Employee was on the ground spotting a dump truck to prepare for dumping a load of fill dirt in excavation area. Employee's back was to the dozer that came up an incline and fatally struck employee from behind.
The employee attempted to reconnect a hose when material splashed into EE's left eye. The employee received antibiotic medication to prevent infection
Employee fell forward hitting their face while moving vacuum hose up a concrete ramp.
EE was cutting a piece of tubing on the #1 belt filter press. While cutting the tube, the blade was pinched and the tube twisted. The result was a kick back from the cutoff wheel and EE's finger was cut. EE was wearing the proper PPE, cut resistant impact gloves, face shield, etc. The blade cut through EE's glove.
A miner was operating a Ditch Witch SK 600 when the equipment suddenly jolted. This action caused the miner's finger to get smashed between a support bracket and the handrest. It resulted in a fingertip avulsion requiring sutures. The miner was released from the hospital the same day and returned to work the next morning.
Opening a railcar (boxcar) door, employee pinched left ring finger between the door and the door frame, resulting in the amputation of the left fingertip at the first joint.

The injured employee had been reclaiming product (repeatedly lifting/pulling 55-lb bags) when EE developed pain in left shoulder and both hands. The injured employee was treated with first aid measures and EE's pain did not improve. EE was seen by a doctor on 8/12/2020 and issued work restrictions.
The injured employee was stepping down off of a step stool when employee began to turn. Employee's foot slipped on wet slick concrete and employee fell to the ground. The injured employee's right arm impacted the concrete ground hyperextending elbow and fracturing radial (arm) bone.
Employee slipped and fell while holding the end of a 60-80 lb pump. EE's left hand was caught in between the pump and the concrete ground injuring EE's left middle finger.
Employee was walking down a set of internal stairs when left foot slipped. The employee was holding both handrails. The employee's weight settled on left arm and shoulder and the employee felt a "popping" sensation in left shoulder. As the employee caught self and straightened up, EE again felt shoulder "pop" back into place.
At 3:45PM ,Saturday, Sept. 21, 2019, the operator was attempting to remove a slide gate from a chute with a pry bar. The pry bar slipped and struck them producing a laceration about 2" in length above right eye. Closure of the wound took 9 stitches at a local ER. The employee was released to work without restrictions and worked all of the following shifts.
A reactor floor operator was using an air lance to unplug a line. When the plug of clay in the line broke loose, the air lance became stuck. The operator gave it a tug to free it and felt a pull in groin area. A hernia was diagnosed by the physician.
employee was on a temporary cross tie platform, laid across the tracks that was not secure, to fill a truck. It rolled causing the employee to fall on his back and elbow.
Employee was walking down the stairs to control panel at belt filter when his foot slipped and he twisted his left knee
Employee was adjusting an Aseptrol packaging machine guide. Approximately 1/4" of the tip of the middle finger was amputated by the knife.
Platform and steps had a thin layer of ice resulting from freeze protection measures associated with area safety showers. He was starting the loading station for super sacks and saw that the s teps were icy but did not take any action on the issue. As he was stepping down his foot slipped off the bottom step. He landed on his back on the floor.
Employee was stooping under the #7 treater and backed into a piece of angle iron.
Employee was clearing a bag jam from the automated palletizer. He had left shoulder pain, right leg pain and a tightness in the back. There were no restrictions from his primary job duties so this was considered a first aid case.
Employee opened door to the control room below the new belt filters and activated clay fell into his left eye. The clay fell from the top of the door. Employee was wearing safety glasses with side shields at the time of the accident.
When switching tubs on the treater floor the employee has to step up to change a valve. When he was stepping down he felt a pull in his groin area. Upon examination at the hospital it was found that he had a left inguinal hernia.
An employee was walking back from break to his job and saw another employee laying on the floor.
EMPLOYEE WAS HELPING REPLACE THE SEAL & BEARING HOUSING ON THE PUMP. HE WAS REMOVING THE CARBON RETAINER RINGS ON THE SEAL USING A CHISEL & HAMMER. WHILE HITTING THE CHISEL WITH THE HAMMER HE STRUCK HIS LEFT RING FINGER & FRACTURED THE DISTAL PHALANGE.
EMPLOYEE TRIPPED OVER AN ELEVATED PART OF A SIDEWALK.
BOXCAR DOOR JAMMED. DOOR BECAME FREE AND PINCHED THREE FINGERS BETWEEN DOOR AND RAILCAR. THIRD & FORTH FINGERS ON RIGHT HAND REQUIRED TEN STICHES.
EE REMOVING EASEOUT STUD ON WEST ROLLER MILL WITH 18" CRESENT WRENCH. WHEN BROKE LOOSE HE JAMMED 2ND/3RD FINGER ON ROLLER MILL HOUSING.
STACKING ON THE TWO-TUBE PACKER, HE REACHED AND PICKED UP A RAG OFF THE SCALE AND TURN TO PUT DOWN ON THE PALLET AND FELT A PULL IN HIS SHOULDER.
EMPLOYEE LOST RIGHT MIDDLE FINGER TIP WHEN HIS GLOVED HAND CAME IN CONTACT WITH AN OPERATING ROTARY AIRLOCK FEEDER. HE WAS ATTEMPTING TO CLEAN A CLAY POWDER BLOCKAGE AT A CLEAN OUT PORT.
EMPLOYEE WAS ATTEMPTING TO TIGHTEN PACKING NUT ON SOUTH PLUNGER. FACKING NUT POPPED UP OUT OF BOX AND SLUNGER YOKE ASSEMBLY MASHED HIS FINGER AGAINST WRENCH HEAD.
