Employee was tightening the compression ring on a bearing with a co-worker. The co-worker was using a punch hammer and sledge hammer when a piece of metal broke off of the punch hammer and went into the injured employee's left arm.

Contract employee was shearing belt with shears when they slipped from grip and lacerated a thumb. The following were not a factor: rules, regulations, mining systems or equipment, training, protective clothing or devices. Attitude may have been a factor.

Employee was picking up lid on PC 79 and smashed left hand causing laceration.

Employee was trimming edge of belt and inadvertently cut left thumb with belt knife. Medical Treatment.

Employee was pulling floor pump pipe on C floor. Pipe became loose striking employee's left foot causing fracture. Restricted Duty.

Employee was reclaiming belt. The employee slipped next to reclaim sled causing a fracture to employee's left foot.

Employee was looking for obstruction between overland belt & chute, while climbing off belt, slipped and fell to ground. Cause of injury no 3 point contact while climbing down. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 11/22/2016

Employee was shoveling on plant feed belt & picked up big load of mud on shovel, when turned to put on belt strained back. Cause of injury to big of load & body positioning. Employee had unloaded supplies off fork truck. Went back & stepped onto fork truck, got to second step on ladder, turned & foot slipped, toe went into mud on ground & fell forward. Employee was lifting a 13 HP flyte pump when felt a pop in shoulder in left arm. Cause of injury wet & muddy conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/27/2016.

Washing guarding at slope tail, coal loosened under guarding around tail roller dropping guard on left hand. Cause of injury, hand placement. Was working in slope swinging a sledgehammer busting concrete. broke through concrete & smashed finger between handle & concrete. Cause of injury not paying attention work area. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. This is a Contractor for REDBIRD construction.

Strands were found broken on the regular lay slope hoisting rope. Rope was taken out of service and corrected. This was the secondary escape way and was rerouted underground to another hoist until rope was corrected.

Working on C side & had left hand behind bracket & took impact tool to run bolt in. Bolt went through bracket and smashed left small finger. Cause of injury hand placement. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor.

Roof fall occurred on the 5 seam air course. Location is 750W 525S. An action plan was submitted and approved by MSHA to install EP's at approved locations. The fall will be referenced as 33A

Walking down slope East side & slipped on wet cement floor & struck knee on metal plate. Cause of injury floor wet & on incline. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor.

Employee was walking along the first submain North take-up area at the drive. The employee slipped falling on right knee and shoulder. Lost time as of 2/16/2017.

Was spreading rock dust that was on travel way with drag attached to ram car. Was watching drag when a piece of T-Channel on rib hit me. Cause of injury T-channel loose on one end sticking out. Was moving fan spacer with another employee, put end on trailer & when helper put end down finger was caught between spacer & trailer. Cause of injury was communication with helper. Was washing out #3 feeder in #2 reclaim when a large chunk broke free & wedged between feeder belt & catwalk. Catwalk raised & lost balance falling from platform. Was down in the #1 reclaim tunnel on the #4 feeder landing changing a nozzle on hose to a wand. Feeder broke free causing surge of material to pin against hand rail of the platform. A methane ignition occurred on the 12WLW MMU-059 at approximately 7:45 pm on 06/08/2016. The ignition was on the headgate drum while shear was mining to the headgate end. This 7000-1 is being submitted to terminate citation #9103414.

Was helping end loader operator look for leak under machine, machine suddenly dropped pinning between end loader frame & mine floor. Cause of injury machine not blocked. Compliance with company rules/regulations was a factor, miner proficiency may have been a factor. Started missing work 6/8/16.

Employee was shoveling in the slope. EE noticed back starting hurting. Cause of injury bad body positioning while on incline. Was in process of weekly examination of bleeders & was walking in 10WTG when I stepped over loose rock & twisted knee. Cause of injury was wet conditions. Alleged incident was to have occurred while attempting to wash down the slope. Walking backwards and slipped on oil. Cause of this injury was employee awareness. Employee was picking up a nitrogen bottle to put in the 4 bucket of scoop for transport. Employee twisted & felt a stabbing & burning in stomach. Cause of injury was twisted body with heavy load. Employee was making pre-shift examination when ee stepped out of vehicle at the 11WHG and stepped on a piece of concrete block & fractured foot and ankle. Was pulling a charge pump onto landing when I slipped & holding onto the pump so I wouldn't fall I strained my back. Cause of injury was wet conditions. Changing out DA ram cylinder. Very tight fit in relay bar. Laying on stomach with 12 # sledge arms extended forward driving ram to relay bar pocket. Felt a twinge in shoulder. EE was carrying a screw prop on right shoulder beside stage loader. EE tripped on belt rail. EE tried to throw prop off shoulder and ran head into pump & landed on left shoulder. Cause of injury was wet/muddy conditions. Incident did not involve any aspect of compliance with rules, regulations,& PPE. Miner proficiency may be a factor. Started to miss work on 2/11/16.

Employee was taking a piece of metal to grinder. Stepped over a roll of lagging & caught foot on lagging causing ee to trip & fall. Landed on right shoulder & arm. Cause of injury not having a clear walk way. Incident did not involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may be a factor. Started missing work 2/25/16.

Employee was working on Longwall & tripped. Employee fell with arms stretched outward & landed on right wrist/hand. Cause of injury was gob in front of shield feet. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may be a factor. Started to miss work 2/5/16.

Employee was riding out in a man trip when man trip hit a hole in travel way causing employee to be bounced around in compartment. Cause of injury was an undetected hole in travel way. Incident did not involve any aspect of compliance with rules and regulations & PPE. Mine equipment involved & miner proficiency may be a factor. Started to miss work 2-9-16.

Employee was walking in front of headgate shields. Bent over in low height. Straightened up by stage loader and noticed sharp pain in lower back. Cause of injury was low top & muddy conditions. Incident did not involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may be a factor. Started missing work 2/4/16.

Employee was rearranging items on a pallet in preparation to send underground. The employee moved the pallet and was struck in the chest with a pallet board causing injury.

IE stated that he was helping move/install water pipe using a come-along tied to a rope. The rope broke, allowing come-along to fly back and strike employee

Working on setting up a long wall recovery and bent down to position a bridge and felt pain lower back.

Employee hand slipped off of shield canopy and struck a piece of tin laceration his right wrist resulting in 5 stitches.

Employee tripped while walking next to scoop injuring left shoulder. Employee became a lost time as of 9/24/15.

An unintentional roof fall occurred in the 5 seam bottom area at the 525 n. and 825 w.in the mine examiners weekly air course.

Employee was changing a sieve on B-1 deslime screen when deflector slipped and pinned his right hand between the feed box.

Employee was going through man door with hand tools in both hands when he noticed the door was about to slam shut. He tried to stop it and got his finger caught between the door and the frame. He received 10 sutures.

An unplanned roof fall occurred at the 1585' mark in the 10 west tailgate entry number 2. The area had been mined in October 2012 and the fall occurred in a three way intersection. The area had previously been supported with 8' torque tension bolts in t-3 channel as the primary roof bolt with 12' cable bolts between the existing rolls of the primary bolts.

Employee was hanging a belt roller when roller fell out smashing his hand. The employee had 5 sutures in palm of left hand.

Employee state he was using flight straps to uncover winch when his right middle finger became caught between the winch and the strap.

Employee was handling rock dust bull hose on RC-09 when hose inadvertently struck the employee on the left side of the face and hand causing laceration. Lost time injury

Employee was walking at the face of the 12WHG in the #3 entry to sign examination tag. Employee slipped and brushed right hand against pie pan causing laceration. Medical Treatment.

Crew member was operating chainsaw with chaps on, clearing right of way for power line supply of fresh water for prep plant. When cutting saplings the EE made contact with the chainsaw in his left calf causing a laceration req. 10 stitches to close.

Employee was using channel locks to open a metal band to access T3 channel. Metal band while opening struck employee's right arm causing a laceration. Medical Treatment.

Employee smashed right pinky finger between shear drum and pipe while changing drums out on longwall shear. Medical Treatment.

Employee was using channel locks to loosen pinner steel to remove from drill pod. Employee accidentally touched the control levers causing the steel to turn striking the employee's left arm causing fracture. PPE and mining equip/systems not a factor. Employee was not paying full attention to task at hand therefore compliance with rules and miner proficiency was a factor.

Employee was using a utility knife on the surface in the staging area cutting thru a zip tie causing a laceration to right index finger. Medical Treatment.

Employee was in the Main West travelway xcut 47 replacing damaged roof bolts, slipped causing injury to right foot. Restricted Duty.

Employee was removing belt blocks after completion of a belt splice on the 2nd North East Submain belt. Employee moved the ratchet and the belt block struck employee on the right side of lower jaw.

Employee was working in the underground shop, assisting in picking up a transmission for PV57. Employee lost footing slipped injuring right shoulder. Lost time as of 1/28/2015.

Employee was walking to the roof bolter lost footing and fell to the mine floor striking nose. Medical Treatment

Employee was loading a ramcar with 6" pipe. Employee slipped allowing the left knee to bump the 6" pipe. Restricted Duty as of 01/06/2015.

Employee was using cum-a-long to move a gear case. Employee's left index finger came in contact with chain hook causing laceration. Medical Treatment.

Employee states he felt pain in groin area when he picked up a stack of bow tie plates while loading roof bolter. Lost time injury.

Employee was placing T3 Channel on battery scoop. T3 Channel slipped and struck employee's left hand causing fracture to middle left finger. Lost time injury.

Employee was plastering stopping and a piece of rock next to stopping struck employee's left shoulder. Lost time as of 2/9/2015.

Employee was assisting moving a prop tripped and the prop landed on employee's middle right finger causing a fracture. Lost time accident.

Employee was walking off of the longwall face and stepped in a hole injuring lower back. Lost time as of 1/28/2015.

Employee was cutting a piece of steel and a small piece of metal bounced in to left eye. Medical Treatment.

Employee was installing 10' cable bolts on the 13 West Head Gate and smashed left ring finger between the shaft of the bolt and the mast of the roofbolter. Restricted Duty.

Employee was removing metal bands from T3 channel and the metal band cut employee's right arm. Medical Treatment.

Employee was operating a Gopher Fatboy air drill in the belt entry of the First Submain North beltline. The drill steel curved striking employee on the left side of face.

Employee was loading a bundle of 8' timbers and one 8' timber rolled off of bundle striking employee's left foot. Lost time injury.

Employee was removing a stopping left hand was struck by concrete block causing a laceration.

Employee was pulling oxygen tank on cart back to the appropriate area and felt pain in right knee. Lost time as of 10/10/14.

While entering a PV the employee sat down and accidentally forced SCSR into his own rib cage fracturing one rib. Medical Treatment.

Employee was at the 10WLW VFD area at the belt drive utilizing the mine phone. Employee while on the mine phone tripped over a golf cart twisting left knee. Restricted Duty as of 8/25/2014.

Employee was cleaning spillage under the refuse bin and was struck in the back of neck by small debris causing laceration. Medical Treatment.

Employee was working on the longwall face and was struck in the forehead by a chain causing laceration. Medical Treatment.

Employee was removing material door frame and a concrete block struck right hand fracturing middle finger. Medical Treatment

Employee was attempting to rearrange tin shed, tin slipped causing laceration to left index and middle finger. Lost time as of 08/11/2014.

Employee was walking to the face of the 10WLW and twisted left knee. Lost time injury as of 8/26/2014.

Employee was working in the 10 West tailgate lost footing causing left foot to turn resulting in injury. Restricted Duty.

Employee was operating the longwall shield and a piece of rock struck employee's chin causing a laceration. Medical Treatment.

Employee was installing lifeline and while cutting a portion of the lifeline he accidentally cut left forearm. Medical Treatment.

Employee was removing a lower rub guard on a roof bolter and smashed right index finger causing a laceration. Medical Treatment.

Roof Fall occurred in the #1 entry of the 10WLW of the belt entry. The fall location starts outby at 5553' to 5586' and dimension is 12'T x 33'L x 18'W. The roof fall will be cleaned and re-supported according to the MSHA approved plan submitted and approved on 17JUL14.

Employee was removing rock from the walkway and his right hand was struck by a piece of rock causing a hand fracture. Restricted Duty.

Employee was working with tin shed in order to direct water for pumping. The tin came in contact with employee's left hand resulting in a laceration. Medical Treatment.

Employee was rearranging tin shed and left arm came in contact with tin causing a laceration. Medical Treatment.

Employee was walking in the #2 entry at the face in the 12WHG and stepped on the continuous miner cable injuring left foot causing small fracture. Restricted Duty.

An unintentional roof fall occurred in the 10WLW travel road between xcuts 36 and 35 at footage marker 4975 in the #2 entry. The dimensions of the roof fall is approximately 50'L x 10'T x 18'W. The roof fall was cleaned up. Roof fall is referenced as #31A.

Employee was operating the continuous miner and a rock from the roof struck employee in the left arm causing laceration. Medical Treatment.

Employee was hanging six inch water pipe and brought arm down on rib bolt plate causing laceration to right arm. Medical Treatment.

Employee was operating a Kubota on the travel road. The front tire caught a notch in the rib due to the employee glancing over in a x-cut while driving and the Kubota turned over on its side. The employee struck head on the roll over protection bar. Restricted Duty.

EE was operating a continuous miner on MU 3 section finishing the clean up in 4 right. According to unit EE's the injured EE was under supported top when found. The injured EE was concentrating on the clean up prior to the accident. Apparently a rock from the mine roof located at 4C fell out bounced off the floor then flipped over striking EE.

Employee was utilizing a utility knife trimming on a conveyor surface belt and inadvertently cut left arm causing laceration.

Employee was building crib and smashed end of right ring finger with crib block. Medical Treatment.

Employee was working in the belt slope at the 6 seam chute. Employee slipped and struck forehead on mine floor causing a small laceration. The employee received 3 sutures. Medical Treatment.

Employee was closing up an amon door on a Fairchild scoop and caught the middle right finger between the amon door and the frame. Medical Treatment.

While performing maintenance employee was removing primary scraper from Main North drive and his left middle finger was pinched between the scraper and the chute. Medical Treatment.

Employee was walking from parking lot to Prep Plant, slipped causing slight fracture to right foot. Restricted Duty.

Employee was using a cutting torch in the underground shop and a spark fell in left ear. Lost time as of 03/14/2014.

Employee was riding in mantrip #8 in the 10WHG travel road. The mantrip struck a pothole causing the employee to feel pain in lower back. Lost time injury.

Employee was assisting in removing pin from center section of Coal Scoop 33 and his right thumb was struck by sledge hammer handle causing hairline fracture.

Employee was operating the continuous miner and a piece of rock from the immediate roof struck employee on the left ear causing a laceration.

The employee was guiding a top idler frame into place with a sledge hammer and inadvertently struck his own left middle finger causing a laceration. Medical Treatment only.

Employee was operating a shuttle car and was struck by small piece of rock on the right side of face causing laceration. Medical Treatment only.

Employee states he was pulling on rib bolt to remove from the predrilled hole and smashed his right hand between the wrench and rib bolt as the rib bolt became dislodged. Medical Treatment only.

Employee was operating a ram car and ran over a concrete block in the travel road causing pain to lower back.

Employee was installing cable bolts and as he was drilling the hole the steel bowed (angled) and struck employee in the right arm.

Employee was making a shuttle car cable splice and inadvertently cut thru his own glove injuring left index finger. Medical Treatment only.

Employee was loading scoop with roof bolting material and a rock rolled out from the hog panel and struck the employee's left arm. Medical Treatment.

Employee was working on a scoop that was separated at the middle located in the underground shop. A different scoop in the area trammed and bumped the rear end of the separated scoop causing it to move bumping the employee's left shoulder. Restricted Duty.

An unintentional roof fall occurred in the 6 seam slope belt between footage markers 1390 and 1420. The roof fall was approximately 10' wide x 20' long x 5' tall. The operator's site specific roof fall clean-up plan in the slope was submitted and approved by MSHA. The fall reference number is 30F.

Employee was using a shovel clearing the continuous miner conveyor and felt pain in right shoulder. Lost time as of April 9, 2014.

Employee states he pushed roof bolt up into the drilled hole in the mine roof for installation. The roof bolt became loose and struck employee's left arm. Medical Treatment only.

An unintentional roof fall (30E) occurred in the 10WHG footage marker (approximately) 7295, xcut 51, between #2 and #3 entries. The dimensions of the roof fall are 10'T x 18'W x 16'L. The fall will not be cleaned up.

An unintentional roof fall occurred in the 10WHG footage marker (approximately) 7285, xcut 51, #2 entry. The dimensions of the roof fall are 18'W x 10'T x 30'L. The fall will be cleaned up and a roof control addendum has been submitted and is currently under MSHA review.

The employee was entering the roof bolter from the front end, while leaning under the ATRS, and his forehead came in contact with ATRS. Medical Treatment Only.

Employee states he was removing a link from the continuous miner chain and a piece broke off striking the lower right leg. Medical Treatment only.

An unintentional roof fall occurred in the Main North Return xcut 37 footage marker 5592 entry #2. The dimensions of the roof fall is approximately 17.5'W x 12'T x 40'L. The fall will not be cleaned up and a plan was submitted and approved to re-route the lifeline to #1 entry. Roof fall #30 - C.

Employee states he stepped off the roofbolter and his arm came in contact with roofbolt plates causing a laceration on right arm. Medical Treatment only.

Employee states while swinging the left side boom of the roofbolter in towards the center he inadvertently pinched left thumb between cylinder and the machine.

Employee states while digging a hole in the rib and floor with a dutchhead hammer he swung the hammer and inadvertently caught his finger between the mine floor and hammer handle causing a laceration to one finger on the right hand. Medical Treatment only

Employee states while using a ratchet wrench to loosen bolts on the bottom of the pod on the roof bolter, the bolt loosened and the employee struck left hand on the pod resulting in a laceration. Medical Treatment Only.

An unintentional roof fall occurred in the 10th West Head Gate in the #2 entry between survey footage marker 5934'W to 5956'W. The fall was approximately 15'W x 18'L x 15'T. A Roof Control plan addendum is being submitted to MSHA to address mining in zero limestone areas due to the roof fall. Roof Fall #30-B

While packing stands, the employee's thumb got smashed between the stand and a man door, causing a fracture to his left thumb.

Employee was roof bolting the last row of bolts in the face and as the mast was lowered down the wrench came in contact with the employee's mouth. Medical Treatment only employee received sutures.

Employee tripped while walking through 26 xcut in the 10WHG and fell to the mine floor injuring left shoulder. Lost time as of 5/6/2014

An unintentional roof fall occurred in the 10th West Head Gate unit at #36 xcut in the #3 entry MMU 001. The roof fall was approximately 20' Wide x 22'Long and 17' Tall. The fall was cleaned up and a roof control addendum was submitted and approved for mining in no limestone areas.

Employee was tightening a nut on a dryer in the prep plant and cut his left wrist due to his hand slipping while tightening on the nut. Medical Treatment only employee received 5 sutures.

Employee was placing trolleys off monorail into ramcar and the employee's right hand 5th digit was pinched between the trolley and monorail. Medical Treatment Only. Employee received sutures.

Employee picked up scoop cover to perform permissibility and felt pain in the lower back. Reportable Lost Time as of 03/05/2013

Employee was using a utility knife to cut thru a piece of rope and inadvertently the utility knife came in contact with the employee's left hand causing a laceration. Medical Treatment only (8 Sutures)

The employee just completed a new water pump installation and was preparing to fix a small leak when the 4" PVC coupler struck the emloyee in the right rear back and rib cage area.

Employee was helping to turn water off at water tank when his foot slipped off the concrete pad, causing him to stumble and fall.

Employee was putting a concrete block next to the rib on the stopping. The block caught on the rib and when employee attempted to move the block, it fell on his ring finger, left hand. Medical treatment injury.
