Employee was installing the top rib bolt in the #3 Heading on the 004 MMU and felt something pop in his left shoulder. He started missing work on 6/23/2014 pending the results of an MRI.

While bolting the ribs, EE got right 4th finger tip in the slide/hinge on boom and cut and bruised finger.

Employee was bolting top and caught his left hand between the drill steel still and wrench and cut his hand. Sutures required.

Employee was removing the drill steel for a test hole and the steel fell and struck him on the left hand and cut his hand. 3 sutures were required.

A continuous miner operator was aligning the miner up on the centerline and struck himself with the miner. He was diagnosed with a non-displaced hairline fracture to the pelvis.

Employee was bolting top and caught his hand between the canopy and drill steel. Fractured a bone in his hand. Started missing work 9-25-2012

Employee hit his elbow against a stack of roof plates and cut his elbow while tramming a roof bolter. Sutures were required.

Employee struck his left index finger against the roof bolter head and cut and broke his finger. Sutures were required.

Employee was bolting top and his glove got caught between bolts and bolter chuck and twisted his wrist - Fractured wrist. Started missing work 7/21/11

Employee hit shoulder on roof bolter canopy & fractured shoulder. Started missing work 5-17-2011.

Employee hit head on top and cut the top of his head. 5 sutures required.

Drill steel fell from hole and struck hand. Fractured bone in hand.

Drill steel hung in hole and then fell and struck his left hand. Diagnosed with a fracture in his left hand.

Drill steel fell out of hole and hit his hand and cut his left thumb. Sutures were required.

He was drilling a hole in the mine floor with hand drill and caught his finger on the drill steel keeper pin. His right ring finger was cut. 9 sutures required.

EE let his drill canopy down onto him too quickly and it knocked him down. Has contusions on his leg and neck. EE started missing work 4/29/10.

A piece of rock slid off miner and struck him on the ankle. He was diagnosed with a small hairline fracture on 11-23-09. He is working regular job.

EE was tramming miner and was struck by the mine tail and it knocked the EE down.

Employee was bolting top, was swinging in boom to put up inside bolt and felt something pull in right shoulder.

Employee drill stell fell out of the top and struck EE's left 5th finger, finger was broke.

Drill steel fell from hole and hit EE in face. 2 sutures required.

Employee was operating bolt machine, pushing bolt into hole and the bolt hit crack and bent sharply and the plate struck him on left side of his face, injuring left cheek.

Employee was bolting top, his glove caught in chuck and twisted hand.

Tramming bolter, he caught his right ankle/foot between rib and the deck of bolter. He is working restricted duty.

Bolter head hung on roof bolt, when he lowered boom, it fell on foot.

Employee was using acetylene torches and a piece of hot metal got into his right eye.

EMPLOYEE OPERATES A ROOF BOLTER & HIS KNEE BEGAN TO GET SORE.

Steel hung in hole, fell out striking his right hand.

Bolting top of mine caught fingers on hydraulic hose fitting, cutting fingers.

EMPLOYEE WAS STRUCK BY A PIECE OF DRILL STEEL AS HE WAS ROOF BOLTING. HE SUSTAINED A FRACTURE TO HIS LEFT WRIST. EMPLOYEE BEGAN MISSING WORK ON 05/05/05.

EMPLOYEE WAS DRILLING HOLE IN ROOF OF MINE, DRILL HUNG UP, HE STUCK HIS HAND UP TO UNHANG IT, SLIPPED, AND WITH THE OTHER HAND HE HIT THE CONTROL LEVER ON MACHINE AND THE HEAD OF THE MACHINE MASHED HIS HAND.

EE WAS BOLTING IN #4 HDG, WHEN EE WAS INSTALLING A ROOF BOLT AND GLOVE CAUGHT ON BOLT TWISTING ARM AROUND THE BOLT

Employee was struck by draw rock that caused a laceration. Stitches were used to close the wound.

EMPLOYEE WAS INSTALLING A BOLT IN ROOF WHEN BOLT ROTATED AND STRUCK HIM ON THE LEFT WRIST. MISSED WORK ON 7/22/04 BECAUSE AVAILABILITY OF DR. TO CAST ARM.

Employee informed operator of a cut that received stitches on 5-28-04. Injury occurred 4-22-04 and was not reported to w/c until 5-28-04.

EE WAS ROOF BOLTING WHEN A PIECE OF DRAW ROCK FELL FROM ROOF & STRUCK HIS RIGHT LEG BELOW KNEE.

EE WAS STANDING BY BACK CARRIER AND HIS FOOT SLIPPED AND HIS FOOT WAS RUN OVER BY CARRIER.

A PIECE OF DRAW ROCK FELL FROM ROOF & STRUCK HIM ON THE LEFT LEG.

EE WAS UNDER A ROOF BOLTER MACHINE WHEN TRAM CHAIN FELL & STRUCK HIM THE MOUTH CHIPPING HIS TOOTH.

EMPLOYEE'S LEFT FOOT WAS STRUCK BY ROOF BOLTER AND IT WAS TURNED FORWARD.

DRAW ROCK FELL FROM BETWEEN BACK ROW OF BOLTS AND ATRS AND STRUCK EMPLOYEE ON LEFT KNEE & LEG.

EMPLOYEE WAS INSTALLING ROOF BOLTS AND TURNED AROUND, TWISTING HIS RIGHT KNEE. EMPLOYEE BEGAN MINING ON 9/10/01.
