Employee experienced a medical emergency that was called in to MSHA at the time of occurrence. There was no injury to the individual, the employee experienced an event related to low blood sugar. They were transported by ambulance to the hospital where they observed them and diagnosed this condition.

Employee twisted while exiting a forklift injuring their back.

Locker room at Asbury; felt something in right eye. Used eye wash station and also the squeezable tubes of saline solution to try and wash out right eye. Couldn't seem to get out was was in eye.

Employee was treating prior to the event for pain in EE's knee that was not work related. EE was walking down a set of stair when EE reported a "pop" in EE's knee. EE did not fall, EE did not slip and EE did not trip. The employee was wearing a knee brace at the time and had no restrictions from EE's physician.

Employee was cleaning bathroom and was stung by a bee. Taken to hospital on 7/21 and released on 7/21. Returned to work 7/24 (Monday).

Employee had a heart attack

Employee was found unresponsive on the Rollermill #1 3rd level. The employee had been seen just two minutes before and showed no indication of a health problem.

Injury was unknown thought to be non- work related infection due to a previous non-work related infection. Doctor supplied letter stating that this was a new injury and W.C. excepted the claim.

Employee was operating a weed wacker in front of the lab area. He disturbed a nest of yellow jackets that was in the ground. He couldn't hear them swarm because of the noise of the weed wacker. He was struck many times on the head, neck, arms and leg areas.

WHILE DUMPING SACKS OF RAW MATERIAL OUTSIDE, THE WIND WAS BLOWING AND SOMETHING FLEW INTO THE EMPLOYEE'S EYE.

EE HAD COMPLAINED OF INDIGESTION TO HIS WIFE BEFORE COMING IN TO WORK. FIFTEEN MINUTES AFTER STARTING HIS SHIFT, HE COLLASPED TO THE FLOOR. CPR WAS ATTEMPTED AND AMBULANCE/EMT WERE SENT FOR. R ESUSITATION ATTEMPTS WERE ULTIMATELY UNSUCESSFULAND EE WAS PRONOUNCED DECEASED AT APPROX. 7:00 PM ON 9-28-98. THE CAUSE OF DEATH WAS A MASSIVE HEART ATTACK.

WALKING & WIND BLEW DIRT INTO EMPL LEFT EYE. EMPL RECEIVBED MEDICAL TREATMENT FOR REMOVAL OF FOREIGN OBJECT FROM HIS LEFT EYE & RESULTING IRRITATION.

EMPLOYEE IN EMP7Y BIN USED FOR OVERSIZED MATERIAL. WAS CLEANING SIDES OF BIN WHILE STANDING IN BOTTOM OF BIN. LEADMAN STATIONED AT DOOR OF BIN WHEN EMPLOYEE IN BIN SLUMPED TO THE BOTTOM OF THE BIN. LEADMAN G2T ASSISTANCE FROM NEARBY WORKER AND REMOVED UNCONSIOUS EMPLOYEE FROM BIN. VICTIM HAS HISTORY OF HEART PROBLEMS.
