Employee passed out inside mill. Had lack of sleep due to issues at home. Also possible potassium deficiency and several layers of clothes.

EE was cleaning windshield of loader when ee twisted and leaned, reaching to clean the middle of the windshield. EE felt a sharp pain in shoulder which progressively got worse. EE was seen at the ER, diagnosed with a strain to the shoulder. Follow-up resulted in the ee being put off work.

Ee experienced a non-occupational health issue that required transport from the mine. MSHA considered this a reportable event.

Ee was standing talking with an electrical contractor supervisor about schedules. When the conversation was over, the contractor supervisor turned to leave and saw the ee fall over. Bystanders began care, EMTs continued care, and ee was flown to NNRH ER. Ee received care at NNRH and was transferred to UofU Medical Center. Ee died 08/08/18. Please see attached email.

Ee was backing up forklift with door opened and latched. Ee had hand holding door frame; strong wind blew door shut, pinching the ee's right thumb between door and frame, causing fracture to tip and laceration. Ee required stitches and was released to full duty.

EE was driving underground in open cab. Ventilation blew some fine dirt into an eye. Employee rinsed the eye and continued working. The next day the eye was irritated. Employee was seen at the clinic and diagnosed with a corneal abrasion. Employee was given prescription eye drops and released to full duty.

Employee suffered a Personal Medical Event while on site. Was transferred to local hospital. Initial diagnosis was unknown. Today 3/7/17 received diagnosis, "Brain Bleed Stroke". Employee doing well with prognosis being full recovery.

Ee was found unresponsive in an underground equipment wash bay at approximately 1:20 am on 11/25/16. Emergency response toned out, ee transported out of mine to HGH where EE was pronounced dead. Preliminary report from Humboldt County Sheriff on 11/29/16: death from natural causes, no environmental influences identified.

Ee was reaching behind the seat of lube truck and felt a sting on the left pinky finger. Seen at ER and diagnosed with a spider bite. Put off work for 4 shifts due to wound care issues.

Ee was squatting to retrieve a sample container and felt a pop in EE's knee with immediate pain. EE was seen at the clinic, diagnosed with a strain and released to full duty. When the pain continued, an MRI was done and EE was diagnosed with a torn meniscus on 04/05/16, making this a reportable injury. Surgery scheduled for 04/20/16. EE is full duty until surgery date.

Survey supervisor out surveying and did not return to crew. When partner went to check on status, ee was found lying on ground unresponsive. Attempts to revive the ee by emergency personnel were unsuccessful.

On Thursday, 28 May 2015, at about 3:19am Officer stated that she was not feeling well. As she was walking away from a vehicle she collapsed. Barrick EMTs were notified, she was then transported to a Reno hospital by helicopter. The reason for collapse is unknown at this time. MSHA was notified as a precaution.

Ee was working with cable, doing regular work in the open pit. He was walking and felt a pop in his right calf. He was seen at the ER and diagnosed with a torn calf muscle and released to non-weight bearing. Mine site unable to accommodate and the ee is put off work.

Ee was feeling bad, and when the feeling got worse, he went to the EMTs and the site nurse who toned out emergency transport. Transported to medical facility. Awaiting surgery for a non work-related condition.

Ee was crushing filters in bay 2 of the shop when dust blew in and he felt something in his left eye. He was seen by a doctor and diagnosed with a corneal abrasion from a foreign body in the left eye, given an antibiotic for the eye, and released to full duty.

While employee was leaving gold room, dust particles flew in his left eye.

Employee unresponsive in water truck. Transport by ambulance, pronounced dead at transfer to county ambulance, declared by Eureka County sheriff. Investigation determined non-occupational.

Employee was an electrical/instrumentation technician the Autoclave. A reportable shift in hearing was identified in annual testing and confirmed by a doctor of audiology on 01/13/14. Written notification was given to the employee and retraining on the HCP has been completed as of 01/23/14.

Natural causes.

Ee was walking from his tool box across the shop. Some cleaner was being sprayed onto a part and overspray went into ee's eyes. Seen at ER, diagnosed with a small corneal abrasion, and released to full duty. Prescription eye drops given.

Affected employee was experiencing physical discomfort and disorientation while leaving the open pit, notified his Supervisor that he required attention.

Injured ee and other ee had an altercation. Injured ee struck by other ee on side of head. Injured ee worked remainder of shift, woke up next morning with increased neck/head pain. Seen at ER and put off work.

Before Lunch experienced dizziness and light headed feeling along with nausea and vomiting, rested and ate lunch but symptoms got worse.

The Employee was using a chop saw to cut a piece of I-beam, the Employee was wearing a Face Shield & Safety Glasses and the wind was blowing. The Employee lifted up the Face Shield and a piece of debris went into the Left Eye causing a Corneal abrasion.

Employee put on work gloves while at the gyro crusher and felt a bite on his left little finger. He reported the bite and went back to work. The bite began to swell and hurt, he went to the ER and was diagnosed with an infected spider bite. He was placed off work.

Employee put on fire jacket before loading furnace. When she took off jacket to go to break she felt burning and noticed a bite on her left arm. She was seen at the ER, diagnosed with an infected insect bite and prescribed antibiotics. Released to full duty.

EE was sitting at the computer and felt burning on his left leg. He notified his co-worker. They called rescue one who then responded to the gate house. He then became nauseous. Rescue one arrived and they called appropriate Barrick staff who determined to transfer EE to the local hospital.

Individual was found unresponsive, pulse less, and breathless in wash bay. CPR was initiated on scene. Individual was transported to Northeastern Nevada Regional Hospital via Crescent Valley Ambulance. Initial determination is that this is a health related incident.

Operator reported that he inhaled fumes from an unknown origin.

Working on a connection in windy conditions when foreign object flew behind his safety glasses and imbedded in eye.

The death was caused by a medical condition not related to a mine accident.

Employee stated on first report of injury "Going into heading and feeling light headed". Was transported to ER by ambulance, adminstered oxygen, and released to full duty for 05/06/2009.

Employee was sitting on a platform loosening lug nuts, when he turned to stand up, he felt pain in his lower back. He was placed on restricted duty and diagnosed with a low back strain.

Employee was getting into Bobcat Skid Loader and while turning to sit down felt the strain in groin area. Noticed the knotted area. Result: Hernia. Hernia was repaired on regular scheduled days off.

DRIVING TRACTOR TO MINE SITE IN HEAVY & GUSTY WIND. A GUST OF WIND CAUGHT TRACTOR HOOD & FORCED IT TO BECOME UNLATCHED. THE HOOD STRUCK THE EMPLOYEE IN THE HEAD. MINE APPLIED FIRST AID & THEN HE WAS TRANSPORTED TO THE E.R.

Injured employee opened air door on vent shaft to send cage down. When door opened, wind caught dust and dirt, blowing it into injured employee's right eye. Upon examination, a rust ring was found and removed.

THE EMPLOYEE HAD LEFT THE BREAK ROOM AND WAS WALKING BACK TO THE STRIP CIRCUIT. HE WAS ALMOST TO THE DOOR AND HE STATES HE BECAME DIZZY. HE BENT OVER AND PUT ONE HAND ON HIS KNEE. THE NEXT THING HE REMEMBERS, HE WAS GETTING UP OFF THE GRATING. HE HAD ONE LACERATION ON THE UPPER PART OF HIS NOSE AND TWO BELOW HIS NOSE. HE WAS TRANSPORTED TO THE E.R. FOR EVALUATION AND TREATMENT.

THE EE WAS OPERATING A ROCK BREAKER ON THE 1600 ORE PASS WHILE HIS ROCK BOLTER WAS IN THE SHOP FOR PERIODIC MAINTENANCE. AT 2:30 P.M. HE WAS INFORMED THAT THE ROCK BOLTER WAS AVAILABLE. A CONT RACT EE SAW HIM AT APPROX. 2:55 P.M. AT 3:05 P.M. HE WAS FOUND LYING ON THE GROUND JSUT INSIDE THE AIR DOORS AT THE 1600 STATION. HE WAS NOT BREATHING AND CPR WAS INITIATED. PROUNDED DEAD 440

THE EE WORKS AS A MINE GEOLOGIST. HIS JOB REQUIRES HOURS OF WALKING ON ROCKY UNEVEN SURFACES. IN JUNE HE WENT TO THE DR. FOR LEFT FOOT PAIN. WE WERE NOTIFIED ON 7-9 THAT THE EE WAS RECEIVING M EDICAL TREATMENT FOR LEFT FOOT PROBLEMS RELATED TO HIS EMPLOYMENT AS A GEOLOGIST.

EMPLOYEE SQUATTED TO READ PAPERWORK THAT HAD BEEN PLACED ON THE GROUND, PINCHING THE TENDON OF HIS RIGHT BIG TOE BETWEEN THE STEEL TOE PLATE AND THE SOLE OF HIS SAFETY BOOT.

EMPLOYEE WAS WALKING ACROSS AN I-BEAM TO A PLATFORM AND STRUCK THE KNEE ON THE HANDRAIL.

EMPLOYEE WAS KNEELING DOWN TO MARK A HOLE IN THEDECKING WHEN HE BECAME DIZZY AND COLLAPSED. THE PHYSICIAN DIAGNOSED PROBLEM AS NON-OCCUPATIONAL INCIDENT. EMPLOYEE HAD A SEVERE SINUS INFECTION FROM A COLD.

ON 8/4/99 THE EE WAS WORKING ON EQUIPMENT IN THE WIND. HIS RIGH TEYE WAS IRRIATED & RED. THE IRRIATION CONTINUED AND HE WAS SEEN AT THE ER ON 8/6/99 WITH A DIAGNOSIS OF AN EMBEDDED FOREIG N BODY ON THE RIGHT CORNEA

EMPLOYEE WAS GRINDING OVERHEAD WELD ON LOWER WIND GIRDER IN 25MPH WINDS. A FOREIGN BODY(GRINDINGS) BLEW INTO HIS EYE THROUGH FACE SHIELD AND SAFETY GLASSES.

AS THE EE ENTERED THE NORTH SIDE OF THE PUMP SHOP, A GUST OF WIND PREVENTED THE DOOR FROM CLOSING. HE GRABBED THE EDGE OF THE DOOR TO PULL IT CLOSED. AS HE DID, ANOTHER GUST OF WIND SLAMMED TH E DOOR SHUT, PINCHING HIS FINGERS BETWEEN THE DOOR AND THE DOORFRAME. THE EE WAS GIVEN FIRST AID ON SITE AND TRANSPORTED TO ELKO. AS THE RESULT OF INJURY, THE EE LOST THE TIP OF HIS LEFT MIDD

EMPLOYEE WAS CHECKING INSIDE OF A LARGE TOOL BOXWITH THE LID OF THE TOOL BOX IN AN UPRIGHT POSITION. A GUST OF WIND BLEW THE LID CLOSED, CAUSINGIT TO STRIKE THE EMPLOYEE ON HER HARD HAT AND SH OULDER, KNOCKING HER HARD HAT OFF IN THE PROCESSTHE EMPLOYEE SUFFERED FROMA A NECK AND SHOULDER SPRAIN, AND WAS PLACED ON LIGHT DUTIES.

RESULTS OF HEALTH MONITORING PROGRAM AUDIOMETRY TESTS REVEAL A SIGNIFICANT DECREASE IN HEARING RANGE. EE REFERRED TO A SPECIALIST FOR FURTHER EVALUATION. TREATMENT TO INCLUDE HEARING AID FOR T HE RT EAR. EE TO BE RATED PER AMOUNT OF HEARING LOSS AND FOR PERMANENT PARTIAL DISABILITY. THIS INFORMATION TO BE FORWARDED WHEN AVAILABE. EE WAS DRIVING LOADER AND SLUMPED OVER THE WHEEL. HE WAS TRANSPORTED TO THE HOSPITAL VIA AMBULANCE AND SUFFERED CARDIAC ARREST ENROUTE TO THE HOSPITAL.

EMPLOYEE WAS BLOWING DUST OUT OF HIS TOOL BOX AND A FOREIGN OBJECT ENTERED HIS EYE.
