Employee stated that he was removing the bolts from the lip saver on a loader bucket. Holding bolt head with right hand while using air wrench with left hand. The bolt spun in his hand causing laceration to right middle finger. Reportable due to requiring sutures.

Mostrando los 6Employee stated that he was laying on his left side using a cutting torch to cut out bogie pins on dozer roller frame. A piece of metal slag popped off and into his right ear. To ER doctor-burn, ear drops. To comp doctor on 12/6 & 12/13 - puncture ear drum. To ENT doctor on 12/16-same diagnosis. Talked to MSHA/Denver 12/22 and report as first aid case per them. No lost time.
Putting on downspout bucket on end of auger, truck operator moved auger too quickly, pinched end of ring finger.
Employee stated that he was operating a dozer pushing out shot rock/material. He backed the dozer up over a rock ledge. When the rear of the dozer sat down, it jostled the employee causing his left side strike the arm rest on the seat, resulting in a fractured left rib.
Employee stated that he was operating a dozer pushing out shot rock. The right front corner bit of the dozer blade slipped out of the tight rock causing it to jar the employee in the cab resulting in pain in back area.
Employee stated that he was using an impact wrench to tighten the bolts on the cutting edges which he had replaced on a D11R dozer. He said he felt a burning sensation in right shoulder area and some numbness in his right hand toward the end of the work day. He did not report this as a work related injury until 8-11-06 when his doctor said it was work related.
