Driller and Assistant were pulling casing. The assistant turned the casing loose. The driller pulled casing up, the casing stuck and then released causing the casing to jerk in an upward motion hitting the driller assistant in the mouth.

Testing revealed perforated ear drum. 01/07/2014 EE underwent surgical procedure to repair ear drum.
07/10/2011 employee received minor burn to right shin while cutting metal. Treated on-site first aid. 09/15/2011 seen by doctor for infection. 09/17/11 employee notified supervisor of medical treatment.
Miner was grinding in front of welders while re-lining a pan feeder. He had his back to the welders. Welding flash came from behind his face shield and reflected off of the inside causing burns to the miner's eyes.
EE was operating tramway and had to reach down to secure a door on an incoming tram bucket.Doors on the tramway buckets are designed to latch automatically after they are emptied into the discharge silo;however,unlatched doors occur frequently due to rocks or sand in the latch mechanism. When EE reached to secure door, it caused a muscle strain in his lower back.
Employee was using a drill press & was moving the metal shavings with a wrench. He began talking with another employee & unconsciously grabbed the metal shavings that were curling toward his chest with his gloved hand. The metal shavings cut through the glove & lacerated his left index finger, which required sutures.
Four employees were performing preventative maintenance on the aerial tram system used to transport stone from the quarry to the plant. While tensioning the steel cable used to carry the tram buckets, the hydraulic cylinder assembly pulled loose from its anchor point. One employee was fatally injured and another suffered a broken leg, after being struck by components of the system.
On 02/23/06 employee received a puncture wound to his right hand, between his little and ring fingers. This incident was witnessed and caused by another employee. Victim was holding a plastic panel up with both hands while a driller from the opposite side of the panel drilled through the panel and part of victim's right hand.
Employee said that while operating the bag machine, his right shoulder became more painful from an old work-related injury. He felt burning pain & stiffness in it whenever he had to raise it overhead to reach an empty bag. Finally became so painful tht he could not push empty bag onto the machine's filler spout on his right side. Had torn rotator cuff surgery on 8/26/05.
Employee was making adjustments to feed system above bulk bagger; he asked other employee to test system. He informed operator he was clear of machine. As machine was started, his leg became caught between stationary and moving part of machine putting pressure on lower leg. Employee had a stress fracture to his right ankle.
THE RAIL ON THE BAGGING MACHINE HAD BECOME SHARP DUE TO CONTINUOUS INSERTION OF THE BAGS FOR LOADING SAND. EE INSERTE BAG AND HIS RIGHT THUMB WAS LACERATED ON THE SHARP EDGE OF THE RAIL.
HE HAD BEEN REMOVING PALLETS FROM THE STRETCH WRAP MACHINE AND PUTTING THEM INTO STORAGE. HE WENT TO LINE UP THE MACHINE WHEN HE WALKED BETWEEN THE MACHINE AND PALLET CONVEYER. WHEN IT WAS ACT IVATED IT PINNED THE LOWER PART OF HIS LEG BETWEEN THE PALLET AND THE FRAME OF THE MACHINE. LOWER PART OF HIS LEG BEGAN TO SWELL UP.
USING BAGGER AND PALLETIZER REPEATEDLY WHEN LEFT SHOULDER BECAME SORE. REPORTED 5-15-01, WENT TO DOCTOR 5-3-01.
