Employee was using tapping tool to tap threads in a coupling. He was using a crescent wrench to apply motive force instead of supplied T-wrench. When he applied force the tap shattered and his right hand, which was guiding the tap, made contact with the sharp edge of the shattered tap, resulting in a laceration that required 6 stitches.

Employee was stooping under the #7 treater and backed into a piece of angle iron.

Operator indicates that he slipped on top step when climbing to look into vacuum chamber. Operator complained of pain in right shoulder. Employee has been under doctor's care and it has been determined on 7/19/2006 that employee needs to be on restricted duty and therefore triggers recordability. Was not considered a recordable until this date.

EE was attempting to remove bearing from bag packing unit to replace it.The bearing is located such that to remove it requires reaching into the structure approximately two feet and removing the bearing horizontally. He had removed the set screws and attempted to loosen the bearing by pulling to the side with his right arm. As he pulled on it, felt sharp pain in R shoulder.

Employee was clearing a bag jam from the automated palletizer. He had left shoulder pain, right leg pain and a tightness in the back. There were no restrictions from his primary job duties so this was considered a first aid case.

EE was getting out of the backhoe He stepped on the backhoe track which had wet dirt on it and he lost his footing and fell. His hip/back hit the track causing contusion.

Employee attempted to lift a pipe threader and load it without the forklift and strained his back.

Employee was removing an impeller from the shaft of a pump. The impeller slipped in his hand and cut his right pinky finger. The employee had just removed his gloves to take a measurement. He neglected to put his gloves back on before handling the impeller again.

Mechanic was climbing down off scaffolding, he felt a pain in his lower back.

Employee was using a car stop as a hammer to move a rail car door slide back onto the slide rail. When the employee struck the door slide, the car stop ricocheted and hit the employee's safety glasses, resulting in a laceration under right eye on right cheek, which required 5 stitches.

Employee was standing next to FedEx delivery truck. A 500 lb wooden crate was being delivered. The driver lowered the crate on the truck lift. As he was moving the crate from the lift with a hand truck, the crate fell over and scraped against the employee's right hand. A piece of wood (approx. 2.5 inches in length) punctured the skin on the top of the hand near the thumb.

Employee delivering water cooler to work station had parked truck beside train track. While standing behind truck at tailgate, railcar hit front passenger side of truck, causing water coolers to launch off bed of truck. One cooler struck employee, causing him to fall to ground, resulting in pain to his right shoulder/flank, hip, and knee. MD placed him on work restrictions.

Employee was attempting to catch a sample on top of a silo when his left middle finger contacted a screw conveyor resulting in an avulsion which required sutures.

Shippers were installing Seabulk liner into a container which requires removal of two tie-off studs. The studs have to be removed partially with a grinding tool and then complete removal with hammer. When he struck the studs, the hammer bounced back, hitting employee in left corner of mouth and left side of face.

Clay build up on the track at 2A bulk made it difficult for the car to roll. The operator had to use a car jack several times to get the car to move. Employee also restacked 80-100 pallets that was knocked over by a hopper car on the same shift. Employee presented with right chest pain the following day, 10/5/05, and sought medical treatment on 10/6/05.

Employee states he was fitting a bag onto a loading shoot; once full, he detaches it and clinches it and ties it closed which he believes lead to the increasing pain and swelling of his right wrist.

Employee was checking the belt number on the drive belts and the conveyor started in motion. Employee's left hand went through the nip created by the drive belt and the sheave, resulting in laceration to left index finger and compound fracture to left middle finger.

EE was driving off road truck and backed under the backhoe bucket to get loaded he did not stop when the bh operator blew the horn and the bh operator had to stop the truck with the bucket. He said this made his back hurt.

A 2A Bagger operator lacerated four fingers on his right hand while assisting a co-worker in the closure of a boxcar door. Additional Comments: Engelhard/MIDGA Operations question the recordability of this incident as EE test positive for drug. Drug test was administered within 8 hours following the accident.

WALKING TO MAIL DROP, TRIPPED ON CARPET & FELL TO FLOOR. TRIED TO BREAK FALL BY PUTTING HANDS OUT, HITTING CABINET & COUNTER. STRUCK HER FACE ON THE COUNTER. HANDS WERE BRUISED & BOTH KNEES HIT CABINET DOORS & KNOBS. ONE KNEE STRUCK THE DOOR KNOB & THE OTHER THE CABINET. CONTINUED TO HAVE COMPLICATIONS WITH KNEE. MRI: TORN MENISCUS IN LEFT KNEE; SURGERY SCHEDULED 1/18/06.

MECHANIC WAS WELDING STEAM COILS IN BOTTOM OF RT-77 TANK FOR LONG PERIODS OF TIME. WHEN HE STOOD, HIS RIGHT LEG POPPED AND HE WAS UNABLE TO STAND ON HIS RIGHT LEG DUE TO PAIN.

Employee states while working on the big bagger, the neck of the bag came off the spout causing kaolin to spill on top of the one ton bag. Employee states he felt a pull in his back while bending over to clean up the kaolin spill.

Employee opened door to the control room below the new belt filters and activated clay fell into his left eye. The clay fell from the top of the door. Employee was wearing safety glasses with side shields at the time of the accident.

When switching tubs on the treater floor the employee has to step up to change a valve. When he was stepping down he felt a pull in his groin area. Upon examination at the hospital it was found that he had a left inguinal hernia.

While working a shut-down providing construction management, a leaking hose was spraying slurry on asphalt on a slight incline creating a slick surface. While trying to get the attention of the production workers to report the problem, she shifted wait from one leg to another and lost balance. The fall caused her fibula to break.

Employee was driving a 10 wheel dump truck on haul road at Dixie Mine. Road surface was wet clay. As vehicle was negoiating a curve, the vehicle started slidding. Vehicle rolled over.

Worker was changing the tags on low voltage wires in a drive cabinet. He placed his right hand on the edge of the cabinet and was using his left hand to replace the wire. When he leaned down to see where to place the wire, his hair touched the 480 terminals and he received a shock, causing him to fall on his chin.

BITTEN BY SCORPION WHICH HAD CRAWLED INTO HIS BOOT

Employee was welding, opened hood to inspect work. He had safety glasses on. Dust dropped down into his face from unknown source. He tried to clear dust from his eyes so he could see, was unable to clear vision completely. He made it to eye wash station, flushed eyes. Eyes still irritated and at that time he sought medical attention.

A tractor operator got stuck in the mud & called for assistance from another operator/the injured operator. The injured operator was attempting to connect the cable to his tractor when he states having sustained an injury to the back of his right leg. EE reported accident on 4/4/05 & diagnosed with a large herniated disc by a specialist on 4/13/05 & was placed on disability.

Employee was hanging a Big Bag on hooks when his right little finger was caught by the forks on the fork lift. He abruptly tried to remove it and sustained an amputation to the distal end of his right little finger.

Employee was leaving work area to go to break room. As he took 3 steps after leaving work platform, his feet slipped out from under him. This caused him to fall to floor & after landing on his right elbow, he injured right shoulder. Floor was somewhat slick due to rain & high humidity conditions. He suffered torn ligaments, tendons to right shoulder (torn rotator cuff).

Employee sustained a right eye injury which he believes occurred while working in the vicinity of an employee who was welding. However, employee was diagnosed with a corneal foreign body and thinks he may have encountered some trash that blew into his right eye while working around calciners.

EE WAS TIGHTING BOLTS IN THE BOTTOM BEARING OF A BAUER MILL WHEN HE FELT A PULL IN RIGHT GROIN AREA AND FELT A SMALL KNOT IN THAT AREA. EE SOUGHT MEDICAL TREATMENT ON 12/01/2004 AND WAS RELEASED TO REGULAR WORK.

WORKER WAS CUTTING LOOSE RING HEADER. WHEN THE RING BROKE LOOSE IT MOVED CAUSING THE ANGLE IRON TO SPRING BACK, HITTING THE END OF THE RING FINGER ON HIS LEFT HAND.

EMPLOYEE WAS WORKING ON THE GROUND FLOOR BETWEEN FOUR NEW TANKS. A WORKER OVERHEAD CUT INTO AN ABANDONED HOSE. WHEN THE END OF THE HOSE WAS CUT IT SWUNG DOWN, SPRAYING WORKER WITH SMALL AMOUNTS OF SULPHURIC ACID THAT WAS LEFT IN THE HOSE. EMPLOYEE GOT TO THE SHOWER IMMEDIATELY.

WORKER WAS WELDING PIPE OUTSIDE OF TANK AND WAS MOVING TO ANOTHER LOCATION APPROX. 5 FEET AWAY. DEBRIS FELL DOWN FROM OVERHEAD FALLING THRU HIS SAFETY GLASSES AND INTO HIS EYES. RINSED HIS EYES AT AN EYE WASH STATION. DOCTOR TOOK SMALL SPECK OF METAL OFF HIS EYE. WE HAVE PROVIDED METAL MAGNETIC STRIPS TO PLACE AT RIM OF HARD HAT TO TRY TO CATCH METAL FLAKES THAT FALL FROM OVERHEAD.

WORKER WAS GRINDING INSIDE OF TANK, WEARING SAFETY GLASSES UNDER FACE SHIELD. WHEN HE STOPPED GRINDING AND LIFTED HIS FACE SHIELD, DEBRIS FELL INTO HIS EYES. HE RINSED HIS EYES OUT AT AN EYE WASH STATION. DOCTOR TOOK A SMALL SPECK OF METAL OFF HIS EYE. WE ARE PROVIDING GOGGLES TO BE WORN UNDER A FACE SHIELD WHILE GRINDING & INSTRUCTIONS ON SHAKING OFF FACE SHIELD BEFORE LIFTING.

EMPLOYEE WAS USING A BOX CUTTER KNIFE TO CUT A PLASTIC TIE OFF OF A PALLET OF MATERIAL. HE WAS HOLDING THE PLASTIC ABOVE THE TIE WITH HIS LEFT HAND, CUTTING WITH HIS RIGHT HAND TOWARDS HIS LT. HAND. THE BOX CUTTER SLIPPED OFF THE TIE, STRIKING HIS LT.WRIST, RESULTING IN A LACERATION. 6 STITCHES WERE REQUIRED.

An employee was walking back from break to his job and saw another employee laying on the floor.

Slick spot on concrete floor

Employee reported a slip and fall on a concrete ramp entrance into the maintenance shop. He requested no treatment at that time. An x-ray and MRI found no damage so no treatment resulted. Around Christmas, he reported continued soreness and stiffness and referral was made to an orthopedic doctor who began treatment with cortisone injection on 2/15/05.

Ee was repairing a broken cable on bulk loading chute. When he went to lower the chute, he hit the wrong button and raised the chute instead. His right hand was caught in the cable resulting in laceration and fracture to the right index finger. He received treatment at ER.

REPORTABLE HEARING LOSS LEFT EAR. TESTED 7/1/04 CONFIRMED BY DR DOCUMENTATION 2/7/05

Ee was replacing rings on underside of tank car when he slipped & lost his footing. Ee used hands to cushion fall & right hand contacted a bolt used to anchor the platform to the concrete pad. Contact resulted in a tear between his right thumb and index finger. Ee was taken to ER where he received 6 sutures & a tetanus shot.

A LAB OPERATOR WAS OPERATING A MIXER IN THE LAB. WHEN HE WENT TO REMOVE THE CUP FROM THE MIXER, THE MIXER CAUGHT HIS RUBBER GLOVE AND CUT HIS LEFT MIDDLE FINGER.

EE was removing a pressure washer from an upper level floor & it was being lowered down the steps to bottom floor for repairs. The EE used the hoist to raise the front of the unit to get it off the handrail when the hook connected to the handle & slid over pinching his rt. little finger causing an avulsion to the distal end.
