Amrize Paulding Plant Metal/Non-Metal

The employee was holding a spacer against a wedge while another employee was hammering the wedge into place. During the activity, the spacer being held by the employee slipped causing the employee's finger to be smashed between the spacer and another piece of hardware.

On 7/20/2023 at approximately 11:00 A.M. an employee working on site at Holcim Paulding Plant sustained an injury to the upper left leg. The employee was working to remove the Torch Scribe Stand when discomfort was felt in the upper left leg. The employee was taken for medical evaluation.

The employee was moving a pipe and smashed their left middle finger between the pipe and the guard on the forklift. Initial first aid measures were applied. The employee sought a medical evaluation on 12Jul23 through company case management system. Evaluation concluded nondisplaced fracture of proximal phalanx of left middle finger along with laceration and abrasion around nail.

The employee was unloading a railcar. When the railcar was empty, the pump remained running and valve open to keep vacuum on the hose. The employee disconnected the hose and began to move it away from the railcar before capping the hose. During this movement, the hose lost its vacuum and material splashed onto the employee.

EE finished unloading a tanker truck, the unloading hose was unhooked from the truck. The employee then removed the adapter and moved the hose prior to capping the hose. There was a loss of vacuum on the hose and a small amount of material spilled on the EE and ground. As the employee was maneuvering around the spill the employee slipped and landed on their right knee.

An employee was climbing a short set of stairs into the office building when EE missed a step causing EE to fall forward. EE caught self on left hand causing a wrist sprain.

The Injured Employee lost their balance due to stepping on a rock. While falling, they turned and put their hand down to brace the fall, scraping the elbow and hurting the wrist/thumb on their left hand. This injury was being managed through various visits to Dr/Clinic but became Reportable on 8/5/2021.

Employee was cutting bolts of a tripper valve. Grinder kicked back cutting left middle finger above first knuckle.

After unplugging a dust collector, a Production Assistant closed the system but the access door had been damaged so did not close properly at the top. After the system was restarted, dust was blown out of the system and struck the employee in the face. The employee visited the local hospital and was examined by an eye doctor, where employee was prescribed medication for eyes.

Employee was reaching for a valve at waist level and felt a pop and pain in the lower back. EE never touched the valve.

Cleaning air conditioners and changing filters in the MCC area. As employee was pulling air hose to unit, coil in the hose caught. When the air hose caught, they tripped, fell, and injured left shoulder.

Process operator was repairing a leaking cement line gasket, when dust got underneath safety glasses and into left eye. Employee immediately reported the injury to supervisor. Irrigated the eye for 30 mins. Believed they removed all dust. Next morning employee woke up with an irritated eye. Went to physician who prescribed medication for possible eye infection.

Employee was completing an S.O.P. cleaning out the #1 kiln D.A.A. conveying system. Employee had to position self on the floor to gain access to the south & east side bolts that attach the hopper onto the bottom of the rotary valve. It is while EE was removing the hopper that EE believes EE came in contact with the hydrated lime product causing a skin burn on EE's pants line.

A millwright was working outside of the maintenance shop on a hopper that the millwright was building for the CKD briquetter. While drilling a pilot hole for a liner, a 1/8" drill bit broke. Once the 1/8" drill bit broke, it pierced the leather gloves at the seam. The seam was between the thumb & index finger. The drill bit went into millwright's flesh about 1/4" deep.

A millwright was working on a pinion bearing replacement job on our raw mill and felt a pain in knee. EE does not know what caused the issue. This incident was initially reported as a first aid and later became a medical.

Employee was in the process of removing bridge from crusher throat. A rock shifted and temporarily pinched EE's left foot. EE was able to kick off the rock and remove self from the area. EE's foot was bruised.

Two employees were a team lifting a window air conditioner up from the floor to place it on a stack of pallets. During the lifting process EE felt a strain in the groin area.

EE WAS TRIMMING A PIECE OF SHEET STEEL SO IT WAS EASIER TO HANDLE. THIS PIECE WAS 1/4"X 2'X 4' EE WAS HANDLING THE STEEL WHEN IT SLIPPED OUT OF HAND AND LANDED ON LEFT FOOT RIGHT BEHIND STEEL TOE. Originally this was a 1st aid however due to the swelling not going down ee was checked out an via an x-ray 10/17/16 it was determined ee fractured second metatarsal.

While cleaning at #2 belt tail, employee was using yellow hose to vacuum up water, mud, and rock. The yellow hose was stuck, so employee used a utility knife to cut the hose loose. During the cutting process the knife slipped off the hose and stabbed EE's left leg in the shin.

As employee was using a high pressure fire hose to clean under the main rock conveyor, he started to stand up. As he was starting to stand up, the water pressure increased causing him to lose his balance. He caught himself from falling backwards by extending his right arm behind him. When he did this, he heard a popping noise that sounded like knuckles cracking.

Contractor operating a forklift on the kiln floor while replacing refractory struck a fuel line to the other operating kiln. This resulted in fuel being spilled and eventually igniting. Fire was contained and extinguished by facility personnel by 11:15 pm. Local fire department arrived as well due to emergency call protocol.

EE was cleaning the base plate for #1 kiln uphill thrust roll. He was using a scraper to clean heavy oil debris off of the base plate on his knees. While in this position he felt pain in left knee cap area. After 1 hour he noticed swelling and increased pain and notified supervisor at 7am. Employee took ibuprofen and elevated leg while applying ice.

Employee was working in a fume hood attempting to unscrew a dispenser from the bottle of nitric acid. The outlet to the dispenser had a screw cap in place but contained a trace amount of acid that was not seen by her. The slight shock to the dispenser from the force of turning it, caused a drop of acid to fly up and hit her in the left eye.

Staging a job for kiln roll replacement. While staging the saddle they began to lift it which has a d-link on each end & when it stood upright while still in contact with the concrete ground it began to spin. EE grabbed a hold of the saddle by instinct w/gloved hand to try and stop it. The momentum carried his hand along w/saddle pinching it between a skid steer which was 1' away

#2 Mod screw was overload. After locking out & ensuring the screw was de-energized, employee removed guard & proceeded to spin the screw backwards using large sheave on gear reducer. Hands were inside the sheave spokes & turned counter clockwise. On the last turn the tip of his little finger got caught between the drive belt and the bottom of the sheave.

Walking out of the kiln employee ran into a chain result in a laceration above left eye, required 4 stitches.

While removing a pump follower plate for maintenance, employee caught finger between the plate and the pump shaft. Result in a facture and laceration. Right hand pinky finger.

EE was walking through the off load area when he felt what he describes as a pop in his foot. He was walking on a flat surface, clear of any debris. EE reported the incident, and continued with work. When he visited the podiatrist on 2/25/11 he discovered that two bones in his right foot were broken. EE returned to work the following Monday 2/28.

Employee was in the man lift and was attempting to loop the welding lead over the kiln shell when the connector came loose stricking the employee in the mouth. Sought medical treatment and received a couple stitches.

At ~ 10:15pm 2/7/09 a natural gas explosion/fire severely damaged the lab and office bldg. Natural gas leaked from either a heater or gas supply line to the heater. No injuries or fatalities resulted from the incident. The adjacent cemnet plant heard the explosion, called the fire Dept. and made the first MSHA notice. A second MSHA notice was made by Systech to (800) 746-1553.

Employee was using the shop plasma cutter. Laid the plasma handle on his left thigh as he bent down to move the angle iron. The plasma gun has a yellow safety switch caught on a piece of clothing and pushed it forward, then the red main trigger was depressed while he was moving. With both of these activated, the plasma cutter turned on and burned him in two areas.

Employee was removing dump cylinder from haul truck, while removing the cylinder, it swung forward and pinched his hand between fuel tank and cylinder. (misc mail 8/7/08 #2- He received medical attention and 5 stitches.)

Employee was pulling/prying on a bar trying to free up the #2 clinker drag when he heard a pop sound and felt a strain in his lower right side rib area. Employee was going to take it easy over his vacation and he would contact us before his next scheduled day back if there were any problems. Employee did not seek medical attention until 1/2/08.

While employee was passing throught the #2 kiln area (on his way to the tankhouse) he got dust in his eye.

Employee was operating the bobcat inside and outside the baghouse, removing piles of CKD dust that had built up. EE said that he noticed his eye was feeling irritated. At some point in time, dust flew beneath his safety glasses and entered his eye.

Employee was swinging a sledgehammer to beat a door loose. While doing so he started to feel back pain that continued throughout the day. He did not seek medical treatment until 5/14/07.

Employee pinched finger between loader forks and bottom of fork support.

Employee was using a cutting torch and a hot piece of slag fell between his cutting goggles and safety glasses hitting his left eye.

A rock was in the road. The operator ran over the rock, jarring his back.

Employee strained shoulder while pulling a 2" water hose to position for cleanup activities. The hose was buried in material build up and had to be pulled out. Employee sought medical attention on December 5, 2006 and was placed on restricted duty until further medical follow-up.

Employee was rolling the kiln (working on firing it back up) when he disengaged the transmission and the housing blew apart into many pieces striking him on his legs which knocked him off the pier. He was transported by LifeFlight to Parkview hospital in Fort Wayne. He has 2 deep severe lacerations to his upper and lower leg, gashes on his hips and a broken right ankle.

A 20mph gust of wind blew dust particles in the employee's eye. After trying to flush it out at the eye wash station the employee noticed that there was a particle imbedded in his eye. He then went to the ER, had the particle removed.

Mechanic was moving the runner in place for installation by rolling it slowly across the floor. There was a sharp spur on the side of the runner that caught the mechanic's right hand (palm) piercing through his glove and consequently puncturing the skin. The mechanic was driven to the emergency room for stitches.

Employee inadvertently stepped into low sump pit covered with water and injured ankle. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 4/24/06.

Employee was using a jackhammer to break away chunks on the silo's cone. When some chunks broke loose it pivoted the jackhammer upward snapping Tracy's arm upward. It appeared the right elbow was hyper-extended.

Employee was using a 45 pound jackhammer when jackhammer became stuck in cement. After jackhammer was freed, employee reported that his right wrist was a little sore at the end of the shift but that it grew increasingly worse overnight. Employee reported injury at the beginning of his shift the following day.

Employee was working on empting #1 kiln hot air cyclone when it flushed shooting dust out of a broken cap. The hot dust burned him on his neck and face.

Employee was going down the stairs on the S side of #2 coke tank to inspect #2 drag. After stepping off of the last stair his foot landed on a piece of clinker resulting in his ankle turning over.

Was moving a horizontal slide gate with a cumalong to select a different discharge hopper. When applying force the cumalong handle broke resulting in his left elbow striking the railing behind where he was standing.

Employee was pulling hose up whe he felt pain in his back.

Employee was reaching into the northeast hopper on the #1 Dracco when the material above the door flushed. He had his head turned to the side as he performed the work and the falling material deflected off his shoulder striking him in the face. Some of it managed to get under the side shields of his safety glasses and entered his eye.

While measuring the silos something flew into his eye.

Employee attemped to weld. He had troubles with ground clamp and had to re-position it several times in order to get a good path. While doing so he may not have had the welding hood fully lowered before striking an arc. He did not report injury immediately.

Vacuuming up cement with hose off Vac. Trk. Cement got into his boots and under his socks, creating burns. He didn't notice the burns until he was home in the evening, though he felt some irritation during the day. He did not report incident until 6/1/05 and at such time we sent him for medical treatment.

A bulk customer needed to have three bags of Federal White Type I (94 lbs) loaded onto the tractor. While lifting the last bag head high, the ee felt a strain in his middle back. During the shift, the employee took a OTC pain reliever to make it throught the shift. He reported the incident on April 26th once the ee expressed interest in seeing a chiropractor.

Employee was prying a knife gate open with a pry bar. The bar slipped and smashed his right little finger. The employees finger was cut and broken, and required surgery.

While working on # 2 kiln feed pump EE's foot slipped on the floor that was covered by slurry while prying open pump cover thus injuring his lower back. He was seen multiple times at a Chiropractor.

Employee was driving haul truck and hit a bumb in the road, jarring his neck.

Employee was removing nuts on a cover for the tail wheel of #1 Clinker cooler frag. While pulling on a wrench he felt something pop in his right shoulder. He felt pain in his arm and his hand swelled.

EE was welding on a clay wash plow in the shop. He was chipping slag off a weld when a piece flew up under his safety glasses hitting him in the eye. EE saw a physician for a corneal abrasion for which he was prescribed an antibiotic ointment.

Ee was trying to insert a 2ml glass pipette into a dispenser when the pipette broke. The broken end of the pipette cut his palm & three of his fingers. He was taken to local hospital & received five stitches on his pinky finger.

The employee was emptying the number one fringe feeder. As she removed some small pieces of material a large chunk broke free striking her left hand.

THE EMPLOYEE WAS DRIVING A PIN INTO A DRIVE CHAIN. HE STRUCK HIS THUMB WITH THE HAMMER.

THE EMPLOYEE WAS SPINNING A RUNNER ON A PUMP. THE RUNNER BROKE LOOSE AND CAUGHT THE EMPLOYEE'S FINGER BETWEEN THE BELT AND SHEAVE, LACERATING THE FINGER REQUIRING STITCHES.

THE EMPLOYEE WAS CHECKING A WATER VALVE WHEN A SLIVER OF METAL BLEW OUT OF THE PRESSURE RELIEF VALVE AND IMBEDDED IN THE EMPLOYEE'S RIGHT ARM.

EE WAS CUTTING A BOX USING A BOX CUTTER (UTILITY KNIFE). THE EE OVER RAN THE BOX AND CUT HIS ARM.

EE WAS DESCENDING A SHORT 4-RUNG LADDER FACING AWAY FROM THE LADDER INSTEAD OF BACKING DOWN IT. HIS HAND SLIPPED AND HE FELL DOWN 4-5' LANDING ON HIS LOWER RT. LEG.

EMPLOYEE WAS SWITCHING A FLOP GATE ON #2 FINISH MILL SEPERATOR WHEN GATE HANDLE KICKED BACK AND CAUSED HAND TO BE PINCHED BETWEEN GATE AND LATCHING MECHANISM. EMPLOYEE CUT MIDDLE FINGER ON HAN D AND REQUIRED AN ADHESIVE TO BE APPLIED BY A DOCTOR.

EMPLOYEE OPENED THE DOOR ON THE BACK OF KILN #2 AND WAS POKING OUT THE BUILD-UP. HE REACHED INTO THE KILN TO IDENTIFY AN UNKNOWN OBJECT WHEN A PIECE OF MATERIAL SHIFTED AND TRAPPED HIS ARM AGA INST THE STEEL DOOR FRAME. HE WAS FREED BY PUSHING THE MATERIAL INTO THE KILN.

EMPLOYEE WAS CLEANING OFF THE SHEAVE ON THE #2 COOLER, #4 FAN. HE WAS SLIDING THE BELT WITH THIS HAND, IN ORDER TO TURN THE SHEAVE (AND THE FAN). HE LOOKED AWAY FROM THE BELT TO INSPECT THE SH EAVE, WHEN THE MOMENTUM OF THE TURNING FAN CAUSED HIS FINGER TO BE PULLED IN BETWEEN THE BELT & SHEAVE. THIS RESULTED IN CUTS TO THE RING & MIDDLE FINGER TO HIS RIGHT HAND.

DUMPING CLEANED UP MATERIAL FROM 55 GAL DRUMS INTO DUMPSTER. MINER WAS STEADYING BARREL WITH LEFT HAND WHEN HE HEARD A POP IN HIS SHOULDER. THE FOLLOWING DAY, SHOULDER WAS PAINFUL SO TREATMENT WAS SOUGHT. WAS GIVEN SLING TO WEAR UP TO 48 HOURS, THEN INCRREASE IN RANGE OF MOTION IS ADVISED. NO OPERATION OF MOVING EQUIPMENT OR OVERHEAD LIFTING FOR 7 DAYS.

WHEN EE STEPPED ON TOP OF THE TRUCK TO SAMPLE IT HE FELT HIS BACK BEGIN TO SPASM. ABOUT AN HOUR LATER IT BEGAN TO HURT MORE. HE REPORTED TO THE MANAGER THAT HE MAY HAVE TO SEE A CHIROPRACTOR. HE DID SO AT ABOUT 12:30. THE CHIROPRACTOR TOLD HIM THAT HE HAD STRAINED HIS BACK & SHOULD TAKE A COUPLE OF DAYS OFF WORK HEAL.

AT FIRST BREAK EE NOTICED EYE IRRITATION. HE HAS NO IDEA THE SOURCE BUT THOUGHT IT WAS AN EYELASH. EE IS A MOTOR MECHANIC AND HAD DONE NO GRINDING NOR ANY TASK THAT WOULD CONTRIBUTE TO GETTING A FOREIGN BODY IN HIS EYE. AWOKE THE FOLLOWING DAY AND HAD MORE IRRITATION SO WENT TO CLINIC.

HIT HEAD ON DOOR JAMB WHEN CLIMBING INTO TRUCK - INCIDENT NOT REPORTED UNTIL 6/18/02.

3 EE'S WERE ASSEMBLING A SHELVING UNIT. WHILE TRYING TO GET 2 PIECES OF METAL TOGETHER INJURED'S HAND SLIPPED & HE RECEIVED THE LACERATION TO HIS THUMB. HE WENT TO THE LOCAL HOSPITAL & RECEI VED 4-5 SUTURES ON HIS THUMB.

EE WAS INSPECTING A ROTARY VALVE. AFTER REMOVING AN INSPECTION PORT, HE WIPED DIRT FROM THE AREA. AT THIS TIME, HIS GLOVE WAS CAUGHT IN THE VALVES ROTATING COMPONENT. HIS RIGHT RING FINGER WAS DRAWN INTO THE VALVE, SEVERING THE TIP ABOVE THE FIRST KNUCKLE.

EE POURED 5 GALLONS OF OIL INTO A BUCKET, PICKED UP BUCKET AND FELT SHARP PULL BELOW LEFT SHOULDER BLADE.

MINER WAS CHANGING A HAMMER VALVE & WHEN PULLINGON THE BAR, INSERTED IN HOLES IN NUT TO LOOSEN & TIGHTEN THEM, HE FELT A PULL IN LEFT SHOULDER DID NOTHING MORE THAN COMPLETE ACCIDENT FORM (2001-017) NO TREATMENT. NOTIFIED COMPANY THAT PAIN CONTINUED & SOUGHT TREATMENT 4/3/02

EE TRIED TO EXIT A FORKLIFT, THOUGHT DOOR WAS STUCK SO SHE USED LEFT HAND TO POUND ON IT TO GET IT TO OPEN. HURT WRIST IN PROCESS.

EMPLOYEE PARKED LOADER ON AREA OF VERY UNEVEN GROUND AFTER HEAVY RAINS. WHEN DISMOUNTING FROM LOADER HE PLACED LEFT FOOT ON THE GROUND, SLID ON SLIPPEERY GROUND AND TWISTED FOOT AT THE SAME TIME.

EE WEARS ELECTRICIANS STEEL-TOED BOOTS WITH SOLETHAT TENDS TO GET CAUGHT IN TREDS DUE TO DEEP RIDGES & PLIABLE MATERIAL. 8/27/01 SHOULDER BEGAN TO HURT OVER WEEKEND NO PRIOR SYMPTONS

EMPLOYEE WAS KNEELING DOWN. HE DID NOT NOTICE ANYTHING FLY IN HIS EYE. HE JUST NOTICED SOMETHINGCAUSING IRRITATION.

EE BENT OVER THE COMPUTER TO RESET MACHINE WHEN HE HEARD SOMETHING SNAP.

MINER WAS USING A PORTA-POWER AND BLOCK OF WOOD TO SPREAD RAILS ON CLINKER COOLER DRAG BOX TO EXPAND THE GAP BETWEEN TWO ANGLES. A LOUD POP WAS HEARD. THE PRESSURE CAUSED THE WOOD TO BE FLUNG OUT HITTING MINER ON FINGER, WHILE PORTA-POWER &REST OF ASSEMBLY FELL INTO DRAG BOX. THERE WAS AFAILURE OF THE HYDRAULIC FITTING.

EE SLIPPED ON ICE & SNOW WHILE WALKING TO BREAK AREA. EE DID NOT GO TO DR UNTIL 12/15

EE WAS TIGHTENING THE HAMMER VALVE WHEN THE VALVE WENT LOOSE THRUSTING EE FORWARD SMASHING HER FINGER IN BETWEEN BAR & CEMENT LINE

EE WAS WELDING IN CLASS AND HOT SPLATTER FLEW INTO HIS EYE. THERE WAS NO DISCOMFORT SO HE DID NOT NOTICE IT UNTIL HE LOOKED INTO THE MIRROR.

EE WAS STEPPING FROM SIDE TO SIDE TO HIT OPPOSITE CORNERS ON A SLIDE UNDER IRON TAKE TO WALK THE SLIDE IN A CLOSED POSITION. HE WAS HITTING THE CORNERS WITH SLEDGE HAMMER. IN DOING SO, HE JAMB ED LEFT INDEX FINGER AGAINST THE ANLE IRON ON THE STRUCTURE.

THE MOVING FRAME BEAMS ARE APPROX 20' LONG AND MUST BE RE-SLUNG DUE TO THE LIMITED ACCESS AROUND THE COOLERS. MINIMUM OF TWO PEOPLE SHOULD MOVE PART.

EE WAS CLEANING IN THE COOLER WHEN DUST BLEW IN THROUGH THE ENTRY DOOR AND A CRACK. EE WAS WEARING ANSI APPROVED SAFETY GLASSES.

EE WAS TAKING OUT THE LINERS ON A FINISH MILL BYUSING A HAMMER. A PIECE OF THE LINER BROKE OFF &HIT HIM IN THE FOREHEAD, BETWEEN HARD HAT RIM & TOP OF SAFETY GLASSES

EMPLOYEE WAS REACHING THROUGH THE DRAG TO CHECK THE BULKHEAD FLOPGATE UNDER THE CLINKER COOLER. WHILE PRYING HE FELT A SNAP INSIDE HIS RIGHT ELBOW THAT CAUSED SEVERE PAIN.

EE WAS PULLING ON VACUUM HOSE THAT WAS STUCK ON FITTING. RATHER THAN LOOSENING HOSE FIRST, EE CONTINUED TO PULL TO DISCONNECT. HOSE SPONTANEOUSLY CAME UNDONE HITTING THE EE IN THE FACE. THE FO RCE HIT THE BRIDGE ON SAFETY GLASSES, THE BRIDGECUT THROUGH SKIN ON NOSE.

SHOVELING & JACKHAMMERING HARDENED CEMENT UNDR A FINISH MILL. EE CONT'D TO WORK & PAIN BECAME WORSE
