Employee was caught between long wall shield tip and mine rib. Compliance with rules and regulations was not a factor. Lost time as 11/07/2016

Working on C side & had left hand behind bracket & took impact tool to run bolt in. Bolt went through bracket and smashed left small finger. Cause of injury hand placement. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor.

Was helping end loader operator look for leak under machine, machine suddenly dropped pinning between end loader frame & mine floor. Cause of injury machine not blocked. Compliance with company rules/regulations was a factor, miner proficiency may have been a factor. Started missing work 6/8/16.

Employee was roof bolting on the 10 East Headgate at face. Roof bolting steel slipped injuring middle left finger. Restricted Duty.

Employee was spraying shear drum with water using the road hose when rock flew off drum and smashed hand on pan line. Cause of injury was hand positioning while cutting rock. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may have been a factor.

During roof bolt installation employee injured the 2nd and 3rd digit on left hand causing laceration. (Medical treatment)Compliance with rules and regulations was not a factor.

Employee was handling rock dust bull hose on RC-09 when hose inadvertently struck the employee on the left side of the face and hand causing laceration. Lost time injury

Employee was trying to break a tail gearbox loose when a bolt from the lifting eye broke striking him above the left eye causing laceration. Employee had sutures above left eye (Medical treatment). Compliance with rules and regulations was not a factor. Miners proficency and job skills may have been a factor.

Crew member was operating chainsaw with chaps on, clearing right of way for power line supply of fresh water for prep plant. When cutting saplings the EE made contact with the chainsaw in his left calf causing a laceration req. 10 stitches to close.

Employee was using channel locks to loosen pinner steel to remove from drill pod. Employee accidentally touched the control levers causing the steel to turn striking the employee's left arm causing fracture. PPE and mining equip/systems not a factor. Employee was not paying full attention to task at hand therefore compliance with rules and miner proficiency was a factor.

Employee was in x-cut loading supplies from the side of roof bolter. When the other roof bolter operator tried to raise the ATRS jack he inadvertently made contact with other employee toes of right foot with stab jack. Lost Time as of 12/26/2014

Employee was installing 10' cable bolts on the 13 West Head Gate and smashed left ring finger between the shaft of the bolt and the mast of the roofbolter. Restricted Duty.

Employee was operating a Gopher Fatboy air drill in the belt entry of the First Submain North beltline. The drill steel curved striking employee on the left side of face.

Employee was working on longwall face and pressurized hose struck him in the left arm and left eye. Restricted duty.

Employee was working on the longwall face and was struck in the forehead by a chain causing laceration. Medical Treatment.

While employee was advancing shields, reached down to pick up rock dust bag and the employee right leg came into contact with shield and pan line. Went on restricted duty on 3-26-14

Employee was using a cutting torch in the underground shop and a spark fell in left ear. Lost time as of 03/14/2014.

Employee states he was pulling on rib bolt to remove from the predrilled hole and smashed his right hand between the wrench and rib bolt as the rib bolt became dislodged. Medical Treatment only.

Employee was installing cable bolts and as he was drilling the hole the steel bowed (angled) and struck employee in the right arm.

Employee states he pushed roof bolt up into the drilled hole in the mine roof for installation. The roof bolt became loose and struck employee's left arm. Medical Treatment only.

Employee was struck in the forehead by a cutting wheel that had broken in half. The employee received 1 stitch as a result of a laceration. Compliance with rules and regulations, involvement with mine equipment or mine systems, or protective items are not a factor. Job skills and miner's proficiency may be a factor.

Employee states while swinging the left side boom of the roofbolter in towards the center he inadvertently pinched left thumb between cylinder and the machine.

Employee was roof bolting the last row of bolts in the face and as the mast was lowered down the wrench came in contact with the employee's mouth. Medical Treatment only employee received sutures.

Employee was holding T-channel in place and when the ATRS on the bolter applied pressure, employee's right middle finger became caught between the roof and the T-channel. Medical treatment injury

Employee states he was operating the continuous miner when he was struck in the chest by a rock. Medical treatment injury. Revised: As of 10/22/2012 became reportable lost time injury due to surgery.

Employee was roof bolting and as he pulled a drill steel from the roof, his forearm came in contact with T-channel. Medical treatment injury.

EE stated that while bolting, he extended the boom and it caught on the track and threw him upward. When he used his right arm to brace himself he felt pain in his right shoulder and elbow.Reportable lost time injury as of 8/15/12 due to surgery.

Employee was roof bolting and inadvertently smashed his finger performing his job. Medical treatment injury.

EE was bolting when a plate slid down and struck his right wrist. Medical treatment injury.

Employee was advancing shields when a secured DA Ram shifted, striking employee's left ankle. Medical treatment injury.

EE was in the process of advancing longwall shield. A damaged DA Ram had just been replaced and was secured to the advancing shield awaiting removal. When the shield was advanced, the damaged DA Ram shifted,striking EE's foot. Medical Treatment injury

Employee was installing a roof bolt when his right thumb became caught between the bolt and drill mast. Medical treatment injury.

Employee was installing a cable bolt when it became jammed and struck employee in the face. Lost time injury.

Employee was driving a construction tractor under a tin shed. The canopy struck the tin shed, causing it to fall in the cab and cut employee's right arm. Medical treatment injury.

Employee was raising a roof bolt into the roof with the chain hanger plate when his right little finger became caught between the bolt and plate. Medical treatment injury.

Employee was standing next to the shield and when an attempt was made to raise the feet on the shield to walk the shield out of a muddy area the shield jarred and came in contact with employees right arm. Lost time injury

EE was helping retract the long wall panline when the panline separated causing the gobside dogbone to come out of it's pocket with the keeper striking the EE's right leg.

Employee was pulling shields over the lead drum when the drum picked up a rock and struck him in the nose. Medical Treatment

Employee stated he was roof bolting, dropped his steels from the roof and reached for them with his left hand while his right hand was still on the control lever. He stated the motion of reaching caused him to push the lever, pinning his left ring finger between the steels.

EE was operating a dozer when he began to feel a tightening from his neck to his tailbone. He drove down off the coal pile and existed the dozer. The tension became worse so he was taken the the ER by ambulance.

Employee was installing a cable bolt when he inadvertently struck the joy stick, causing the bolt to spin and struck employee on his right wrist and forearm. Restricted duty injury.

EE had bent a roofbolt and prepared to drop boom when he accidently back spun the wrench striking his right wrist with the bolt.

Employee was pulling shield 120 from shield 119. Shields 119 and 120 had a loose relay bar so when employee pulled shield 120, it pinned it into the pan, also pining employees left ankle between the relay bar and the pan. Lost time injury.

EE was roof bolting and has he removed the first pin he slipped and his right hand went to the pinner boom. The steel slipped out of the roof, striking his right index finger.

Employee was installing a roof bolt when he placed his hand between the roof and plate. Medical treatment injury (10 stitches).

EE had been bolting and swung the bolter arm in at the same time the other operator moved the machine in. The cat caught the platform, causing the platform to jump. This threw the EE into the canopy. Employee became a lost time injury on November 15th due to surgery.

EE was drilling when the auger hung up. When the drill came out, the handle swung around and hit EE on the right side , knocking him off his feet. Lost time injury.

Employee was roofbolting. As he prepared to put the second steel on, his intention was to raise the mast. His gloves were wet and his hand slipped off and bumped the clamp lever which closed on his middle finger and thumb. Medical treatment injury

Employee was roofbolting when a roofbolt bent. It smashed his finger between the bolt and the mast.

Employee was roofbolting when the t-channel plate struck his left wrist. Medical treatment injury (5 stitches).
