Employee was cleaning out a screen inlet chute from clogged material that resulted in a laceration to the right ring finger. When the EE removed their hand, EE made contact with the top of their right ring finger and a sharp metal edge of the chute inner lining. The EE received six stitches and was allowed to return to work with restrictions.

EE reported on 9/16/22 but did not want medical treatment. On 10/4/22 EE stated EE did want to see Dr. for strain to Rt. shoulder from pulling silo slide gate pipe handle on bin number three. Slide gate was stuck closed, and the EE overexerted to open the gate. On 10/7/2022, Dr. did not prescribe medication and no restrictions but referred EE to PT.

Bagging Operator was helping a forklift operator align the lifting eyes on a super sack tote in the storage warehouse. The Employee's right ring finger was caught in between the forks and the building c-purlin resulting in a laceration requiring stitches.

Employee was working near Bucket Elevator 205 near the Screenhouse Plant when the Employee walked into a 2-inch angle iron support on an H Brace I-beam that was supporting a belt conveyor structure. The angle Iron was at approximant 5 foot high. The employee was wearing hard hat. Employee was prescribed muscle relaxants for a stiff neck.

A Maintenance Technician was tasked to remove a piece of angle iron that was stuck in the belt conveyor tail pulley of BC-101. The EE tried to roll the conveyor belt backwards to let the angle iron drop out of the tail pulley by rolling the motor drive belts at the head pulley. In doing so the EE caught three fingertips on left hand. The Employee was wearing gloves.

Employee was a passenger in a moving work truck, in the backseat and was reaching down to pick up a set of keys when employee overextended arm and felt a sharp pain in left shoulder.

Employee was using a vacuum hose attached to a vacuum truck to remove sand near a guarded tail pulley. The hose clogged and the employee was picking up the hose to dislodge the material to unclog it when the hose clog released and the hose fell to the ground, the employee then felt a sharp pain in EE's back.

EE was moving bagged uniforms inside the office when a paper name tag made contact with their left eye. FA provided on-site. EE later sought medical attention and received prescription medication.

Employee claims left leg slipped behind them while exiting the restroom. Employee claims left knee made contact with the floor. Employee stated that there were no physical hazards present. Employee did not seek medical attention until 12/24/18, and provided a work restriction form on 12/26/2018.

Miner reported 03/15/2018 that they had sustained a work related injury to the back. The miner was moving/lifting a 4" rubber flex hose for the dust conveyance system in order to free a clog on the morning of 03/14/2018, when they noticed they were experiencing back pain.

Injured employee had just completed rigging a load on the 300 ton Link belt crane. EE made eye contact with the operator and asked if EE could come and talk to EE. While up on the deck talking to the Crane operator EE fainted and fell to ground and broke left clavicle. Employee was taken to Hospital where EE was found dehydrated and taking 3 meds for blood pressure.

EE reported feeling ill. Taken to local ER. Transported to Shannon Hospital for potential heart attack. No heart attack. Elevated heart enzymes. Dr deemed non-work related. Contacted Denver MSHA. Stated classified as "degree 8".

EE states was climbing stairs when left knee buckled and twisted.

03/17/14 Miner reported falling while descending screen house stairs, pain to right knee. 04/03/14 Miner requested to see doctor, released full duty. 05/02/14 Miner had follow up doctor apt. 06/12/14 Miner saw orthopedic doctor. Company was not notified of specialist 06/12/13 until 08/04/14. Injury meets reportable status on 06/12/14.

Miner was dismounting Bobcat 130 skid-steer loader when he lost his footing. Miner attempted to catch himself when he heard his shoulder pop, and he went to the ground. Miner was treated and released for a dislocated shoulder at the Heart of Texas Memorial Hospital ER.

Miner was moving vacuum hose while cleaning spilled material from the ground when he noticed pain in his back.

Miner was removing wet sand from the head box of the Truck Scale Conveyor when the rope holding the box in a raised position loosened causing the box to fall on the miner's right forearm.

Miner was removing wet sand from shakers on A Deck. When employee turned to move to the next access panel, his left toe caught on the I-Beam causing his knee to sprain/strain.

Employee manually handling 100# bags of sand and strained back. Did not report until 08/25/2010. Placed on restricted duty by doctor 08/26/2010.

Employee was brushing screens when a shaker door closed on his left shoulder. The employee was taken to the doctor and released without restrictions. The employee complained of continued pain and was seen by the doctor again on 03/18/2010 when he was placed on restrictions pending a MRI.

Replacing elevator bucket conveyor belting on #2 elevator, rock fell from belt during placement of new belt and struck miner's left thumb causing a fracture.

First aid incident 03/19/08 when tagline came free from load employee was handling and stumbled backward into guardrail. Seen by doctor & released to regular duty that day. 03/27/2008, employee back spasm getting into vehicle. Dr states result initial incident. Instructed by doctor stay home from work Friday, 03/28/2008. Employee returned work Saturday, 03/29/2008.

Miners were adjusting the timing of gates on the Lambert Palletizer Model 1200. Miners must be on equipment while in operation to make these adjustments. The guard had been removed from the left gate. The effected miner's left foot was pinned between the gate and the shaft when the gates were opened.

Miner states that he heard a load "squall". He shut off the belt to go see what was making the sound. He saw fire and smoke coming from the head box of the #4 belt. He attempted to fight the fire, but the fire grew. He called 911 at 2:10 pm. The fire was put out by the Brady Fire Department around 3:00pm.

Miners was lifting a damaged sand bag from a pallet, twisted and felt sudden pain in is back.

Employee was operating trackmobile. He stepped down to the ground and twisted his right knee. Note: This report is being file because employee has sought medical attention and has not returned to work.

Employee was spotting railroad hopper cars using a track mobile. He jumped down from the top rung of the ladder to exit the cab and fell backward when he hit the ground. He injured his vertebrae.

EE WAS USING A UTILITY KNIFE TO CUT CONVEYOR BELTING MATERIAL TO USE AS A LINER FOR A BELLY PAN. EE WAS PULLING THE KNIFE THROUGH THE MATERIAL WHEN IT SLIPPED OUT OF THE CUT AND WENT ACROSS HIS RIGHT UPPER LEG. EE RECEIVED STITCHES AND RETURNED TO WORK.

EE WAS OPERATING BAGGING MACHINE FROM A SEATED POSITION. HE ROSE TO STEP DOWN AFTER FINISHING A PALLET OF SAND BAGS. HE DID NOT PLANT HIS RIGHT FOOT SQUARE AND ROLLED THE ANKLE AS HE PUT HIS WEIGHT ON IT, CAUSING A SPRAIN OF THE RIGHT ANKLE.

THIS IS A REPORT OF POSSIBLE OCCUPATIONAL INJURY. ON 8-16-2004 THE MINER RECEIVED NOTICE THAT A CT SCAN INDICATED EARLY STAGE SILICOSIS. THE FILING OF THIS FORM 7000-1 IS NOT TO BE CONSTRUED AS ADMISSION BY OPERATOR THAT AN OCCUPATIONAL RESPIRATORY ILLNESS HAS OCCURRED.

EMPLOYEE WAS WALKING ALONG THE TOP OF THE RAILROAD CAR WHEN HIS LANYARD HUNG UP ON A RAILING PULLING THE EE BACK & CAUSING HIM TO FALL. EE WAS BRUISED & SCRAPED ON LEFT ARM.

Employee was standing on top of an enclosed rail hopper car straddling the top hatch door. He stopped over door and lifted it open with both hands injuring his right elbow.

MAINTENANCE CREW WAS INSTALLING A NEW RUBBER PIPE. HE OFFERED TO HELP AND POSITIONED HIMSELF SITTING ON TOP OF THE 20-40 BIN, GRASPING THE 8" PIPE AT THE BOTTOM. WITH 3 OTHER WORKERS THEY AT TEMPTED TO RAISE THE VERTICALLY-ORIENTED PIPE, BUT REQUIRED ADDL. POSITIONING TO FIT IT ONTO THE BIN FLANGE. AS THEY LIFTED, EE FELT A "POP" IN HIS SHOULDER AND A SEVERE PAIN.

EMPLOYEE WAS ASSISTING MAINTENANCE CREW INSTALL CONVEYOR BELT SCALE. HE WAS LIFTING HEAVY OBJECTS INCLUDING TROUGHING ROLLER SETS. EMPLOYEE NOTICED PAIN IN LOWER BACK TOWARD THE END OF THE SHI FT. HE SOUGHT MEDICAL ATTENTION AND WAS UNABLE TO RETURN TO WORK THE FOLLOWING WORKDAY.

EE WAS BRUSHING SCREEN WHEN FOOT SLIPPED CAUSING HIM TO TWIST BACK.

DRIVER WAS IN THE PROCESS OF UNLOADING HIS TRUCK WHEN HE STUMBLED, FALLING AGAINST THE FENDER OF THE TRUCK TRACTOR. POSSIBLE TRIP OVER SLIGHT RISE IN DECKING AT DUMP DRIVE OVER. FATIGUE.

EARLY MORNING DARKNESS AND DEW CONDENSATION. EE WAS EXITING LOADER TO PICK UP PERSONAL RADIO AT EE DAY ROOM. HE STEPPED OUT ONTO DECK TURNED TO DESCEND LADDER & SLIPPED ON OILY GRIME SLID OFF LADDER HITTING LEG, LOST GRIP ON HAND RAILS AND FELL TO GROUND. LANDED ON LEFT SIDE CAUSING AN INJURY TO SHOULDER.

TAKING TOOLS OUT OF TOOL BOX & LID FELL ON HIS RIGHT WRIST

TAKING ALTERNATOR OFF OF TRUCK BECAME DISORIENTED MOVED TO SHADE AND WET DOWN BECAME ILL AND VOIMITED.

PULLING PALLET OUT OF PALLETIZER MAGAZINE AND APPARENTLY DISLOCATED RIB ON LEFT SIDE.

SAND IN LEFT EYE.

BRUSHING SCREENS AND NOTED THAT GATE ON SCREEN MAY BE HUNG. STEPPED UP ON EDGE OF SCREEN ELEVATING FEET ABOVE STAIRS, APPROX. 1'. REACHED TO FEEL IF GATE HUNG AND SLIPPED OFF SHAKER. APPARENTL Y TWISTING BACK.

ATTEMPTED TO ROLL DRIVE BELTS TO CHECK FOR WEAR., WHEN FINGER WAS CAUGHT BETWEEN BELT & SHEAVE CAUSING INJURY TO LEFT RING FINGER.

USING BAGGER AND PALLETIZER REPEATEDLY WHEN LEFT SHOULDER BECAME SORE. REPORTED 5-15-01, WENT TO DOCTOR 5-3-01.

HE WAS UNHOOKING CRANE BALL FROM CONVEYOR COVER, CAUGHT INDEX FINGER BETWEEN BALL & COVER-MASHED LEFT INDEX FINGER.

EMPLOYEE WAS LOADING STEEL BALLS ON BELT, ONE BELT SLID AND MASHED RT MIDDLE FINGER TIP AGAINST ANOTHER BALL. FRACTURE TO FINGERTIP UNDER FINGERNAIL.

AFTER LOADING A BULK TANKER, HE LIFTED THE LOAD OUT SPOUT AND PULLED A MUSCLE BELOW HIS LEFT SHOULDER BLADE.

RIGHT SHOULDER STRAIN MOVING BAGS ONTO PALLETIZER. REPORTED 7/23. WENT TO DOCTOR 8/2.

USING UTILITY KNIFE TO CUT A PIECE OF RUBBER TO LINE A CHUTE. KNIFE SLIPPED CUT LEFT FOREARM REQUIRED STITCHES.

WORKING IN SCREENHOUSE, FELL FORWARD, BENT LEFT RING FINGER BACK, DISLOCATED IT AT MIDDLE KNUCKLE.
