Employee was bolting top in the #5 Heading a piece of rock fell and caused him to fall forward onto bolter and struck his chest. Diagnosed with fractured ribs.

The continuous miner got stuck in the left pillar wall after some rock fell on the head of it. The miner eventually caught enough traction and trammed out of the cut.

Employee was backing a roof bolter out of a cut and a cable bolt on the bolter caught the rib and sprung the other end toward the operator. The bolt end hit the operator and cut him above his left eye. Sutures required, No Lost Time.

Employee was dipping water and bending over and strained his back. He continued to work at his regular job and then started missing work on 8/30/2014 due to his back not getting any better.

Found an unintentional roof fall in the #3 Entry, break #8, of the #26 Beltline.

Had an unintentional roof all in the #3 Heading between breaks # 13 to 14 on the 4th Left panel off of Pratt Fork Submains.

Found an unintentional roof fall in Break #4 from #3 Heading to #4 Heading in the 1st Left off of Right Shell Submains. Dimensions of fall are approximately 60' long by 20' wide by 6' to 8' thick.

Employee was observing a crew setting roof support beams while his hand was lying on the continuous miner and an employee flipped a beam over and it struck his left 5th finger. Diagnosed with a fractured finger.

Employee was bolting top and caught his left hand between the drill steel still and wrench and cut his hand. Sutures required.

Employee was helping put drums on miner and drums rolled onto hand and fractured 2 bones in his hand. Non-displaced stress fractures.

Found an unintentional roof fall on the #23 Beltline. The fall was 120' long by 20' wide, and 12' thick

Employee was pulling a dust hose through a man door & twisted his left foot. He was diagnosed with a stress fracture of his left 5th tarsal bone.

Found an unintentional roof fall in the #2 entry, break #27 panel with the #21 belt line. The fall was dangered off and supplementally supported with cribs. This was a continuation of another fall.

He was putting a bolt in a standoff chain and caught his finger in the hole and pinched the end off. Started missing work 5/7/12.

Found an unintentional fall on #23 beltline between breaks #52 to #54. Fall is 120' L. x 20' W. x 7' H.

Employee was splicing belt and a piece of rock hit his left forearm and fractured his arm.

Employee hit his elbow against a stack of roof plates and cut his elbow while tramming a roof bolter. Sutures were required.

Employee struck his left index finger against the roof bolter head and cut and broke his finger. Sutures were required.

Employee was bolting top and his glove got caught between bolts and bolter chuck and twisted his wrist - Fractured wrist. Started missing work 7/21/11

Employee hit shoulder on roof bolter canopy & fractured shoulder. Started missing work 5-17-2011.

A Pici contract employee was cutting & repairing the lifeline & fell down and cut his right index finger. Sutures required.

EE stepped up and out of a mobile bridge carrier and felt a pain in his lower back. Started missing work 3/14/11. Dr diagnosed as a reoccurrance of an old injury that occurred on 8/17/02 and employee lost time then too. Changed to 8/17/02 - This is additional days off to injury

EE was walking in belt entry and caught his neck against a loose roof strap. Sutures were required.

Employee hit head on top and cut the top of his head. 5 sutures required.

Found an unintentional roof fall in #5 entry of #16 return at break #52. Fall is approx. 12' long x 20 wide x 8' high.

Had an unintentional roof fall at break #22 on the #21 belt. Fall is 20' x 20' long x 10' high.

EE started missing work due to self-sickness. We rec. notification on 11/16/10 that he has filed for Occupational Disease - Coal Work Pneumoconioses

EE was pulling up cable slack and cut his left arm on a piece of rock, 8 sutures were required.

Drill steel fell out of hole and hit his hand and cut his left thumb. Sutures were required.

EE let his drill canopy down onto him too quickly and it knocked him down. Has contusions on his leg and neck. EE started missing work 4/29/10.

He stepped out of rail motor and the supply car hit him on the ankle. Fracture ankle. Started missing work 1/19/2010.

A piece of rock slid off miner and struck him on the ankle. He was diagnosed with a small hairline fracture on 11-23-09. He is working regular job.

Bolting top and a piece of rock hit his hand. 11 sutures on palm of hand.

Employee hit hisleft middle finger with a pick hammer. Sutures were required

He was leaning a stiff jack against the tailpiece and caught his finger between jack & tailpiece. Sutures were required.

EE slipped and fell outside and fractured his thumb.

A rock slid down drill steel and caught his left hand & fingers. 13 sutures required (no lost time).

EE was beside another worker who was using a hammer and a piece of metal from hammer flew into his eye. Started missing work 9/26/2008.

He was installing a roof strap and the strap twisted around and stuck his left ear. 2 sutures required

Caught hand on roof bolt and broke finger. *He started missing work on 10/23/08 due to having surgery.

Using a pry bar and it slipped and hit him in the mouth. Had 3 sutures on tongue.

Employee had lid off bolt machine, slid lid back on and caught left 4th/little finger between lid and bolt machine.

Employee was making a belt splice and cut left index finger with knife.

Employee was walking through mud to retrieve cable puller (come-a-long), slipped and twisted right ankle.

He was removing a brake line off of a rail motor & got brake fluid & grit in his left eye. An antibiotic was prescribed.

Employee was throwing a track switch outside and twisted his right knee.

Employee moved bolt machine up, reached to get bolt, and rock struck him on the back.

Taking canopy off scoop, chain came loose and he cut his left middle finger. Sutures required - no lost time.

Employee was climbing up on rail runner to work on it and fell off, landing on his lower back.

Employee's arm was slammed in mantrip door.

Found a roof fall during weekly exam-fall is at break #84, #1 entry of left fork mains old works fall is 20' wide x 20' long x unknown height. This is a continuation of another roof fall.

Employee was bolting top, his glove caught in chuck and twisted hand.

Tramming bolter, he caught his right ankle/foot between rib and the deck of bolter. He is working restricted duty.

Drilling test hole, he caught left hand between drill steel and canopy and cut left hand.

Riding mantrip and did not get head down and hit head on a roof support beam. Required sutures - No lost time.

Taking lid off scoop. The lid started falling and employee grabbed lid, resulted in pulling a muscle in his left shoulder.

Bolter wrench dropped out of hole and struck him on the left thumb. Sutures were required.

Employee was operating bolt machine, he was swinging head and canopy caught the top, causing rock to fall, hitting on left ring finger.

Employee was walking to scoop, hit head on roof bolt, jarring his neck.

A chain hook broke and struck EE on the left arm. 4 sutures were required.

Employee got his ankle caught between a track rail and a roof bolting machine and fractured his left ankle.

Employee was getting a rock off of the bridge conveyor. The rock fell out of the conveyor and struck his left leg. Started missing 1/8/2007.

He raised his head and struck a beam accross the track and cut his forehead. He received sutures.

Turned water valve on and valve top blew off and hit EE under his right eye, causing a contusion and got grit in his eye. Antibiotices were perscribed for the eye.

Haning Hi-line and stained lower back. Started missing work on 9/25/06.

An electric meter blew up while ee was troubleshooting a tram breaker and burned his hands. Started missing work 9/6/2006.

EE strained back while pulling on belt (converyor). Started missing work on 9/1/06. He did not report injury until 9/14/06.

EE was shoveling head drive and rock fell from top, striking him in lower back.

Tool box lid fell while ee was reaching into box and broke ee's right forearm. He started missing work 7-25-2006.

EE was putting a pin in a rail motor coupling and caught his right 5th finger. He received stitches.

EE slipped off of a rock he had stepped on and fractured his right ankle. Worked light duty on 3/21, 3/22, 3/23 and 3/24 and started to miss work on 3/27/2006.

An ex-employee filed a claim for black lung benefits on 11-7-2005 as per a notice from aig workman's comp. The injury date was his last day of employment with this company.

In the process of putting a filter on waterline, employee was holding the waterline when another employee hit it with a crib block to knock it on and pressure built up and it blew off and hit thumb.

EMPLOYEE WAS PULLING ON A PIECE OF BELT AND FELT PAIN IN LOWER BACK. HE WENT TO DR ON 05/05/05 AND WAS DIAGNOSED WITH A STRAIN AND TOLD NOT TO WORK

Unintentional roof fall occurred at #93 to #94 x cut on #12 beltline. area was supported 6 7 5' resin bolts.

Original injury occurred on 4-19-04. after reviewing medical records it is determined that cut to little finger on right hand was treated and stitches were required. ee was installing a belt roller when he caught his right little finger between a rail and the roller.

EMPLOYEE WAS DOING HEAVY LIFTING OVER AN EXTENDED PERIOD OF TIME AND FELT PAIN IN LOWER BACK. EMPLOYEE BEGAN MISSING ON 3/20/03.

EMPLOYEE WAS RIDING IN A COVERED MANTRIP THAT WAS STRUCK FROM REAR BY A FLAT CAR.

EMPLOYEE TWISTED HIS BACK LOADING SUPPLIES ON A ROOF BOLTING MACHINE. EMPLOYEE BEGAN MISSING WORK ON 9/3/02.

EE WAS STRIKING A KNOCKER WHILE MAKING A BELT SPLICE WHEN A SMALL PIECE OF METAL STRUCK HIS RIGHT THUMB AND BECAME LODGED. STITCHES WERE REQUIRED AFTER METAL WAS REMOVED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE N0. 1 ENTRY OF 004 SECTION SUBMAIN. AREA THAT FELL MEASURED APPROX. 100' LONG BY 14' WIDE & UP T0 6' HIGH. AREA DIDN'T IMPEDE TRAVEL OR AIRWAYS & HAS BEEN TIMBERED OFF.

AN UNINTENTIONAL ROOF FALL WAS FOUND IN THE SUB MAIN #5 ENTRY ON MMU 200-0 MEASURING 12' WIDE BY 50' LONG AND APPROX. 5' THICK. THIS FALL DOESN'T AFFECT VENTILATION OR TRAVELWAYS AND HAS BEEN TIMBERED OFF.

EE WAS UNCOUPLING A HYDRAULIC HOSE WITH TENSION ON IT & WHEN HE UNHOOKED IT THE HOSE STRUCK HIS FACE.

UNINTENTIONAL ROOF FALL OCCURRED IN #3 ENTRY OF 005-0 MMU.

EE WAS PICKING UP METAL BEAM WHEN THE WEIGHT SHIFTED AND HE STRAINED HIS LOWER BACK. EE BEGAN MISSING WORK ON 2/04/02.

EE WAS STRUCK BY A ROCK WHILE EIDING A MANTRIP. HE PLACED HIS HAND ON SIDE OF MANTRIP AND EACH STRUCK HIS INDEX FINGER AT RIGHT HAND.

EE WAS SPOT-BOLTING WHEN HE SLIPPED AND FELL WHILE TRAMMING ROOF BOLT MACHINE ON HIS LEFT FOOT.

EE WAS TALKING ON MINE PHONE AND WAS STRUCK BY APIECE OF DRAW ROCK THAT FELL FROM BETWEEN BOLTS.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY AT 9X-CUT OF THE 005-0MMU. PANEL. FALL WASAPPROX. 50'LONG X 20'WIDE & 5-7'HIGH. AREA WAS RESUPPORTED.

EE CUT HIS RIGHT LEG WITH A UTILITY KNIFE & DOCTOR PUT HIM OFF WORK BEGINNING 11/15/01.

EMPLOYEE WAS PULLING ON CABLE & FELT PAIN IN BACK.

EE WAS TAKING A PART OUT OF A CONTINUOUS MINER &A SLATE BAR THAT WAS UNDER THE PART STRUCK HIM IN THE FACE WHEN THE PART SLIPPED OUT.

EMPLOYEE WAS WORKING ON A SCOOP WHEN A COVER SLIPPED OFF AND STRUCK HIM ON LEFT SHOULDER.

EE WAS CROSSING A MBC BRIDGE UNIT WHEN HIS FOOT SLIPPED AND HE FELL INJURING HIS RIGHT TESTICLE. HE HAD OUT-PATIENT SURGERY BECAUSE OF INFECTION 5-23-01.
