Mostrando los 5A contracted carrier loaded with cement was traveling on the plant road. The driver was speeding and ran off the road causing the truck and trailer to tip onto its side. The driver sustained a laceration to the head and was transported to a local hospital where stitches were required and a cracked vertebrae was discovered. Driver was also not wearing a seatbelt.
Employee was backing away from dryer crusher and their elbow got caught on the clamshell door and when it did it put their wrist in a bind around the control lever guard of the walk behind skid steer they were operating.

While offloading material from a haul truck, a hydraulic cylinder broke, causing the bed to fall unexpectedly. The employee suffered a strained neck and was initially treated with First Aid. After five days, they sought medical attention, were diagnosed with an irritated nerve, and returned to work with restrictions.
