slid down the handle and the tip of the right ring finger struck the back of the trowel...(4) stitches and released to return to work with no restrictions

EE was entering the plant oil house when a red wasp stung EE on the bridge of the nose. EE immediately began to swell. We transported EE to the local hospital where EE received steroids and medicine to counter inflammation through an IV. EE was released to return home soon after the swelling started going down. We are now aware of EE's allergy to certain insect stings.

The gate controlling feed to the roller mill was being closed using a pipe wrench on a shaft fitted with a nut. Resistance was significant requiring downward pressure. Either resistance ended without warning resulting in head striking structure or grip was lost on wrench handle allowing it to rebound striking forehead resulting in (9) stitches.

Cleaning out screw conveyor by pushing material down an opening when pushing too hard resulted in sharp shooter shovel penetrating too deep and employee jerked back on the handle causing the right little finger to strike the flight resulting in a bruise relieved by drilling a tiny hole in the nail to relieve pressure and three days light duty no lost time.

EE was separating sheets of 1/4" steel enough to get forklift forks between them. While pressing down on a 36" pry bar to obtain initial separation he strained his lower back. ER doctor instructions included no lifting until seen by Company doctor the following week. Meds to counter inflammation were also prescribed. Returned to normal activity 12/16/14 with no lost time.

assisting repairmen with repairs to screw conveyor in hydrate system...hot and humid...wearing long sleeve shirt which apparently rubbed against wrist and top of hand...exposure to hydrated lime resulting in lime burns to top of right hand and wrist...medical treatment sought Sunday...silver sulfadiazine and antibiotic is the result...no lost time or restrictions

Foreman and Hydrator operator were in lunchroom when they herd a "pop". In seconds they were outside to investigate. The area near the Hydrate MCC and bag storage area were on fire, and the fire was growing. A brief attempt to control with extinguishers was abandoned and 911 was activated. No injury.

Employee was bagging 10 lb bags of hydrate when bag burst. Employee was wearing safety glasses, but some hydrate entered area behind glasses. Result was irritation to both eyes with the right side the worst. On site washing using water and eye wash plus several daily trips to eye doctor resolved the situation ending with two days restricted duty, but no lost time.

(See Attach) EE states he was lifting a railcar shaker and he thinks the injury could have possibly occ. during perf. of that task. EE did not report this to co. officials until he returned from vacation, approx. 1 week after possible occur.It is still unclear if this occurred during perf. of the EE's duties while on co. property. No Witness, W.C. will issue letter of denial.

Roller mill operator was finishing the routine cleaning of #1 roller mill dust collector system...was standing on the deck beside the collector chute, reached inside access door and down the chute with right hand feeling for material... made contact with operating roll feeder...result is partial amputation of two fingers and laceration to a third

Foreman inspecting plant when he fell landing on his right hand. He first sought diagnostic evaluation Friday, 03/25, and was referred to another clinic for additional evaluation, which was Thursday, 03/24. This revealed a broken bone in the heel of the hand. The Orthopaedic said it would heal with no treatment. There are no restrictions and no lost time.

EE was simply stepping up concrete steps to gain access to the stone mill operating room when he felt a pull or tear in his lower left calf. Days later diagnosed as a pulled muscle following an MRI, resulting in light duty and prescription muscle relaxers to be followed by two weeks physical therapy. EE continues to perform his normal job each day.

Employee was working on top of bulk hydrate tank re-bagging a dust collector...he is very explicit in that he is unaware of what happened that resulted in his left elbow being swollen and hurting...he can't recall striking anything or being stung (wasp were in the area)...he has had fluid drawn from the elbow and is taking a antibiotic...he continues to work unrestricted.

Leadman opening discharge gate (rail car) using steel bar to turn the cap stand so as to slide the gate back. His foot was on the bar to gain leverage. He had mud on his boots, which caused his foot to slip off the bar. The bar sprang up and struck fingers on right hand. Hairline fracture midway through middle of bone of right ring finger.

Deceased truck driver (customer) found dead in cab of tractor (Freightliner) by another truck driver at our truck wash location. Death is believed to be from natural causes to be determined by Arkansas State Crime Lab.

Two employees were setting a newly fabricated lid for unit II mill feed screw...this lid was fabricated from .5 inch stock...One employee dropprd his end first and the impact caused the bottom piece to bounce upward and strike the other employee's left hand area resulting in a hairline fracture to the tip of the left middle finger...no lost time or light duty.

Employee was bagging Hydrated lime in 50 pound bags using a machine manufactured for this purpose...he was removing a filled bag when it split spewing hydrate onto his face...some got under his left eyelid irritating it...today, he made his third trip to the Company Doctor before the doctor pronounced him totally cured...he lost NO time and his left eye was NEVER affected.

picked up a piece of steel grating and turned as he stepped on a small amount of spilled and muddy 140 mesh limestone...he caught himself and prevented a fall but appearently wrenched his lower back resulting in a muscle strain...this has resulted in light duty and physical therapy...progressing very well

EE was wearing his safety glasses. Was performing his bid job as a bagger. Wind started blowing rather hard for the 1st time in weeks & some foreign material got in his right eye irritating it. After 24 hours the redness didn't recede so he was sent to a doctor. Medication is working wonders as the redness is going away as of today 9/15/05. EE missed 9/19-20. Worsened over weekend.

EMPLOYEE WAS HELPING WITH PRESSING A NEW BEARING INTO A ROLLER MILL ROLL BODY. EE & CO-WORKER WERE USING A PORTAPOWER CHAINED TO THE ROLL BODY. THIS CHAIN IS A LOAD BEARING CHAIN BUT TOO MUCH TORQUE WAS APPLIED & THE CHAIN BROKE & STRUCK EE'S RIGHT FOOT ON TOP JUST BEHIND THE STEEL TOE. X-RAY REVEALS BROKEN BONE (ONE) IN TOP OF FOOT. RESULTED IN HARD CAST/LIGHT DUTY (NO LOST TIME).

INJURED AND CO-WORKER WERE REMOVING DRIVE GUARD FROM SECONDARY CRUSHER IN OLD QUICKLIME SYSTEM. AS THEY WERE RAISING GUARD IT CAUGHT ON SOMETHING AND THE JARRING ACTION RESULTED IN MUSCLE SPASMS IN LOWER BACK, A TRIP TO THE DOCTOR, ANTI-INFLAMATORY AND PAIN MEDS AND A THREE DAY LOST TIME ACCIDENT. EMPLOYEE RETURNED TO WORK 7/19/04 WITH NO RESTRICTIONS.

EE WAS HELPING WITH SWITCHING. CARS HAD BEEN POSITIONED. LOCOMOTIVE WAS DISCONNECTING FROM CARS. EE WAS STANDING TOO CLOSE AND AIR LINE FROM LOCOMOTIVE STRUCK HIM ON LEFT LEG CAUSING AN ABRSIO N AND CONTUSION. SWELLING AND SORENESS LED US TO HAVE EE SEEN BY A DR. DUE TO DISCOLORATION AROUND THE LEFT ANKLE, HIS AGE, AND GENERAL HEALTH TH DR. WANTED HIM OFF HIS FEET A FEW DAYS. SUPT.

THIS EE WAS WORKING AS A BAGGER & WAS BAGGING HYDRATED LIME. THIS IS A TASK THE EE HAS BEEN DOING FOR MONTHS. IT WAS A VERY WINDY DAY & GOT SOME DUST/GRIT IN HIS EYES. THE EYES WERE IRRITATED EVEN FOLLOWING SEVERAL WASHINGS. EE WAS SENT TO DOCTOR TO HAVE HIS EYES CHECKED & ANY FOREIGN MATERIAL REMOVED. EE RETURNED TO WORK LOSING NO TIME.

ACTUALLY, THERE IS NO ACCIDENT OR ANY OTHER SINGLE EVENT. THERE WAS AN ONSET OF PAIN THAT EE REPORTED AS BEING GREATER THAN WHAT HE HAS EXPERIENCED IN RECENT MONTHS. EE FIRST SAW PERSONAL DR. WAS SENT BY US TO AN ORTHO. AND IS HAVING AN MRISO WE CAN DETERMINE WHAT, IF ANYTHING, IS ACTUALLY INJURED AND IF IT IS JOB RELATED.

EMPLOYEE WAS PULLING ON A PIPE WRENCH AND STRAINED LOWER BACK...EMPLOYEE CALLED IN FRIDAY AND MISSED WORK THAT DAY...SAW COMPANY DOCTOR AND RECEIVED PRESCRIPTION FOR MUSCLE RELAXERS...RETURNED TO WORK MONDAY AS SCHEDULED.

EMPLOYEE WAS HELPING MAINTENANCE MAN REMOVE THE DRIVE GUARD FROM THE QUICKLIME SECONDARY CRUSHER AT OUR OLD PLANT SITE. THIS IS A LARGE AND HEAVY GUARD. THE UNBOLTED GUARD SLIPPED FROM ITS MOU NTING BRACKET FALLING ONTO EMPLOYEE'S FINGER. THIS RESULTED IN A FRACTURE OF THE TIP OF THE FINGER AND A LACERATION REQUIRING 5 STITCHES. EMPLOYEE RETURNED TO WORK AFTER LEAVING HOSPITAL.

EE WAS DESCENDING LADDER FROM ELEVATED LOADING PLATFORM TO TOP OF TANKER TRAILER TO CLOSE HATCHES SO TRUCKER COULD PRESSURIZE THE TRAILER. SAFETY LANYARD CAUGHT ON ROUGH SPOT ON HANDRAIL CAUSI NG EE TO BE JERKED CAUSING HIM TO LOOSE BALANCE.ATTEMPTING TO REGAIN BALANCE EE EXTENDED FOOT OUT & INTO OPEN HATCH OF TRAILER. THE SHIN OF RIGHLEG STRUCK EDGE OF HATCH. WOUND CLEANDED, STAPL

EE WAS MAKING A PUNCH USING OUR PLANT STATIONARY GRINDER. THE GUARDS WERE IN PLACE! HIS LEFT HAND SLIPPED BETWEEN THE INSIDE OF THE LEFT WHEEL AND LEFT SIDE OF THE MOTOR. THE INSIDE EDGE OF TH E GRINDING WHEEL CUT INTO THE TOP OF HIS LEFT HAND. A TENDON HAD TO BE STITCHED BACK TOGETHER AND STITCHES WERE USED TO CLOSE THE WOUND. I EXPECT THE EMPLOYEE TO MISS THREE TO FOUR WEEKS.

EE WAS ALIGNING LOADEING SHUTE OVER OPEN HATCH OF RAILCAR WHEN HE SLIPPED ON THE TOP OF THE CAR AND FELL WITH HAND EXTENDED TO BRAKE THE FALL. HE LANDED ON THE TOP OF THE CAR BUT BROKE HIS RIG HT HAND ONE BONE.

EE WAS CUTTING OFF TEMPORARY WALKWAY SUPPORT. AS THE LOST CUT WAS MADE. THE BRACE SURING AROUND AND STRUCK EE.

EMPLOYEE WAS HELPING RE-ASSEMBLE PARTS OF PLANT LOCOMOTIVE WHEN HE KNOCKED A SPRING, RESTING ON A GEARBOX OVERHEAD, OFF STRIKING HIM IN THE HEAD. THIS CAUSED SOME PAIN AND NUMBNESS BETWEEN SHO ULDERS AND RIGHT ARM. DOCTORS INCLUDING SPECIALISTS HAVE FOUND NOTHING TO INDICATE NERVE OR SKELETAL DAMAGE.

EE WAS LOADING BAG TRUCK...SNOW WAS FALLING VERY HEAVILY...WHILE EXITING TRUCK HE SLIPPED RESULTING IN A SPRAINED ANKLE/FOOT.
