Contracted service employee (seasonal) was performing landscape maintenance/mowing on site and was stung by bee/yellow jacket. Reporting under our Mine ID as contractor is an independent contractor and does not meet the requirements to have their own mine ID under 30CFR Part 50. However, the reportable should not count against 4100320 as the person is not employed by Alcoa.

Employee says that yesterday, 6/11/2018 around 9am, right leg began to hurt just below the knee and on the outside part of the calf. Employee says doesn't know the exact time the pain started and isn't sure on exactly what cause it. Employee says the pain feels like a constant Charlie horse and is having difficulty walking on the leg.

Employee was scoping a job in a manlift. Employee was reaching out and up to simulate what needed to be done (not lifting anything) when EE felt pain in back. The pain did not continue and EE completed the job. After leaving work and over the weekend the pain progressed and reported pain when arrived to work on Monday 2/13.

RED LINE CASE employee states that he had injury about 8-10 years ago involving left shoulder, states that he is having increased difficulty with left shoulder, increased pain and difficulty with movements of arm no complaints of new injury or specific instance or action caused current pain.

Employee was standing at work station in Machine Shop and felt something in his right eye, corneal abrasion seen, no foreign object seen, eye drops prescription given

employee bent over to unload a supply box, started feeling pain in left lower back, felt spasms and tightening in back

Red Line Case Employee was squatting and when he stood up his knee popped.

Employee's shift started at 8am. At approx 10am, the employee was injured when the wind caused a door to slam on his right middle finger. When the employee was taken to the clinic for treatment, he was given a post accident drug screen and breath alcohol test. The employee failed the breath alcohol test and was subsequently terminated as a result.

While attempting to survey the source side of the gauge with a survey meter a gust of wind blew water draining from the upper floor on to the right front side of my hard hat and into my eye.

RED LINE CASE Employee was walking on grating and felt popped knee.

Employee was walking to control room and his knee gave out. Employee did not trip or twist his knee. He was walking and his knee gave out.

Screen box blew out and shot liquor out.

Was standing about 8 feet away when a screen box blew out and shot liquor and scale out.

Screen box blew out spraying liquor.

Screen box blew and liquor sprayed all over.

Screen box blew out spraying liquor.

Employee was ascending stairs to control cab of gantry crane and started to have low back pain. No slip trip or fall at the time.

"Red Line Case" No Mechanism of injury Stiffness in elbow for two months prior to report date

4/11/2013 employee experienced muscle spasms while at work. No specific task attributed to causing spasm. Reported to Medical Dept 5/13 conservative treatment. 5/17 patient no better so put on restricted duty effective 5/17/2013.

Employee had just finished the morning safety meeting and was gathering tools for the job when he began to have chest pains. He was transported by ambulance to a local hospital.

Employee had finished the workday and was in the compound getting ready to leave to go home. The employee started having chest pains and his left arm went numb. He was transported to a hospital where it was discovered that he had a stroke.

Employee was working in Digestion Dept performing routine preventative maintenance. He was exposed to a plume of steam that contained caustic mist. He started coughing and walked to Medical to report.

Employee was kneeling to access an electrical junction box and twisted his knee and has noticed pain and discomfort in knee since the incident. There was no recordable treatment until 5/20/11 when restrictions were given.

Employee went to weather station for daily check and while opening the gate was stung by a wasp/hornet on the scalp. Site of bite has become infected and patient was prescribed Rx medication for the infection.

Employee reported to Medical Dept on 7/8/2010 to report injury stating that he squatted down to go under trough and felt a pop in left knee. Rx medication and work restrictions applied 7/8

Employee was exposed to hydrochloric acid (HCL) fumes while working and developed eye, throat and respiratory irritation along with a headache. The exposure resulted from a spill of approximately 100 gallons of HCL at the HCL storage tanks. The employee was working in an adjacent building.

Employee reported to medical dept on 12/11/2009 stating soreness in shoulder. Treated conservatively. Continued working without meds or work restrictions. Diagnosed with torn rotator cuff and scheduled for surgery 1/6/2010. LWD started 1/5/2010.

Employee reported to work area and bent over to pick up newspaper to read and felt pain in his back and could not move.

Employee felt sudden irritation to eye and quickly removed safety glasses and reported to medical. Eye was swollen. Taken to emergency room and diagnosed with insect sting and treated with prescription medication.

Employee was working as the ground safety person in the 510 switch yard as an observer. He noticed something in his eye at break and does not know what may have gotten in his eye. He was not performing work that was dusty.

Employee stood up from table in break room and his left ankle did not support him and he twisted his ankle.

Employee was opening a drain valve and was strung twice by yellow jackets.

EMPLOYEE SUFFERED A FOREIGN BODY TO THE RIGHT EYE THAT REQUIRED THE USE OF PRESCRIPTION MEDICATION. HE WAS WALKING OUT OF THE MACHINE SHOP AND STOPPED TO PICK UP SOME DEBRIS FROM THE GROUND. AS HE PLACED THE ITEM INTO A DUMPSTER, THE WIND BLEW AN UNKNOWN FOREIGN BODY INTO HIS RIGHT EYE.

Employee sustained a strain to his left knee that required the use of a knee immobilizer. He was walking from the lime area toward the railcar loading area when his knee popped and kept popping on consecutive steps. He was taken to the plant medical facility via plant ambulance and then referred to his own doctor. He returned to work with the knee immobilizer.

Employee was weed-eating when he was stung/bitten by an unknown insect on his (R) forearm.

EMPLOYEE KNELT DOWN AND CRAWLED UNDER A BRACE WHEN HE FELT PAIN IN HIS LEFT HIP JOINT. THE EMPLOYEE WAS TREATED AT THE LOCAL MEDICAL CLINICAND PLACED ON RESTRICTIONS TO RETURN TO WORK.

EMPLOYEE WAS PROCESSING THE PRESS OPEN WATER DRAIN ON #26 AND CAUSTIC VALVE DRIPPED IN THE BOOT OR HOLE IN SEAM OF BOOT.

EE WAS OBTAINING METAL OUTOF A STORAGE RACK WHENA STRONG WIND BLEW DUST &DEBRIS INTO HIS AREA. THE EE WAS TREATED AT THE LOCAL CLINIC & RETD TO WORK

EE STATES HE WAS INSPECTING DOZER/TRACTOR & WAS IN SQUATTING POSITION. HIS RIGHT EYE BECAME BLURRY LATER. DENIES ANY FOREIGN BODY OR TRAUMA AN EYE SPECIALIST TOLD HIM HE NEEDED SURGERY SO REPO RTED CONDITION TO OUR MEDICAL DEPT. ON 9/10/99 SAYING HE WANTS WORK COM SINCE HE NOW HAD SO HAVE SURGERY

EMPLOYEE WAS DOING A REBUILD ON #6 UNIT AND WAS CONTACTED BY DRIPS FROM AN OVERHEAD STRUCTURE. THE DRIPS WERE FROM AN UNKNOWN SOURCE. HE WAS CONTACTED ON BOTH SHOULDERS, THE LEFT SHOULDER HAVING A SCOND DEGREE BURN.

EE SUSTAINED A "DUST IN THE EYE" INJURY WHICH REQUIRED THE USE OF A RX MEDICATION. HE WAS WASHING HIS FACE AFTER WORKING AND SOME REFRACTORY DUST BLEW INTO HIS LEFT EYE.

EE REPORTED CHEST PAINS AT HOME ON FRIDAY EVENING. REPORTED FOR WORK ON SAT MORNING TO WORK AS TEMPORARY SUPERVISOR. AS HE WAS MAKING JOB ASSIGNMENT HIS CHEST PAINS INCREASED AND HE WAS TRANSP ORTED TO DR OFFICE AND THEN TO BAPTIST HOPSITAL IN LITTLE ROCK. EE CONTINUES TO THE UNDER DR CAR AFTER SUGERY.

EMPLOYEE WAS TAING OFF P.C. HYDRATE, WHEN DUST BLEW INTO HIS RIGHT EYE. EMPLOYEE RECEIVED FIRST AID. ON DECEMBER 31, 1998, EMPLOYEE RECEIVED MEDICA; TREATMENT AND CONTINUES UNDER DOCTORS CARE. WHILE CONTINUING TO WORK.

EE RECD A SMALL 2ND DEG BURN TO HIS RIGHT SHOULDER. SOMETHING DRIPPED FROM A FLANGE OR VALVE--EE NOT SURE WHERE OR WHEN HE WAS CONTACTED

EE SUFFERED A FOREIGN BODY TO HER LEFT EYE. THE EYE WAS EXAMINED AGAIN THE FOLLOWING DAY AND A CHEMICAL BURN WAS SEEN. A PRESCRIPTION MEDICATION WAS PRESCRIBED FOR PROPER TREATMENT.

EE RECEIVED A SECOND DEGREE CHEMICAL BURN TO HIS RIGHT LOWER LEG. HE HAD BEN WORKING ON THE PRESS FLOOR AND DIDN'T NOTICE ANY BURNING TO HIS LEG. WHILE SHOWERING OUT HE SAW SEVERAL SPOTS.

EE WAS WORKING OUTSIDE DURING A STORM WITH HIGH WINDS AND GOT A FOREIGN BODY DUST IN HIS ELFT EYE. EE WAS WEARING SAFETY GLASSES. HE WASHED OUT HIS EYE AND RECEIVED MEDICAL TREATMENT WITH A PR ESCRIPTION MEDICATION.

EMPLOYEE WAS IN THE PROCESS OF RETRIEVING A WATER LANCE IN THE BASEMENT OF R-25. AS HE WALKED IN ANKLE DEEP SLURRY AND WASH WATER, IT SPLASHED UP ON THE BACK OF HIS LEGS. THIS RESULTED IN SE COND DEGREE BURNS TO BOTH LEGS.

EE WAS CUTTING DOWN PIPING IN BLDG 420. THE AREA WAS SO DUSTY THAT DUST WAS BLOWING INTO HIS EYES. HE REPORTED TO THE PLANT DISPENSARY WHERE HIS LEFT EYE WAS TREATED WITH PRESCRIPTION MEDICATI ON.

EMP SUFFERED AN ABRASION TO RIGHT EYE FOR WHICH PRESCRIPTION EYE OINTMENT WAS ADMINISTERED. HE WAS RIFING A BICYCLE TO R-85. SOME DUST WAS FALLING IN AREA AND HE FELT IRRITATION IN EYE. WASHED HIS EYES AND REPORTED TO MEDICAL.
