Climbing down ladder felt pop in right knee

Employee was loading a job box in the back of a service truck, job box slipped off forklift smashing left thumb.

Moving wooden pallet in shop caused lower back strain.

Employee was walking and twisted ankle.

Employee was exiting driver side of service truck and tripped on broken concrete and rolled ankle. Initial doctor's requested imaging on injured ankle. Results came back 6/25/25 and the employee was put on restricted duty.

Employee was Pulling drip line on pad 4 rehandle. Coupler gave out causing employee to fall backwards. During fall employee twisted left knee and was not able to put full weight on that leg after fall.

Employee was using a pin from a clevis as a hammer to loosen tow rope from clevis. Employee went to an ER and received four stitches in left middle finger and medical glue on left ring finger. X-rays showed no fractures.

Stepped off of a Track Dozer while the dozer was on a tow haul trailer and missed stepping on the trailer and falling to the ground

Exited the cab of a Hydraulic Shovel to use the restroom and the jacket of the employee snagged on the door and slammed on the employees left middle finger. Employee was seen at the ER and had X-Ray imaging done on the affected finger. X-Ray showed a hair line fracture on the affected finger. Employee received a return to work no restriction on next scheduled shift (11-25-2024)

Employee was unloading tools from tool box and putting them on a tool cart. Employee felt discomfort in the lower abdomen while pushing tool cart. Employee notified supervisor.

Employee was walking slope on a leach pad and plumbing solution pipes. While walking on the slope the material was wet and caused the employee to slide and then tumble down the slope. The employee had a strained neck and right ankle due to the incident. The Doctor recommended that the employee take three days off of work.

A safe rescue line attached to the back of an employee standing on the ground got entangled on a handrail of a Bulk truck while pulling through an ammonium nitrate silo. With the line being tangled, it got tight and jerked the employee off their feet and into the ground causing injuries to left shoulder and right bicep.

Employee was dismounting a track dozer and tripped forward and felt the knee pop and was taken to Hospital in Ely, NV. Xray performed with negative findings and released to full duty with. Followed up with Ortho and MRI was completed on 04/23/2024. MRI results read on 04/24/2024 with the ACL and meniscus was damaged. Released to restricted duty on.

Progression of hearing loss at high frequencies bilaterally noise induced.

Employee was driving a boom crane out of mine traveling on the 8060 area of mine when they lost traction and the crane began to slide/roll back down the hill until it ran into the left-hand berm/hillside and tipped onto it right side. The employee was wearing a seatbelt but sustained a concussion, compression fracture in lower back, and multiple contusions.

Employee was cleaning the welding area of the deployed geomembrane liner with their back to 2 other crew members who were rolling up a scrap piece of geomembrane material. The rolled up scrap geomembrane got away from the two crew members and contacted cleaning employee on back of ankle causing employee to fall to their knees.

Employee was tightening bolts when right index finger got smashed by wrench resulting in a fracture of the right distal phalanx.

Technician began experiencing symptoms of heat illness while working.

While removing a filter the wrench slipped off and struck employee on the lower lip causing a laceration resulting in the employee receiving 3 stitches

Employee initially reported that dirt/debris had gotten into EE's eyes during windy day. First aid was refused on site and C1 was completed. On 01/04/2022 employee went to optometrist and found that a microscopic object was lodged in left cornea. Object was removed.

The operator of the excavator hit the left rear corner of the 605 haul truck with EE's bucket while swinging to dump the bucket load into the truck.

A haul truck operator was traveling at an excessive speed down a ramp when the haul truck encountered a containment channel. The truck hit the channel and bounced the operator in their seat, which caused a lumbar fracture in the lower back.

Operator was dismounting water truck when EE missed the last step and sprained knee when EE's foot hit the ground hard.

Employee was changing track pad and fractured/lacerated right thumb when pinched between 2 pads on dozer.

While moving drill sample bags the employee stumbled, twisting their back.

Employee fell during a line out meeting and broke a front tooth.

Installing pin on articulation lock. The pin slipped and pinched EE's right ring finger.

Surveyor was walking across uneven ground and twisted a knee.

The blast crew was returning material to the magazine when the blast crew employee got a finger caught between the truck door and magazine door.

Driving haul truck and hit some washboard. Steering wheel jerked injuring left wrist.

A mechanic injured two fingers on the right hand while loading a hydraulic cylinder on a flatbed trailer with another employee.

While walking through the maintenance shop the employee succumbed to syncope. Employee fell unconscious without injury. The shop was well ventilated, well lit and the walkway was unobstructed and even.

While walking over uneven ground and carrying a heavy box the employee strained the left side of abdominal wall.

While walking down hill to a drill site being constructed employee twisted right ankle.

While loading a Gehl forklift onto a flat top trailer, the operator attempted to better center the forklift. In doing so EE got to far to the right side and the right side of the forklift slid off the trailer edge. The operator sustained a cut on right forearm requiring 21 stitches.

Employee placed right hand on door frame of track drill. Wind blew door shut injuring the thumb.

Passenger seat of unit was lifted to gain access to the electrical panel. The seat and panel cover closed, pinching left middle finger.

Warehouse tech receiving a shipment of Haul truck wheel chocks (roughly weighing 25 - 30 pounds) from a tote. Picking up last wheel chock felt, employee felt back tighten up, could not sit or stand without pain. Employee was treated at local ER, issued a full release back to work. Upon follow up appointment on June 18th, employee was unable to return to work.

Employee was helping change a bucket on a skid steer, overexerted and felt a pain in the left side of their groin. Employee taken to Urgent Care where it was determined a bilateral hernia. Employee released to comfort at normal duties. EE has not missed any work due to injury. Employee went to doctor on 3/21/2019 to schedule surgery.

Supervisor dropped employee off at the pad to run dozer. Employee walked toward dozer when employee felt their feet slip on the snow pack and fell twisting ankle. Initial trip to hospital showed no break, employee returned to doctor due to pain in ankle and the stress fracture was found on Feb 19, 2019

A mechanic was fixing a leak on the steering hose of a Komatsu 830e Haul Truck. A second employee shut the truck down so the pressure would drop off. The mechanic turned off the master switch and locked out the switch before commencing work. The pressure did not completely bleed off the hydraulic hose. When EE loosened the fitting, the hose broke free and struck EE.

Employee was grinding on a water tank when debris was blown into their eye by the wind. Accident date is 4/5/18. Treatment was First Aid. Injury became reportable on 4/6/18 when employee returned to clinic and received prescription antibiotics. Report was filed on 7/5/18 when prescription was confirmed. Employer was notified on 4/6/18 of prescription.

Contractor was walking through hallway located in the warehouse facilities when they fell to the floor. There is a small transitional ramp in the area that may have caused them to tumble.

Operator was walking to equipment from van on ready line and slipped injuring their left wrist. Weather conditions were snow packed and icy from haul trucks staging between shifts.

Two employees were dismantling an elevated section of pipe from a catwalk. An employee was removing bolts from the pipe flange while the other held the other end up with one hand to alleviate pressure from the remaining bolts. When the second to last bolt was removed, the pipe swung down and pinched the employees left thumb between the flange and hand rail. 10 stitches.

Unrolling plastic liner material, employees right foot slipped in front of the roll of material and ee twisted ankle.

Driller helper smashed and cut finger on the mud tank and hand rail.

Driller was changing a bit and when ee went to remove the bell housing off the bit. EE cut the top of right index finger on the bottom of the steel on a burr.

The incident occurred while washing a work truck with a high pressure water cleaner located at the mine operator's vehicle washing facility. While washing the truck, water under high pressure exited a section of the hose, passing though rubber gloves, resulting in an "injection" injury to the individuals right index finger.

While employee was dismounting dozer 401 at the end of shift, employee missed the hand rail with left hand resulting in employee falling approximately 6 feet to the ground. Employee was seen at the ER, x-ray was positive for a fracture.

Improper use of haul truck braking system. Total loss of haul truck.

Driver inattention caused Haul Truck to strike berm. In the process operator struck head against inside right window. Sustained cut on head requiring stitches.

While tripping in drill pipe, the helper had just installed the clam shell and was pulling the cable tight when the pipe shifted into the gap on the top row of pipe pinching employee's finger between the clam shell handle and next piece of drill pipe.

3 lb hammer slipped from grip while driving pin from overshot. Hammer head impacted right foot just behind steel toe.

Employee was changing a tail light on a Komatsu haul truck when EE was startled by a deflating tire. The employee dismounted the ladder quickly missing the last step resulting in a fractured right ankle.

Ee was changing cutting edges on a bulldozer using an air impact wrench when debris went into EE's right eye. EE reported this to EE's supervisor on 01/04 and then went to the eye doctor. A metal foreign body was removed from the eye and antibiotic eye drops were prescribed. Released to full duty.

Ee was walking to his piece of equipment, slipped on some ice, and fell, landing on his right knee. He was seen at the clinic and diagnosed with a strain to the knee. He was given a brace to wear on the knee and released to restricted duty.

Employee states while riding in a 4 person ATV, Kubota UTV, the driver attempted to turn around. The ATV slid on ice and rolled onto its side causing the employee to strike his head.

Ee was tightening bolts on track wheel when the rock breaker fell and struck him on the right forearm. He was seen at the ER, xrays were negative for fracture. Diagnosed with a contusion, placed in a splint and released to restricted duty.

Ee hit soft spot in road, was jostled in the seat of the haul truck, and felt immediate pain in his low back. Seen at the ER, referred to back specialist who put the ee on restricted duty and prescribed physical therapy.

Ee was operating a light vehicle that had just been serviced. He began to feel ill, and notified his supervisor. He was given oxygen on site, and transported to the ER where he was diagnosed with carbon monoxide poisoning. He was given more high flow oxygen and was released to full duty.

Ee was stepping into passenger van, leg slipped and scraped/struck the edge of the van causing a large laceration. Ee was seen at the ER and stitches were required to close the wound. Ee put off work.

Employee was unloading structural steel from trailer. The material shifted and pinched his left middle finger between the steel and the trailer. The pinch resulted in a cut and required stitches to close. The employee was wearing gloves.

EE was out surveying and the wind kicked up some dust, causing some irritation in the left eye. Eye was rinsed but continued to hurt. EE was seen at ER. No foreign body found in eye, but ee was prescribed an antibiotic, making this reportable for treatment. EE released to full duty.

Employee was organizing tooling bins, moved hammer out of his way and placed his hand on the square stock that goes across the bin. He bent over to move the bazooka tube and the hammer slid and crushed his little finger between the square stock and the hammer.

EE was leaving site at end of shift, stepped off step and felt pop in left foot. Saw doctor 07/31/15, diagnosed with strain, given a boot to wear and released to full duty.

Ee stumbled on steps of haul truck and caught right shin on the fire extinguisher bracket. Seen at the ER for the laceration, and stitches were required to close the wound. The ee was released to full duty.

While lining up to be loaded, the ee backed into the shovel, clipping the bottom of the shovel bucket with the dove tail on his haul truck. He strained his left shoulder on impact. He saw a doctor on 03/09 who released him to restricted duty.

Employee was working on removing a pivot arm from a Komatsu 830 haul truck. He was working with another individual and when they were placing the jack stand under the frame of the truck, his hand was caught between the jack stand and the fuel tank.

While inspecting a shovel, right pinky finger came in contact with a collector ring. Ee experienced a shock and was transported to NNRH. Hospitalized overnight for observation and released on 02/05/15. Ee was seen on follow-up and put off work in order to keep small wound on pinky clean and dry. Released to work full duty effective 02/17/15.

After lifting pipe fitting, ee felt pain in lower back. He was seen initially on 12/16 and released to full duty, no follow-up. Pain continued, and ee was prescribed physical therapy on 12/22. Medical investigation completed, claim determined compensable 01/07/15. Reportable for medical treatment.

EE was troubleshooting a ladder on a dump truck when the ladder went in down mode, smashing the ee's left wrist. X-rays taken and ee referred to orthopedist where he was placed on restricted duty. The ee is on pre-scheduled vacation until Dec. 28.

Ee was working with cable, doing regular work in the open pit. He was walking and felt a pop in his right calf. He was seen at the ER and diagnosed with a torn calf muscle and released to non-weight bearing. Mine site unable to accommodate and the ee is put off work.

Ee stepped out of a service truck onto a rut that was covered with snow. Ankle turned, popped twice. Ee was diagnosed with a strained ankle and put off work.

Haul truck jarred by boulder that got hung up in the shovel bucket and unexpectedly broke free, hitting the truck's tailboard. Ee treated at the hospital, diagnosed with multiple strain injuries and released to full duty. At follow-up on 10/15/14, he was prescribed physical therapy and continued on full duty making this reportable for treatment.

Ee was removing a filter on a haul truck when he pushed on the wrench, his right shoulder popped. Diagnosed with a strain and put on modified duty which the site cannot accommodate. This employee is a maintenance training student who works a maximum of 3 days per week.

Ee was driving haul truck away from shovel, hit a soft spot in the road and was jostled in the cab. He felt pain and it continued to get worse. He was seen at the ER and diagnosed with a strain and released to full duty. Further follow up resulted in a restricted duty return to work release.

Ee was loading broken bags of cement onto forklift on Monday, August 18. He felt pain in his back that day but thought it would go away. It did not, so he reported the injury to his supervisor on Tuesday the 19th. He sought treatment on Friday, August 22nd and was diagnosed with a back strain and released to restricted duty.

Ee was driver of a crew van that hit a large rock in the haul road. He was momentarily blinded by the sun in his eyes, and did not see rock. He was seen by a doctor, diagnosed with a neck and low back strain, and put on restricted duty for one day.

The ee also strained his right shoulder pulling himself into the cab when he fell. The employee will be lost time.

Ee tripped/stumbled on stairs prior to entering the mine ops building at the start of shift. Fell to knees, striking them on grating. Laceration to right knee required sutures. Employee stated several times that this issue was not work related, that she simply stumbled.

Ee had placed chock blocks at equipment, turned to walk to van and stepped on a rock, slipping and hitting his hand, dislocating his left ring finger. He was seen by the doctor, the finger was put back into place and splinted. The ee was released to full duty with the splint.

After using the restroom, the operator noticed one of his chock blocks was falling out of the holder. He readjusted the block into the holder and felt some pain in his right collar bone and neck area. Reported to supervisor. Seen at doctor on 05/15. Confirmed work related on 05/28/14. Diagnosis was right shoulder strain.

Haul truck was being loaded at electric shovel. Shovel hit dovetail of truck while swinging into bed. Ee jarred in cab, c/o of numbness to right arm and hand, numbness and pain in right shoulder. Initially diagnosed with strain, released to full duty. MRI reviewed on 02/13/14 showed labrum tear treated with injection and physical therapy. Ee continued on full duty.

Ee slipped and fell on the ice. He was seen at the ER and diagnosed with a contusion to the tailbone. Site investigation and investigation by workers' compensation determined this was a work related injury on 12/31/2013, making this a reportable for lost time.

EE was walking up stairs of haul truck, both hands on handrails, lost footing, stumbled and hit right knee on grating. Seen by provider, released to restricted duty on next scheduled shift.

Ee was walking on the leach pad and the ground shifted when he stepped down and he fell, injuring his left knee. He was seen by the doctor and diagnosed with a meniscus tear and a strain. Put off work pending release by an orthopedist.

Ee made claim of low back pain at start of shift after picking up chock block. Seen at the ER, diagnosed with a strain and released to modified duty. After investigation of the claim by workers' comp carrier and mine site, it was determined to be work related on 10/07/13.

Ee was loosening a bolt, bolt broke and ee's hand hit the corner of angle iron, lacerating right index finger. Sutures required to close laceration. Released to full duty.

Employee was getting on the drill, lost his footing and fell, catching himself with his left hand. Seen at the clinic, diagnosed with strain, and released to work with restrictions.

Working on pad crew, ee stepped on HDPE pipe main line with left leg. Pipe rolled causing employee to fall on to left knee. He was seen at the ER, referred to Dr. where he was diagnosed with a soft tissue tear and scheduled for surgery.

The employee filed a workers' compensation claim for left wrist pain. He stated that over time, his wrist hurt due to his work as a tire technician. His total employment history is that of a tire technician. The claim was accepted as work related on 07/30/2013, and the employee is getting therapy for this cumulative trauma issue.

Employee was unhooking a chain on the bushing fork of a drill. His finger was caught between the drill steel and the kick out arm roller. He was taken to the ER, diagnosed with a crush injury to the left middle finger. Transported to SLC where surgery was performed and a pin placed in the finger.

Employee was welding for most of the shift. He went home, slept, and woke up with gritty, painful eyes. He was seen by an eye doctor, diagnosed with flash burn, and released to full duty on next scheduled shift. He was given a prescription eye drop.

Drill assistant and driller were loading mud tub onto pipe truck for moving. DA signaled driller to raise the tub so he could move the tub into position. DA moved the tub, lost his footing. Driller noticed that the tub was where it needed to be and started to bring the tub down. This resulted in the tub being placed onto the DA's right foot.

Employee was lifting aluminum stands into the bed of a pickup truck and he felt pain in his back. He was seen at Golden Health on 01/14/2013 and put off work due to a strained back. Released to return to work full duty 01/17/2013.

Employee was standing on the deck of a tow/haul guiding a piece of equipment. He stepped backwards off the edge of the deck, falling about 5 feet to the ground. He was transported to the ER where he was diagnosed with a concussion and transported to another hospital for further observation. Released from hospital on 12/31/12 and referred to neurologist for release to work.

Employee was lifting hood on truck for preshift inspection, the hood was hung on something and the employee pulled harder and felt pain in his pectoral muscle.

EE stated he was walking back to his truck after a break, stepped on a rock and twisted his ankle. Seen at the ER, diagnosed with a strain and released. EE requested more care. Investigation confirmed on 08/27/2012 that this was a work related injury. Reportable for lost time.

Employee was replacing a pin on a track frame. A piece of metal went through his coveralls into his right leg. Xrays identified a small piece of metal embedded in the leg. The wound was stitched and the employee released to full duty.

Employee just stepped off bus at mine site to go to work and there was rut in the ground. She stepped in the rut and fell, landing on both hands, injuring her wrist. Seen at clinic on follow up and there may be a dislocation. Referred to orthopedist.

Employee states that he lifted a heavy object and hurt his back.

Employee was lifting a ramp onto a tow/haul trailer when he felt a pull in his lower back causing pain that dropped him to his knees. Seen at the ER, originally released to full duty, put off work effective 12/29/2011 and sent to physical therapy on follow-up 12/28/2011.

Employee was operating a dozer and smelled diesel fumes in the cab. He began getting sick, upset stomach, stomach pain, chest pain and coughing. He was seen at the ER, diagnosed with bronchitis that was exacerbated by inhaling diesel fumes. He was admitted to the hospital for observation for 2 nights. Released on 12/15 to follow up for release to work on 12/19.

While working on a dozer, the employee's hand was pinched between the blade and a cylinder. The cylinder moved, pinching the employee's left ring finger causing a fracture and an avulsion. He received stitches and was released to sedentary duty.

Employee was walking up stairs, boot stuck and he landed on his knees. Taken to ER, released to full duty. Follow up on 11/18/11 resulted in restricted duty on 11/19/11. Investigation complete by workers' comp carrier and on site and determined reportable 11/22/11.

Employee drove haul truck into a pothole that was hidden by mud. He was slammed back into his seat which caused pain in his lower back. He was seen at the ER and released to full duty. On follow-up on 10/13/11, he was placed on sedentary duty, making this reportable for restricted duty.

Employee was disconnecting a hose from another hose. They came apart suddenly and one came back and struck him in the mouth.

IW was using air to clean steel cup on blast hole drill. The cup pressured up and released air, expelling fine sand, gravel and oil. IW was peppered by debris. He was examined at he ER and given one week of physical therapy to assist in removing debris from his left arm. Released to full duty.

IW saw fuel leaking under truck, ran under the truck to stop leak and hit his head on the ladder, jamming his neck and back and falling to his knees. He was seen at the ER, diagnosed with concussion and neck/back strain. Released to restricted duty, and then placed off work due to pain.

Employee was bringing some pipe over to be installed and set the pipe on his shoulder to carry it. When he set it on his shoulder, he set it down too hard, causing a contusion.

While employee was changing bucket teeth on a shovel, a small piece of metal broke off the the hammer or the tooth and embedded in his left hand. Taken to clinic, referred to orthopedic surgeon. Metal was surgically removed on 10/20/10 and the incident became reportable that day. Employee released to full duty on next scheduled shift of 10/23/10.

Employee was dismounting the haul truck during a pre-shift inspection. He slipped on the deck and slipped down a couple of steps, hitting his elbow on the metal railing. The bruise had to be drained on a subsequent medical visit on 09/28/2010, making this reportable for medical treatment. Employee released to full duty.

Operator of the lube truck stepped out and onto a rock, twisting his ankle. Intially the operator felt the ankle was ok and continued to work. He was later seen by a doctor and restricted from working on August 19th due to a severe sprain.

Employee was unhooking 2 rolls of liner that was being placed on a stock pile. The loader operator released the tension on the chain so employee could unhook the straps. Rolls were split on two levels one on top and one on the ground. The bottom roll moved and tightened the chain pinching the employee's ring finger between the loader fork and the chain.

Wheel chock fell from top of truck and caught employee's ring finger between the chock and the edge of the truck.

Employee was holding a hydraulic filter housing while another mechanic removed the mounting bolts. When the last bolt was removed the filter housing dropped and the employee's left ring finger was pinched between it and the frame, resulting in a fracture and laceration requiring stitches.

Employee was descending the stairs of a haul truck, missed the last stair and stepped down hard on his left foot on the landing. Employee felt a pop below the left knee. Employee was diagnosed with a torn left calf muscle and placed on restricted duty.

Co-Workers working together in close quarters, one holding cable while the other cut through rubber lining with grinder. Grinder became bound up in rubber and jumped coming into contact with coworkers right index finger.

Driller was thawing out casing with propane torch. 30LBS propane bottle fell or was knocked off from bucket it was set on and landing on a pointed spearhead rupturing the bottle which started a fire. The driller ran out. He had burns on his hands and to a lessor degree on his face. The fire was put out and he was transported to mine first aid and to Hospital.

While dismounting a dozer employee slipped on a frozen tire groove and twisted his left ankle.

Sustained an abdominal injury (umbilical hernia) while reaching high into wheel group of a Komatsu 830E haul truck while standing on a ladder to remove a wedge which weighs approximately 15 lbs. Surgery performed on 29-Jan-2009 to repair the hernia.

Employee was using a power drill to remove some screws from a pulverizer when the drill spun and struck employee in the mouth causing his front tooth to break. Mine was notified on 11/26/2008 that medical care was needed/

Employee was closing a hinged hood on a lube truck and smashed finger in the hood.

While running out drill pipe on a truck employee rolled a piece of pipe onto his right instep. The result was a contusion and swelling on the right foot. The employee received a 7 day light duty release.

Crew was tripping pipe and helper reached over hydraulic wrench to secure it to the drill pipe. As the hydraulic ram attached to the wrench retracted, the skin on the helper's upper arm was drawn into the ram creating a large abrasion / blood blister.

Employee was backing under the shovel when she struck the shovel bucket with the bed of the truck causing a whip lash injury.

While loading a bucket tooth into the back of a service truck, the left ring finger was caught between the tooth and the truck bed resulting in a laceration requiring sutures.

Employee was walking past the Wash Bay on the way to the Ready Line and slipped on the ice striking her back and neck.

While taking flow meter readings at Mooney, slipped on liner. There was snow on the liner. He had climbed to the top of the snow covered berm (liner) and fell, injuring his right shoulder.

Employee bent over to pick up a 2" ball valve that weighs less than 5 pounds. When he stood up, he felt a sharp pain in his lower back. Diagnosis was a strained back muscle.

While cutting a piece of perforated pipe with a box cutter in an upward motion, the box cutter released from the pipe and lacerated the left palm. 4 sutures required for wound closure.

Employee pulled his back muscles when a door he was against popped opened and the employee had to catch himself.

Drill helper lost his balance and reached back to derrick to steady himself as driller was running the tophead down. Two fingers of helpers left hand were pinched by tophead rollers resulting in lacerations.

Maintenance Tech was walking under a mezzanine when a motor cover fell and struck employee's left shoulder. Employee was initially treated on the 22nd and given a full duty release. He returned to the Dr. on the 27th due to persistent pain and was placed on light duty and further tests were ordered to see if there was further injury than initially indicated.

Employee was operating the Lube Truck when he lost control on the road at the narrows and overturned. Employee was wearing his seat belt at the time of the incident. It was determined at a later time that he had suffered a seperated shoulder and was placed on modified duty.

Employee was lifting a wheel chock that was covered in mud and felt a pian in her shoulder.

Helper was modifying sampling tools with cutting torch. When he adjusted the tool, the torch in the left hand passed over the back of the right hand. Result was a second degree burn on the back of the right hand about 21/2 inches across. Helper was wearing cotton gloves - not welding gloves.

Mechanic was replacing the fittings on the circle of the 14H blade. After replacing the fittings, he put the line up bar into the hydraulic swivel valve to secure it. The valve slipped off the bar when he attempted to bolt the valve back into place. He reached out to grab the valve to prevent it from falling. The valve landed on the palm of his hand, overextending his arm.

Cold frost on surface of things making the tongue on the trailer to the water tank slippery.

The EE was beginning maintenance work on a belly dump. When the EE attempted to pick up a block of wood and use it to chock the trailer, he felt and heard a pop as he lifted. The block weighed approx 50 lbs. The EE reported the injury on 11/21/06 and sought medical attention the same day. Diagnosis was an upper back strain.

Truck Driver pulled into the RBM ready-line and was dismounting his truck when he stepped onto a rock and rolled his ankle.

Two employees were attaching a clamshell to a piece of drill pipe. One employee rolled the pipe without looking where the other employee's fingers were located. The rolling pipe pinched the other employee's right pinkie finger. Stitches were required.

While tripping pipe on an angle hole, driller swung pipe onto pipe truck with the jib boom. The pipe came down faster than anticipated and it struck the helper in the right ankle. Result was a fractured ankle.

Employee was climbing down off of one of the Company's 16G Graders, when he went to grab the handle on the hood the hood moved. Employee jumped back to keep from falling and landed off at an odd angle on his left foot. INJURY WAS INITIALLY NON REPORTABLE. He received a partial tear to his achilles tendon which was discovered on June 5th and operated on on June 8th 2006.

Employees were emptying a scrap metal bin when the forklift they were using began to tip when the metal bin was lifted into the air. The injured employee stepped up onto the forklift to act as a counter-weight. When the load was released and the lift came down they employee fell to the ground and landed on his right wrist.

EE was holding the pull-back cable while he was drilling. His glove was pulled between the sheave and the cable. His 4th finger on his left hand was cut in 2 places. A total of 2 stitches were required.

Employee had pulled into the fuel island and was dismounting his haul truck. As he stepped down off of the ladder he stepped onto a frozen clump of mud and twisted his ankle. Twisting also caused a bone in his foot to fracture.

Slipped and fell on muddy ground while lifting a piece of pipe. Pipe fell on arm, bruising right forearm.

Employee was inspecting a Drill Steel after removing from the drill for damage when he stepped in two separate drill holes that were hidden by snow. Area had multiple open holes and is frequently used to test drills after repairs have been completed. Employee twisted his mid-back and right knee.

OUR EXPERIENCED TIRE MECHANIC WAS REPLACING THE INSIDE DUAL TIRE ON A CAT 785 HAUL TRUCK WHEN THE SNAP RING SLIPPED FROM HIS HAND & CAUGHT THE TIP OF HIS RT. RING FINGER BETWEEN THE RING & THE WHEEL. THE INJURY WAS TO THE FLESH ON THE BOTTOM OF HIS FINGER. EE WAS TAKEN TO AN E.R. WHERE THE WOUND WAS CLEANED & HE WAS GIVEN A PRESCRIPTION FOR PAIN MEDICATION.

Employee got right thumb caught between ram and pipe wrench. Laceration requiring 3 stitches

The Geotemps employee was driving a pickup with the windows open. He drove into a pebble lime storage area striking a 2ft high pile of lime. The lime scattered with some coming into the cab of the pickup. It appears the employee inhaled some of the lime dust. Two hours after the incident he complained about chest pains and was transported to the emergency room.

Employee drove through a road that was covered in lime. Window in truck was open and employee inhaled dust. Inhalation was the cause of burnt lungs. Employee was put on 3 days of light duty. Employee went back to hospital on 8-14-05 was put on light duty for 5 days. Laid off atter the 4th day. Post accident drug result-negative.

Fuel truck parked too close to haul truck 2-3' operator stepped back from bottom rung on haul truck ladder and bumped into the bumper ont he fuel truck. The operator hit the rear of his right leg just above the knee. The tendon became swollen causing nerve pain.

WHILE PERFORMING A PRE-SHIFT EQUIPMENT INSPECTION ON A CAT 777 HAUL TRUCK, A WORKER, EMPLOYED THROUGH A TEMPORARY STAFFING SERVICE, CLOSED THE PASSENGER SIDE DOOR ON HIS RIGHT MIDDLE FINGER. THE RESULTING INJURY (LACERATION) REQUIRED FIVE STITCHES TO CLOSE.

EE WAS WAKING A WATER RUN, FELL ASLEEP AT THE WHEEL AND RAN OFF THE ROAD

CHANGING PIPE ON LEACH PAD. ROCKY SLOPE ON LEACH PAD CAUSED THE EE TO SLIP AND FALL BACKWARD, LANDING ON A SOFTBALL-SIZED ROCK AND CAUSING A LARGE BRUISE ON THE LEFT BUTTOCK. GIVEN PRESCRIPTION FOR PAIN PILLS AT ER; X-RAYS NEGATIVE. (DELAY IN FILING INJURY REPORT BECAUSE COMPANY THOUGHT ONLY 1X INJECTION FOR PAIN WAS GIVEN AT ER.)

Worker was repairing a tire for a Cat 777 haul truck. The tire was "flat on the ground," with the tire hand still attached. The worker needed more slack in the air hose and so he "whipped" the hose, pulled, but did not realize that it had become tangled in the levers that control the hand. This caused the tire to shift and pin his leg against another tire also "flat on the ground."

Cold weather, icy frozen ground conditions. Worker had both feet on the ground when she lost her footing, fell backwards to the ground striking her tailbone on the frozen surface.

EE IS A HAUL TRUCK DRIVER. HE WAS DESCENDING THE LADDER FROM THE CAB AREA OF THE TRUCK. THE WEATHER CONDITIONS WERE WINDY, ICY, WITH BLOWING SNOW. EE SLIPPED AND FELL APPROX. 8-10 FEET, LANDING ON HIS BACK IN A SNOWDRIFT. NOTE: THIS BECAME REPORTABLE BECAUSE OF PRESCRIPTIONS. EE WAS BACK TO WORK FOR NEXT SCHEDULED SHIFT.

THIS WAS EE'S 2ND DAY OF WORK FOR EKCO DRILLING. HIS DUTIES INCLUDED CATCHING SAMPLES & MAKING CONNECTIONS. EE COMPLAINED OF A SORE BACK AND WAS TRANSPORTED TO ELKO. HE TOLD US THIS HAD BEEN AN ONGOING CONDITION SINCE PLAYING HIGH SCHOOL FOOTBALL.

CONDITIONS WERE VERY WET AND MUDDY. EE WAS FUELING AND SERVICING HAUL TRUCKS ACROSS FROM THE LIME PILE IN VERY DEEP MUD WHEN HIS CLOTHING BECAME WET AND MUDDY. EE STARTED TO FEEL BURNING ON HIS LOWER LEGS AND IMMEDIATELY SOUGHT FIRST AID ATTENTION. EE SUFFERED CALCIUM OXIDE CONTACT DERMATITIS.

WHILE OPENING WINCH VALVE, BODY MOVED INTO 6" PIPE.

WHILE WELDING ON SHOVEL BUCKET RECEIVED FLASH BURNS TO BOTH EYE AREAS.

WHILE DESCENDING BLAST HOLE DRILL RIG, MISSED LAST STEP. TRIED TO CATCH HIMSELF WITH LEFT ARM AND DISLOCATED SHOULDER.

Moving pipe; lack of knowledge of proper method, severe strain, placed on restricted duty, no lost time.

EE did not hook up ladder properly, and so he fell with the ladder when he was climbing it.

At the time of the incident the operator had his head turned to the right watching his apron on the scraper close. He had picked up speed but had not engaged his cushion hitch when he hit a bump. According to other scraper operators the ground conditions were not abnormal or excessively rough.

WHILE MOVING A COMPRESSOR, THE COMPRESSOR TIPPED OVER AND LANDED ON RIGHT LEG.

While lifting a brake drum, he felt a strain in the lower back.

While starting to take strip solution sample, ee slipped. He had his hand on a valve and when he slipped the valve came open and the hot solution started coming out of the hose attached to the valve. He was sprayed with hot solution on his chest and abdomen.

VANS WERE PREPARING TO DEPART THE MINE SITE AND RETURN TO HOME (ELY, NV). EE WAS SITTING IN THE VAN AND STARTED HAVING TROUBLE BREATHING. HE BECAME UNRESPONSIVE, WITH NO VITAL SIGNS. CPR WAS S TARTED. LIFE FLIGHT ATTENDANTS ARRIVED AND CONTINUED CPR UNTIL A DOCTOR AT THE HOSPITAL IN ELKO ADVISED THAT THEY COULD STOP CPR.

WHILE GOING DOWN LADDER USING 3 POINTS ON CONTACT, STEPPED DOWN WITH RIGHT FOOT ONTO A ROCK. IN TURN TWISTED RIGHT ANKLE WHEN HE STEPPED ON ROCK.

WHILE UNLOADING DRILL PIPE ONE PIECE OF PIPE WAS CROOKED ON THE PIPE RACK. ATTEMPTED TO STRAIGHTEN PIPE ON PIPE RACK WITH A BAR. AS THE BAR WAS PULLED A PIP WAS FELT IN THE LOWER BACK.

WHILE BACKING DOWM RAMP IN A HAUL TRUCK, RT REAR OF TRUCK CONTACTED A CUT, HAUL TRUCK TRAVELED UP THE CUT & THE TRUCK LAID OVER ONTO THE DRIVERS SIDE.

3 MEN WERE ASSIGNED TO REMOVE A CATWALK FROM THE CRUSHER AREA. CATWALK HAD BEEN REMOVED & WAS BEING LOWERED TO GROUND WITH A CRANE. CATWALK BECAME UNSTABLE & CONTACTED EE'S HAND PINCHING IT BE TWEEN THE CATWALK & THE CRUSHER. EE HAD PLACED HIS HAND IN AN UNSAFE MANNER WHICH RESULTED IN HIS INDEX FINGER BEING PINCHED BETWEEN THE CATWALK & THE CRUSHER. THIS RESULTED IN 8 STITCHES.

WHILE UNLOADING A SHIPMENT OF UNR 811, FUMES OVERWHELMED EE. UNR 811 IS A MERCURY INHIBITOR CHEMICAL.

DROVE OFF ROAD IN A PICK-UP TRUCK AND INJURED HIS BACK.

WHILE DISMOUNTING TOP LANDING ON WASH BAY, RIGHTFOOT CAUGHT IN TOP LANDING , RIGHT LEG AND KNEE GOT TWISTED MOMENTARILY

WHILE WORKING ON A 3/4 TON PICK UP FRONT AXLE IN THE VISE ONT HE WORK BENCH, THE AXLE FELL OUT OF THE VISE AND STRUCK EE S RIGHT FOOT JUST BEHIND THE STEEL TOE. EE WAS TRANSPORTED TO HOSPTIAL X RAYS WERE NEGATIVE FOR FRACTURES, EE GIVEN 3 DAYS LGIHT DUTY.

WHILE LIFTING LIGHT VEHICLE ON TO THE TIRE MACHINE. EE FELT PAIN IN HIS BACK. EE WAS TREATED BY CHIROPRACTOR AND RELEASED TO FULL DUTY.

EMPLOYEE STEPING OFF LUBE TRUCK WHEN HE SLIPPED AND FELL.

EE BENT OVER TO WALK UNDER BLADE. WHEN HE STOOD UP HIS BACK POPED.
