Mostrando los 28EE attempted to retract a magnet carriage into place. The carriage did not fully retract as prompted. EE performed LOTO and opened the hatch to the magnet to then find the magnet half closed. EE attempted to maneuver the carriage to return it to an open position simultaneously the cylinder actuated closing the carriage on EE's right hand. EE suffered a contusion.
While unplugging a mill circuit, the miner removed a guard, and the motor belt contacted miner's right thumb and severed it at the knuckle.
A Temporary employee was in process of loading an incoming truck, when the mast chain drive became loose by hitting a bump on the roadway. They decided to try and put the chain drive back in place by placing their left hand inside the chain pulley, when the pulley snapped back in place, catching the left finger, causing a fracture.
Mechanic was in the process of removing the sump lid to gain access to the sump by using an electric winch to move the 400 pound lid out of the way. Upon freeing the lid, it moved and caused left hand to be caught between the edge of the lid and an electrical panel mounted to the wall, resulting in a contusion type injury requiring stitches
A Calcium Department Employee was vacuum cleaning up lime that had accumulated below the briquetting area. When removing the lid of the vacuum system, lime dust particles were released into his face and into his eyes. As a result of getting lime particles in his eye, the employee received a corneal abrasion.
Employee was adjusting carousel brake on an overburden drill. He was leaning against the carousel with his left hand as he applied an impact wrench to the brake adjuster. The carousel moved enough to pinch his left hand. He jerked his hand out and a rough surface caused a partial skin avulsion to his palm. The doctor decided to apply stitches.
Worker was operating overburden drill near a highwall, when three (approximately 2-3") rocks struck side windows on cab, causing fractures in the glass. The fractures released very small glass particles into the cab which entered the worker's eyes, resulting in corneal abrasion.
A hydraulic chain wrench was being used to loosen the hammer on an overburden drill. The threaded portion of the end of the chain wrench broke, swung around, and struck the employee in left shin, causing a small puncture wound.
While arc welding inside the dryer at the secondary and crusher dryer, EE's eyes were exposed to flashes reflecting off the very shiny stainless steel inside dryer. Flashes came in from back of welding helmt. Reportable due to prescriptiong antibiotics to prevent infection.
EMPLOYEE CHARGED & DISCHARGED 5 FURNACES DURING THE DAY WEARING GOGGLES. HIS EYES FELT DRY DURING THE DAY. ON HIS DRIVE HOME THE SUNLIGHT WAS IN HIS EYES & HIS EYES BECAME SEVERELY IRRITATED & STUNG. HE WAS TREATED AT AN E.R. & THE FOLLOWING DAY AT OUR OCCUPATIONAL DR.
Employee was walking to the tool room to return some equipment. She had to pass through the fabrication area. She tried to shield her eyes from the grinding, but felt something in her eye. A foreign body was removed; she was treated for conjunctival infection and given prescription medications as treatment.
EMPLOYEE FILLING SUPERSACKS WITH LIMESTONE IN THE SCREEN STONE DEPT., GRABBED THE SACK W/HIS LT. HAND, BACK FACING SACK & PULLED TO SWIVEL SACK EVEN W/PALLET BELOW WHEN THE PIN FROM THE CLEVIS CAME OUT DROPPING THE SCALE, SACK FRAME & 2000 LB. SACK STRIKING THE EE ON THE BACK LEFT SHOULDER.
EMPLOYEE WAS UNDER THE HEAT EXCHANGE FOR KILN #4 ATTEMPTING TO REMOVE THE BELLOWS FROM THE HEAT EXCHANGE TUBES USING AN AIR HAMMER. ONE 30' TUBE LOOSENED IN THE TUBE SEAT AND FELL STRIKING HIS LEFT THUMB WHICH WAS HOLDING THE AIR HAMMER. HE RECEIVED SEVERAL STITCHES AT THE LOCAL HOSPITAL AND WAS GIVEN MEDICATION.
EE INJURED RIGHT HAND USING POWER WRENCH TO CLOSE DOOR ON RAIL CAR. EE WAS TREATED BY MD AND WAS RELEASED TO MODIFIED DUTY NO USE OF RIGH HAND. EE WAS GIVEN A PRESCRIPTION FOR ANTIBIOTIC WHICH HE DID NOT HAVE FILLED. UPON FOLLOW UP MD PLACED EE OFF OF WORK DUE TO INFECTION.
EE USING HAND DRILL TO DRILL HOLE IN STEEL. DRILL BIT CAUGHT ON WELD AFTER MAKING HOLE. DRILL TWISTED OUT OF EE'S HAND AND STRAINED MIDDLE FINGER OF RIGHT HAND MIDDLE FINGER BECAME SLIGHTLY SW OLLEN BY NEXT DAY. SENT EE TO MD WHERE HAND WAS WAS EXAMINED. EE RELEASED TO MODIFIED DUTY LIMITED USE OF RIGHT HAND FOR ONE WEEK.
EMPLOYEE WAS WELDING A GUARD IN THE WELDING BOOTH IN THE MAINTENANCE SHOP. HE STOPPED WELDING AND LIFTED HIS HOOD TO OBSERVE HIS WORK. THE WELD POPPED AND A PIECE OF SLAG WENT BEHIND HIS SAFET Y GLASSES AND INTO HIS LEFT EYE. HE RECEIVED PRESCRIPTION MEDICATION FOR HIS EYE.
WHILE ATTEMPTING TO PUSH A PIN AND BEARING ASSEMBLY FROM ITS HOUSING USING A MECHANICAL PRESS AND A BOLT AS A SPACER, THE BOLT AND HOUSING ASSEMBLY KICKED OUT STRIKING EE ON THE RIGHT SIDE OF HIS HEAD.
EE WAS MOVING BRUSH WHEN FELLOW EE NEARBY WAS USING A CHAINSAW TO CLEAR BRUSH AND TURNED AROUND AND GLANCED EE'S FOREARM WITH CHAIN.
WHILE DRILLING OUT THE DIESEL GUN PART FOR #3 KILN LIGHT-OFF, THE DRILLED LIME PARTICLES BLEW BACK AND MADE CONTACT WITH EE S RIGHT FOREARM. THE LIME CALCIUM OXIDE STRUCK TO THE ARM AND DISOLV ED IN THE PERSPIRATION CAUSING SKIN BURNS.
EE WAS TRYING TO UNPLUG DUST COLLECTOR & PLACED HAND IN PIPE TO DUST COLLECTOR CHUTE AND FINGER TOUCHED BLADE OF ROTARY AIR LOCK.
EE EXPERIENCED EYE IRRATIATION.
EE DRILLING A HOLE IN A PIECE OF VERTICAL MOUNTED PLATE STEEL. MAGNET CAME LOOSE. FROM STEEL CAUSING DRILL TO SLIP DOWN ONTO RIGHT RING FINGER.
EE WAS CLEANING UNDER A DRYER WITH AN AIR CHISLE. THE HOSE ON ANOTHER EE AIR CHISLE CAME OFF AND BLEW SOME MATERIAL UNDER HIS CO-WORKERS SAFETY GLASSES AND INTO HIS RIGHT EYE. THE OPTOMOLOGIST HAS TO REMOVE A PARTICLE FROM EE'S EYE AND HE WAS GIVEN A PRESCRIPTION MEDICATION.
OPERATING ELECTRIC POWERED FLOOR JACK (HANDHELD)CARRYING CROWN PULLER ON FURNACE FLOOR, WHEN THEWHEEL HIT DIP/GROOVE IN FLOOR AND HANDLE WAS JERKED OUT OF HANDS AND HIT LEFT THIGH.
EE WAS CUTTING BROKEN CROWN AT 200 TON PRESS. CALCIUM PIECE STUCK TO BLADE AND FELL ON LEFT HAND WHILE REACHING FOR CALCIUM ON CUTTER SURFACE.
REPLACING PAD ON PUSHER BAR IN THE AUTOMATIC PACK CENTER. PAD IS MOUNTED ON RETRACTABLE FLAPPER. WHEN EE WENT TO REMOVE OLD PAD AND IN THE PROCESS OF TIGHTENING UP THE NEW PAD, THE ELECTRIC EY E WAS ACTIVATED. WHEN SAW MOVEMENT(HAND) AND ACTIVATED THE FLAPPER AND PINCHED EE HAND.
EE WAS GRINDING THE TIP OF A SHEAR PIN AND IT APPEAR THAT HE PIN WENT INTO THE GRINDING WHEEL IN THE ARE TO THE SIDE OF THE WRENCH THE PIN CAME AROUND THE WHEEL AND HIT THE EE ON THE TOP OF TH E LEFT HAND X RAY ON THE HAND WERE NEGATIVE. EE WAS PLACED ON RESTRICTED WORK ON 1-31-00. 1
