Employee was an air impact with reducer and socket. They went to the place the tool in place to tighten a bolt and felt a pop in their elbow. They were given self care treatment, but on 3/16/2026 they sought medical treatment and were given medication for treatment.

Miner was struck on left side of face by an extension for manual jack.

Employee was attempting to blow off a turntable in the packhouse between the grate using an air lance. As the employee was pulling the air lance from the grate they felt a sharp pain in the right shoulder. The employee received medical attention, and been place on restricted duties.

An employee was trying to clear a lump of hardened material from above the silo rotary feeder. They used an air lance through a port to get the material to start flowing. The silo became pressurized with the use of the air lance and cement came out of the port and blew into the employee's eye. The employee received a chemical burn to their eye.

An employee was using an air hammer to clean bolts at the secondary sizer. They were sitting on a 10 inch lip of the sizer bending over. The employee reported back pain and sought medical treatment the next day. They were placed on restricted work duties.

Employee was air lancing a chute and leaning against a railing. The employee leaned into the railing to back the air lance and banged their ribs on the railing. Employee was placed on restricted duties.

Contractor with Metal Surgery (E230) was grinding a weld on a girth gear, when a metal shaving flew up underneath the employee's face shield and safety glasses and got in their eye. The employee sustained a scratch to their cornea.

EE was using a compressed air hose removing a blockage in a lime dust system when the hose became crimped and then releasing air outwards, blowing dust from off the work landing into the EE's right eye.

Employee was installing a vibrator on a coal car while using an impact gun. The exhaust air pressure blew debris into right eye. Employee thought it was dust and it would come out on its own. Ended up being 2 small pieces lodged in eye that was removed by the eye doctor.

Electrician was working in a shallow trench tightening 2" conduit, kneeling next to a ground wire. Backhoe operator, 25 feet away, snagged the ground wire with bucket, lifting the ground wire out of the ground and pinning the knee of the electrician against the ground wire and conduit. Knee strain with restrictions

Employee was working on clearing a blockage through a 4" port on the CKD silo cone. Material flushed into employee's face knocking off his safety glasses. Both eyes came into contact with dust.

Employee stated that while operating a jet hammer, material moved, causing the jet hammer to cock up and pinch left thumb between the jet hammer and scaffolding railing.

Employee was grinding on shale crusher west bearing base.

Tramming Drill from gate 3 to storage lot behind Mobile Shop. The door latch vibrated loose and pinched little finger on left hand between door and door jam.

Using automatic welding equipment to build up grinding table inside raw mill. Employee received flash burns to both eyes.

EMPLOYEE WAS CUTTING METAL, WENT TO PULL GOGGLES OFF AND LET TORCH SLIP, BURNING HIS THUMB.

HAND DRILL SLIPPED TWISTING THUMB.

EE WAS INSERTING SELF TAPPING SCREWS INTO METAL PLATE. W/DRILL GLOVED FINGER GOT CAUGHT IN DRILL.

EMPLOYEE WAS CHIPPING CONCRETE WITH A 15 POUND JACK HAMMER. THE HAMMER BOUNCE UP AND STRUCK HIMIN THE MOUTH. INJURY CONSISTED OF 3 LOOSE TEETH THAT HAD TO BE PULLED.

EE WAS BLOWING DOWN IRON ORE IN 235 HOPPER AND A PIECE OF METAL BLEW INTO LEFT EYE.

AIRLANCE GOT STUCK IN VESSEL AND EE WAS JERKING, TRYING TO REMOVE IT. PULLED MUSCLE IN LOWER BACK ABOVE HIP.

EE WAS TRANSFERING CEMENT FROM GRP4 SILOS TO GRP5 SILOS USING A 6" FULLER KINON PUMP. HE WAS CHECKING FOR AIR ON CARBON SEAL OPENED EXHAUST VALVE (1/4") APPROX .15 PSI. HE WAS LOOKING AT END OF EXHAUST LINE & CEMENT DUST BLEW IN BOTH EYES

FILLING PACKAGE CEMENT BAG WHEN SEAMS BURST CAUSING DUST TO BLOW INTO HIS FACE

EE WAS USING AN AIR IMPACT GUN TO LOOSENB BOLTS ON CLUTCH ASSEMBLY ON #10 MILL MOTOR. AIR HOSE CAME LOOSE & STRUCK EE ON LEFT JAW.

EE WAS PUTTING GAS TORCH IN FRONT OF KILN WHEN DUST BLEW OUT THE FRONT OF KILN CAUSING THE DUST TO BURN HIS RIGHT WRIST.

EMPLOYEE WAS USING WAMA GUN TO CLEAN BYPASS DUCTWHEN HE STRAINED HIS RIGHT SHOULDER.

TAKING UP BUCKET OF MUD AND GOT TO THE TOP.BUCKET DIDN'T CONNECT ON TRACKS AND STARTED JERKING. FELL TO BOTTOM, THREW MACHINE OFF FOUNDATION ANDTHREW ME SIDEWAYS.

INDIVIDUAL WAS USING A PNEUMATIC IMPACT WITH A SWIVEL EXTENSION AS HE WAS JOCKEYING FOR BETTER POSITION IN THE SOCKET SLIPPED CAUSING HIS FINGER TO BE PINCHED BETWEEN THE IMPACT A& THE BOOM

EE WAS BLEEDING AIR FORM HYDRAUIC FITTING ON SUNSTRAN DSYSTEM FROM HYDRALIC FILTER. AS THE PUMP WAS BEING PUMPED TH EFITTING GAVE WAY.

FITTING BLEW OFF OF AIR HOSE ALLOWING HOSE TO BLOW DUST INTO EE S EYES.

A SECTION OF HOSE ON THE VACUUM TRUCK COLLAPSED & EE WAS CUTTING OFF A SECTION OF THE HOSE WITH A KNIFE WHEN HIS ARM WAS SUCKED INTO THE HOSE

EMPLOYEE WAS USING SHOTGUN BLAST TO LOOSEN MATERIAL IN #2 COOLING TOWER WHEN BARREL BURST INFLICTING A SMALL CUT ON EMPLOYEES LEFT INDEX FINGER DID NOT SEE PHYSICIAN UNTIL 12-14-93

PROLONGED USE OF JACKHAMMER CAUSED NUMBENSS AND SWELLING OF HANDS AND ARMS.

WHILE USING A WEEDEATER TO TRIM AROUND THE SEWAGE LAGOON EE CAME IN CONTACT WITH POSION IVY.

EE WAS WELDING FROM BELOW WHEN DIRT DISLODGED AND FELL ONTO HIM ENTERING HIS RIGHT EYE FROM AROUND HIS WELDING SHEILD AND SAFETY GLASSES.
