The employee was spotting empty barges forward when a wire rope became caught on the wrong side of a kevel. While attempting to reposition the wire rope, the rope under tension struck the employee in the ankle, causing the employee to fall to the deck.

Employee was removing a liner from inside number 1 raw mill, when the liner dislodged from the wall it slide down the mill bar the employee was using pinching their left index finger.

The employee was using a dial indicator to check runout on a motor sheave. After positioning the dial indicator, the employee manually pulled on the belts to rotate the sheave. During this process, the employee's finger became caught between the belt and sheave.

2 employees became trapped in elevator between ground and 1st floor. Employees were able to exit around 3:12 am without injury. Elevator was locked out pending elevator company release.

Miner was performing housekeeping duties when their arm was sucked into the vac truck hose

Employee was walking by an electric cooling fan when some dust was blown behind EE's safety glasses.

IE was closing a 15' x 15' raw mill door with another employee and the IE right middle finger was caught in between when the door (as it rebounded) and the nut on the locking mechanism. causing a smashed finger

Employee was using a utility knife and cut the side of the right hand. Employee went to Urgent Care for stitches.

Employee was in an elevator when it stopped between floors. Employee safely out of elevator after 45 min. Elevator was locked out pending release by elevator company.

While walking to the dumpster, employee felt that they stepped wrong and twisted the right knee. No trip, fall, contact with an object, vehicle, or equipment, or hazardous surface condition was reported. Employee resigned 7/22/2026 to start a new job before completing follow-up medical evaluation or receiving a full-duty release.

Employee was stuck in an elevator for approximately 3 hrs between floors. elevator technicians were called to move the car to the next landing. No injuries of illnesses as a result. MSHA hotline was called at time of event. Inspector already came to investigate

The worker was trying to shut an entry door that broke and the door handle hit them in the right side of their ribs. Didn't meet reportable criteria until 7/3/2026

EE reports they were starting a conveyer belt and it wouldn't start so they were walking check the emergency stop cables and they were walking down the line and they stepped in some slippery material and they fell and hit their butt and right elbow on a piece of concrete.

While a CAT 777 truck was pulling away after loading, a CAT 992G loader bucket contacted the top of the truck's box. The unexpected impact caused the truck operator to tense up and shift abruptly in their seat, resulting in reported mild back pain.

electricians were attempting to restore power to the Line 2 Clinker Cooler main switchgear when an arc flash occurred. switchgear doors were closed but gaps along the sides of the panel boards allowed radiant heat to escape from the equipment. while positioned slightly to the side of the switchgear, the employee sustained burns to exposed areas of the neck, left wrist, and ear.

The employee identified a material blockage in a rotary feeder and attempted to clear the obstruction using an air lance. While maneuvering the air lance inside the chute, the running rotary feeder pulled the air lance and the employee's hand toward the feeder. The employee's hand became caught resulting in a laceration. No LOTOTO was applied.

Employee was attempting to remove a ceased impellor from a pump casing when a small fragment of metal sheered and punctured the employees left hand.

Two employees were moving pallets of DEF (Diesel Exhaust Fluid). One employee was using a pallet jack and the other employee was operating a forklift. The pallet jack became stuck under a pallet of DEF. The employee pulled forcefully on the pallet jack, it came free and the employee stumbled back hitting EE's back on the end of one of the raised forks of the forklift.

A rail operator parked a fuel truck, applied the parking brake, and used three points of contact to exit the cab. Upon stepping onto flat, dry ground, the employee turned away from the vehicle and experienced sudden pain in the lower right leg and knee, resulting in restricted joint movement. EE was placed on restricted duty.

Employee was setting brake on rail car when felt discomfort in shoulder. Employee refused medical care until 7/7 and was referred to a specialist. Employee was seen by specialist on 7/29 and Diagnosed with shoulder tear.

While performing geotechnical soil borings for new project driller got EE's tip of the left pointer finger smashed. Finger was caught in-between the vice and drill rig while attempting to remove pin to open the vice up to pull augers. Finger needed first aid which was three stiches and driller returned to work after receiving first aid.

A TDF attendant was unstacking tires. The EE pulled a tire off the stack, causing dust to hit EE's face, and enter EE's left eye causing irritation. The EE wore safety glasses, first aid was administered EE reported discomfort on 5/7 and EE went to and urgent care facility. No foreign body was discovered, EE was given no restrictions. The doctor prescribed antibiotic.

Two maintenance mechanics entrapped in the Mill Bldg. elevator after the car stalled between the 2nd & 3rd floor landings. Both employees are uninjured and remain in constant communication with site personnel. Otis Elevator has been notified, a technician was dispatched to the site to safely extract the employees and evaluate the equipment. MSHA was notified immediately.

While operating a John Deere 315 Skid Steer to transfer cement powder out of the mill building as part of a cleaning task, an operator wearing steel-toed boots rested their feet outside the footwell with the bucket slightly raised. This resulted in injuries to the right big toe.

EE working on primary pit crusher when somewhere in the line of equipment was struck by lightning which resulted in the current flowing through the wrench EE was using on the equipment. The electrical current exited EE's arm. EE was knocked onto the ground. Emergency services called and EE was taken to the ER.

Employee had a Reportable hearing loss for noise exposure

EE was dismounting last rung of ladder on a front-end loader and sustained a muscle strain in their right thigh.

The employee had CKD dust in EE's boots, which caused a chemical burn. Because EE wasn't able to wear boots, EE had restrictions on work starting on Apr 14.

The employee was cleaning up used vac hose (6" corrugated piping) when they pushed some filled piping over the the toe plate and as it was going over the end whipped up and slung cement dust into their eyes. Prescription drops prescribed.

Resubmitting new report due to glitch from previous report (E-Doc #3673211). An altercation between two employees. One employee was struck in the face multiple times and received closed fractures to the facial bones

Improper use of PPE, using leather gloves instead of impact gloves. Body positioning, hand placement.

While clearing out full mechanicals on K22, PA had some CKD flush out through the port they were poking through. The dust contact on the PA's skin caused some irritation; later, the PA was diagnosed with second-degree burns in the neck region.

Debris and other unnecessary work material were present in the work area.

Employee was stepping down off of the fan deck of a cooling tower and rolled an ankle. They went to urgent care the following morning and it was determined to be fractured.

Employee was in elevator and it got stuck between levels 1 and 2 on the preheat tower. MSHA hotline was called as required. Fire company and site rescue team were able to safely rescue employee at 7:50 am.

Employee had finished up using a grinder, too EE's face shield and welding coat off when they felt something in the eye. EE was seen by the local clinic, it was a small piece of metal that was removed with a cotton swab and they gave EE antibiotic eye drops.

Finger was caught between two bricks. Employee had a fracture with stitches.

Employee was operating an air hoist when their hand got caught in the chain of the hoist. It smashed is left thumb and index finger causing lacerations.

A contracted carrier loaded with cement was traveling on the plant road. The driver was speeding and ran off the road causing the truck and trailer to tip onto its side. The driver sustained a laceration to the head and was transported to a local hospital where stitches were required and a cracked vertebrae was discovered. Driver was also not wearing a seatbelt.

Employee was using a pry bar to clear a plugged chute. When the rock broke free, the employee hit their hand on the side of the chute, sustaining a fractured finger.

A employee sustained a bursting laceration to the middle finger of their left hand when it became pinched between two pieces of 3/4-inch steel.

Employees onsite smelled smoke at the beginning of the shift. Production supervisor went to see where smoke was coming from and observed a fire adjacent to mine property. At the end of the shift around 6:00 pm the fire had spread to mine property, so production supervisor call the fire department. No employee injuries and no mine asset damage.

Employee was opening and removing an inspection door on a LOTOTO clinker cooler fan. The door fell forward onto the EE's hand breaking a finger

While assisting in barge movement on the lower dock, an employee was struck by a 2" Three Strand twisted polypropylene rope snapback after the rope failed under tension. The force of the line propelled the employee's arm into their chest and mouth, resulting in a fractured tooth requiring a crown, along with contusions and to the lower arm and hand.

The employee performed an annual hearing test at the start of their shift. Results shows that the employee has a reportable threshold shift.

Employee was backing away from dryer crusher and their elbow got caught on the clamshell door and when it did it put their wrist in a bind around the control lever guard of the walk behind skid steer they were operating.

Employee was retrieving tools from the RM500 milling machine and, as they were descending the ladder, their pant leg got snagged on the ladder step and EE lost EE's balance, falling approx. 5 feet to the ground. They landed on their right wrist, resulting in a wrist injury.

Employee's finger was crushed while attempting to remove a piece of rubber from a conveyor

Employee was climbing down two steps off the ANFO rig, placed EE's left ankle on the ground (approximately 12 inches from bottom step) and it rolled when EE released the handrails on the step. Immediately reported to management and eventually found to be non-displaced fracture of the lateral Malleolus of the left fibula.

Sections of the back between 01SB-02SB and 10EB-11EB fell past the anchorage zone at some point in time and was discovered on 12/3/25. The entire area has been bermed off since 2020 so the area is considered inactive. All entrances to the entire bench cell have been barricaded against entry at this time.
