No 27 Strip Coal

Employee stated was climbing up ladder to get on hauler. EE was standing on the bumper or step above so as to set thermos onto the catwalk. As EE did, hand slipped off the hand rail and EE fell backwards onto the ground. EE landed on back resulting in nine fractured ribs to the right side.

Employee was walking across shot rock to the foreman's truck when their right foot slipped off of a rock and into a crevice. This caused the foot to bend backwards. Went to ER that night to get checked due to increasing pain. X-rays were negative but lost time started on 07/25/17.

Employee stated that he was operating a hauler and was being loaded with mine spoil. The loader operator accidentally dropped a large rock into the hauler bed causing it to jar the employee. This motion caused pain in his neck area. Went to ER later in shift to get checked. Missed the next work shift, thus reportable.

Employee stated that he was getting up into the hauler. The first step was a little high off of the ground. He grabbed the handrail to pull himself up the ladder. When he did, he felt a sharp pain in his right shoulder. Went to ER that night to get checked. Kept off of work for the next two days.

Employee stated that he was operating excavator moving spoil material. A rock rolled out of the spoil and hit a foot long stick laying on the ground. The stick flipped into the air and through the windshield cab guard and busted the windshield. A foreign body got into his right eye. Would not flush out so he went to doctor to have removed. Given eye drops (used for 2 days).

Employee stated that he was operating dozer in the shot when he got off for a moment. When he was getting back on the dozer, his foot was on the step on the push beam and his hand was on the hand hold. As he was pulling himself upon the track, his hand slipped off causing him to fall back onto the ground. Had some back pain, went to ER, and missed one shift of work on 5/2/2014.

Employee stated that he was helping the mechanic put the bottom foot plate on the left leveling jack on the drill. When he raised up, he hit the back of his head on the leveling jack causing a head injury. Has not missed any work shifts but is reportable due to being on light duty.

Employee stated that he was removing the bit ring from the drill bit/sub with a hammer. As he was striking with the hammer, a piece of metal flew off, struck and imbedded in top of right hand behind thumb. Became reportable on 5/16/13 when he decided to have it surgically removed and lost 2 days work.

Employee stated that he was using a pry bar to lift up on the dozer track so as to put it back together. When he let loose of the bar, he felt pain in between the shoulder blades. Went to doctor on 8/20/12. No lost time but is on work restrictions.

Employee stated that he was changing cutting edges on dozer. He was using a grinder, cutting torch, and air wrench. Sometime during this process, a piece of metal got into his left eye. Noticed it later that day but could not rinse it out. Went to doctor on 1/26/12 & had it removed from eye.

Employee stated that he was pushing out shot rock with his dozer. As he was backing his dozer, he ran over a rock causing the dozer to shift and become unbalanced. This motion jarred the employee resulting in low back pain. Went to doctor on 1/24/12 & became reportable.

Employee stated that he was digging rock off of the coal seam. The loader struck some tight rock and came to a sudden halt causing a whiplash type motion to the employee's neck area.

Employee stated that he was removing the bolts from the lip saver on a loader bucket. Holding bolt head with right hand while using air wrench with left hand. The bolt spun in his hand causing laceration to right middle finger. Reportable due to requiring sutures.

Employee stated that he was getting out of his mechanic's truck. As he stepped onto the ground, he stepped onto a ridge of dirt and sprained his right ankle.

Employee stated that he was aligning sprocket and chain up with hole in the pull down cylinder. He was moving a shim in the hole when the sprocket fell down on top of his left middle finger causing a laceration. No lost time but reportable due to having to glue/suture laceration.

Employee stated that he was laying on his left side using a cutting torch to cut out bogie pins on dozer roller frame. A piece of metal slag popped off and into his right ear. To ER doctor-burn, ear drops. To comp doctor on 12/6 & 12/13 - puncture ear drum. To ENT doctor on 12/16-same diagnosis. Talked to MSHA/Denver 12/22 and report as first aid case per them. No lost time.

Reported 9/8/10 - attempting to hook up hose from tanker to tractor to blow off product, slipped and fell backward hit catwalk and landed on tail bone.

Employee stated that he was helping the mechanic change the cutting edges on a dozer blade. He bent over to pick up an air impact wrench. As he picked it up, he felt a sharp pain in his back going down into his left leg. He did not report the injury until 11/09/2009 when he incurred lost work days.

Employee stated that he was operating a loader in soft material. The swaying/rocking motion of the loader in this material caused pain in lower back area radiating down his legs. Missed first work shift on 5/21/08 thus being a reportable injury.

Employee stated that he was helping the mechanic to lower the pull bar on a fuel tanker so as to hook it to his dozer in order to move the tanker. As they were pulling down on the bar, it fell and struck the employee's leg causing a deep laceration above his right knee.

Employee stated that he was operating a loader in the strip pit when the loader bucket struck the highwall causing loader to stop abruptly. This jarred the employee causing low back pain. Not reportable until lost time incurred on 10-24-06.

Employee stated that he was operating a dozer pushing out shot rock. The right front corner bit of the dozer blade slipped out of the tight rock causing it to jar the employee in the cab resulting in pain in back area.

Moving an Auger around on a shot loader when a rock fell down off the high wall striking left thigh & forehead

Loading powder truck, he bent down to go under the anfo bin, straightened up, felt something pull in neck.

Employee stated that he had parked his truck, set brake, & got into truck bed to look for a pin in tool box. The truck started sliding and slid into rear end of a parked tractor trailer. Employee was thrown into the side of the tool box.

Job mechanic had removed the old filtering system from the diesel fueling station so as to install a new larger system. Some spillage resulted. As he proceeded to weld on the bumper of the fueling station, the diesel spillage caught on fire and spread along the station. He could not control the fire in time by extinguishers, thus destroying the fueling station.

Employee stated that he was loading a truck with bags of slurry. He reached down and picked up 2 bags of slurry (40lbs each bag), one in each hand. He raised up and turned to go toward the truck with the slurry bags. As he did, he felt pain in his lower back area.

EMPLOYEE WAS OPERATING A HAULER WHICH WAS USED TO HAUL SPOIL FROM THE PIT AREA TO THE DUMP AREA. DURING THE HAUL, THE SEAT IN THE HAULER BOTTOMED OUT JARRING HIM, RESULTING IN PAIN IN HIS LOWE R BACK AREA. LOST TIME WAS INCURRED STARTING 3-4-04.

EE STATED THAT WHILE HIS HAULER WAS BEING LOADED WITH SHOT MATERIAL DIRT OFF A SPOIL BANK BROKE LOOSE AND FELL DOWN AGAINST THE HAULER CAUSING IT TO BOUNCE THE EE AROUND INSIDE THE CAB. THIS RESULTED IN A WHIPLASH TYPE OF INJURY TO THE EE NECK/SHOULDER AREA.

EMPLOYEE STATED THAT HE WAS OPERATING A HAULER IN THE MINE PIT. AS HE WAS BEING LOADED, THE FRONT END LOADER TIRE RAN UPON A ROCK ON THE GROUND CAUSING THE LOADER BUCKET TO STRIKE THE CAB SHIE LD ON THE HAULER. THIS RESULTED IN THE HAULER JARRING THE EMPLOYEE.

TWO EMPLOYEES WERE REMOVING THE BOTTOM ENGINE GUARDS WHEN THE END OPPOSITE INJURED FELL TO THE GROUND CAUSING THE GUARD TO HIT HIM IN THE RIGHT HAND. THE GUARDS WERE HEAVIER THAN NORMAL DUE TO MUD AND DIRT. DUE TO THE LOCATION OF THE GUARD,A CRANE WAS NOT AN OPTION AND HAD TO BE REMOVED BY HAND (TWO MEN).

EE STATED THAT HE WAS CHECKING THE LOADER FOR AN OIL LEAK. AS HE OPENED THE DECK LID TO LOOK, HE CAUGHT HIS FINGER BETWEEN THE DECK LID AND HANDLE, MASHING HIS FINGER. WHEN THIS HAPPENED, HE J ERKED HIS FINGER OUT CAUSING A FRACTURE TO THE TIP OF HIS RT. RING FINGER. NO LOST TIME INVOLVED BUT REPORTABLE DUE TO FRACTURE.

EE WAS CHECKING A LOADER RADIATOR THAT WAS OVER-HEATING. THE RADIATOR CAP WAS NOT PROPERLY SECURED. IT WAS HOT TO THE TOUCH SO HE STEPPED ON TOPOF IT WITH HIS FOOT. AS HE DID, THE CAP BLEW OFF AND HOT FLUID/STEAM BLEW ONTO HIS FACE & ON THE RIGHT ARM.
