Employee was walking from the car to the time clock to start shift on Thursday morning when EE collapsed. Nearby employees heard EE fall and began CPR, called EMS, went for the nearest AED machine, and sent employees to escort EMS personnel to EE when they arrived. EE passed away the following Tuesday night (8/11).

MSHA Inspector issued citation of 25dB average threshold shift on the 2000,3000,4000 Hz range-left ear. Audiologist, Industrial Hearing, identifies no reporting required. Requested health conference to vacate - MSHA inspector requesting 7000-1 form to terminate citation paperwork.

It is unknown at this time but cause of death appears to be a heart related. Employee was operating a Cat 777F haul truck and struck berm and truck came to rest on berm approx 400 feet from initial contact.

Employee walking on clean conveyor walkway and heard pop in knee. No days away from work and working full duty and Meniscus tear not discovered until 6/3/13 after MRI.

Employee suffered heart attack in parking lot of quarry before start of shift.

Employee arrived at work not feeling well. As the afternoon progressed he began to feel worse. He was driven to the emergency room at local hospital.

Employee suffering with chest pains and numbness in arm. Emergency personnel was contacted and employee transported to local hospital where he was diagnosed with a heart attack.

Employee arrived at work complaining of chest pain and arm pain. Pain increased during evening and 911 was contacted.

EE WAS WELDING ON A PIECE OF HEAVY EQUIPMENT ON A HOT, HUMID DAY. OUTSIDE TEMPERATURE EXCEEDED 95 DEG. EE HAD SCARRING ON BOTH LEGS FROM AN ACCIDENT BACK IN EARLY 1980'S. THE BURN/SCARRING BEC AME AGGRAVATED FROM THE HIGH HEAT AND HUMIDITY. EE'S LEGS BEGAN BLEEDING AND SUSTAINED BLISTERS.

EE WAS CROUCHED DOWN W KNEES BENT. EE WENT TO STAND UP & FELT PAIN IN HIS RIGHT KNEE

EE WAS ON STAIR LEADING TOO FUEL PUMP AREA. EE WAS SWORMED BY BEES WHICH CAUSED HIM TO JUMP BACKWARDS. THE EE STRUCK HIS LEFT ELBOW WHICH SWOLLED UP I DATE LATES.
