Employee was struck in the mouth by a socket from an air impact gun resulting in a laceration to his upper lip and damage to his teeth.

Injured employee sustained a laceration to his left thumb when it was struck by a piece of drill steel that fell from the mine roof while he was roof bolting. The wound required 11 sutures to close.

Williamstown contractor employee sustained a laceration to his right upper thigh while using a hand held grinder to cut a section of the impact bed of the 1 North #2 belt when the grinder kicked back and struck his leg. Sutures were required to close the wound.

While rib bolting on the miner on the 10 right section, the employee pinched his left hand finger between two drill steels while adding an additional steel to finish drilling his rib bolt hole.

Employee stated that while roof bolting on the left side of the Sandvik continuous miner on the 9 Right section the roof bolt that he was inserting into the hole started to bend. Employee then attempted to grab the roof bolt and caught his left hand between the roof bolt and the mast of the bolter causing a laceration and a fracture to his left forth finger.

Injured employee was in the process of connecting a finishing steel to a starter steel while roof bolting when a fellow employee at the center of the roof bolting machine attempted to reposition the head. This caused the drill steels to move suddenly and strike the injured emplyee's right hand resulting in a bruise.

Injured employee sustained a laceration to the left side of his face while using a hand-held grinder to cut a roof strap along the 8 Right belt when the grinder kicked back and struck his face. The laceration required sutures to close.

Employee was installing a roof bolt in the rib on the 1 West section and as he was inserting the bolt into the rib he hit the top of his left hand on the drill guide. Employee felt pain and a knot raised up on his left hand. Employee worked fifteen days following the incident was off three days came back and worked seven days before being taken off work on Nov. 21,2013.

Injured employee was using an air powered hand drill to install 12' cable bolts in the crosscut #1-2 at 11 block of the 7 Right section. While drilling the roof bolt hole a jack being used to support the roof strap kicked out and struck the employee on the left side of his head.

Employee was using a cutting torch and piece of slag went into his right glove burning his right hand. This incident became reportable on 1/6/2014 when medical reports confirmed a reportable injury.

EE was standing on face side of LW belt at -1 block directing his EEs while pulling end of belt they'd just installed a splice on. They'd attached a side grip on face side & pulling w/DC winch. EE knelt from a position 8' inby the grip instructing winch Op. to keep pulling when grip slipped off belt striking the bottom of a belt stand then EE's lt forearm & the top of his hard hat.

Injured ee was roof bolting on 1 No mains installing 6' roof bolts. After installing the bolt, he lowered his drill head & the wrench caught on the bolt head. While reaching to grab the wrench it fell & struck his right hand which he had placed on the drill head resulting in a laceration to hid right index finger requiring 7 sutures to close.

The dozer operator entered onto the clean coal stockpile in preparation to push coal to the number 325 feeder to load a train. The stockpile had a void over the feeder and the d-9 dozer that he was operating slid sideways into the void. The operator struck his head inside of the cab, this injury resulted in 4 staples on his head.

Injured EE was roof bolting on the left side of a twin head roof bolter on 1 North Mains. While holding a section of drill steel and attempting to reposition the head, the EE inadvertently moved the canopy and caught his left hand between the drill steel and the canopy resulting in a fractured left hand.

While mining in the number 3 heading of 14 Left, approximately 160' in the cut, the employee stated that he had placed a roof strap on top of one of the jacks of the miner. While backing the miner up to correctly position the roofbolter, the strap fell from the jack and struck the employee's right hand.

Employee stated that he was letting the bolt canopy down when he caught his finger between the second stage of the canopy jack and the bolter frame.

EE stated that while installing an eye bolt, he was holding the bolt and swung the boom of the bolter to the bolt and pushed the wrench onto the bolt but did not remove his finger. The finger tip became caught between the bolt and the wrench causing a avulsion to the finger and a fracture. The avulsion required stitches.

He had put bolt in hole and was putting bolt onto wrench. Then stated that coat sleeve had got caught between the bolt and wrench, he began to spin the head to align the bolt into wrench, sleeve became caught between the 2. it caused his right arm to twist around the bolt wrench. The ee continued to work until 4/27/11, he then began missing work due to injury.

The roof bolt crew moved from #7 to #6 when the bolts in the tray snagged the line curtain and began pulling supplies off. The off side operator began freeing the curtain while the operator trammed forward to give slack in the curtain. The bolt machine tracks rolled over the tip of the employees right foot partially amputating the tip of his big toe.

Employee was installing a roof bolt. While doing so a drill steel fell out of the hole being drilled and struck him on the left middle finger.

EE was positioning a stainless steel chute over the top of the heavy media cyclone sump. The chute was hanging by 2 electric hoists & being lowered into the sump. The area was tight causing it to hang up & it had to be shaken or jarred loose. In an attempt to free it, EE put his hand on the hoist block & pushed. The block then twisted & caused a laceration.

Employee was attempting to re-rail the pump car at the "Hole" switch using air bags & a 3" x 5" block of wood. As he inflated the air bags the block of wood dislodged and struck him in the face.

EE was working at the surface and was un-screwing a cap on a tanker truck. EE began to bleed the pressure from the pressure relief cap on top of the tanker truck assuming there was minimal pressure present. The cap flew up and struck the EE in the face. This caused a laceration to the lips and a fractured maxilary bone. EE was treated and released with no lost time or restr. duty.

Employee was struck in head with nail that ricocheted.

Employee was drilling and trammed his hammer into the rib causing a rock to fall and strike him in the arm.

Employee was trying to pull in the #1 shield from the #2 shield sims controls. The #1 shield had a chain hooked from the pontoons to the stageloader/face conveyor. As pressure was applied with the stageloader and the shield a connecting link on the chain failed and the employee was struck on the lower left leg just above the ankle by a section of the chain.

EE stated that while he was installing the inside bolt in #4 entry of 5 Rt future longwall setup face his drill steel plugged up & he dropped the drill head off to check the chuck section of bolter. As he dropped the drill head off the steel stuck in the hole. He placed his rt hand on drill head to check the suction & a section of drill steel fell & struck his rt hand.

The employee stated he was bolting top and was taking the steel out of a drill hole. When he took the wrench off the steel, the steel fell. He reached for it and felt a pain in his shoulder. He went to the doctor and x-rays were negative. He has been diagnosed with calcification tendonitis of the right shoulder.

The employee stated he was bolting and was pushing the bolt wrench onto the bolt and had his hand on the bolt when he accidently hit the rotation on the machine. His glove was caught between the wrench and the roof bolt and this caused the glove to the pulled off his hand and hyperextended his fingers.

Employee stated he was drilling a hole to install a roof bolt. While doing so he got the 8 ft steel hung up in the hole. He attempted to dislodge the upper steel and it fell from the hole striking him on the left hand.

The employee stated he was operating a bolter and taking the steel out of the drilled hole when he let the drill pot down his right foot was under the frame of the drill pot.

EE stated that he was trying to replace a dogbone in the face conveyor while laying on his side. While trying to adjust the panline to allow the dogbone to line up a shield that was not pressurized against the top was pulled in and pinned the injured employees left leg between the pontoon of the shield and the panline. He did not being missing work until 4/9/08. require knee su

The employee stated he was installing a cable bolt at break five when he started to push the bolt into the hole the plate caught on a track tie and caused him to feel pain in his shoulder. The employee did not start missing work until 1/2/08. He was diagnosed with arthritis and severe degenerative changes in the shoulder.

The employee stated he was moving the continuous miner to help free up a roof bolt machine that was hung up when the miner itself got hung up. The employee & 3 other employees were putting blocking under the cat pads & the operator was on the other side of the miner with the remote. The operator let the stab jack up & mashed the EE's finger between the cat pad & the crib block.

The employee finished the shift and continued to work until September 8, 2007 when he sought medical attention and was told he had a broken bone.

The employee was observing the slack being taken up on the bolt machine cable reel. He stated that a loop of cable was around a rib board pulling tight as he was stepping across the cable and it struck him on the inside of the left ankle. The employee did not start missing work until 3/8/07.

EMPLOYEE STATED HE WAS KNEELING DOWN USING A BAND SAW TO CUT UP OLD RAILS. WHEN HE ATTEMPTED TO STAND HE FELT A PAIN IN THE MIDDLE OF HIS LOWER BACK. HE ASKED A FELLOW EMPLOYEE TO ASSIST IN GETTING HIM TO HIS FEET.

The employee stated he was bolting top using stack steel. When he lowered the steel from the roof the top piece of steel hung in the hole and as he reached for the steel it dropped and struck his right little finger.

The employee was sent to move and set a pony belt drive on the construction section. He instructed the crew to hook a 5/16" chain around the drive and the scoop rope and hook to the chain and tighten up. He was standing beside the winch when the chain broke and struck the employee.

Lowering a piece of 20"-diameter polyethylene (plastic) pipe to rerig/reposition. Chain slack allowed the 90 deg ell to rotate, striking EE on the top of his hard hat - "stoving" his neck.

EMPLOYEE STATED HE WAS DRILLING A ROOF BOLT HOLE IN AN AREA THAT HAD BEEN CUT FOR OVERCAST DEVELOPMENT. THE AREA HAD OLD BOLTS EXPOSED IN THE ROOF. WHILE DRILLING THE HOLE HIS DRILL HEAD CONTACTED THE BOLT & IT BROKE. WHEN THE BOLT BROKE IT FEEL ON THE ROTATING DRILL HEAD & THEN STRUCK THE EE IN THE MOUTH.

The employee stated he was holding MRS cable while another employee was tramming the MRS to pull the next jack when the operator hit the wrong buttons to move the cable unexpectedly and this slapped the cable out of the holder and caused it strike him on the side of the head.

EE WAS USING A CUTTING TORCH TO CUT THE CAT PAD PINS OFF OF POWER CENTER. TORCH SPUTTERED AND WENT OUT. EE STATED SHUT THE TORCH GAS OFF AT THE HANDLE AND USED HIS LEFT GLOVED HAND TO BRUSH ANY DEBRIT AWAY FROM THE TIP. AS EE DID THIS, LEFT HAND GLOVE IGNITED INSIDE, CAUSING A BURN TO THE LEFT HAND, PALM AND BACK.

The ee stated he was bolting top and started the bolt into the hole with a rib board on it when the board came down and caught his left thumb against the board and the plate.

Employee was drilling a roofbolt hole on the right side of the miner when a piece of coal broke out of the top striking him on the left side of his head and face. The top hit his glasses causing a laceration to his left cheek.

Cleaning components from transmission using air cleaning gun and dirt went under safety glasses and into right eye.

EE STATED HE WAS TRYING TO UNPLUG THE HEAD ON THE ROOF BOLTER. HE PLACED HIS HAND ON THE SLIDE AND HIS COWORKER WAS TO ACTIVATE THE ROTATION LEVER BUT HIT THE WRONG LEVER AND DROPPED THE HEAD, CATCHING EE'S HAND, CAUSING A PINCH TO EE'S THUMB.

THE EMPLOYEE STATED HE WAS TRAMMING THE ROOF BOLTER UP THE TRACK AT NO.2 HEAD WHEN HIS LT. FOOT GOT CAUGHT BETWEEN THE CANOPY FRAME OF THE BOLTER & A ROCK LEDGE. THE EE DID NOT START MISSING WORK DUE TO THIS INJURY UNTIL 7/12/04 WHEN IT WAS DETERMINED HE SUFFERED A FRACTURED FOOT.

EE stated he was standing between the rib end and the buggy watching the cable reel up after repairing the level wind and the buggy cable hit him in the stomach.
