Employee coming downstairs of a haul truck, their knee popped resulting in 2 tears that require surgery.

Employee was entering mine site for start of shift and felt something hit their eye. Prescription medication was later given to prevent any infection due to evidence of a foreign object that had been in the eye.

EE was finishing blowing off load of Cyanide into ADR tank. Went to back of truck and Cyanide Crystals fell from top of Storage tank and hit EE, the trailer, and the ground. The wind was blowing toward back of tanker. EE was not sure if EE had inhaled any or not but had a co-worker watch EE disconnect then go get the Supervisor. EE then called mayday.

EE working on conveyor system at ground level had foreign debris blown into his left eye by a strong gust of wind, resulting in a scratched cornea.

An employee was walking down a ramp and twisted their knee. The employee was transported to the hospital where they were released to restricted duty.

Employee was demobilizing from site when the employee sufered a heart attack. At the time of the event the employee had been assisting the forklift operator by guiding him into place. Employee died of natural causes due to heart disease.

Employee was walking across the yard towards the batch plant when a large gust of wind came up & blew dust in her eyes. She had help flushing her eyes out with saline solution. The next day she came to work & her eyes were still irritated so she was taken to a Dr. where more material was removed from her eye.

Wind was blowing extremely hard - when employee got out of haul truck at fuel island & felt something hit her eye.

Strong wind gusts blew dust through the duct system which ended in an employee's eyes.

Operator got out of his dozer to check dump "toe stakes" and the wind blew lime dust in his eye.

Stated during the shift, the wind blew dust behind his safety glasses.

AN ALTERCATION ENSUED BETWEEN THE MACHINIST AND A WELDER, THEN THE WELDER STRUCK THE MACHINIST IN THE FACE.

GRINDING OUTSIDE ON VERY WINDY DAY.

EMPLOYEE WAS OPERATING A LOADER. HE TURNED ON THE AIR CONDITIONER AND A PIECE OF DEBRIS BLEW OUT AND INTO THE EMPLOYEE'S EYE.

EMP WAS INSTALLING LINER MATERIAL WHEN WIND BLEWDUST/DIRT IN RIGHT EYE.

EMPLOYEE RECEIVED FIRST AID FOR A CENTUSION & SMALL LACERATION RECEIVED WHEN HE PINCHED HIS THUMB ON 02-02--93 THE INJURY BECAME INFECTED & HIS DOC GAVE HIM LIGHT DUTY ON 02-09-93 TO LAST THRO UGH 02-12-93

A GUST OF WIND BLEW DUST CAUSING A FOREIGN PARTICLE TO GET UNDER THE EMPLOYEES PRESCRIPTION SAFETY GLASES AND INBEDDED IN RIGHT EYE.

EMPLOYEE STATES HE WAS SITTING DOWN EATING HIS LUNCH OUTSIDE THE ADR/CRUSHER MAINENANCE OFFICE, WHEN THE WIND BLEW A LADDER OVER AND IT STRUCK HIM ON THE HEAD, LACERATING HIS SCALP.

EMPLOYEE STATES HE WAS WELDING AND THE WIND WAS BLOWING, SO HE IS NOT SURE WHAT BLEW INTO HIS EYE, BUT ON 7-7-90 HIS EYE BEGAN TO SWELL SO HE SOUGHT TREATMENT IN TONOPAH. DR. REMOVED A SMALL P ARTICLE FROM HIS RIGHT EYE AND GAVE HIM AN RX FOR PAIN AND RX SALVE AND RELEASED EMPLOYEE FOR WORK. EMPLOYEE WAS ON SCHEDULED DAYS OFF ON 7-7&8 AND RETURNED TO WORK ON 7-9-90. REPORTABLE DUE

EMPLOYEE STATES SHE WAS WALKING FROM ONE ROOM TO THE NEXT ROOM WHEN HER RT KNEE GAVEWAY THIS WAS NOT REPORTED UNTIL MIDSHIFT ON 02-28-90 RPORTABLE DUE TO RESTRICTED WORK EMPLOYEE FAILED TO REP ORT INJURY UNTIL 02-28-90 SO THE CLAIM WAS CHALLENGED TO STAT INDUSTRIAL INSURANCE WHICH ACCEPTED CLAIM & NOTIFIED US ON 03-20-90

EMPLOYEE STATES3THAT HE FELT A PAIN IN HIS BACK, AS HE WALKED FROM THE READY LIVE TO THE CHANGHOUSE. ALSO, HE CLAIMS HE DID NOT SLIP, TRIP OR FALL.
